Analysis of Free Flap Reconstruction of the Tongue (유리피판을 이용한 설재건술의 분석)

  • Purpose: Advanced carcinoma of the tongue is a devastating disease which may cause severe speech or swallowing dysfunction. But, none to date has provided all of the complex functions of the tongue. The purpose of this study is to review our experiences with individuals who underwent glossectomy followed by reconstruction using free tissue transfer. Methods: Between February 1998 and February 2005, twenty-four patients underwent glossectomy followed by free tissue transfer reconstruction. The defects of tongue caused by partial or subtotal glossectomy were reconstructed by means of radial forearm or lateral thigh free flap with nerve innervation. Especially for the patients who underwent total glossectomy, we reconstructed deglutition muscles anatomically with nerve reinnervation, a procedure that allows the grafted muscle to maintain good tongue bulk without obvious atrophy. Results: Patients were reviewed to determine their functional outcome as it related to speech, deglutition, and aspiration. All patients achieved oral intake of a soft diet and acceptable speech. Conclusion: Although reconstruction following glossectomy using free tissue transfer is not ideal, this procedure is safe and reliable, and provides predictable results. A future challenge is the development of a surgical procedure for reconstruction of a tongue that maintains mobility and sensation using neurotized flaps.

A Study on the Social Image and Make-up Characteristics of Korean Women in 1970s (1970년대 한국여성의 사회적 이미지와 메이크업 특성 연구)

  • The purpose of this study is to compare the social image and the external image of a Korean woman in 1970s and find out that the make-up can provide an important clue to examine the image of a woman in a given period. The research scope covered 10 top news of the daily newspapers and articles of women's magazines. A focus was made to an analysis on words and photos from them. The relationship of each image scale was examined by comparing the linguistic image scale and the visual image scale. The results were as follows : First, a frugal and tidy image. It was the look of our tidy, simple, traditional and classic woman. Second, an image of a cute and pure lady of refined manners. In 1970s, women were supposed to be 'a loving wife', a cultured female image with a faithful role of a 'wise mother' and a lady of refined manners as the best value. Third, a frivolous and decadent image. Double-faced image of a woman which included the women, who had to live as the lady of refined manners during the daytime and seductive woman during the night. Fourth, an image of a contemporary working woman. It was the image as a chic, confident and dignified working woman requested by the society of the times. Namely, it can be understood that women had a make-up of a soft and gorgeous tone as an expression of a will to keep a confident and female aspect in the course of working in the society by the women experiencing 1970s, the turbulent period. Consequently, it is possible to understand that the make-up was utilized as a means to express an ideal beauty of the time pursuant to the historical background or feature.

  • The purpose of this study was to identify the change and characteristics of 20th men's fashion. The result of the Study as follows; 1 In the period 1890's-World War I there was a display of opulence and extravagance Therefore men's fashion pursued with elegance and grace. The rise of sports and an erosion of rules of Victorian society were evident influences before 1914. So men's silhouette resembled athletic appearance and less formality was shown in masculine attire. 2. During the period World War I-1920's the new equality of social relations between the sexes had an influence in fashion. Unisex look evolved so boyish shaped became the feminine ideal, men wanted to boyish fitted leaner stylings and natural shoulder suits. In the twenties youth, who seemed to have a social dominance, asserted their own testes in fashion. New style of men's fashion such as oxford bags and Ivy League Look appeared. After World War I "comfort" is the aim of fashion so casualness was reflected in fashion: vests often were discarded and man wore a soft, detachable collars. 3. In the thirties depression marked a shift in mood from gaiety and progressivism of the twenties to the conservative, even reactionary values. The role of men and women returned to more traditional attitudes, Every men's general appearance was bold and manly: his chest was broad and well-built, his shoulders square and muscular. The effects of World War ll were not on style but on textiles and clothing construction: lighter weight fabrics appeared and construction was simplified 4. Men's dress changed only in detail and not in essence. But there was a continued increase in the variety of clothes and color in men's fashion.

THE HEALING OF MANDIBULAR FRACTURE WITHOUT INTERMAXILLARY FIXATION IN A LONG-TERM DISABLED BED PATIENT: REPORT OF A CASE (장기간 침대요양 장애환자에서 악간고정술 없이 치유된 하악 골절: 증례보고)

  • Treatment of the mandibular fracture consists of reduction and fixation. Primary wire & Arch bar are perhaps the ideal method for intermaxillary fixation. But, daily feeding, swallowing, speech, and in some instances, respiration is difficult to maintain during the period of intermaxillary fixation, owing to muscle weakness, emotional disorder and poor oral hygiene in a position of the long-term bed disabled patient with multiple injuries. Therefore, Intermaxillary fixation is not applied in the disabled bed patient, the alternative methods must be obtained. In the case of the mandibular fracture, because of the absence of weight bearing, osseous union may eventually occur even without immobilization if the patient is maintained without wound infection on a controlled soft diet. For the purpose of the prevention of the wound infection, the establishment of an drainage on the oral lacerated wound is necessary for the removal of the hematoma & seroma in the fracture site. This is the report of a case that was managed conservatively without the intermaxillary fixation in the long-term disabled bed patient with a mandibular compound fracture.

  • There are various types of foreign body reactions, such as inflammation, edema, fluid collection, hematoma, infection, abscess and granulomas. There are various imaging findings according to types of foreign bodies and depending on the lapse of time. Therefore, correct diagnosis of a foreign body reaction is difficult and easily confused with soft tissue neoplasm. The MRI is ideal for the detection of foreign bodies regardless of radiolucency or acoustic impedance. It is especially very useful in the evaluation of the surrounding tissue reaction. The authors report a case of a 26-year-old female patient with both forearm swelling due to self-injection of a mixture of powdered tablets and saline. The lesion shows numerous internal T1 and T2 dark signal intensity micro-spots with surrounding fluid collection, which are diagnosed as foreign bodies with surrounding inflammatory changes during an operation.

Case Report : Latissimus Dorsi Flap for Secondary Breast Reconstruction after Partial TRAM Flap Loss (유경 횡복직근피판술 후 발생한 부분 피판괴사 및 지방괴사의 넓은등근피판을 이용한 재건 치험례)

  • Purpose: The transverse rectus abdominis musculocutaneous(TRAM) flap is the most commonly used autogenous tissue flap for breast reconstruction. Postoperatively, partial flap loss or fat necrosis are relatively common and it may result in a smaller breast volume with marked contour irregularities. These defects are not easy to reconstruct with local tissue rearrangement or with breast implants. The current authors present the results of 2 patients who underwent Latissimus dorsi(LD) flap reconstruction to correct partial flap or fat necrosis that developed after TRAM flap breast reconstruction. Method: Case1: A 50 - year - old woman with left breast cancer visited for breast reconstruction after radical mastectomy. Initially, breast reconstruction with pedicled TRAM was performed. Postoperatively partial flap necrosis was developed. Secondary breast reconstruction using LD flap was done. Case2: A 51 - year - old woman with left breast cancer visited for breast reconstruction after radical mastectomy. Initially, breast reconstruction with pedicled TRAM was performed. Postoperatively fat necrosis was developed. Secondary breast reconstruction using LD flap was done. Results: Secondary breast reconstruction using LD flap survived completely and produce successful reconstruction. There was no significant complication in both patients. Conclusion: LD flap provides sufficient, vascularized skin and soft tissue. The flap can be molded easily to replace deficient tissue in all areas of the breast. These attributes make it an ideal candidate for salvage of the partially failed TRAM flap breast reconstructio.

  • The purpose of this study is to survey about the images of the sexy clothes that young women sympathize in modern society, that is, to make a research into the cognitive dimensions of 'Sexy image' and the constructive peculiarity of clothing. The researches into dimension of image by sexy clothing make that female mainly consist of in their twenties during May to October in 2003. The independent variables were: 1) The physical beauty dimension, 2) The negative estimate dimension, 3) The characteristic devaluation dimension,4) The elegance/ high grade dimension,5) The freshness/ beauty dimension,6) The sex appeal/ exposure dimension. Judging from each characteristic of costume in classified by articles, that is, colors, texture, shapes, details, and others-phenomenon of outerization of under clothing, print. We can know the clothes showing up our nice figures are the sleeveless design tied with strap, soft texture as silk chiffon and the colors that are not strong. And there were no embarrassed, ugly and pitiful clothing. Expressing sexy beauty by clothing is the result that the wearer produces intentionally by compounding design factors, it's the ideal when the observer's reaction and the wearer's intention correspond. Therefore, it will be helpful in expressing ideally if the subject, what kinds of sexy beauty the compounding design factors express, is studied carefully. Also it is expected to be used in grasping modem consumers' desires that want to look sexy, finding out how to express, dealing with the consumers' desires correctly, and the reference materials of the planning goods, sales promotion, and selecting of a medium of advertisement according to the consumers' desires.

Variable Applications of Facial Artery Musculomucosal Flap for Oral Cavity Reconstruction (구강내 재건을 위한 안면 동맥 근점막 피판의 다양한 적용법)

  • Background : The purpose of this study is to review our experience with the variable applications of the facial artery muscolomucosal(FAMM) flap for oral cavity reconstruction following treatment for head and neck malignancies. Methods : We performed retrospect medical record review from January 2012 to June 2013. This flap technique was applied in 3 patients to reconstruct the defects of oral cavity induced by treatment for head and neck malignancies. Results : The defects and fibrosis occurred due to variable causes related to previous treatment for head and neck malignancies such as tumor resection and chemoradiation. Two patient suffered from defect on hard and soft palate and one patient suffered from trismus related to fibrotic scar band. We performed oral cavity reconstruction using FAMM flap even though these all patients had history of neck dissection surgery or radiation therapy. Conclusion : FAMM flap is an ideal option for reconstruction following treatment not only for primary tumor resection but also for defects induced by variable causes. Preoperative radiation and neck dissection surgery, which are previously considered as some challenges, are no longer contraindication in our cases.

A new retaining method of cement-retained restoration with linguo-horizontal insertion of fiber post (시멘트 유지형 임플란트 보철물의 신개념 유지 방식: Fiber post의 설측 수평 삽입에 의한 유지력 보완)

  • The retaining methods of implant prosthesis were classified into a screw-retained and a cement-retained type. A screw-retained prosthesis has many advantages, such as retrievability, preventing residual cement, while their disadvantages include the possibility of screw loosening and fracture, on the contrary advantages of cement-retained prosthesis are relatively low cost, but they are difficult to retrieve. To combine the advantages of both type, screw-cement retained prosthesis (SCRP) type have been introduced. But they still require ideal implant placement. So we introduce fiber post retained prosthesis without residual cement for preventing soft tissue trouble due to excessive cement.

Features and Issues of an Urban Community by Analysing Residents' Awareness and Attitude (주민의 인식과 태도로 본 도시 공동체의 현황과 과제 - 대구 동구 안심지역을 사례로 -)

  • Urban communities have been recently highlighted as an alternative idea in the field of urban regeneration, social economy and so on. Urban communities should not be understood as an ideal, but the way urban communities work in reality also should be analysed. This paper aims to find out what urban communities really mean and who are involved in urban communities. The paper explores the features and issues of unban communities by surveying residents in Ansim area, Dong-gu, Daegu city. The paper analyses the relation between participation and community features: residents' socio-economic features, residence features, social relationship with their neighbours and the sense of community. The survey outcome shows that middle income, higher educated residents living in apartment complex are relatively more involved in community activities. Moreover, those who have more relationship with their neighbours have participated in community activities more than those who do not have any. As a consequence, this paper carefully states that the urban community is middle class-oriented and such feature could cause other residents' limited opportunities to access information about their own community and lead to social exclusion. This paper suggests that urban community groups need 'soft solidarity' between social classes.
