  • Objectives: The aim of this study was to examine factors influencing health-related quality of life of waged workers. Methods: This study used data from the Seventh Korea National Health and Nutrition Examination Survey. Data were analyzed with the $x^2-test$, t-test, ANOVA, and multiple regression. Results: The significant predictors of health-related quality of life were depression, subjective health status, age, activity limitation, education, worker status, monthly income, drinking, smoking, physical injury, stress, and moderate physical activity. Conclusions: Personal health characteristics, psychological characteristics, socioeconomic characetrictics and working characetrictics affect health-related quality of life. Health-related quality of life is also affected by social structural problems such as socioeconomic factors and employment instabilitye. In addition to the development of health care programs to improve health-related quality of life of twaged workers, policy changes are needed to improve the social structure.

Pattern of Hepatitis A Incidence According to Area Characteristics Using National Health Insurance Data

  • Objectives: Over the past several years, the incidence of hepatitis A infection has been increasing rapidly in the young-adult population in Korea. We examined the effects of area-level socioeconomic status and environmental hygiene on the incidence of hepatitis A. Methods: This study is based on the registered national population of Korea and the national health insurance data from 2004 to 2008. A total of 73 459 individuals were confirmed to have had hepatitis A. The standardized incidences of hepatitis A in 232 districts adjusted for sex and age of people were calculated for each year, and the rate ratios of the incidence rates were estimated according to area-level socioeconomic status and environmental hygiene using multiple Poisson regression models. Results: The incidence rates of hepatitis A infection were 15.6 (per 100 000) in 2004, 19.0 (per 100 000) in 2005, 27.2 (per 100 000) in 2006, 25.1 (per 100 000) in 2007, and 61.7 (per 100 000) in 2008. The analysis of the area-level effects showed that residential areas of the less deprived than other regions, areas with higher levels of education, and heavily populated areas were significantly associated with increased risk. Conclusions: There is a very strong possibility that both area-level socioeconomic status and environmental hygiene play a role in increasing the risk of hepatitis A infection in Korea. Therefore, to reduce hepatitis A infection, we need a nationwide strategy that considers these area-level characteristics.

베트남 참전군인에서 우편으로 검진대상자를 모집하기 위한 예비조사 (Pilot Study on Recruiting Medical Checkup Participant by Mail Survey among Korean Vietnam Veterans)

  • Objective : The aim of this study was to identify the validity of recruiting medical checkup participants of Vietnam veterans using a mail survey, and to identify the 'Vietnam service related characteristics' and 'general characteristics' of Vietnam veterans groups, Method : In this study, a total 900 veterans were randomly selected from the list of Vietnam veterans from f964 to 1973. The veterans were classified into 5 groups, taking into consideration their registered status in the Ministry of Patriots and Veterans Affairs: the VRD (veterans who have agent orange-related diseases), VSD (veterans who have agent orange-suspected disease), VM (veterans who peformed meritorious deeds during the war), VR (veterans who were registered with the Ministry of Patriots and Veterans Affair) and OV (other veterans) groups. By means of postal surveys, the veterans' intention to participate in the medical checkup for our research, and their socioeconomic and general characteristics were investigated. 52 surveys were returned due to the subjects not residing at the listed address, and were excluded from the analysis. Result : 699 of the 848 veterans (82.4%) responded to the survey, of which 619 (88.6%) intended to participate in the medical checkup for our research. The S veterans groups all had similar ages, Vietnam service period, Agent Orange exposure, troop characteristic and wartime class, with the exception of VM, who were older, and with a greater number of officers than the other 4 groups, There was a big difference in the health statius among the Vietnam veterans group. The VM and OV were much healthier than VRD, VSD and VR groups, The socioeconomic stati of the VRD, VSD and VR groups were lower than those of the VM and OV groups. Conclusion : Although there were some limitations, the recruitment, by mail, of medical checkup participants from Vietnam veterans is a valid and feasible method. The VM and OV groups were much healthier, and with higher socioeconomic stati, than the VRD, VSD, and VR groups.

Regional Contextual Factors and Self-Rated Health: a Multilevel Study of Korean Adults

  • Objectives: To examine whether the socioeconomic characteristics of communities (contextual effects) are related to the self-rated health of community residents after controlling individual characteristics (compositional effects). Methods: A linked data set including information on individuals from raw data of 1998 Korean National Health and Nutrition Survey(KNHNS) and information on the regions where the individuals lived from the socioeconomic statistical indices of Si-Gun-Gu (city-county-ward) in 1998 was established. The contextual factors of communities were generated from these socioeconomic indices through factor analysis. The contextual effects of community over and above the individual characteristics on the self-rated health were investigated using multilevel analysis. Results: The contextual factors of the community expressed as the factor scores have influence on the self-rated health of their residents above the compositional factors. When the communities were categorized into 5 groups (highest, high, middle, low, lowest) according to each of their factor scores, for factor 1 reflecting urbanization reversely, the residents of the communities that had the high, middle, and low factor scores showed significantly poor subjective health status than the residents of the lowest (most urbanized) group. For factor 2 reflecting community services and health resources, the subjective health status of the residents gradually became poorer when the group went from the highest to the lowest, and the low and lowest groups showed a significant difference. For factor 3 reflecting the manufacturing industry, as compared with the communities that have the highest factor scores, the other 4 groups showed significantly poorer subjective health status. And for factor 4 reflecting the scale of the regional government, as compared with the middle group, the rest of the 4 groups showed significantly better self-rated health. Conclusions: There existed regional contextual effects on their residents' health in Korean adults. To make policies tackling these contextual effects possible, more elaborate researches to find more specific factors and to explain the mechanisms of how health is influenced by the contextual factors are needed.

사회경제적 특성 및 유병에 따른 의료서비스 이용 행태 (Utilization Behavior of Medical Services According to Socioeconomic Characteristics and Prevalence)

  • 본 연구는 제6기 국민건강영양조사 자료를 이용하여 만 25세 이상 성인을 대상으로 사회경제적 특성 및 유병상태에 따른 의료서비스 이용행태를 파악하기 위한 연구이다. SPSS 23.0을 이용하여 빈도분석 및 기술적 통계분석, 일원배치분산분석, ${\chi}^2$-test, 회귀분석을 실시하였다. 연구결과는 남자보다 여자가, 65세 이상, 고졸이상, 무직이 많았고 소득분위는 모두 비슷한 수준으로 나타났다. 교육수준, 직능, 소득분위를 함께 고려한 사회경제적 상태(socioeconomic status; SES)에 대한 점수는 7-8점이 가장 많았고, 대부분 응답자의 주관적 건강상태는 보통으로 느끼고 있었다. 평균유병개수는 $1.07{\pm}1.497$개이며 외래서비스 평균이용횟수는 $0.50{\pm}.0745$회이며, 입원서비스 평균이용횟수는 $0.12{\pm}0.454$회로 외래서비스 이용횟수 보다 낮았다. 일반적 특성에 따른 유병 및 주관적 건강상태는 유의한 차이가 있었고, 연령이 높을수록, 교육수준과 직능, 소득분위 및 SES점수가 낮을수록 평균 유병개수는 많았으며 주관적 건강상태는 나쁘게 인식하였다. 현재 앓고 있는 질환에서 입원서비스는 중증일수록, 외래서비스는 만성일수록 많이 이용하는 것으로 나타났다. 즉, SES점수가 높을수록 전반적인 의료서비스의 이용이 낮았고, SES점수가 낮을수록 의료서비스의 이용은 높게 나타났다. 이와 같은 결과를 토대로 사회경제적 특성이 낮은 집단에게 질환을 예방할 수 있는 적절한 보건교육 프로그램을 개발하여 이들이 질환에 노출되었을 때 적정한 의료서비스를 이용할 수 있도록 지역사회기반의 적절한 보건서비스 체계의 구축 및 시행의 필요성을 시사 할 수 있겠다.

지역사회 사회경제적 박탈이 공공도서관 대출 책수에 미치는 영향 (The Effects of Socioeconomic Deprivation on Public Library Book Circulation: A Community-level Study)

  • 본 연구에서는 지역사회적 특성이 해당 지역 구성원의 정보행위에 영향을 미친다는 지역 정보지평 이론에 근거하여, 지역사회의 사회경제적 박탈 수준이 공공도서관 도서 대출에 미치는 영향을 분석하였다. 이를 위해 다차원적 빈곤 지수에 기반하여 사회경제적 박탈 지수를 구성하였으며, 수도권 지역사회의 2015~2018년 박탈 지수를 산출하였다. 동 기간의 수도권 지역사회의 공공도서관 대출 책수는 국가도서관통계시스템을 통해 수집하였다. 박탈 지수와 대출 책수 간의 관계를 파악하기 위해 다층모형 로버스트 선형회귀분석을 실시하였으며, 통제변수로는 지역사회 특성과 도서관 특성을 활용하였다. 분석결과, 지역사회의 사회경제적 박탈 수준이 높아질수록 해당 지역 구성원의 1인당 공공도서관 대출 책수는 감소하는 것으로 나타났다. 또한 도서관의 소장도서 수가 많은 지역에서 박탈 수준이 높아지면 대출 책수의 감소폭이 큰 것으로 나타났으나, 그렇지 않은 지역에서는 그 효과가 미미하였다. 나아가 지역사회 단위면적당 도서관 수는 박탈 지수의 효과를 조절하지 않았지만, 전반적으로 대출 책수에 긍정적인 영향을 미친다는 것을 알 수 있었다. 이러한 연구결과는 정보격차를 해소한다는 기존 공공도서관의 역할에 새로운 시각을 제시하는데, 특히, 공공도서관 운영 및 서비스 전략 수립에 지역사회의 사회경제적 특성을 고려하여야 함을 시사한다.

1990년 서울특별시 O/D자료를 이용한 중력모형적용에 관한 연구 (A study on the Application of Gravity Model using 1990 Seoul O/D Data)

  • Among trip distribution models, The BPR type Gravity Model is the one which is the most widely used. The key issue associated with this model is a functional form of friction factor which should be calibrated in the process, and interpretation for socioeconomic factor known as K-factor(Kij) which is used to adjust the difference between observed zone-to-zone trips and the estimated trips. In this study, the BPR type Gravity Model has been fitted to 1990 O/D data for Seoul. Two type of function form for friction factor has been employed : one is a form of Generalized function and the orther is UTP function. With above two function, the parameters for travel distance(Skin-tree) are prepared. The relationship between socioeconomic factor and trips is identified by calculating and analyzing the characteristics of Kij. Consequently, both of the friction factor functions are statistically signified. However, it show an overestimation tendency when estimated with UTP function. It is found that the Generalized function is suitable for the city of Seoul, and also, in case there are a lot of trips correlatively, the socioeconomic factor is close to 1, on the other hand if it's small, it shows a bias which is dispersed around 1.

당뇨병과 관련인자들에 대한 연학적 연구 -서울시내 일부 종합병원 환자를 중심으로- (The Epidemiological Study on Diabetes and Related Factors)

  • This research is intended to reveal the correlations between diabetes and dietary and other related factors. We were studied for 257 diabetics, in t도 9 general hospitals, located at Seoul, From August 109 to September 3, 1986. The result of this study can be follow ; 1) Distribution of the subjects according to their general characteristics ; The sex ration of the subjects is 1.06 ; 1.00 (male ; female), and the average age is 54.7 $\pm$10.9 years. Among the subjects 28.8% have a family history of diabetes ; the closer the genetic relationship, the more hereditory factors play a part. 2) Distribution of the scores according to their socioeconomic status ; As the socioeconomic status goes p, the intake of calories and fiber increases ; however, the frequency of eating out and the degree of physical activity decrease. 3) Relationship between obesity prevalence and each variable ; Among the subjects 27.2% are obese, and 60.3% have been obese in the past. Even though there is no significant relationship between the food habit and obesity, the number of people who are obese shows a significant increase as the socioeconomic status improves. 4) The effect of sex on each variable ; The intake of calories, sugar, alcohol and coffee is higher in male subjects, as is the frequency of eating out. In regular exercise and physical activity, there are significant differences between male and female subjects.

Value of a statistical life estimation of carcinogenic chemicals for socioeconomic analysis in Korea

  • Objectives To protect public health from risk, the Minister of Environment in Korea legislated an act concerning the registration and evaluation of chemical substances. In this study, we estimated the value of a statistical life (VSL) of carcinogenic chemicals to evaluate the socioeconomic analysis in Korea. Methods The estimation of the health benefit can be calculated through an individual's VSL and willingness to pay (WTP). To estimate the VSL and WTP, we used a contingent valuation method through a web-based survey. Results The survey is conducted with 1434 people living in Seoul and six large cities. An analysis of the survey is essential to review the distribution of the characteristics of the target population. The statistically significant variables affecting the WTP are location, age, household income, quality of life. Through the review of data, we secured statistical validity. The WTP was estimated as 41205 Korean won (KRW)/person, and the estimated VSL appeared as 796 million KRW/person. Conclusions There is a case in which the amount of statistical life value is estimated in connection with domestic environmental policy, fine dust, etc. However, there are no cases of evaluation for chemical. The utilization of this result is possible for conducting other study with chemicals.

Daily Life Satisfaction Among Thai People: Socioeconomic Comparison

  • This descriptive study evaluated the satisfaction in daily life of Thai people according to participants' socioeconomic characteristics. The accidental sample included 676 Thai people aged 20 and older who lived in Bangkok and urban Saraburi. The satisfaction in daily life questionnaire (${\alpha}=0.86$) was used to collect data. The ranking for each aspect of daily life satisfaction arranged from high to low are: family life (mean=3.39), human relations, housing, food, job, health, and income (mean=2.99). Females were found to have a significantly higher satisfaction with daily life than males (mean=22.81 vs. 21.91). Those who lived in Saraburi province were found to have a higher satisfaction with daily life than those in Bangkok. Married participants, participants age 50-59, participants from two-generation households of parents and unmarried children, participants who stated that they took better care of their health now compared to one year ago, and participants who consulted a doctor in the past month were found to have higher satisfaction with daily life than their counterparts.
