A Review on Quality Control of Korean Medicinal Herbs Through Packaging Standardization (국내 한약재의 유통상 품질 문제점과 포장표준화를 통한 대응방안 고찰)

  • The production, distribution, and consumption of Korean medicinal herbs have increased every year because of an increase of health considerations. But there are no sufficient standardization procedures in packaging and distribution systems related to these herb products. The standardization of packaging material and dimension could be a good addition for establishing proper quality control methods for Korean medicinal herbs. Medicinal herbs deteriorate under physical, chemical and biological hazards such as oxidation, textural change under high moisture content, color change by light, sanitation problems by insect invasion and contamination. Packaging standardization with proper functional packaging materials is critical to ensure the qualities of medicinal herbs. Also, storage and distribution efficiencies can be improved by achieving packaging dimension standardization. This review summarizes recent research trends related to quality evaluation methods and major quality deterioration factors of Korean medicinal herbs. This paper also offers reviews on some of problems and challenges in current packaging and distribution systems of medicinal herb industries and briefly suggests advanced quality control methods through packaging standardization.

Tasks for the Separation of Prescribing and Dispensing medicinal herbs in Traditional Korean Medicine (한의약분업과 관련된 여러 가지 문제)

  • Preconditions for the separation of prescribing and dispensing medicinal herbs in Traditional Korean Medicine are classification of medicinal herbs for general public and special medical uses, establishment of national medicinal herb distribution company of governmental base, restriction in purchase of medicinal herbs for special medical use, partnership between doctors and pharmacists of Traditional Korean Medicine, and coverage of herbal medicine-based medication in national health insurance, etc. The number of Traditional Korean Medicine Pharmacists which was born during 'the herbal medicine conflict' initiated in 1993, goes over 1,000 and will increase by 120 annually. The number of Traditional Korean Medical Doctors is over 17,000 and increases by 850 annually. So in order to engage partnership between two groups, the government have to arrange the number of outputs of each group. Standardization and classification of diagnosis and diseases in Traditional Korean Medicine is a matter of course in the separation of prescribing and dispensing medicinal herbs. Related societies and academies need to do researches with governmental fund first. After these works, we can launch a task force team for implementation of process for the separation of prescribing and dispensing medicinal herbs in Traditional Korean Medicine properly. Entering the national health insurance system for full coverage of Korean Medicine care service will be essential for the patients. Implementation the separation of prescribing and dispensing medicinal herbs in Traditional Korean Medicine would be the core of health insurance coverage for medication.

Study on The Regulation on Poisonous Medicinal Herbs (독성 한약재의 법적 규제에 관한 연구)

  • Objective: Poisonous Medicinal herbs can be considered as a risk factor to public health unless they are prescribed by Doctor of traditional Korean medicine. The proper method to manage them should be prepared to prevent risk factors caused by misuse and abuse of the poisonous medicinal herbs and enhance public health. Methods: In this dissertation (paper), the definition, scope, management status, data about pharmacology and toxicity and media release regarding adverse reaction were understood after organizing documents, laws and regulations concerning poisonous medicinal herbs. Also, management methods are suggested by analyzing related examples and regulations in China, Japan and Hong Kong, where the use of herbal medicine is general. Results: Methods for items for poisonous medicinal herbs, safety information management, management based on standardization of traditional processing methodology and reorganization and revision of related laws and regulations are established. Conclusion: Proper laws and regulations are not yet established to manage poisonous medicinal herbs in Korea. In this regard, it is urgent to establish laws and regulations which can apply independently. The purpose of the laws and regulations should be to enhance management of poisonous medicinal herbs and prevent incidence of addiction and death, improving the public health.

  • Aconiti Root(AR), has been used for about 2000 years, since recorded for low grade in 'Shinnongbonchokyung(Divine Husbandman's Herbal Foundation canon)'. It also recorded for Poisonous Drug in 'Myunguibyullok', 'Yaksungron', etc, and they advised when the AR is using for clinical desease it has to be processed. There are more than 13 processing methods, in records that related 39 parts which is recorded in ancient documents. In China, there are no standard of herb that using for clinical, cause of the traditional processing methods was cut since 1960s. In these days, they are using AR that recorded in Aconiti Tuber part of the processing records, in Guangdong and Shantung. In addition, the modification of Processed Aconiti Root(PAR) is also used for goods in Hongkong, Macao, and Southeast Asia. We studied the PAR for enhancing the importance of it, and multiply using. In this study, we search for the history of processing of AR, traditional theories of processing and clinical adaptation.

  • From the tuberous root of Aconitum carmichaeli Debx.(Ranunculaceae), the main root is called as common monkhood mother root and the later root is called as the prepared aconite root. From the prepared aconite root. Looking at the processing method of the prepared aconite root, it is divided into Yeombuja (prepared aconite root processed in salt) and heuksoonpyeon (baekbupyeon) following the processing method after removing the soil and this is a way of processing the prepared aconite root without damage it. The recently produced raw prepared aconite root is easily damaged, thus it shall be preserved in salt to have the crystal shape on the surface of the prepared aconite root and store and transport in firmly solidified yeombuja condition. Therefore, yeombuja shall remove the salt before use and requires processing for use but heuksoonpyeon or baekbupyeon may use immediately. For the succession of the unique processing techniques of our ancestors, there has to be studies on the techniques. Prepared aconite root is generally used as holy medicines to cure the yang depletion syndrome, kidney-yang deficiency syndrome, and obstruction of qi in the chest syndrome. However, they are the substances with toxicity. It is contemplated that the contents of processing are broadly understood through the document on the processing method, and based on such foundation, the systematic set and proof on the documents are made along with the addition of the contemporary scientific theory and technology to develop the traditional processing technology to maximize the treatment effect and safety of prepared aconite root. In this study, the historic data and records on the processing method of latteral root of aconitum carmichaeli Debx will be rearranged to contribute to the standardization of medicinal herbs, maximization of efficacy and minimization of the side effects.

The Effects of Bojungikgi-tang on Streptozotocin-induced Diabetic Gastroparesis Rat Model (보중익기탕(補中益氣湯)이 streptozotocin 유발 당뇨병성 위부전마비 백서에 미치는 영향)

  • Objective : Diabetic gastroparesis is a complication that is defined as delayed gastric emptying and upper gastrointestinal symptoms and often occurs in long-standing diabetic patients. Bojungikgi-tang (BJT) is a traditional oriental herbal formula that has long been used for the treatment of digestive disorders. The purpose of this study was to investigate the effects of BJT on streptozotocin (STZ)-induced diabetic gastroparesis rat model. Methods : Sprague-Dawley (SD) male rats (250-270g) were divided into 13 groups including normal group, STZ-induced diabetic control group, BJT diet (7 various concentrations), and insulin-, glibenclamide-, metformin-treated group were used for the experiments for the comparison. Diabetic gastroparesis was induced by intraperitoneal injection of STZ. The water intake, food intake, body weights and fasting blood glucose levels were measured. After 4 weeks the animals were sacrificed and gastrin, leptin, insulin, hemoglobin A1C (HbA1c), lactate, lactate dehydrogenase (LDH), bilirubin, creatinine, albumin and lipid levels were evaluated. Results : Intraperitoneal injection of BJT for 4 weeks resulted in increased levels of gastrin in blood and decreased leptin and lactate concentration in STZ-induced diabetic gastroparesis rat model. BJT did not affect insulin, fasting glucose, HbA1c, and lipid levels in STZ-induced diabetic gastroparesis rat model. Conclusion : These results indicated that BJT would have protect effect on diabetic gastroparesis through the improvement effect of gastric motility and fatigue syndrome in STZ-induced diabetic rats. This study shows that BJT might be effective for treatment of diabetes and its complications such as gastroparesis.

The effects of Socheongryong-Tang on LPS-induced lung inflammation rats model (소청룡탕이 LPS로 유도된 폐손상 동물모델에 미치는 영향)

  • Objectives : In present study, we investigated a therapeutic effect and optimum dose of Socheongryong-Tang (SCT) on LPS-induced lung inflammation rats model. Methods : Male Sprague-Dawley rats ($260{\pm}10g$) were divided into 12 groups : Group 1 included the normal rats, and Group 2-12 were administrated LPS by intranasal injection to induce experimental lung inflammation. After 1 day of LPS administration, Group 3-9 were treated with SCT ${\times}1/4$, ${\times}1/2$, ${\times}1$, ${\times}3$, ${\times}6$, ${\times}12$ or ${\times}18$, respectively. Group 10-12 (positive control) were treated with dexamethasone 1 mg/kg or acetylcystein 1.5 mg/kg or diclofenac sodium 0.4 mg/kg, respectively. After sacrifice, bronchoalveolar lavage fluid (BALF) was isolated. The levels of IL-$1{\beta}$, TNF-${\alpha}$, mucin glycoprotein 5AC (MUG5AC) were measured in BALF using enzyme-linked immunosorbent assay (ELISA). Results : LPS injected rats exhibited outstanding lung inflammation manifestations, including increased amount of total cells and neutrophil, and upregulated inflammatory cytokines level in BALF. However, the administration of SCT ${\times}1/4$, ${\times}1/2$ and ${\times}1$ decreased total cells and neutrophil, and suppressed the production of inflammatory cytokines, including $IL-1{\beta}$ and TNF-${\alpha}$, and MUG5AC in BALF. Notably, inhibitory effect of SCT ${\times}1/2$ and ${\times}1$ on the level of TNF-${\alpha}$ was markedly better than that of positive controls, dexamethasone and acetylcystein. Conclusions : Taken together, these results suggest that SCT ${\times}1/2$ and ${\times}1$ has therapeutic effects on LPS-induced lung inflammation rats model.

Morphological Analysis Study for the Development of DB on the Medicinal Herbs Manufacturing Process - with focus on the manufacturing method of Rehmanniae radix - (본초 제조 공정의 DB화를 위한 형태소 분석 연구 - 숙지황 제조 공정을 중심으로 -)

  • Objectives : Treatment method using drugs has already been used in Korean medicine for a long time. Moreover, database has been developed and utilized for more efficient management of the treatments that use drugs. Most of such database related to knowledge on drugs is composed of origin, efficacy, temperament, ingredients and examples of application of the standardized drugs. Communication with knowledge information in other specialized areas is also accomplished by using the efficacies and ingredients with the drugs. In this study, we aimed to make data structure of the terminologies that represent the manufacturing process of herbs. However, in spite of the fact that the manufacturing process of the drugs imparts effect on their efficacies and ingredients, details of the manufacturing processes are quite limited to simple text sentences, thereby resulting in substantially lower level of utilization and difficulties in systematic researches on various factors included in the manufacturing processes in comparison to other knowledge on drugs. Methods : This Study extracted the factors necessary in the development of database by executing morphological analysis of the manufacturing process of herbs. Results : The factors are 'Order', 'Act', 'Raw material', 'Tools', 'Supporting materials', 'Intensity', 'Duration Time', 'Interval', 'Focus', 'Repetition Number', 'Untill'. We were able to tell the difference of the manufacturing process with a simple structured query language and the factors. Conclusions : Morphological analysis of medicinal herbs manufacturing Process contributes to standardization with information of the manufacturing process. And it helps to creates a quality management system through the Database.

A Study for the Standardization of Elsholtzia ciliata (Thunb.) Hylander and Elsholtzia splendens Nakai ex F. Maekawa

  • The purpose of present study is to clarify the differences between EIslwitzia Ciliata (Thunb.) Hylander(향유) and Elsholtzia splendens Nakai ex F. Maekawa (꽃향유) for standardization and the proper usage as medicinal herbs. The major ingredients of both species were isolated by distillation and extraction. The qualitative and quantitative analyses of major distillates were carried out by the use of GC/MS. There was a significant difference between the components of Elsholtzia ciliata and Elsholtzia splendens in the aspects of major components. Several common ingredients were identified as linalool, cumene, elsholtzia ketone, naginata ketone isomer, naginata ketone, myristicin, and sesquiterpene alcohol. Comparison between Elsholtzia. ciliata and Elsholtzia splendens was done in the aspect of major compounds. Myristicin (33.7%) has been shown to be the major component in Elsholtzia ciliata whereas naginata ketone isomer (26.1%) was believed to be a major ingredient in Elsholtzia splendens. The elsholtzia ketone was also one of the major differentiating factors between Elsholtzia splendens and Elsholtzia ciliata, and the quantity is 15.1% in Elslwltzia splendens compared to 2.87% in Elsholtzia ciliata. Moreover, in the Elsholtzia splendens, 4- vinylguaiacol and isoosmorhizole were absent, but both compounds were present in the EIsholtzia ciliata.

Introduction of the International Standardization of ISO in the Production and Quality of Herbal Medicines and a Review of Countermeasures (한약재 생산 및 품질부문의 ISO국제표준화 등재현황 소개 및 대응방안 고찰)

  • Recently, the international standardization of ISO in the field of Oriental Herbal Medicine has been progressing rapidly under the direction of China's promotion. China's intention to promote international standardization is to extend its influence to the world and beyond the domestic market. The Oriental medicine system in East Asia has similar roots in academic terms, but the medicines that can be supplied and received in each country are different and have developed independently. The international standardization of medicinal herbs is expected to function in a direction that weakens such differentiation and independence. From a commercial point of view, international standardization is no different from creating evaluation criteria for oriental medicinal products, and it is expected that its potential impact on domestic and overseas markets and producers will be large. In particular, the international standardization centered on China can lead to favorable evaluation criteria for China, which may further negatively affect the market competitiveness of domestic raw materials, which have been pushed back by Chinese manufacturers. If the domestic production base is weakened, not only will the farmers suffer but the supply and demand of raw materials will also be manipulated, safety management control will be reduced, and the development of oriental herbal products using domestic raw materials will be hurt. Therefore, in the promotion of international standardization, it is necessary not only to reflect the value of Korean herbal medicine but also to provide strategic responses to protect the domestic production base. However, in the case of recent initiatives, there is no precedent in analyzing influence on the production partners and the related industries. In addition, there are few related papers and reports on the subject, so the publicity process has not been done sufficiently. In response to this, this study will examine the countermeasures against the international standardization of herbal medicines through reviewing its present status and evaluating the agenda of the Korean initiative.
