A Study on The Training Curriculum for The Emergency Medical Technician (응급구조사(應急救助士) 교육과정(敎育課程)에 관(關)한 연구(硏究))

  • It was enacted 'Emergency Medical Act' in January, 1994 beginning the emergency medical service system from 1982, and while it was established the emergency medical department in junior college providing the detailed agenda about emergency medical technician and the regulation relative to the application of a law on the emergency medical act in 1995, the fire school and the National Medical Center are enforced the curriculum. It is very important subject faced for the construction of emergency medical system to produce a number of emergency medical technicians to be sufficient to the role of emergency aid. In this study it is analysed the training curriculum for the emergency medical technician and presented the improvement plans. 1. Though it needs the qualification level of first and second class in the selection process, the more important thing needs the detailed qualification level by term of one's service and the skills of business accomplishment. 2. In the examination management, (1) written examination is composed of the questions to understand how much faithfully they carry out the practical business as the emergency medical technicians, (2) it is added practical examination as the item to appraise the situation disposal ability. 3. It is necessary to prescribe the activity in the medical institution and ambulance arrangement through the development of 'Business Treatment Guide'. 4. For the regional balanced disposition of emergency medical personnel it is selected balanceably the educational institution by eight medical service areas, and considering the characteristics of region it is necessary to manage, in the practical business training course, another special course such as the mountains medical aid and sea medical aid. 5. In the period of education the first class needs the practical business training period of a certain period after passing examination, and the second class needs the extension of the period. 6. As the problems to improve in the curriculum [1] in the first class course (1) intensification of practical educaiton (2) reinforcement of curriculum (3) the development of standardized curriculum etc., [2] in the second class course (1) varieties of curriculum (2) intensification of basic first aid treatment education.

A Study on the Compliance of the Occupational Safety and Health Act by Busan and Gyungsangnam-do Province Working Environment Measurement Institutions (부산·경남지역 작업환경측정기관의 산업안전보건법 준수 실태 및 준수율 제고를 통한 측정기관 종사자 건강보호 방안 고찰)

  • Objectives: This study aims to investigate whether working environment measurement institutions(WEMIs) are conducting safety and health education, risk assessment, and oversight of special management materials, and whether working environment measurement and special medical examinations are being conducted as prescribed in the Occupational Safety and Health Act(OSHA). Methods: In of February 2021, a questionnaire was prepared and distributed to 33 WMEIs registered with the Ministry of Employment and Labor(MoEL) in Busan and Gyeongsangnam-do Province. The responses were collected and then analyzed. Results: The findings show that 5 WEMIs(15%) complied fully with OSHA. Risk assessment was conducted by 13 WMEIs(39%) and safety education by 11 WMEIs(33%). Eighteen WMEIs(55%) conducted working environment measurement, and 29 WMEIs(88%) conducted special medical examinations. The implementation rate of the risk assessment in the health industry(85%) was higher than the one in the special technology industry(11%)(p<0.05). The implementation rate of the special medical examination in the examiners(54%) was not as high as the one in the analysts(91%)(p<0.05). Conclusions: The MoEL needs to check whether basic OSHA requirements are being observed during regular inspections by WEMIs. These findings indicate that it is necessary to prepare a plan to improve the rate of compliance with OSHA regulations.

  • Radiographic osteolysis after total shoulder arthroplasty (TSA) remains a challenging clinical entity, as it may not initially manifest clinically apparent symptoms but can lead to clinically important complications, such as aseptic loosening. A thorough consideration of medical history and physical examination is essential to rule out other causes of symptomatic TSA-namely, periprosthetic joint infection-as symptoms often progress to vague pain or discomfort due to subtle component loosening. Once confirmed, nonoperative treatment of osteolysis should first be pursued given the potential to avoid surgery-associated risks. If needed, the current surgical options include glenoid polyethylene revision and conversion to reverse shoulder arthroplasty. The current article provides a comprehensive review of the evaluation and management of osteolysis after TSA through an evidence-based discussion of current concepts.

A Study of Major Issues in the Act (Draft) on Remedy for Damage from Medical Accident and Medical Dispute Mediation, etc. (의료사고 피해구제 및 의료분쟁 조정 등에 관한 법률(안)의 주요 쟁점에 관한 고찰)

  • In this paper, the researcher looked into major issues in the "Act (Draft) on Remedy for Damage from Medical Accident and Medical Dispute Mediation, etc." which was proposed by the Health & Welfare Committee, the National Assembly of the Republic of Korea, and which was pending with the Legislation & Judiciary Committee. Then the researcher pointed out worrisome problems therein and presented suggestion" to improve problematic situations. First of all, the researcher examined the following items which are major points in the aforementioned Act: 1) Establishment of Korea Medical Dispute Mediation and Arbitration Center, 2) Procedures for mediation and arbitration of medical disputes, 3) Establishment of Medical Injury Compensation Association, 4) Introduction of proxy payment for damages, 5) Compensation for no-fault medical accidents, 6) A system concerned with special cases on criminal punishment. Next, the researcher closely reviewed the following possible issues: 1) Limit of arbitrary mediation, 2) Postponement of the system concerned with special case on criminal punishment, 3) Examination of reasons for rejection, 4) Function and role of the Appraisal department, 5) A possibility of being reduced to an evidence collection procedure for lawsuit, 6) A possibility of no-fault compensation rather than injury compensation, 7) Operational issues related proxy payment for damages. Lastly, the researcher presented suggestions on how to improve each problematic issue.

  • The personal health record platform can store and manage medical records, health-monitoring data such as blood pressure and blood sugar, and life logs generated from various wearable devices. It provides services such as international standard-based medical document management, data pattern analysis and an intelligent inference engine, and disease prediction and domain contents. This study aims to construct a foundation for the transmission of international standard-based medical documents by mapping the diagnosis items of a general health examination, special health examination, life logs, health data, and life habits with the international standard terminology systems. The results of mapping with international standard terminology systems show a high mapping rate of 95.6%, with 78.8% for LOINC, 10.3% for SNOMED, and 6.5% when mapped with both LOINC and SNOMED.

Study on the Professionalism of Medical Technologists' Work in Health Examination Centers: Focusing on the General Health Examination (건강검진센터에서 임상병리사의 업무 전문성 연구: 일반건강검진을 중심으로)

  • "Health examinations" means medical examinations conducted by health examination institutions, including a physical examination, consultation, physical measurement, diagnostic laboratory test (also known as clinical laboratory test, clinical pathology test), pathology test, and Imaging test for checking the health condition and the prevention and early detection of diseases. The types of health examinations include general health examinations, comprehensive health examinations, special health examinations, and other health examinations. The proportion of the general health examination work of medical technologists working at health examination centers has been estimated to comprise more than 50% of the total work. The cost aspect of suspicious diseases can be estimated to be more than 25%. The cost of diagnostic laboratory test has been estimated to be approximately 5%, and the proportion of medical diagnosis use is 70%. The results show that it is time to revisit whether medical technologists are receiving appropriate social, economic, and reasonable policy treatment in health examination work. The future of medical technologists will further expand their professionalism in diagnostic laboratory test and quality control in precision medicine and telemedicine in the future. Therefore, medical technologists will need to participate proactively in government policy decisions related to national health examinations and make efforts to improve treatment.

A Study on the Development of a Computerized Nutritional and Health Guide Program Based on Periodic Health Examination at Work Sites (근로자 정기건강검진을 기초로 한 영양 및 건강관리 전산화 프로그램 개발에 관한 연구)

  • In this study, a computerized nutritional and health guide program for workers was developed. The dietitian at the work site could utilize periodically conducted medical examination data to develop an effective health care counseling model based on the developed Nutritional and Health Guide Program. A personal computer (Pentium II PC MMX-150, 32MB RAM, 2.95 GB HDD) with Microsoft Visual Basic 6.0 Enterprise Edition and Microsoft Access 97 installed, was used. The Nutritional and Health Guide Program consisted of seven main menus and 43 sub-menus. Included in the main menu were Basic Information, Periodic Health Check-ups, Visitors' Consultations, Nutritional/Health Tips, Nutritional Education according to Diseases, Help and Exit. In the Periodic Health Check-up menu, dieticians could input the health examination data of employees and touch for the recommended treatments for diseases such as obesity, diabetes, high cholesterol, hypertension and hepatitis. The Visitors'Consultation menu has been designed to compile health information about the employees who sought consultations. The Nutritional/Health Tips menu was designed to provide 14 kinds of programmed nutritional educational media and information. In the Nutritional Education According to Diseases menu, the dietitian could judge the subject's willingness to obtain treatment based on the Stage of Change Model. According, the content of the administered respective nutritional education was classified by stages. The Help menu, provide a chart of the method and procedure used as nutritional guidelines, by which the results of the health examination were classified as people in good health and those requiring special medical attention. The results of the evaluation of this program showed highly positive rates for usefulness (4.09), convenience (4.04), lettering size (4.02), interest (3.93), design (3.49). It also showed that 97.5% of the subjects thought that this program would be helpful for implementation of their company's nutritional educational program. Therefore, this menu could help dietitians plan, conduct, and evaluate their nutritional guidelines for employees. It is expected that The Nutritional and Health Guide Program developed in this study will play a role as a scientific and effective guide in conjunction with health examination results.

  • This research is undertaken for the purpose of providing basic data to improve the occupational health service for future. 415 workers of 46 firms in Seoul are included in this data points. Results from this analyzing are the following. 1) The level of knowledge, attitude and practice of workers on the occupational health is moderate. 2) The cognition of necessity about special health examination gains the highest point(3.94), but the understanding of the occupational health educational program gains the lowest point (.85) in the knowledge region. 3) In the attitude region, the highest point is marked by the workers' prudence of expending time on medical examination(3.45). The lowest one by the workers' feeling of satisfaction of the follow-up service after medical examination(1.79). 4) The workers' participation in medical examination is the most active(3.15), and that in occupational health education in the least active(.86) in the practical region.

Legal Review of Similar Medical Practice (유사의료행위에 관한 법적 검토)

  • This study aims to review legal problems of similar medical practice and suggest methods of improvement. Similar medical practice refers to all medical practices conducted in the state that human qualification is not fulfilled. It may cause serious damages on health and lives of national people. Currently, similar medical practices are recognized as unlicensed medical practices and prohibited based on the Medical law and additionally punished by then special law in Korea. However, the current Medical Law does not provide clear and accurate concept of medical practices so that it is difficult to regulate similar medical practices. The issue of complementary and alternative therapy related to similar medical practices is also in special state different from other countries. In addition, since similar medical practices lack of evidences in terms of safety, the dangerousness of accidents is high and it may affect badly on health of national people and health care policies. Methods of improvement in order to resolve problems regarding similar medical practices are: first, concept and scope of medical practice should be clear, accurate and concrete. Second, complementary and alternative therapies related to similar medical practices need to be strictly examined and the supervisory right should be given to doctors should be given even though a part of it is allowed. Third, research institutes specialized in the field should be established for scientific examination of complementary and alternative therapy and objective research results should be open to the public. Finally, since damage cases caused by similar medical practices by non-medical personnel, national management and supervision for similar medical practices should be reinforced.
