A CFD Simulation Study on the Isolation Performance of a Isolation Ward (CFD를 이용한 격리병동의 격리성능 검토)

  • Purpose: In this study, we performed ventilation simulations for a standard isolation ward including three intensive care rooms, one anteroom(buffer room), and its recommended ventilation equipments. The purpose of this study is to predict outflow of pathogenic bacteria from patient breath to verify the reliability and the safety of the isolation ward. Methods: We suppose three scenarios of the movement of medical staff. The leakage of patient's breath to out of the ward is predicted in these scenarios using CFD simulations. Results: The patient's breath leakage rate to out of the ward in scenario 1 according to room air changes per hour(ACH : 6 and 12) is predicted to be 0.000057% and 0.00002%, respectively. The patient's breath leakage rate to out of the ward in scenario 2 according to room air changes(ACH : 6 and 12) is predicted to be 0.00063% and 0.00019%, respectively. The patient's breath leakage rate to out of the ward in scenario 3, which is the worst case(6 room air changes) is predicted to be 0.1%. Implications: Through the ventilation simulation like that in this study, the reliability and the safety on isolation performance of various plan of isolation ward are predicted quantitatively.

A Study on the direction for Facility Improvement of Nationally Designated Negative Pressure Isolation Ward through Post Occupancy Evaluation (거주 후 평가를 통한 국가지정 입원치료병상 시설 개선 방향 도출에 관한 연구)

  • Purpose: The negative pressure isolation ward is a key facility in preparedness and response to infectious diseases. For the sustainable operation of the facility, appropriate facility improvement is required. The experience of medical staff responding to infectious diseases in the COVID-19 pandemic provides effective informations for facility planning. Methods: The post occupancy evaluation (POE) was conducted by interviewing medical staff who is working on Nationally designated negative pressure isolation ward in general hospital. Floor plan analysis was conducted before field surveys for identifying facility characteristic and spatial composition. After that, field surveys were conducted at 3 hospitals, and interviews and fieldwork were conducted together. Results: It is necessary to increase the standard size of ward area from 15m2 to 20m2. The size of the doffing room has to be planned for accommodation of two or more people. Equipment storage, clean storage and waste storage also should be properly planned. There were almost no problems with the circulation in the ward. There was not enough space for medical staff. Implications: For a sustainable and safe negative pressure isolation ward planning, it is necessary to exploit learning from the medical staffs who have many experiences of coping with infectious diseases.

  • According as the aging procese have been high, last stage patient become more important. Therefore, todays hospice ward of hospital is glandized, specified too. However, its study is few performed in domestic and its architectural planning data is few obtained. Therefore, this study aims to represent the standard of the architectural planning of the hospice ward in hospital. As its method, the present conditions, the interview with staffs, space programs of 4 hospice ward were investigated and analyzed, add to 2 free standing hospice were selected, theirs architectural space programing were studied.

A Study on the Functional Efficiency according to Composition of Single Bedroom in Ward - Focused on Distance from NS to Bed - (종합병원 병동부 1인 병실 적용에 따른 기능효율성에 관한 연구 - 간호거점에서 병상간의 거리를 중심으로 -)

  • Purpose: This study examines distance from nurse station to bed according to composition of single bedroom. Methods: This study has two groups of target sample ward planes. The first group has diverse patients bedroom. The other group is 36 single bedrooms and 2 3-bedrooms. The guideline of distance measurement was instituted for consistency. This study has 3 main concepts of the guideline. 1) The distance is shortcut from NS to bed. 2) The traffic line is center line between walls and center of door. 3) The start point is center of NS and the end point is bed. Results: The result of this study can be summarized into two points. The first one is that single bedroom has good privacy and longer distance from NS to bed. The second one is that weak point of single bedroom was asked ward operating system and architectural planning like as patients room, bed per unit, location of NS and plan type. Implications: Consideration Should be taken into account for the effective bedroom composition and allocation in ward. This Stuy hopefully serve as a stepping stone for the standard design of space program in ward planning.

A Study for Establishment of High Dose Radioiodine Therapy Patients' Release Standard

  • This study, which is proceeded in the department of nuclear medicine, aims at preventing unnecessary radiation exposure to the patients and the people near the patients by understanding and presenting the realities about the isolating period for the high dose radioiodine patients after total thyroidectomy in the 7 general hospitals in metropolitan area. Theoretically, the physical half-life of the high dose radioiodine is 8 days. Radioiodine lower than 100 mCi usually is eliminated all in 2 days 1 night considering the biological half-life and the amount of excreting radioiodine The hospitalization standard of the patients treated with high-dose radioiodine therapy has been established according to the fact above. Investigation of the data and questionnaire from the hospital have proven that some hospitals didn't even measure the acceptable dose because of the faith in the vague data. Besides, the some of those inevitably let the readmitting patients exceeding the acceptable dose be in the general ward, not in the isolation ward, because the number of the isolation rooms is relatively smaller than the patients. Thus, we want to contribute that patients understand the realities and the hospitals consider the relevant problem actively so that the problem will be settled by this journal.

  • The Nursing Care Integrated Service is an inpatient service provided in a ward equipped with all kinds of professional nursing services, nursing environment improvement, and patient safety management through team nursing staff placement through appropriate nursing staff placement. The subjects of the study were 92 patients who agreed to understand and participate in the research purpose as the inpatient ward of a general hospital in Gyeonggi Province, for the study method, frequency, percentage, mean, standard deviation, t-test, ANOVA were applied using SPSS/WIN 21.0 program and the post test was Scheffe test. In addition, the correlation between nursing satisfaction and service evaluation was analyzed by Pearson's correlation. This paper is meaningful in that it reaffirms the importance of satisfaction and presents basic data for improving the quality of service of nursing care integrated service ward inpatients.

Comparision Between Noise Levels of Hospital Wards and the Nurses Efforts for Noise Management in Selected General Hospital (종합병원 병동별 간호사실의 소음정도와 간호사실들의 소음인지도 및 소음관리노력 비교)

  • This study was performed to find out the differences between noise levels of hospital wards and the nurses efforts for noise management in some general hospitals. The hospital wards selected were the intensive care unit(ICU), the emergency room(ER), the nursery room(NR), the internal medicine(IM), the general surgery(GS) among the 5 general hospitals located in Seoul. The data were collected from August 3 to September 13, 1999 through questionnaire survey and noise measurement in each nursing station of hospital wards. Data analysis was done by SPSS 8.0 package among the 305 questionnaires and 24 hours monitored noise levels. Frequency, Chi-square and ANOVA test were used. The study results were as belows: 1. The noise level measured by 24 hours monitoring survey were exceeded on the standard limit in all the hospital wards. Data also showed that noise levels were significantly different in each ward among the three shifts working duties. 2. The subjects were all female nurses. They were mostly working in the ICU ward(28.9%). They were 26~30 years old (43.9%), junior college graduates(57.0%), working for 1~5 years(55.1%) as staff-nurse(85.6%). There were no significant differences between hospital wards and general characteristics of nurses. 3. The noise levels perceived by nurses were regarded as 'Highly noisy'(56.4%), especially during the 11:30 and 15:30 (30.2%) o'clock. Data also showed that noise education was not ever given to nurses(89.9%). Nurses also responded that they hardly put an effort to reduce noise level(54.8%). However, there were significant differences between wards and noisy working time, experience of noise education and level of effort for noise reduction. 4. Nurses also perceived the ventilator alarm and EKG-alarm as the most disturbing sounds in the ICU, human voice and telephone ringing in the ER, human voice and EKG-alarming in the NR, human voices and telephone ringing in IM and GS both wards respectively in order. There were significant differences between hospital wards and noise making factors. 5. Nurses were shown that they regarded highly 'Sound reduction of the human voice', 'Careful handling on medical instruments', and 'Immediate appliances on alarming materials' as the practical method for noise management. There were significant differences between hospital wards and behavioral practical efforts for noise management. According to that results, the statistical differences were shown in the 24 hour monitored noise levels in each ward. Also, nurses perceived the noise severity differently and they approached variously on the practical efforts for noise reduction in each ward. Thus, author thinks that concrete and systematic endeavor will be necessary for noise reduction and management in hospitals for better working and healing environment for both of patients and staffs.

Are Medical Personnel Safe from Radiation Exposure from Patient Receiving Radioiodine Ablation Therapy? (갑상선 암의 방사성요오드 치료 시 의료진은 방사선 피폭으로부터 안전한가?)

  • Radioiodine ablation therapy has been considered to be a standard treatment for patient with differentiated thyroid cancer after total thyroidectomy. Patients may need to be hospitalized to reduce radiation exposure of other people and relatives from radioactive patients receiving radioiodine therapy. Medical staffs, nursing staffs and technologists sometimes hesitate to contact patients in radioiodine therapy ward. The purpose of this paper is to introduce radiation dosimetry, estimate radiation dose from patients and emphasize the safety of radiation exposure from patients treated with high dose radioiodine in therapy ward. The major component of radiation dose from patient is external exposure. However external radiation dose from these patients treated with typical therapeutic dose of 4 to 8 GBq have a very low risk of cancer induction compared with other various risks occurring in daily life. The typical annual radiation dose without shielding received by patient is estimated to be 5 to 10 mSv, which is comparable with 100 to 200 times effective dose received by chest PA examination. Therefore, when we should keep in mind the general principle of radiation protection, the risks of radiation exposure from patients are low and the medical personnel are considered to be safe from radiation exposure.

Protocol Development of Cardiopulmonary Resuscitation Nursing Tasks targeting Patients with Ventricular Fibrillation Generation (심실세동 환자의 심폐소생술 간호업무 프로토콜 개발)

  • Purpose: In order to improve resuscitation rate of CPR through providing qualitative nursing while performing CPR in hospital ward and to enhance quality of emergency nursing in the ward, the author embarked on the research as a way of developing ventricular fibrillation protocol about CPR. Method: Collected data were analyzed by using the SPSS/PC14.0 program while the routes of final protocol developed in this research are as follows. Result: Based on analysis results of literature study and CPR electronic hospital records, a total of 22 items and 53 specific contents were confirmed through the gathering of opinions from panels, and the allotment of roles and tasks with a standard of 2 nurses was designated. As the result of specialists' verification on validity, the final protocol composed of a total of 23 items and 45 specific contents was confirmed. At the result of the pertinency evaluation of confirmed protocol, it was evaluated as relatively pertinent with its average score 2.89-3.32. Conclusion: The protocol developed in this research is seen as to contribute to nurses participating in CPR to avoid the overlapping of tasks and to develop CPR through effective teamwork of medical teams by presenting clear roles and tasks.

Customer Load Pattern Analysis using Clustering Techniques (클러스터링 기법을 이용한 수용가별 전력 데이터 패턴 분석)

  • Understanding load patterns and customer classification is a basic step in analyzing the behavior of electricity consumers. To achieve that, there have been many researches about clustering customers' daily load data. Nowadays, the deployment of advanced metering infrastructure (AMI) and big-data technologies make it easier to study customers' load data. In this paper, we study load clustering from the view point of yearly and daily load pattern. We compare four clustering methods; K-means clustering, hierarchical clustering (average & Ward's method) and DBSCAN (Density-Based Spatial Clustering of Applications with Noise). We also discuss the relationship between clustering results and Korean Standard Industrial Classification that is one of possible labels for customers' load data. We find that hierarchical clustering with Ward's method is suitable for clustering load data and KSIC can be well characterized by daily load pattern, but not quite well by yearly load pattern.
