Analysis of paramedic students' needs for the major theme of emergency medical technology Using Borich need assessment and The Locus for focus model

  • This study aims to provide basic data for reinforcing the learning competency of paramedic students by analyzing the performance, importance, and demand for the major curriculum of them. The participants of the study was 217 students from the Department of Emergency medical technology from 3 universities in Chungnam, and the survey data collection period was from December 13 to December 24, 2021. As a result of the study, 'Education for Ambulance management', 'Education for maintaining professionalism after graduation', 'Education for In-hospital patient monitoring' are highly required by Borich need, and 'Education for medical oder from a doctor, Education for han dover to In-hospital medical staff', 'Education for non-traumatic emergency patient treatment', 'Education for In-hospital patient monitoring', and 'Education for In-hospital medical assistance' are the top priority areas of the LF model. It is judged that it is necessary to reinforce the curriculum corresponding to in order to strengthen the learning capabilities of paramedic students.

Community Care for Cancer Patients in Rural Areas: An Integrated Regional Cancer Center and Public Health Center Partnership Model

  • Purpose: The accessibility of medical facilities for cancer patients affects both their comfort and survival. Patients in rural areas have a higher socioeconomic burden and are more vulnerable to emergency situations than urban dwellers. This study examined the feasibility and effectiveness of a cancer care model integrating a regional cancer center (RCC) and public health center (PHC). Methods: This study analyzed the construction of a safety care network for cancer patients that integrated an RCC and PHC. Two public health institutions (an RCC in Gyeongnam and a PHC in Geochang County) collaborated on the development of the community care model. The study lasted 13 months beginning in February 2019 to February 2020. Results: The RCC developed the protocol for evaluating and measuring 27 cancer-related symptoms, conducted education for PHC nurses, and administered case counseling. The staff at the PHC registered, evaluated, and routinely monitored patients through home visits. A smartphone application and regular video conferences were incorporated to facilitate mutual communication. In total, 177 patients (mean age: 70.9 years; men: 59%) were enrolled from February 2019 to February 2020. Patients' greatest unmet need was the presence of a nearby cancer treatment hospital (83%). In total, 28 (33%) and 44 (52%) participants answered that the care model was very helpful or helpful, respectively. Conclusion: We confirmed that a combined RCC-PHC program for cancer patients in rural areas is feasible and can bring satisfaction to patients as a safety care network. This program could mitigate health inequalities caused by accessibility issues.

A Study on the Architectural Planning of the Space and Area Composition of Health Examination Center in Regional Public Hospitals (공공병원 건강검진센터의 공간과 면적구성에 관한 건축계획 연구)

  • Purpose: Health examination centers of regional public hospitals are essential facilities for strengthening preventive medical services in local communities. This study is to organize architectural planning data related to function, space, and area composition of health examination center by the size of the regional public hospitals. Methods: The literature review on the function and spatial composition of the health examination center and the drawings of the regional public hospitals were analyzed. Results: The result of this study can be summarized into several points. 1) The function of the health examination center consists of a diagnostic area, patient area, and staff area based on general examination and comprehensive examination. 2) The type of spatial composition of the 300-bed public hospital examination center is a spatial linkage type with examination rooms of the other departments where general examination and comprehensive examination are undifferentiated. 3) The examination center of public hospitals with 500 beds or more is composed of an independent space-separated type with self-installed examination rooms, and general examination and comprehensive examination are divided. 4) The examination center of public hospitals with 300 beds around, the diagnostic area occupies most of the total area, around 80%, but in public hospitals with 500 beds or more, the proportion of diagnostic area drops to 50-60%, and patient area increases to 30-40%. Implications: The examination center planning of public hospitals requires an architectural planning approach to the function and spatial composition according to the size of the hospital.

Understanding the Current Trend of Home Care Assessment in the Post-COVID-19 Era by Comparing Outcome and Assessment Information Set (OASIS)-D and E (미국 환자사정도구(OASIS) 개정 사례를 통한 포스트코로나 시대의 가정간호 환자사정 동향 파악)

  • Purpose: This study aimed to comprehensively analyze the recent revision of the Outcome and Assessment Information Set (OASIS)-D to version E in the United States to identify the most recent trends in home health care and provide suggestions for implementing home health care in Korea. Methods: This study is a comprehensive literature review. We compared all items in OASIS-D and OASIS-E and identified the most significant changes. Additionally, we have discussed the context in depth. Results: The addition of items related to health literacy and medication reconciliation, strengthening assessment in the cognitive and mood domains, and improving items related to pain assessment were identified. These major revisions resulted from a greater emphasis in the healthcare field on social determinants of health. Additionally, the COVID-19 outbreak highlighted the importance of monitoring and managing the symptoms of home care patients in the community. Conclusion: The key items of the OASIS amendment and their background analysis proposed in this study could serve as a cornerstone for the revision of home health care assessment tools or the development of systematic common assessment tools in Korea.

Survey on Nursing Care Delivery Systems of University Affiliated Hospitals in Korea (종합전문요양기관의 간호전달체계에 대한 실태조사 연구)

  • Purpose: This study investigated nursing care delivery systems in 44 university affiliated hospitals and satisfactions with the systems perceived by 226 unit managers (head nurses) of general medical surgical wards. Methods: Data were collected with questionnaires consisting of checklists asking the unit managers their nursing care delivery systems and their satisfactions with the systems. Results: Four models of nursing care delivery systems (primary, modified primary, team, and functional models) were drawn from the participants' responses. Among the four key models 35% of the units adopted team model whereas 24.3% adopted primary model and 22.6% adopted modified primary model. In spite of 35% of team model being under use, 60.6% (n=137) of the unit managers answered the nursing delivery system of their units as team model and only 6.2% (n=14) answered their units having primary or modified primary models, instead of 46.9% combining both. In regard to the satisfaction, critical thinking ability of staff nurses (members in their units) was the most dissatisfactory area regardless of models of service delivery. Conclusion: Introducing team model supplemented with core concepts of primary model (primary team delivery model) into nursing practice will reform the workplace and therefore deliver safe health care services to patients.

An Action Convergence Research to Improve Evaluation Index of Pneumonia Adequacy in Regional Public Hospital (지역거점 공공병원의 폐렴 적정성 평가 지표 개선을 위한 융복합 실행연구)

  • This is an implementation study towards the development of a strategy to improve the pneumonia adequacy evaluation index of regional public hospital and to confirm its effectiveness. One physician, eight nurses, one computer room staff, and one nursing professor working at a regional base public hospital located in N city participated in this study from March to October 2021. An implementation study was conducted based on the conceptual model of Zuber-Skerritt & Fletcher. The stages of planning, implementation, evaluation, and reflection were operated in two cycles, and the effectiveness was evaluated in the second and third quarters. Compared to the pneumonia adequacy evaluation index in 2019, all indicators reached 100% of the standard target in 2021. The results of this study therefore infer that to provide higher quality medical services in the future, it is necessary to apply medical quality improvement activities through practical research under multidisciplinary participation and cooperation and to review the applicability of clinical sites.

Analysis of the Response of School Food Service Operations to the COVID-19 Pandemic in 2020-2021 (2020~2021년 코로나바이러스감염증-19 대응을 위한 학교급식 운영 현황)

  • This study aimed to examine the operations of school food services to prevent the spread of coronavirus disease 2019 (COVID-19) in schools nationwide. The survey data on school food service operations targeting nutrition teachers and nutritionists at 1,023 schools in 2020 and 1,177 schools in 2021 were used. The year 2021 saw an increase compared to 2020 in the following: 'average days to be served with meals for a year (144.5 vs. 184.7)', 'provided meals to all students (92.3% vs. 96.6%)', 'utilization of additional staff for foodservice assistance (33.4% vs. 38.8%)', 'installation of partitions (61.2% vs. 83.8%)', 'provision of general diet (96.1% vs. 99.1%)', and 'use of kitchen utensils (91.3% vs. 95.1%)', 'use of cafeteria water cup (9.9% vs. 31.0%)' and 'use of drinking water in school (46.8% vs. 52.1%)'. Compared to 2020, in 2021, it was confirmed that the school food service operations stabilized due to the increase in the normal school attendance rate and that systems were in place for operations during the COVID-19 pandemic. In the future, it will be necessary to develop manuals and special recipes necessary for responding to infectious diseases, and to operate a manpower pool that can quickly find replacement personnel if required.

  • The Internet of Things, which began by connecting sensors through a network, is developing into an intelligent IoT by combining it with artificial intelligence technology. Elevators are essential for high-rise buildings in the city, and elevators move from floor to floor and perform the functions of transporting goods and moving users. It is necessary to provide safe and convenient services for elevator users in high-rise buildings or special environments (hospitals, etc.). In an environment where rapid patient transportation is important, such as large hospitals, there is a problem that hospital staff and the general public often use the elevator for patients. In particular, when moving patients where golden time is important, the waiting time to board the elevator is a major hindrance. In order to solve this problem, this study proposes an intelligent IoT system for elevator calling using smartphone Bluetooth. First, we experimented with the elevator calling IoT system using smartphone Bluetooth, and as a result of the experiment, it was confirmed that it can authenticate elevator users and reduce unnecessary waiting time for boarding. In addition, we propose an intelligent IoT system that connects with intelligent IoT.

Methods of Record Management for Head of Local Government (광역자치단체장의 기록 관리 방안 연구)

  • This study suggested the methods of record management for the heads of local government, which would be the most valuable among local records. In order to conduct a systematic record management for the heads of local government, this study suggested the methods of establishing a record management system regarding regulation arrangement, production registration, preservation, utilization and services. First of all, in order to estimate the record category of the heads of local government, the study examined the duties of the offices of the deputy heads of local government, secretary's offices and information offices, which have been subsidiary & assistance branches in charge of producing the record. In addition, it investigated the present conditions of record management for the heads of local government through the interviews with secretary offices and information offices belonging to 16 cities and provinces and the claims for information disclosure and found out the following problems. They included incomplete record production, non-registration of produced records, abolition of records and taking them out of designated places with due notice, record preservation period regardless of the term of the heads of local government, varied preservation period for the records of the heads of local government by local self-government, short preservation period of primary records and non-management of home pages after the term of the heads of local government. To solve such problems, the study suggested the regulation arrangement for record management and a record management system. The regulation arrangement could be obtained through the establishment of the administrative organization setup condolence etiquette enforcement regulation and the recorders in local government and the revision of operation rules and through the revision of the reference plan for operation rules enactment of recorders from National Archives of Korea. As for the record management system, the study suggested the establishment of production, registration and preservation system of records for the heads of local government and the utilization and services of their records. In order to produce and register the records, the unit assignments should be founded by department in charge of the duties related to the records of the heads of local government on record management criteria, thus letting the staff surely produce and register the records. In terms of utilization and services of the records, the study suggested the use of websites and drawing up the record list, through which each record viewer would be able to figure out which records have been managed through the list services and which services could be given to the residents, thus letting the residents and the heads of local government who finished their term of duties use the records.

A Study on the Information Usage Behavior of Researchers in the Field of Ocean Science and Technology (해양과학기술 분야 연구자의 정보이용행태에 관한 연구)

  • The purpose of this study is to explain information usage behavior of researchers in the field of ocean science and technology. The study mainly collected primary data for advancement of special library services as well as establishment of personalized information services based on personal characteristics such as age, education level, and area of research. The data collection was conducted for two weeks during January 2014, through a web survey to 348 researchers in national ocean research institutions in South Korea. Total of 115 researchers replied. The analysis showed that the most preferred type of information medium was a scholarly journal. Researchers used more foreign published journals compared to Korean ones, while favoring digital formats rather than printed ones. The top channels for information collection were 'web search' and 'affiliated libraries.' Most pointed out difficulties of data collection were 'lack of variety of digital resources in affiliated libraries' and 'reluctance to use charged information.' Key elements for satisfactory user experience were ranked in the order of 'digital library system,' 'library staff,' and 'library collection' and so on;which proves the close relationship between library service and information usage service satisfaction. The result of an assessment for demands in special libraries showed that 'personalized information search service,' 'project support service,' and 'research direction analysis service' should be implemented in the future.
