Development of Hospice Model Networked with the Existing Welfare Systems in Gimhae City (김해지역 호스피스 보건복지연계 모형개발에 관한 연구)

  • Purpose : With the elongation of the average life span, the main causes of death are chronicle illness including cancers resulting in a large amount of medical resources. And there are still many patients to whom a sufficient medical care is not given. All these bring on the uneven distribution of medical resources together with the increase of medical cost. Hence, an efficient system should be set up to solve these problems. Methods : The hospice draws a great attention as a resolution of high medical cost and uneven medical resource distribution, and has been proved to be an alternative to the existing medical system. So, the characteristics of the hospice has been reviewed, particularly with respect to its scopes and related resources. And by tracking the actual cases, the necessary services and supports are investigated. Results : The intrinsic characteristics of hospice is that it executes not only the medical exercise but also all the subjects related with patients and their families. And the hospice is performed not only by the medical experts but also by all the disciplines including social and spiritual affairs. This indicates that the hospice requires the integrated system comprised of medical, social and other welfare entities. Conclusion : To establish the actual hospice, an efficient and systematic integration of all the existing medical and other welfare resources in the local society is necessary. The most practical way is the networking of resources, which practices the hospice more efficiently without additional investment.

Discharge Nursing Needs of Patients having Surgery with Gastric Cancer (위암 수술 환자의 퇴원 간호요구)

  • Purpose: The purpose of this descriptive study was to identify the discharge nursing needs of patients who received gastrectomy. Method: The subjects of this study consisted of 100 patients who had gastric surgery in 4 university hospitals in chungnam province. The instrument for this study was developed by the researcher based on literature through in-depth interviews. Data was collected from October 4 to October 30, 2000. Result: The results were as follows: Discharge nursing needs were classified in 8 main categories. The 8 categories were 'emotional and spiritual support', 'nutrition and diet control', 'medication', 'treatment planning and follow up care', 'activity level', 'symptom management' 'health seeking behavior' and 'social support'. The total mean score for nursing needs was 3.89. 'Nutrition and diet control' was the highest degree of nursing needs and 'activity level' was the lowest degree of nursing needs. There were significant differences in degrees of nursing needs between different ages(F=2.74, p=.048), level of education(F=2.91, p=.038) and period since diagnosis(F=4.46, p=.037). Conclusion: This study looked at 8 categories of discharge nursing needs. Patients identified 'nutrition and diet control' as the highest need and 'activity level' as the lowest need. Further research needs to be done to identify discharge educational needs for various age groups, educational level and for patients with varing lengths of diagnosis time to surgery.

Validation of Quality of Life Index-Cancer among Korean Patients with Cancer (Quality of Life Index-Caner의 구성타당도 검증 -국내 암환자를 대상으로-)

  • Purpose: The purpose of this study was to validate Quality of Life Index-Cancer (Q.L.I.-C) developed by Ferrans (1990) among Korean cancer patients. Method: This study design was exploratory factor analysis methodology. Q.L.I.-C was translated into Korean and reverse-translated into English. The subjects were 357 Korean patients with various cancers. Data were collected by questionnaires from May to August, 2000 and was analyzed by descriptive statistics, Principal Component Analysis for construct validity and Cronbach's alpha coefficient for reliability. Result: The range of factor loadings was .446~.841. The explained variance from the 5 extracted factors was 63.7% of the total variance. The first factor 'family' was 35.5%, and 'health & physical functioning', 'psychological', 'spiritual', and 'economic' factors were 11.5%, 6.9%, 5.6%, and 4.2% respectively. Because of cultural difference between Americans and Koreans, certain items such as sexuality, job status, and education were deleted from the extraction of factors in this study. The Cronbach's alpha coefficient was .9253 among the 28 items. Conclusion: Q.L.I.-C could be applied in measuring quality of life of Korean cancer patients. It also recommend to do further studiesfor validation of Q.L.I.-C American and Korean versions relating to cultural differences.

A Study on Adolescents' Religious Activities and Life Satisfaction (청소년의 종교활동과 생활만족도에 관한 연구 - 개신교회에 출석하는 고등학생을 중심으로)

  • The purpose of this study is to analysis the effects of religious activities on adolescents' life satisfaction. For this purpose, we surveyed the general trend of adolescents' religious activities and life satisfaction as well as life satisfaction according to demographical variables. In addition, this study analyzed adolescents' life satisfaction according to variables related to individual adolescents' religious activities and to parents' religious activities. The analyses of the results are as follows. First, since the childhood, adolescents have been participated in religious activities habitually or by enforcement or encouragement of their acquaintances. However, most adolescents think that the main purposes of their religious activities are a receiving grace and self-growing. So, they are willing to participate in worship service and spiritual training camp. Second, adolescents' life satisfaction was high when they got a high grade in school or they felt their health condition is good. The life satisfaction of adolescents who belong to economically high class families and whose fathers were college graduates or above is high. Third, the facts that effect on adolescent's life satisfaction are not how long they spend their time on religious activity, but the contents, motives and purposes of religious activities and satisfaction with religious activities. Fourth, adolescents' life satisfaction was high when their parents have same religion to them and their fathers participated in religious activities and their mothers did not participate in religious activities.

  • Due to the increasing social participation of women and the decrease in the size of the typical family, childre's environments have become increasingly more important. In fact, children are one of the main groups who use the community environments. However, children as a group have not been considered in the planning of residential neighborhood environments. A child's spiritual, psychological and physical health develops upon their interaction with the surrounding environment. Residential community should therefore be planned in a child-friendly way. The aim of this study is to introduce possible criteria for developing design guidelines for Children-friendly residential environments. The principal objective is to investigate the actual conditions of children's lifestyles in residential environments and the exploration of design guidelines for children. In this study, a small group workshop panel method was used. The subjects were 4 panel groups which was consisted of households with children. Each of the panel included 2 consecutive workshops. In this study, a list of valuable criteria for design guidelines was identified. In addition, the current residential community environment was found to be inadequate to accommodate for children's lifestyles and children had different housing needs according to their ages. In conclusion, the design guidelines should be developed to provide children with developmentally appropriate community environments.

The Construction of the Domains and Indicators Measuring and Monitoring Children's Well-Being in Korea (한국의 아동 웰빙 지표 구축을 위한 기초 연구 : 아동 웰빙 지표의 영역과 세부 지표를 중심으로)

  • While efforts to measure and monitor children's well-being have gained increasing recognition across the OECD, there has been relatively little research on the subject of indicators of children's well-being in Korea. This study was undertaken in order to develop the domains and indicators which can be used to measure the quality of life of Korean children from birth to age 17 and to put forward a possible well-being index for Korean children. The 35 indicators of well-being were grouped into eight domains and were designed for this study, using previous research on key child indicators for Korea and the child well-being index in foreign countries, such as the FCD-Land Index, Kids Count, the index of child well-bing in the European Union and OECD area. These domains are composed of the following : economic well-being, health, education, spiritual/emotional well-being, children's relationships, civic participation, safety/risk behavior, and housing and environment. The establishment of a Korean children's well-being index requires access to and use of national statistical data analyzed annually by the government as well as a general consensus regarding such issues as scope and its indicators.

A Study on Factors of Job Satisfaction and Burnout of Hospital Dietitians (병원 영양사의 직무만족도 및 소진에 영향을 미치는 요인)

  • This research aims to examine the experiences of hospital dietitians' burnout and the factors of their burnout, such as demographic factors, unique characteristics of hospitals, their self-respect, and organizational factors. In order for this, a questionnaire survey was conducted with hospital dietitians working in general hospitals of over 400 beds in the Seoul Metropolitan area. The average point of self-respect of dietitians is 3.67 out of 5.00. Their experience of burnout turned out to be 3.17 out of 7, the extent of job satisfaction is high, that is 3.35 out of 5.00. The decrease of personal accomplishment is shown to be closely related to the burnout of dietitians. The emotional exhaustion in burnout at the significant level 0.001 is increasing as the annual gross salary is low, their age is young, extra-working hours are long, and is high to the dietitians serving meals and working at entrusted companies. The factors affected by burnout experience vary with individual circumstances. So, to prevent the emotional exhaustion, depersonalization, and reduced personal accomplishment, hospital dietitians' spiritual and physical health needs to be kept up by their self-development and investment.

  • This thesis is a study to develop the tentative objectives and present the professional and courses for the nursing college. The conclusions conducted by the purpose of this study are as follows. First, on the basis of the concepts of the nursing and the view-points of nursing education, the tentative goals for the nursing education are set up as follows. 1. To understand human being's life, dignity and their physical, mental, and social aspects. 2. To understand the basic concepts and the principles of human sciences, social sciences, natural sciences, and medical sciences. To apply their knowledges to nursing practices. 3. To diagnose and assess the problems of individual, family and community in terms of nursing practice. To develop the ability of planning, negotiation, management, and evaluation for the nursing education. 4. To develop appropriate knowlege, attitudes, and skills to promote the clients' health and treat their illness. 5. To accomplish all tasks effectively as a trained and qualified professional nurse through the endless studies. Second, the nursing areas and courses for the nursing college in terms of validity, Importance, continuity, relationship, utility and appropriateness are listed as follows. 1. An Methodology Research of the Quality of Life of the Allogenic Bone Marrow Transplantation Survivors (동종골수이식 환자의 삶의 질 측정도구 개발)

  • This Research was designed to develop an instrument that can be used for measuring and analysing the degree of the quality of life of the Allogenic bone marrow transplantation survivors and to provide basic data to help them attain a better quality of life. The subjects of this study were a total of 101 people who were undergone Allogenic bone marrow transplantation and were over 100 days after transplantation. Data were collected during the period from September 3 to December 2. 2000. In Developing this instrument, the researcher established a conceptual framework based on the result of previous research, interview of patients and medical staff and researcher's experience. The scale for this instrument consisted of 51 items. Responses were obtained from the respondents through a self reporting method and each item had a possible score of 5. The collected data were analyzed using the SPSS program. The reliability of the scale was tested by Cronbach‘s alpha. For the examination of the component factors of the measuring instruments, factor analysis method was used. Examination of the difference in the quality of life as related to demographic variables was done using t-test, analysis of variance(ANOVA). For the examination of the major variables influencing the quality of life, Pearson's Correlations was used. The results were as follows: (1) The reliability of the instrument was 0.90 (Cronbach's alpha) (2) The validity of the instrument was tested by factor analysis. The result of the factor analysis of the 51 items is making up the instruments were classified into the following factor; emotional state, Interpersonal problem, physical problem, spiritual state, social problem, decrease of physical energy, physical problem-eye, physical problem-skin and mucosa, health awareness, elimination problem, worry about health, financial problem, sexual problem. (3) Demographic variables related to quality of life is the time after transplantation. GVHD experience, lose of job. Age at the transplantation, diagnosis, type of the transplantation, marital status, monthly income, educational background, sex, religion are not related to the quality of life.

The Study for the Foundation Build up of the Elderly Welfare Service in Korea. - Focused on the Degree of Life Planning of Aged and Needs of Elderly Welfare Service (우리나라 노인복지서비스 기반 구축을 위한 기초연구 - 노후생활준비와 노인복지서비스 요구도를 중심으로)

  • Korean society is experiencing rapid changes which are certain to shape the lives of the older people and their families. So, the purpose of this study was to improve the quality of the elderly welfare service in Korea. Even though the interest about the elderly welfare in Korea is rapidly on the increase, the reality of Korean Society still leaves much to be desired, quantitatively as well as qualitatively. Pay attent to this point, now going to grasp the situation of not the handicapped elder people but the general elder people actual life condition and demand for welfare. So this study focused on the demand of not out-of home service but in-home care service. It was based on the data from by Korea National Statistical Office, having observed 6,139 elderly people. The results were as follows. They hope to get the elderly welfare services about health examination service, nursing survice, supporting service for household. That was, the most of them wanted in-home care service than out-of home care service. To ensure effective care to the elderly, it needed development and settlement of welfare service in face of their daily living. And, even though they recognized that they should cover the elderly life expenses themselves, the rate was hit that indicated their adult children as a parents supporter. This means that we should consider not only to the elder people but also to the family which contained the old people as the elderly welfare service. As a remedy, we can find the cooperation between elder welfare service at the social welfare organization and counsel, education at the family strengthen center. To improve the quality of the elderly welfare service in Korea, the elderly welfare service should focus not only on the physical and spiritual health maintenance but also active understanding living environment and growing ability to arbitrate between individual and living.
