자폐 스펙트럼장애의 환경 요인 (Environmental Factors in Autism and Autistic Spectrum Disorder)

  • Autism and autistic spectrum disorder are chronic neuro-developmental disorders characterized by social and language impairments and stereotyped, repetitive patterns of behavior. The etiology of autism remains unknown; however, a strong genetic component has been detected and environmental factors may also be involved in their etiologies. In the current study, we reviewed evidence for the presence of prenatal and perinatal factors, gastrointestinal factors, food allergies, metabolic and heavy metal factors, and other nutritional factors that may represent risk factors for the development of autism and autistic spectrum disorder.

  • Autism Spectrum Disorder (ASD) is characterized by persistent deficits in social communication and restricted, repetitive patterns of behavior and interest. Sleep problems are not uncommon in children with autism spectrum disorders. Symptoms of insomnia are the most frequent sleep problems in individuals with ASD. Sleep problems can cause significant difficulties in the daily life of children with ASD and their families. Genetic factor, deregulations of melatonin synthesis, extraneous environmental stimuli and psychiatric and medical conditions may cause sleep problems. The first line treatment of sleep problems in ASD includes managements for potential contributing factors and parent education about sleep hygiene care for child and behavioral therapy. Supplementation with melatonin may be effective before considering other medications, such as risperidone, clonidine, and mirtazapine.

  • Previous research has identified the biological, psychological, and social factors that confer an elevated risk for suicide. Evidence suggests that sleep disturbances are one of the risk factors that predict an increased risk for suicidal behaviors. Both sleep disorders and general sleep complaints are linked to higher levels of suicidal ideation and depression, as well as increased rates of suicide and suicide attempts. The causal mechanism of this association is not clear. For example, it is not known if insomnia is an independent phenomenon that if interrupted could prevent the emergence of a mental disorder, or if insomnia is a symptom of another developing illness. Hypofrontality, HPA dysfunction, and an impaired serotonergic system are potential mechanisms underlying the association of sleep disturbances and suicidal behavior. Future research is necessary to examine the specific mechanism of this association between sleep and suicide, which may lead to an effective intervention and diminished suicide risk.

여자대학생의 스트레스 정도에 따른 식행동 분석 (An Ecological Study on Dietary Behaviors by the Degree of Stress Among Female College Students in Suwon)

  • Stress not only brings disorders in social, family life but brings also changes in eating behaviors so that the imbalance of food intake is induced. Our study was carried out to find out the association between the degree of stress and eating behaviors for the subjects of 262 female college students in the Suwon area. The study instrument was a structured questionnaire composed with questions about demographic data, food preferences, questions related to life styles, and health. All of the collected data was analyzed by the degree of stress score, grouping into high(25〈) and low stress group(〈=25). High stress group showed unhealthy lifestyles, higher percentage of irregular menstruation, smoking, dizziness, higher amount of alcohol consumption. They also showed higher preference rate of spicy foods including hot foods, oil based foods and showed percentage of removing visible fat when eating meats. They more wanted to control weight and prefers snacking compared to low stress group. These results show that stress changes in eating behaviors in a way of unhealthy life styles.

Prevalence of Cognitive Impairment and Related Factors Among the Elderly in Rural Communities of Jeju Province

  • Purpose. The purpose of this study was to survey the prevalence rate of cognitive impairments and to identify the factors influencing cognitive impairment in the elderly in rural communities of Jeju Province. Methods. 590 elderly in 6 rural communities of Jeju Province were interviewed, using a questionnaire consisting of sociodemographic characteristics, health behavior, quality of life, and MMSE-K Results. Prevalence of cognitive impairment was 33.1 % (39.1 % of females, 16.76% of males). Prevalence of dementia was 12.4% (16.3% of females, 2.87% of males). Factors related to cognitive impairment were age, sex, education, standard of living, employment status, and subjective health state. Conclusions. In community health care for the elderly, factors relating to cognitive impairment have to be considered. When planning community health care, priority should be given to the elderly; who need care but live alone; who lack social support; who have a low standard of living; who experience discomfort in the activities of daily living; who believe they are not in a good state of health; or whose life satisfaction is low.

자살의 생물학적 기전 (Biological Mechanism of Suicide)

  • Suicide is a behavior that is intended to cause death by itself and requires medical treatment, resulting in suicidal attempt or completion. Suicide causes loss of life, damages the body, costs a lot of medical expenses, and causes families to fall into sorrow and suffering therefore this suicide is a huge loss to family and society. There have been attempts to reduce and prevent suicide by understanding the mechanism of suicide. The mechanism of suicide can be thought of as psychological mechanism and biological mechanism. In the past, if we considered the psychological and biological mechanisms separately, the development of neuroscience now connects and integrates these two. Psychological factors affect biological factors and biological temperaments also affect perception or thinking about the situation and increase psychological vulnerability. Distant factors in suicidal behavior-such as childhood adversity and family and genetic predisposition-increase the lifetime risk of suicide. They alter the response to stress and other processes through changes in gene expression and regulation of emotional and behavioral characteristics. Distant factors affect the biological system and consequently changes in these systems can increase the risk of suicide. In other words, the distal factor does not directly induce suicidal behavior but rather acts indirectly through developmental or mediating factors. These mediating factors are impulsive aggressive and anxious trait, and chronic use of substances. The mechanism of this disorder is the abnormality of the serotonin system and the abnormality of the lipid level. Proximal factors are associated with the onset of suicide events and include changes in the major neurotransmitter systems, inflammatory changes, and dysfunction of glial cells in the brain. A series of studies, including a variety of research methods and postmortem and in-vivo imaging studies, show the impairment of the serotonergic neurotransmitter system and hypothalamic-pituitary-adrenal axis stress response system for suicidal behavior. These disorders lead to suicidal behavior due to difficulty in cognitive control of mood, pessimism, reactive aggression, abnormality in problem solving abilities, excessive response to negative social signals, severe emotional distress, and cognitive dysregulation of suicidal ideation.

  • Pbjective : A significant proportion of children with autism spectrum disorders (ASD) have regression characterized by loss of previously acquired skills. The purpose of this study was to compare demographic, clinical characteristics and autism-related symptomatology of the children who have regression with children who don't have regression. Methods : The subjects with ASD and their unaffected siblings (SIB) were recruited from the Korean Autism Genetic Study Consortium. Typically developing children (TC) were volunteered from community. The subjects were administered the Korean version of Autism Diagnostic Interview-Revised (K-ADI-R) and the Korean version of Autism Diagnostic Observation Schedule (K-ADOS) to diagnose or exclude ASD. Regression was defined on the basis of K-ADI-R data. The Korean version of Vineland Adaptive Behavior Scale (K-VABS), Aberrant Behavior Checklist (K-ABC) and Social Responsiveness Scale (K-SRS) were obtained from their parents. Results : Regression occurred in 8.33% (n=14) of children with ASD (n=168). Any SIB (n=166) and TC (n=53) did not experience regression. Regression was associated with lower IQ and lower score of K-VABS. There was no difference in autism symptom severity and K-ABC, K-SRS scores, between children with ASD who experienced regression and who did not. Conclusion : Regression seems to be a distinctive feature of ASD. Regression is associated with cognitive and more general functions, rather than symptoms specific to autism.

상호작용형 에이전트의 설명 양식을 디자인하기 위한 프레임워크 개발 (Framework for Designing Explanatory Style of Interactive Agents)

  • 최근 다양한 형태의 에듀테인먼트 시스템 내에서 사용자와 상호작용이 가능한 에이전트를 통하여 사용자에게 참여 동기 유발을 향상시키기 위한 시도가 활발하게 진행되고 있다. 특히, 에이전트와의 상호작용을 통해 사용자에게 사회적 감정에 대한 이해를 향상시킬 수 있도록 하기 위하여 에이전트의 사회적 감정 생성 기법에 대한 연구가 진행되고 있다. 하지만 이러한 연구들은 대부분 사회적 감정 생성에 중요한 영향을 끼치는 개인성에 대해 추상적인 면을 다루는데 그치고 있는 실정이다. 본 논문에서는 개인성에 따라 에이전트가 사회적 상황을 이해하는 경향에 대한 차이점을 중점적으로 다루는 평가 모델을 개발하고 이러한 평가 모델이 에이전트의 감정 생성에 미치는 영향에 대해서 살펴본다. 이를 위하여 인간의 개인성과 상황 인지에 대한 관계성을 구체적으로 다룬 심리학적 설명 양식(Explanatory style) 이론을 기반으로 하여 에이전트로 하여금 개인성에 따라 자신이 처한 상황에 대해 서로 다른 설명 성향을 나타낼 수 있도록 하는 프레임워크를 제안한다. 이는 심리학적 설명 양식에서 언급하는 주요 설명 변수들을 에이전트에 적용 가능한 형태로 모델링하고 설명 변수의 정도에 따라 에이전트로 하여금 상황을 인지하는 성향을 결정할 수 있는 방법론을 제시한다. 그리고 에이전트로 하여금 설명 경향에 따라 상황을 인지 및 자율적으로 해석할 수 있도록 하고 이에 따라 감정 상태를 선택할 수 있도록 한다. 더 나아가 제안한 프레임워크를 기반으로 사용자와 상호작용이 가능하며 자율적으로 상황을 해석할 수 있는 증강 에이전트를 개발하였다. 또한 구현한 증강 에이전트를 상호작용형 원예 체험 시스템에 적용하여 사용자로 하여금 서로 다른 설명 양식을 표현하는 에이전트를 경험할 수 있는 기회를 제공하였다. 궁극적으로 이러한 체험을 통하여 사용자의 감정조절 능력 개발 수단으로서의 개발한 에이전트의 적용 가능성을 검토하고자 한다.

ADHD 초등학생을 위한 학교 중심 사회성기술 훈련 프로그램의 효과에 대한 연구 (The Effectiveness of School Based Short-Term Social Skills Training in Children with Attention-Deficit/Hyperactivity Disorder(ADHD))

  • Objectives: Children with attention-deficit hyperactivity disorder(ADHD) often have difficulties in social behavior. The aim of this study was to evaluate the effectiveness of a short-term training program for improving social skills, self-perception and attention deficits. Methods: The subjects were nine children diagnosed with ADHD with(or without) other mental disorders using the Diagnostic Interview Schedule for Children(DISC-ADHD) module. Children were given eight sessions of a social skills training program. Parents of children simultaneously participated in their own training which was designed to support their children's generalization of skills. Assessments included child, parent and teacher ratings of social skills, self-perception and attention deficit at baseline and post-treatment. Results: Social skills training led to significant improvements in child-reported measures of self-esteem, in teacher reported measures of social skills, and in parent-reported measures of attention deficit. Conclusion: This study suggests that short-term social skills training programs for children with ADHD may improve their social skills, self-perception and attention deficits.

자폐증 아동에 있어서 Carbamazepine의 치료효과 평가 (EVALUATION OF THE THERAPEUTIC EFFECTS OF CARBAMAZEPINE IN AUTISTIC CHILDREN)

  • 자폐증의 임상증상 완화에 대한 Carbamazepine의 효과를 평가하기 위하여 위약으로 대조한 이중맹(placebo controlled double blind) 연구를 실시하였다. 연구대상 아동은 1989년 10월부터 1991년 11월까지 서울대학병원 어린이병원 소아정신과 외래를 방문한, DSM-III-R의 전반적 발달장애의 진단기준을 만족시키면서, 부모가 연구에 동의한 23명의 남아로 하였다. 또한 다른 내과적 혹은 정신과적 질환력을 갖지 않으며, 연구 시작전 적어도 2개월이상의 약물복용이 없는 기간을 갖도록 하였다. 연구 대상군의 Cabamazepine치료군과 위약군으로 할당을 무작위적 할당표에 의해 할당하였는데, 각각 12명, 11명이 해당되었다. 연구 대상 아동의 상태를 모르는(blind) 2명의 평가자에 의하여, 기초관찰 기간과 12주의 약물치료 기간후에 측정한 검사도구의 측정값의 변화와 약물에 대한 부작용을 보았다. 두 검사자간의 신뢰도는 .4875-.6613이었다. 기초 관찰기간시 두군사이의 사회 인구학적 변인 및 검사도구의 값은 유의한 차이가 없었다. 약물 치료 기간 종결시 측정한 검사도구의 값과 기초 관찰기간의 값을 비교한 결과, Autism Behavior Checklist상 전체 점수의 유의한 차이(P<.05), 즉 Carbamazepine치료군이 위약군보다 자폐증 증상의 완화정도가 큰 소견을 보였다. Vineland Social Maturation Scale에서는 두군 모두에서 약물 치료 기간동안 사회성숙도상의 발달이 유의한 정도(P<.005)로 있었다.
