Coronavirus Disease (COVID-19) Outbreak and Its Impact on Spinal Daily Practice : Preliminary Report from a Single (Regional) University Hospital in Republic of Korea

  • Objective : Since the first discovery of the 2019 novel coronavirus (COVID-19), rapid and wide spread of the disease has been reported and the World Health Organization announced that a 'pandemic' has started. Up to date there is little known regarding the impact of this outbreak on spinal specialists' daily clinical practice. We intended to evaluate how COVID-19 has affected the number of spinal disease patients we meet and operate in daily practice. Methods : The de-identified data regarding number of patients visiting the spine clinic at a tertiary referral hospital and a secondary level hospital from January, February and March of 2017 to 2020 were retrospectively reviewed. The number of out-patient department (OPD) visits, number of emergency room (ER) visits as well as number of surgeries performed during the reviewed period were collected and analyzed, comparing 2020 to the previous 3 years. Results : The number of daily OPD visits showed a steady decrease starting from January, and presented a statistically significant decrease by early March 2020, compared to the previous 3 years. During the same period, decrease in number of daily ER visits was statistically significant as well. The number of elective surgeries or number of surgeries for patients admitted via ER during COVID-19 outbreak remained similar to that of 2017-2019 suggesting, despite the decrease of patients visiting the hospital for spinal diseases, those whom required surgery still visited the hospital. The results were consistant among other hospital level. Conclusion : The outbreak of COVID-19 affected our daily practice as OPD and ER visits reduced but did not affect the number of surgeries. We believe that this report will be informative to spinal specialists worldwide fighting the COVID-19 pandemic.

간호실무를 위한 정보체계 개발;척수손상 환자를 중심으로 (Development of Information System in Nursing Practice;spinal cord injury patients)

  • The purpose of this research conducted has been an the development of a computer program to be used for nursing of spinal cord injury patients. The research is also intended to contribute by supplying the basic data, to the development of other programs in relevant areas where similar nursing information system is required. This research used System Development Life Cycle method. Results from the study were as follows : The scope of Information System in nursing practice were based on a wide range of mateials as charts at four hospitals in Pusan that cover the nursing period of spinal cord injury patients from admission to discharge and other sources of reference, the research is done for nursing diagnosis, nursing history, admission and discharge education, and the performing of doctor's order, which were divided into a subsystem that consisted of help, Patient Management, Performing of Docter's Order, and Basic Data Management and then these was analysed systematically using Hierachy Plus Input Process Output, necessary information, data flow diagram, and conceptual designs were developed for chosem system. Furthemore, the system has been programed by a conceptual design that includes the objects of the program and the origins of the sources. The oder to make sure of the proper function, the computer program was actually applied to a nursing practice that had been developed for a hypothetical situation through the two nurses who were in charge of spinal cord injury patients after they were trained to understand the structure and the contents of computer program about the nursing practice from using the computer program developed. Also, the compensatory strategy has been worked out against any problems to arise. It is expected that this kind of nursing practice information system to be used for the spinal cord injury patients that has been developed through the processes shown above will increase the satisfaction for both patients and nurses by enabling them to give more professional advice and service to the development of more information systems for many hospitals.

Effectiveness of orthoses for treatment in patients with spinal pain

  • Spinal pain is a common patient complaint in clinical practice. Conservative treatment methods include oral medication, physical therapy, injections, and spinal orthoses. The clinical application of orthoses is debated because of potential complications associated with long-term use, such as muscle weakness and joint contracture. We reviewed the orthoses most frequently used to manage spinal pain. We review the use of soft cervical and Philadelphia collars, lumbosacral corsets, and thoracolumbosacral orthosis to manage spinal pain. Spinal orthoses can help reduce pain by protecting the muscles and joints of the injured spinal region, preventing or correcting malformations, and limiting trunk flexion, extension, lateral flexion, and rotation. The short-term use of spinal orthoses is known to improve pain and disability during the treatment period without significant adverse effects. Spinal orthoses are expected to alleviate pain and improve patients' lifestyle.

재가 척수손상환자의 청결자가도뇨 실태 및 지식 조사 연구 (Knowledge and Practice of the Spinal Cord Injury Patients for the Clean Intermittent Self Catheterzation)

  • The purpose of this study was to examine the knowledge and practice of the Clean Intermittent Self Catheterization (CIC) conducted by Spinal Cord Injury Patients (SPIC) or their significant others as an activity of daily living at their homes or working places. Forty-seven patients were participated in this study. Three questionnaires developed by researchers for this study were used to collect demographic data, knowledge and practice skill of CIC. Descriptive statistics and ANOVA were used to analyze the data. Nineteen (19) patients experienced Urinary Track Infection (UTI) during the last one years and two of them have had several times of UTI. Knowledge level of the participant was not high enough to perform correct CIC. Some evidences indicated that the knowledge did not apply to their practice. Systematic analysis and development of educational content based on this study results was recommended and the systematic instructional design before actual patient education was strongly recommended.

통증자가조절기 융합교육이 척추수술환자의 수술 후 통증관리에 미치는 효과 (Effects of Patient Controlled Analgesia Convergence Education on Postoperative Pain Management in Spinal Surgery Patients)

  • 본 연구목적은 통증자가조절기(patient controlled analgesia:PCA)융합교육이 척추수술 후 통증관리에 미치는 효과를 조사하기 위함이다. 60명의 척추수술환자를 대상으로, 실험군(30명)에게 수술 전 PCA동영상, 소책자, 및 PCA실습으로 구성한 PCA융합교육을 적용하였다. 구조화된 질문지로 자료수집한 후 기술통계, ${\chi}^2-test$, Fisher's exact test, t-test 및 repeated measures ANOVA로 분석하였다. 연구결과 척추수술 후 실험군의 PCA지식태도, PCA만족도, 통증관리만족도는 대조군보다 유의하게 높았다(p<.001; p=.001; p<.001). 실험군의 수술 후 통증과 추가 진통제사용은 대조군보다 유의하게 낮았다((p<.001; p=.001). 따라서 시청각매체와 PCA실습을 접목한 PCA융합교육은 척추수술환자의 통증관리에 효과적인 실무중재임을 확인하였으며 다양한 간호실무에서 융합중재개발에 응용 가능할 것이다.

경막외 카테터 거치후 발생한 척추 경막하 농양 -증례 보고- (Spinal Subdural Abscess Following Epidural Catheterization -A case report-)

  • It is common practice to use epidural catheter for anesthesia or for postoperative analgesia and other kinds of pain control. However, Intraspinal infection associated with this practice is rare event. We report a case of spinal subdural abscess occuring in patient who had recently received epidural catheterization. The cause in this case is not certain, although infection from the epidural catheter is the best possibility. We recommand an aseptic technique in all procedure for epidural or spinal analgesia.

Sufficient explanation of management affects patient satisfaction and the practice of post-treatment management in spinal pain, a multicenter study of 1007 patients

  • Background: Spinal pain is most common symptom in pain clinic. In most cases, before the treatment of spinal pain, physician explains the patient's disease and treatment. We investigated patient's satisfaction and physician's explanation related to treatments in spinal pain patients by questionnaires. Methods: Anonymous questionnaires about physician's explanation and patient's satisfaction in each treatment and post-treatment management were asked to individuals suffering from spinal pain. Patients who have spinal pain were participated in our survey of nationwide university hospitals in Korea. The relationships between patient's satisfaction and other factors were analyzed. Results: Between June 2016 and August 2016, 1007 patients in 37 university hospitals completed the questionnaire. In the statistical analysis, patient's satisfaction of treatment increased when pain severity was low or received sufficient preceding explanation about nerve block and medication (P < 0.01). Sufficient explanation increased patient's necessity of a post-treatment management and patients' performance rate of post-treatment management (P < 0.01). Conclusions: These results show that sufficient explanation increased patients' satisfaction after nerve block and medication. Sufficient explanation also increased the practice of patients' post-treatment management.

척추수술 환자의 질병경험 (Illness Experiences of Patients Following Spinal Surgery)

  • Purpose: The purpose of this study was to explore the illness experiences of patients who had spinal surgery. Methods: Colaizzi's phenomenological method was used for the data analysis. Patients who had spinal surgery were included in this study. The data was collected between April and October 2022 by conducting a one-on-one, in-depth interview. Results: Five theme clusters were identified based on the illness experiences of patients who had spinal surgery: "An agonizing life changed by pain", "Efforts to alleviate the pain", "Surgery was determined to be the best way to relieve pain", "Recovered daily life after relieving pain caused by surgery", and "Dedicated to living a healthy, pain-free life". The analysis further yielded 20 themes. Patients used various treatment methods to relieve pain, but ultimately chose surgery due to unbearable pain. Patients who had spinal surgery felt grateful that they were feeling good and healthy because they could return to their normal daily lives. After all, the pain was relieved after the surgery. Even though some pain remained unrelieved, the patients had the hope of getting better through exercise and rehabilitation. Conclusion: This study provides an in-depth understanding and meaning of the illness experiences of patients who had spinal surgery and presents new perspectives on clinical practice. The findings of this study are expected to be useful in developing and applying systematic and customized nursing interventions before and after spinal surgery.

척추관절통증증후군 (Spinal Joint Pain Syndrome)

  • Spinal joint pain syndrome is composed of atlanto-occipital, atlanto-axial, facet, and sacro-iliac joints pain. The syndrome is characterized as referred pain which is originated from deep somatic tissues, which is quietly different from radicular pain with dermatomal distribution originated from nerve root ganglion. The prevalence of facet joint pain in patients with chronic spinal pain of cervical, thoracic, and lumbar regions has been known 56%, 42%, and 31% as in order. It is generally accepted in clinical practice that diagnostic blocks are the most reliable means for diagnosing spinal joints as pain generators. The sacroiliac joint has been shown to be a source of 10% to 27% of suspected cases with chronic low back pain utilizing controlled comparative local anesthetic blocks. The treatment of spinal joints ideally consists of a multimodal approach comprising conservative therapy, medical management, procedural interventions, and if indicated.
