본 연구는 만성질환 여성 노인의 삶의 만족도 영향요인을 파악하여 삶의 만족도를 증진하기 위한 기초자료를 제공하고자 시도되었다. 제6차 국민노후보장패널 만 65세 이상 만성질환 여성 노인 1,846명을 연구 대상으로 하였다. 일반적 특성에 따른 삶의 만족도, IADL, 대인관계를 알아보기 위하여 기술통계, t-test, ANOVA를 사용하였고, 삶의 만족도에 미치는 영향요인을 알아보기 위하여 위계적 다중회귀 분석을 이용하였다. 연구 결과 만성질환 여성 노인은 나이가 어릴수록, 배우자가 있는 경우, 교육 수준이 증가할수록, 적절한 수면, 규칙적 운동, 정기 검진을 실천하는 경우 삶의 만족도가 높았다. 삶의 만족도에 수면(β=.11, p<.001), 운동(β=.07, p<.001), 월 소득(β=.13, p<.001), IADL(β=-.05, p<.001), 대인관계(β=.38, p<.001)가 유의미한 영향을 주었으며, 그 설명력은 27%이었다. 따라서 만성질환 여성 노인의 삶의 만족도를 향상하기 위하여 적절한 수면, 운동, IADL, 대인관계를 증가시키는 중재가 필요로 할 것이다.
목 적 : 본 연구의 목적은 침실에서 야간 조명이 강제적으로 사용되는 교정기관 내 정신과적 입원 환자들에게 상업용 활동 추적기와 수면일지를 통해 안대 착용과 완전 소등이 수면 상태에 주는 효과들의 차이를 조사하는 것이다. 방 법 : 본 연구는 3일 연속 밤 동안 수행되었다. 국립법무병원에 입원된 29명의 정신질환자들에게 무작위 연구 번호가 배정되었으며, 첫 날에는 야간 조명 환경에서 평소처럼 취침했다. 29명 연구대상자들은 다른 날에는 야간 조명환경에서 안대를 착용한 채 취침하였으며, 또 다른 날에는 완전 소등 환경에서 안대 없이 취침하였다. 29명 연구대상자들은 상업용 활동 추적기를 착용하면서 취침하도록 요청되었으며, 다음 날 아침 수면일지를 작성하였다. 연구대상자들은 이러한 둘째 밤과 셋째 밤의 침실 조도 환경 변화를 무작위로 겪도록 배정되었다. 결 과 : 조명 하 수면 환경과 안대 착용 수면 환경 비교 시, 안대 착용 수면 환경에서 수면 후 각성 시간이 짧았고, 수면 만족도가 높았다(각각, Z = 3.66, p < 0.017 ; Z = 2.69, p < 0.017). 또한, 조명 하 수면 조건과 소등 하 수면 조건 비교 시, 소등 하 수면 조건에서 수면 후 각성 시간이 짧았고, 수면 효율, 수면 만족도가 높았다(각각, 31.14분 대 15.00분, Z = 2.40, p < 0.017 ; 79.40% 대 88.10%, Z = 3.02, p < 0.017 ; 2.97점 대 4.10점, Z = 3.88, p < 0.017). 그러나 안대 착용 수면과 소등 하 수면 비교에서는 유의미한 차이가 없었다. 결 론 : 본 연구 결과 정신질환자나 교정기관의 수용자에게 수면을 개선시키기 위해 완전 소등 또는 환경 개선이 불가능할 경우에는 안대 착용을 권장하는 것이 주관적인 수면의 질과 만족도를 높이는데 유용한 방법이 될 수도 있겠다.
Purpose: To examine the effects of the environmental nursing intervention using eye-shields and earplugs on the incidence of delirium, environmental stress and sleep among the patients with open-heart surgery in the post-cardiac ICU(intensive care units). Method: This study was a non-equivalent control group non-synchronized design. The subjects consisted of 93 patients who received ICU care after the cardiac surgery. Among those, 32 subjects was assigned in one experimental group with eye-shields, 31 in the other experimental group with earplugs, and 30 in the control group. ICU delirium was measured by CAM-ICU, environmental stress by ICUESS, and sleep by two sleep scales developed by Oh, et al. and Kim. The data were analyzed by the SPSS 13.0 program including frequency, percentage, t-test, $x^2$ test, ANOVA and ANCOVA. Results: There are no statistically significant differences in the incidence of delirium among three groups (p=.139). The ICU environmental stress scores from the subjects of two experimental groups demonstrated lower (F=6.731, p=.002) than the control group. Also, the degrees of sleep pattern (p=.000, p=.000) and sleep satisfaction (p=.000, p=.000) were greater than those of the control group. Conclusion: Although the environmental nursing intervention using eye-shields and earplugs did not demonstrate the effects in decreasing the incidence of ICU delirium, it has affirmative effects in decreasing environmental stress and improving the quality sleep among the patients with open-heart surgery while they stay in ICU. Further nursing implications are discussed.
Purpose: The purpose of this study was to compare the predictors between the early and late postpartum period in parturient women. Methods: The Subjects were 399 healthy postpartum women who visited the obstetric clinic at 5 hospitals for a routine check up after childbirth. They were divided into two groups: one was in the early postpartum period, the first three weeks after childbirth (n=107), the other was in the late postpartum period, four to six weeks after childbirth (n=292). The data were collected by using a self-report questionnaire from July to September, 2006. The data were analyzed with the SPSS 12.0 Win program. Results: There were significant statistical differences in age and parity between the two groups. Postpartum fatigue was significantly predicted by postpartum depression and age in the early period, and postpartum depression, sleep satisfaction, and childcare stress in the late period. The mean scores of childcare stress and perceived infant difficulty in the late period were significantly higher than those of the early postpartum period. Conclusion: Postpartum depression was the most important predictor of postpartum fatigue regardless of the postpartum period. In addition, especially in the late period, sleep satisfaction and childcare stress were significant predictors of postpartum fatigue. Based on these results, primary nursing interventions for reducing postpartum fatigue should focus on postpartum depression management, and nursing interventions for increasing sleep satisfaction and decreasing childcare stress would be helpful for management of postpartum fatigue in parturient women in the late period.
Purpose: This study was performed to analyze the characteristics and effect sizes of intervention studies on foot-reflexo-massage applied to cancer patients. Methods: For meta-analysis, a total of 159 studies were retrieved from search engines such as RISS, nanet, KISS, richis and KoreaMed. 16 studies published from 1990 to 2010 were selected based on the inclusion criteria. The data were analyzed with the RevMan 5.0 program of Cochrane library. Results: 1) The mean score of 1 implement time on foot-reflexo-massage was 25.62 minutes, the average number of days was 4.12 days, and the total number of average intervention frequency was 4.25 times. 2) Intervention studies on foot-reflexo-massage included 9 studies on anxiety (56.3%), 7 for pain (43.8%), 5 for BP/pulse (31.3%), 5 for fatigue (31.3%), 3 for nausea/vomiting (18.8%), 3 for sleep satisfaction (18.8%), and 2 for depression (12.5%). 3) The effect sizes of the intervention studies that showed higher effect size were in order, anxiety (d=-1.76), fatigue (d=-1.43), depression (d=-1.03), nausea and vomiting (d=-0.83), pain (d=-0.77), pulse rate (d=-0.61), blood pressure (d=-0.55), and sleep satisfaction (d=0.43). Conclusion: This study suggests that foot-reflexo-massage can increase sleep satisfaction, whereas decreasing blood pressure, pulse rate, anxiety, fatigue, depression, nausea, vomiting and pain.
Purpose: The purpose of this study was to identify the effects of foot massage on the degree of nausea/vomiting, anxiety, sleep satisfaction and fatigue during chemotherapy. Method: Total 36 subjects who were admitted to C university hospital in G city during 3 days and 2 nights for chemotherapy were selected from January to June, 2003. Seventeen of these subjects were assigned to control group and the rest were assigned to experimental group. Tools were Index of Nausea, Vomiting & Retching (INVR), Spielberger's state anxiety, sleep satisfaction, & fatigue VAS. Data were collected at both pre-test and post-test. Results: Difference of pre-test and post-test was not significant between control group and experimental group on the degree of nausea/vomiting, anxiety and sleep satisfaction. However, the effect of foot massage on the fatigue was significant. Conclusion: Further studies need to identify the immediate effect of foot massage on either nausea and vomiting or anxiety and to develop strategies for lengthening and strengthening the short-term effect of foot massage.
Purpose: Examination of the effects of auricular acupressure therapy on insomnia of Taeumin in elderly women. Methods: Fifty-six women over 65 yr-of-age (experimental group n=28, control group n=28) residing in Seoul or Gyung-gi province participated in the study, which used an unequivalent control group pretest-posttest design. Data collected from May, 2007 to May, 2008 was analyzed by SPSS Ver. 16. Results: Auricular acupressure was associated with better sleep state and self-reported satisfaction of sleep as compared with the control group. Conclusions: Aricular acupressure therapy can be beneficial in relief of insomnia in elderly women.
Purpose: The purpose of study was to examine effects of aromatherapy on stress, sleep, nausea and vomiting of women after laparoscopic hysterectomy. Methods: The participants were 60 women who had laparoscopic hysterectomy: experiment group for aromatherapy (n=30) and control group for routine care (n=30). The experimental group received inhalation aromatherapy for 5 minutes, twice; the first was done right after the operation, the second was at 9 pm before sleep on the same day-while the control group had no inhalation. Data were collected from July to September, 2012 at G hospital. Results: The degree of psychological stress was not significantly different between two groups (t=-1.96, p=.054). Yet, there were significant differences between two groups for degree of physiological stress (t=-3.20, p=.002), the level of cortisol (t=-2.01, p=.049), the score of sleep status (t=2.47, p=.016), the score of sleep satisfaction (t=2.43, p=.018), and the score for nausea and vomiting (t=-2.58, p=.012). Conclusion: Inhalation aromatherapy using the mixed oil of lavender, mandarin, and marjoram was effective in decreasing the level of physiological stress, cortisol, and the score for nausea and vomiting, and also allowed the participants to have a better sleep. Therefore, inhalation aromatherapy could be effective in improving the quality of life of these women during recovery.
Objectives: The purpose of this study was to provide a counseling program basis for the department satisfaction in dental hygiene department according to the subjective factors of happiness in dental hygiene students. Methods: A self-reported questionnaire was completed by 197 dental hygiene students in Gwangju from April 10 to 30, 2017. From collected data, general characteristics, happiness index and department satisfaction of the subjects were investigated. Data were analyzed using the IBM SPSS 18.0 program. Results: The happiness index according to general characteristics showed a significant difference in grade, character, friendship, academic achievement, pocket money satisfaction and health condition. The department satisfaction according to general characteristics showed a significant difference in grade, age, character, motivation for further study, friendship, academic achievement, pocket money satisfaction, health condition, sleep time. Happiness index showed a significant correlation with department satisfaction. The happiness index was higher as the department satisfaction was higher. Factors that affect happiness index are friendship, health condition, relationship satisfaction, perception satisfaction, school satisfaction, curriculum satisfaction and department satisfaction. Conclusions: The happiness index was higher as the department satisfaction was higher. Therefore, it is necessary to develop various programs to increase the happiness.
연구목적: 알코올의존 환자의 수면 장애에 대한 치료에 있어서 기존의 진정수면제 등의 약물은 교차 내성을 일으키고, 수면의 질을 악화시키기도 한다. 광치료는 일주기리듬장애, jet-lag, 순환제교대근무에서 나타나는 수면장애와 노화와 관련된 수면 장애의 치료에 효과적인 비약물적 치료중재이다. 따라서 저자들은 이미 여러 질환의 수면장애에 시도되고 있던 광치료의 회복기 알코올의존환자의 수면장애에 대한 치료적 효과를 알아보고자 하였다. 연구방법: 알코올 금단 증상을 보이지 않는 13명의 알코올의존 환자들을 대상으로 하여 2500Lux의 광상자로 하루에 오전 1시간 씩 3일에 걸쳐 광치료를 시행하였다. 총수면시간, 실제수면시간, 수면잠복기, 수면효율, 각성빈도 등을 수면기록지를 통해 평가하였으며, visual-analogue scale을 통해 주관적인 수면의 만족도를 평가하였다. 오전 8시, 오후 2시, 오후 8시 3차례에 걸쳐 연구대상자의 졸림 정도를 스탠포드졸림척도를 통해 평가하였으며, visual-analogue scale을 통해 불안, 우울 기분 및 피로감을 평가하였다. 수면상태를 반영하는 수행 능력의 평가를 위하여 기초단계와 연구 4일째에 기호잇기검사(trail making test)A, B 및 기호바꿔쓰기검사(digit symbol substitution test)를 시행하였다. 각 측정값은 반복측정분산분석법을 이용하여 비교하였으며 수행능력의 평가는 사전사후차이검증을 이용하였다. 결 과: 수면기록지로 평가된 수면잠복기, 수면효율은 광치료의 효과를 확인할 수 있었으며 기초 단계에 비해 연구 1일째부터 유의한 차이를 보였다. 수면에 대한 만족도도 광치료에 따라 유의하게 증가하였으며 연구 1일째부터 유의한 차이를 나타내었다. 오후 2시에 평가된 졸림은 광치료의 효과를 확인할 수 없었지만 오전 8시에는 광치료에 따라 졸림이 유의하게 감소하였고 오후 8시에는 유의하게 증가하였다. 기호잇기검사와 기호바꿔쓰기검사에서 환자들은 광치료 전후 수행 시간이 유의하게 감소하였다. Visual analogue scale에 의한 피로감의 평가에서 오후 2시와 오후 8시에 광치료에 따라 유의하게 피로감의 감소가 나타났으며 우울기분과 불안은 광치료의 효과를 확인할 수 없었다. 결 론: 본 연구에서는 금단 증상에서 회복된 후에도 지속적으로 수면장애를 호소하는 알코올의존 환자들에 있어 비록 제한적이지만 광치료의 효과를 확인할 수 있었고, 임상 실제에서 알코올중독 환자들의 수면장애에 단기간의 광치료가 효과적인 치료도구가 될 수 있음을 제시하고 있다. 추후 보다 객관적인 수면 평가 도구를 이용한 장기간의 대조군 연구가 필요할 것으로 생각된다.
