한국인(韓國人) 중고교생(中高校生)들의 흡연실태(吸煙實態)에 관(關)한 연구 (A Study on the Smoking Status of the Korean Middle and High School Students)

  • I investigated actual conditions of smoking of teenagers who were randomly chosen middle and high school students. 1. Juvenile smoking 1) Parents' opinions of juvenile smoking Most parents do not want their children to smoke after growth : 88.6% of fathers (middle school students: 88.9%, high school students: 88.4%) and 95.1% of mothers (middle school students: 93.4%, high school students :95.5%). 2) Teenagers' opinions of smoking after growth The rate of students who will smoke after growth is 10.8% (middle school students: 12.0%, high school students: 9.9%): students in agricultural areas show the higher rate than those in cities. 3) Parents' opinions of their children's smoking now 1.5% of fathers want their children to smoke now (middle school students: 1.3%, high school students: 1.6%) and 1.1% of mothers do (middle school students: 0.6%, high school students: 1.5%). This shows that most parents do not want their children to smoke now. 4) Students' opinions of their friends' smoking now Students who want their friends smoke now cover 7.8% (middle school students: 7.1%, high school students: 8.4%). This rates are higher than those of parents shown in (3). And more high school students and more girl students gave the positive reponse than middle school boy and girl students, respectively. 5) Students' views of smoking "Look like an adult" covers the rate of 4.0% (boy: 7.8%, girl:3.6%) 6.7% of middle school students have this view, while 3.7% of high school students have. 16.1% of students had an experience of smoking during the last one year (boy: 29.9%, girl: 8.6%): this shows that the rate of the boy students is more than 3 times greater than that of the girl students and high students who experienced smoking last year covers 20.2%, while middle school students shows 10.9%. 6) Actual conditions of students' smoking The present rate of students' smoking is 22.4% (boy:38.3%, girl:13.8%): the rate of boy students is greater than that of girl students. Students who smoke more than pack of cigarettes a day cover 8.2% (boy: 17.5%, girl: 3.2%): 5.2% of middle school students (boy:11.4%, girl: 2.1%) smoke more than one pack while 10.7% of high school students do (boy:21.5%, girl: 4.2%). This shows that the rate of boy students' smoking is greater than that of girl students' smoking. 7) The rate of smoking of students' parents 75.4% of fathers (city: 74.5%, agricultural area:75.9%) smoke: and more than a half (62.4%) smoke more than a pack cigarettes a day. On the other hand, the rate of smoking mothers is 5.2%(city: 4.3%, agricultural area: 7.3%): the rate is higher in agricultural areas. 8) Opinions of smoking population in the future 61.4% of students answered that smoking population will increase, while 27.0% have the opinion that smoking population will decrease. 2. Opinions of the effects of smoking on health 1) Have you heard that smokers are likely to suffer from tuberclosis? 78.3% of students said yes (boy: 80.8%, girl: 76.4%): it is shown that the rate of boys is greater than that of girls. 2) Have you heard that smokers are likely to get out of endurance? 76.6% of students (boy: 69.3%, girl: 49.7%) answered yes: it is shown that the rate of boys is greater than that of girls. 3) Have you heard that heart-beats get fast when one smokes? 32.5% of students (boy: 35.5%, girl: 30.9%) answered yes: 32.2% in cities(boy: 33.0%, girl: 31.8%) and 33.5% in agricultural areas(boy: 41.8%, girl: 28.8%): and 28.7% middle students and 35.5% of high school students answered yes. 4) Have you heard that smokers are likely to have heart-diseases? 35.1% of students (boy: 34.0%, girl: 34.1%) answered yes: 35.3% in cities (boy: 37.2%, girl: 34.2%) and 36.7% in agricultural areas (boy: 39.0%, girl: 33.9%): 34.8% of middle school students and 35.4% of high school students. 5) Have you heard that smokers are likely to have a lung cancer? 91.4% of students (boy: 93.2%, girl: 89.9%) answered yes: 90.35% in cities and 94.2% in agricultural areas. 6) Have you heard that the life of smokers gets shorter? 94.3% of students (boy:94.6%, girl: 92.2%) answered yes. 7) Have you heard that pregnant smokers will deliver a baby with low birth weight? 29.6% of students (boy: 29.8%, girl: 29.4%) answered yes: the rates of boys and girls almost the same. 8) Have you heard that one feels calm when one smokes? 80.1% of students (boy: 81.8%, girl: 79.2%) answered yes: boys and girls showed almost the same rate. 3. Preventive measures Smoking people continued to increase all over the world because smoking not only mitigated emotional uneasiness such as loneliness, nervousness and so on, but also could be very helpful from the social perspective. This was so because they did not consider harmful effects of smoking on health, and victims. However, because any -one can have physical disorders caused by smoking, people should always keep in mind the following preventive measures. 1) Doctors or teachers should set an example of giving up smoking. Informing patients or students of harmful effects of smoking to persuade their family and relatives not to smoke. 2) Through mass media like newspapers, periodicals or broadcasting, to make people know harmful effects of smoking and not smoke. 3) To prohibit selling teenagers cigarette by law. 4) To prohibit smoking in public places like work places, offices, lecture rooms, recreation rooms, buses, trains and so on. 5) To decrease the rate of life insurance for non-smokers as in foreign countries and to give a warming of the harmful effects on cigarette packets or ads.

국가표본조사자료 기반 청소년 성경험의 개인 및 가족 요인 분석 (Individual and familial factors associated with youth sexual experience based on national sample survey)

  • 본 연구는 우리나라 청소년의 성경험에 영향을 주는 개인 및 가족요인을 파악하고자 우리나라 청소년들에 대한 국가 통계량 생산을 위해 복합표본설계 기반으로 조사되고 있는 제11차 (2015년) 청소년건강행태온라인조사 자료를 활용하였다. 연구 대상자는 2015년 국내 중 고등학교에 재학 중인 68,043명이며, 층화 및 집락, 가중치의 복합표본설계를 고려하여 기본 빈도분석과 성경험 유무에 대한 로지스틱회귀분석을 수행하였다. 본 연구결과에 따르면 개인 (나이, 학교유형, 스트레스 정도, 흡연, 음주) 및 가족 (경제상황, 부모동거) 요인이 청소년들의 성경험에 유의한 관련성을 가지고 있었다. 특히 음주경험과 흡연경험이 있는 대상이 그렇지 않은 대상에 비해 각각 2.03배와 4.01배 높은 성경험 가능성을 나타내고 있으며, 양부모와 동거하지 않는 대상이 양부모와 동거하는 대상에 비해 3.47배 높은 성경험 가능성을 나타내고 있다. 따라서 청소년들의 올바른 성문화를 위해 위험요인들을 고려한 전문적인 교육을 강화할 필요가 있으며, 특히 개인요인 중 큰 위험요인으로 나타난 음주와 흡연에 노출된 청소년들을 보호하기 위해 국가의 더욱 적극적인 규제가 필요할 것이다. 그뿐만 아니라 경제상태가 좋지 않거나 부모들과 함께 거주하지 않는 자녀에 대해서도 부모들의 더 큰 관심이 필요할 것이다.

시멘트 취급 근로자들에 대한 구강위생실태 연구 (A Study on the Occupational Diseases and Dental Hygiene of Laborers in Cement Industry)

  • The purpose of this study was to examine the actual dental hygiene status of workers in cement industry in an effort to serve as a basis for enhancing their dental hygiene. The subjects in this study were 420 laborers from the cities of Curi, Donghae and Samcheok who handled cement. As a result of conducting a survey, the following findings were given : l. The workers investigated found their working environment satisfactory, getting a mean score of 323. They also expressed a high job satisfaction with a mean score of 333. 2. They didn't have a big interest in oral health. By age group, those who were aged between 36 and 40, or who cared less about health management, showed a higher interest in oral health. Besides, the laborers who kept smoking for a longer period, or who drunk more, expressed greater interest. 3. The toothbrushing method was considered most important for periodontal health, by 45.0% of the workers, and the next most crucial one was regular dental examination, followed by refraining from smoking and staying away from sweet food in the order named. 4. Concerning daily mean toothbrushing frequency, 455% brushed their teeth three times a day on the average. More than half them didn't pay enough attention to toothbrushing. 5. Regarding scaling, the large number of the workers, 42.4%, had no experience to get their teeth scaled, 37.6%, the greatest percentage, didn't have their teeth scaled because it seemed to make their teeth painful or cold. 6. As fororal health education experience, 67.6%, the great number of them, had no experience to receive dental health education. The above-mentioned findings suggest that the cement-related workers generally neglected dental health management. They should be encouraged to correct their wrong oral health knowledge or habit to make their oral cavity more healthy. To make it happen, it's required to provide oral health education and promote organized dental health projects.

대학생의 건강증진 프로그램 참여의사에 영향을 미치는 주요 요인에 관한 연구 (A Study on the Factors to Associate with the Participative Intention for Health Promotion Programs in a University)

  • This study purposed to examine the participative intention for health promotion program in a university and to find out the factors to associate with the participative intention. The data were based on the self-reported questionnaires from 746 women who study in E university, and this survey performed May, 1998. This study performed to analyze the participative intention for health promotion programs and the factors associate with health promotion program using $chi^2$-test and trend test by the PC-SAS 6.12. The major findings were as follows: 1. The tendency of participative intention for health promotion programs showed that Influenza preventive program was the highest among the health promotion programs, and the next were Weight control program, Rubella preventive program, Fitness program. On the other hand, Smoking preventive program and Non-drinking program were lower than the other program. 2. The four significant factors on participative intention for health promotion programs were grade, concern for health, and behavior change experience through the health education. On the other hand, the cognitive level for health, experience for health education were not the significant factors associate with the participative intention for health promotion programs. 3. The relationship between factors and each health promotion program showed that Rubella preventive program, Influenza preventive program, Weight control program, Smoking cessation program and Non-drinking program were associated with the grade or the health concern. And Chronic diseases preventive program was associated with the grade and the concern for health. Fitness program and Sex education program were associated with the concern for health and the behavior change experience through health education.

흡연상태에 관한 자가보고 설문의 타당도 평가: 제1기(2009-2011) 국민환경보건기초조사 자료 분석 (Validity Assessment of Self-reported Smoking Status: Results from the Korean National Environmental Health Survey (KoNEHS) 2009-2011)

  • Objectives: The purpose of this study was to assess the validity of self-reported cigarette smoking status and investigate factors associated with the accuracy self-reported and measured urinary cotinine in Korean adults. Methods: We used data from the $1^{st}$ Korean National Environmental Health Survey (2009-2011) among adults aged ${\geq}19$ years (N=6,246). The survey examined self-reported smoking status, and urinary cotinine was regarded as the biomarker of exposure to tobacco smoke. Urinary cotinine was analyzed using a gas chromatography-mass spectrometry (GC/MS) and data analysis was conducted using IBM SPSS version 20.0, which uses the sample weight and calculates variance estimates to adjust for the unequal probability of selection into the survey. Results: We calculated a cut-off point (53.3 ug/L) by using a ROC (Receiver Operating Characteristic) curve. The smoking prevalence was 24.6% based on self-reported data and 28.2% based on urinary cotinine concentrations. When we assessed the agreement between self-reported and urinary cotinine, we found an average agreement of 97.7% among self-reported smokers and 94.5% among self-reported non-smokers. Among self-reported smokers, factors affected the discrepancy were age, household economic status and average number of cigarettes smoked per day. On the other hand, gender, former smoking experience, and exposure to SHS (second hand smoke) were associated with discrepancies among self-reported non-smokers. Conclusion: These results suggest that self-reported data on smoking status provide a valid estimate of actual smoking status. In future research, we will conduct a continuous monitoring study for reliability verification of the data to reduce potential interpretation errors.

국민건강영양조사 제8기 1차년도(2019년) 원시자료를 활용한 흡연, 음주와 안전의식 연구 (A Study on Smoking, Drinking and Safety Awareness using Raw Data in the 8th year of the National Health and Nutrition Examination Survey(2019))

  • 본 연구는 제 8기 1차년도 2019년 국민건강영양조사의 원시자료를 이용한 2차 자료 분석연구이다. 본 연구의 대상자는 질병관리 본부에서 우리나라 가구를 층화집락 추출방식으로 표출한 만 19세 이상 6,606명으로 남자 2,956명, 여자 3,650명이었다. 자료분석은 SPSS WIN 23.0 version을 이용하여 흡연, 음주와 안전의식 간의 차이에 대한 통계적 유의성 검증을 실시하였다. 19세 이상 평생 흡연 경험에서는 성별(𝑥2= 2571.24, p<.001)에서 통계적으로 유의한 차이가 있었다. 19세 이상 평생 음주 경험에서 성별(𝑥2= 216.02, p<.001)에서 통계적으로 유의한 차이가 있었다. 안전의식 중 운전 시 안전벨트 착용 여부와 평생 흡연 여부는 통계적으로 유의한 차이가 있었다(𝑥2= 6581.46, p<.001). 안전의식 중 운전 시 안전벨트 착용 여부와 평생 음주 경험은 통계적으로 유의한 차이가 있었다(𝑥2= 6421.29, p<.001). 따라서, 안전의식 증진을 위해서는 금연과 금주를 시킬 수 있는 성별과 흡연과 음주의 관련 특성을 고려한 생의 주기별 맞춤형 교육 중재 프로그램 개발이 필요하다.

스트레스가 고등학생의 흡연.음주에 미치는 영향 (The Influence of Stress on Smoking and Drinking of High School Students)

  • This research attempted to investigate the real state of adolescent smoking and drinking, and to study how stress influences on smoking and drinking of high school students. This data was collected from 837 first and second grade students in a vocational high school in Go-yang city, Gyeonggi province. Data analysis consisted of frequency, percentage, Chi-square, T-test, step-wise regression analysis, using SPSSWIN. The results of analysis were as follows: First, 51.7% of the students have never smoked, while 48.3% have experienced smoking. Most students started smoking in the second year of the middle school, though 17.6% of the smokers already started in the elementary school. The strongest motive of initial smoking was curiosity, next the inducement of friends, and 10.9% of them smoked to get rid of stress. Their favorite place for smoking was in the order of the entertainment centers, schools, and private academies. They personally purchased cigarettes mostly at the store. Most of them smoked less than 5 cigarettes a day, but 3.7% smoked more than a pack of cigarettes a day, usually with friends rather than alone. Second, the ratio of drinking was relatively high 78.1%, and their initial drinking experience was most frequent at the third year of the middle school. The initial motive of drinking was mostly curiosity and the inducement of friends, but 10.7% of them started drinking to reduce stress and anxiety. The places for buying alcohol and drinking were bars or restaurants. The ratio of habitual drinking was 45.1%, and the ratio of irregular drinking was 53.3%. 1.8% of the drinking students enjoyed drinking everyday. The amount of drinking varied from half a bottle to two bottles of Soju, and the drinking problem was serious to see that 11% of them drank more than two bottles at a time. Because 84.4% drank in groups with friends, a thorough and systematic supervision and education of the peer group drinking is essential. Third, as for the relationship between personal matters and smoking, it was found that there was a positive relation between smoking and sex, but there was no correlation between smoking and school years. Adolescent smoking was also related with scholastic achievement, the number of friends, and the existence of parents. Fourth, as for the relationship between personal matters and drinking, it was found that there was no correlation between drinking and sex, but there was a positive relation between drinking and school years. Fifth, the average index of stress was students themselves(2.58), school life(2.53), family life(2.19), friends(2.00), and the total index of stress was 2.33. Sixth, there appeared a marginal negative correlation between stress and 'drinking and smoking' in Pearson coefficient of correlation r to see the influence of stress on smoking and drinking. The difference of the average index of stress according to smoking and drinking bears meaningful difference for all students, smokers and non-smokers, drinkers and abstainers in students themselves, school life, family life, friends, and total stress. Smoking has an effect on family life most, school life next, and drinking has an effect on school life most, family life next, with the explanation power of 11% and 9% respectively, in a regression analysis to analyze the factors influencing on smoking and drinking among the factors of stress.

대구, 경북지역 고등학생의 조기흡연과 관련된 요인 (Factors Related to Early Smoking of High School Students in Daegu City and Gyeongsangbuk-do Province)

  • 대구광역시 3개 인문계고등학교 364명, 경산시, 경주시, 안동시 인문계 고등학교 각 1개교씩 456명 등 인문계 고등학교 학생 920명을 대상으로 2003년 8월부터 9월까지 2개월간 구조화 된 설문지를 이용하여 조기흡연 경험률과 관련 요인을 분석하고자 연구를 시행하였다.대상자의 조기흡연 경험률은 1.8%였다. 흡연동기에 대한 복수응답결과 ‘호기심’이 20.2%로 가장 높았고, ‘친구의 권유’가 9.0%, ‘스트레스 해소’가 6.2%였다. 지속적으로 흡연을 하는 이유에 대해서는 ‘습관 때문에’가 53.1%로 가장 높았고, ‘스트레스 해소’가 26.5%, ‘친구와 어울리기 위해서’가 8.2%였다. 흡연경험 학생의 금연 동기는 ‘건강 때문에’가 68.4%로 가장 높았으며, 금연을 시도한 경험이 있는 학생들의 금연 실패이유는 ‘의지가 약해서’가 58.7%로 가장 높았다. 흡연경험 학생의 향후 금연계획에 대한 설문 결과 ‘금연계획이 없다’는 학생이 12.6%였으며, ‘언젠가 금연하겠다’는 학생은 26.6%였다. 이경수, 강복수, 황태윤, 김상규 10대상 학생의 현재 흡연율은 10.8%였으며, 조기흡연 경험이 있는 학생에서의 현재흡연율은 13.8%였고, 조기흡연 경험이 없는 학생의 현재 흡연율은 10.4%로 차이는 있었으나 통계적으로 유의하지는 않았다. 조기흡연과 가족 중 흡연자 여부(p<0.01)와 향후 흡연 의향(p<0.05)이 유의한 관련성이 있었다.조기흡연 여부를 종속변수로 시행한 다중 로지스틱 회귀분석에서는 스트레스 점수가 높을수록, 가족 중에 흡연자가 있을수록 조기흡연에 유의한 영향을 미치는 것으로 나타났다.청소년의 조기흡연이 증가하고 있으므로, 조기흡연의 실태를 파악하고 관련요인을 밝혀 조기흡연을 예방함으로써 흡연율과 흡연시작연령을 연장시키기 위한 방안을 마련해야 하며, 조기흡연의 경험이 있는 학생들의 지속적 흡연을 방지하여야 한다. 이를 위하여 학교 내외에서 적용할 수 있는 초등학생에 적합한 흡연예방 및 금연교육 프로그램을 개발하여야 힐 것이다.

한국 성인의 전자담배 사용 관련요인 (Factors associated with Electronic Cigarettes Use in Korean Adults)

  • 이 연구는 한국 성인의 전자담배 사용과 관련된 요소들을 조사하기 위해 수행되었다. 카이제곱 검정, 로지스틱회귀분석을 통해2015년 한국질병통제센터에서 실시한 지역건강조사228,477건을 분석하였다. 전자담배평생 유병율이 남성 11.1%, 여성 0.8%로 나타났다. 현재 전자담배 흡연율은 남성이 24.2%, 여성이 26.1%로 였다. 평생전자담배 흡연경험율은 연령 교육수준, 직업, 소득상태 및 현재흡연상태와 각각 유의미하게 관련되어 있었다. 현재의 보편적인 전자담배 흡연이 현 흡연상태와 유의한 관련이 있는 것은 과거에 흡연했던 그룹보다 현재흡연자 그룹이 현저히 높은 경험을 보이고 있는 것이다. 전자담배의 선호는 높았다. 전자담배는 금연을 목적으로 하는 이들이 전자담배가 일반담배에 비해 금연 효과가 있는 것처럼 인식하고 있었고, 흡연에서 오는 쾌감이나 중독성을 만족시켜 주었다. 따라서 전자담배는 흡연에 대한 딜레마에서 벗어날 수 있는 해결책으로 반영되어 선호도가 높았다. 2014년 전자담배 수입은 전년도 대비 약 3배 증가했고 세계적으로도 전자담배에 대한 관심이 증가하고 있다. 우리는 전자담배의 예방과 중단에 관련된 특징을 고려해야 한다. 흡연자들이 잘못된 흡연관련 건강인식을 개선하기 위한 금연교육 강화와 금연 위해성에 대한 적극적인 홍보가 필요하다. WHO에서도 전자담배의 안전성을 담보하거나 효과가 있다고 인정한 근거가 부족하다고 권고하고 있다. 전자 담배의 사용 동기에 있어서 중요한 환경적인 요인들을 확인하고 전자담배인식과 사용에 대한 연관성을 분석하는 연구들을 모색해야 한다.

한국 청소년의 치아우식증과 치주질환에 대한 예방행위 경험 관련요인 (Related factors of preventive behavior experiences toward dental caries and periodontal disease in Korean adolescents)

  • Objectives: The objective of the study was to investigate the related factors of preventive behavior experience toward dental caries and periodontal disease in Korean adolescents. Methods: The study subjects were 72,060 adolescents in 800 schools who completed 2014 Korean Youth Risk Behavior Web-based survey. Dependent variables included general characteristics of the subjects, oral health behavior, and oral disease experience. The experience rate of sealant and scaling showed the prevention behavior of dental caries and periodontal disease. Data were analyzed by IBM SPSS statistics 20.0. Results: The experience rate of sealant accounted for 26.9%. The experience of sealant was related with gender, school, maternal education level, economic status, vigorous physical activity, smoking, usual stress, fruit consumption, milk consumption, soda consumption, snack consumption, tooth brushing, oral health education experience, periodontal bleeding and pain, mucosal disorders, and bad breath. The experience rate of scaling was 22.6%. The experience of scaling was related with gender, school, city division, learning achievement, maternal education level, economic status, residential type, vigorous physical activity, usual stress, fruit consumption, milk consumption, tooth brushing, oral health education experience, periodontal bleeding and pain, mucosal disorders, and bad breath. Conclusions: To expand preventive oral health behavior in the adolescents, it is necessary to support the systematic policy making and monetary establishment in the future.
