Evaluation of Contact Lens Care Regimens in Removing Tear Lipids on Silicone Hydrogel Lens (콘택트렌즈 관리방법에 의한 실리콘하이드로겔렌즈의 지방침전물 제거효과)

  • Purpose: An in vitro study was conducted to compare the lipid cleaning efficacy of care solutions on balafilcon A silicone hydrogel (SiHy) lens. Methods: Lipid spoilation was performed by incubating balafilcon A SiHy lenses in phosphate buffered saline (PBS) containing oleic acid, oleic acid methyl ester and cholesterol. Spoiled contact lenses rinsed with PBS were cleaned with surfactant cleaner, alcohol containing cleaner and multipurpose solution (MPS) respectively and repetitive spoilation and cleaning was conducted up to 14 times. To observe the cleaning effect of ultrasonic wave on the lipid deposit, each spoiled lens was ultrasonicated and then compared with non-sonicated lens. Lipids deposit on the contact lenses was extracted by methanol:chloroform (1:1, v/v) solution. High performance liquid chromatography was used to analyze and quantify lipid deposit extracts. Results: The effectiveness of alcohol containing surfactant cleaner on the lipid deposits was better than that of surfactant cleaner and MPS, and the cleaning efficacy was significantly higher in the ultrasonic wave treated group. Lipid deposits were not removed completely by contact lens care solutions so that lipid deposits increased continuously and cumulatively. Conclusions: The cleaning efficacy of contact lens care solutions was not satisfactory to remove lipid deposits on the SiHy lens that new cleaning products specially designed for SiHy lenses are needed to develop.

Study of Masking Effect of Soft Contact Lenses on Cornea after Refractive Surgery (각막굴절교정 수술 안에 대한 소프트콘택트렌즈 착용 시 Masking 효과)

  • Purpose: This study investigated the masking effect of the hydrogel lens and silicone hydrogel lens on the cornea with refractive surgery and without surgery. Methods: 24 university students (means age: $23.48{\pm}2.89years$) without refractive surgery (12, control group) and with refractive surgery (LASIK: 8, LASEK: 4, experimental group) participated in the study. Mean refractive errors of right eyes were -2.73 D for control group and -0.24 D for experimental group. The differences in the refractive power and corneal topography map between pre- and post-wearing the -3.00 D lenses were compared, and 2 kinds of hydrogel contact lenses (0.89 Mpa, 0.49 Mpa) and 2 kinds of silicone hydrogel lenses (1.5 Mpa, 0.8 Mpa) were used for -3.00 D lenses. NVision-K5001 (Shin nippon, Japan) was used to measure the refractive power and Keratograph 5M (Oculus, Germany) to measure the corneal topography map change. Results: Variations in the refractive power increased to the plus direction in the experimental group after wearing soft contact lenses. The corneal topography map showed significant changes on the both groups after wearing soft contact lenses (p<0.05). However there were no significant differences in the refractive power and corneal topography map variations by lens materials. Conclusions: Wearing soft contact lenses showed corneal topography map changes. Especially wearing soft contact lenses on the flat cornea after corneal refractive surgery showed greater corneal power changes. Therefore, it should pay attention to refractive change in case of prescribing soft contact lenses to patients with corneal refractive surgery.

  • Interfacial and rheological properties of selected hydrogel formulations were studied to understand the contact-lens comfort in end use. It was concluded that protein adsorption from aqueous solution decreased monotonically with increasing surface energy (water wettability) of tested hydrogels. Also, it has revealed that friction coefficient of polydimethylsiloxane-polyvinylpyrrolidone (PDMS-PVP) was significantly larger that 2-hydroxyethyl methacrylate (HEMA) based hydrogels. Interestingly, in artificial tear solution, friction coefficients of HEMA based hydrogels were larger than silicone hydrogels.

Reliability of Measurements of Back Vertex Power for Soft Contact Lenses Using an Auto-Lensmeter (자동렌즈미터를 이용한 소프트 콘택트렌즈의 굴절력 측정 방법에 관한 신뢰도)

  • Purpose: To assess the reliability for measuring the back vertex power of soft contact lenses by dry blotting and wet cell method using an auto-lensmeter. Methods: The soft contact lenses used for measurement were 5 types that were distributed in Korea, and 4 back vertex powers (-1.50D, -3.00D, -6.00D, -9.00D) were used. and repeatability and reproducibility were evaluated by measuring them with an auto-lensmeter by two examiners. Results: Measured powers by dry blotting method were ranged in mean differences from 0.03D to 0.18D for overall lenses, 0.10D to 0.18D for silicone hydrogel lenses, 0.03D to 0.08D for hydrogel lenses. The mean differences between two examiners were less than 0.10D, and the inter-examiner reproducibility was good for dry blotting method. The mean difference between powers determined by wet cell method were 0.09D to 0.69D, the mean differences between two examiners were 0.02D to 0.59D. The reliability of measurements and inter-examiner reproducibility were less than dry blotting method. Conclusions: The reliability of measurements for all materials was better in dry blotting than wet cell method, the re liability of measurements for silicone hydrogel lenses was low in both methods. In clinical practical which requires quick checking of back vertex power using an auto-lensmeter. dry blotting method is thought to be more efficient than wet cell one.

Understanding of Protein Adsorption Kinetics to Contact Lens Hydrogels (콘택트렌즈용 하이드로젤로의 단백질 흡착 반응속도 이해)

  • Protein adsorption kinetics was studied with the amount of proteins adsorbed to contact lens hydrogels over time scales. Hydroxyethylmethacrylate (HEMA) and silicone hydrogels were dipped in protein solutions (albumin or IgG) and adsorption amounts were measured over time scales. The amount of protein adsorbed to both hydrogel types increased rapidly in 10 min, and remained consistently in 90 min. Decreasing interfacial energetics was taken slowly up to an hour in spite of rapid diffusion of protein molecules. This is due to the fact that water deprivation from three dimensional interphase initially formed by protein diffusion took over an hour. Interpretation of adsorption kinetics on contact lens hydrogels was discussed with understanding of relationship between surface energy and protein adsorption capacity.

Fabrication and Processing Method of Ophthalmic Hydrogel Tinted Lens Containing Indium Tin Oxide-Composited Materials

  • In this study, a multifunctional ophthalmic lens material with an electromagnetic shielding effect, high oxygen permeability, and high water content is tested, and its applicability is evaluated. Metal oxide nanoparticles are applied to the ophthalmic lens material for vision correction to shield harmful electromagnetic waves; the pyridine group is used to improve the antibacterial effect; and silicone substituted with urethane and acrylate is employed to increase the oxygen permeability and water content. In addition, multifunctional tinted ophthalmic lens materials are studied using lens materials with an excellent antibacterial effect (2,6-difluoropyridine, 2-fluoro-4-pyridinecarboxylic acid) and functional (UV protection, high wettability) lens materials (2,4-dihydroxy benzophenone, 2-hydroxy-4-(methacryloyloxy)benzophenone). To solve problems such as air bubbles generated during the polymerization process for the manufacturing and turbidity of the lens surface, polymerization conditions in which the defect rate is minimized are determined. The results show that the polymerization temperature and time are most appropriate when they are $110^{\circ}C$ and 40 minutes, respectively. The optimum injection amount of the polymerization solution is 350 ms. The turbid phenomenon that appears in lens processing is improved by 10 to 95 % according to the test time and conditions.

The Change in Refractive Powers of Soft Contact Lenses Caused by the Deposition of Tear Proteins (누액 단백질 침착에 의한 소프트콘택트렌즈의 굴절력 변화)

  • Purpose: The present study was conducted to investigate whether refractive powers of soft contact lenses were induced by the deposition of tear proteins when wearing soft contact lenses. Methods: The soft contact lenses (material: etafilcon A, hilafilcon A and comfilcon A) with refractive powers of -1.00 D, -3.00 D, -5.00 D and -7.00 D were incubated in artificial tear for 1 day, 3 days, 5 days, 7 days and 14 days, respectively. After incubation, their refractive powers were measured by wet cell method with an auto-lens meter and their protein deposited on the lenses was determined by the method of Lowry. Results: Among three types of soft contact lenses, the most protein deposition was detected in ionic etafilcon A lens material and significant change of its refractive power was manifested. In other words, refractive powers of etafilcon A lenses firstly decreased after 1 day incubation in artificial tear and then gradually increased with increasing incubation period again. The observed change in refractive powers of all diopters of etafilcon A material was beyond the scope of standard error and bigger in the lens with lower optical power. On the other hand, non-ionic hilafilcon A showed less protein deposition as much as about 20% in etafilacon A and statistically significant increase of refractive powers with increasing incubation period in artificial tear. The change in refractive power of hilafilcon A was also beyond the scope of the standard of error when incubating in artificial tear and greater in the lens with lower diopter. The least protein deposit was shown in silicone hydrogel lens material, comfilcon A as approximately 10% of it in etafilcon A, indicating less change in refractive power within the standard range of error. Conclusions: The large change of refractive powers that was beyond the scope of standard error by the deposition of tear proteins on soft contact lenses was differently detected depending on lens materials in the current study. Thus, the deposition of tear proteins induced by longer period of lens wearing may be one of the causes that induces blurred vision, suggesting that soft contact lens wearers with the amount of tear proteins may need to choose proper lens material.

Oral tissue response to soft tissue expanders prior to bone augmentation: in vitro analysis and histological study in dogs

  • Purpose: To determine whether the swelling and mechanical properties of osmotic self-inflating expanders allow or not the induction of intraoral soft tissue expansion in dogs. Methods: Three different volumes (0.15, 0.25, and 0.42 mL; referred to respectively as the S, M, and L groups) of soft tissue expanders (STEs) consisting of a hydrogel core coated with a silicone-perforated membrane were investigated in vitro to assess their swelling behavior (volume swelling ratio) and mechanical properties (tensile strength, tensile strain). For in vivo investigations, the STEs were subperiosteally inserted for 4 weeks in dogs (n=5). Soft tissue expansion was clinically monitored. Histological analyses included the examination of alveolar bone underneath the expanders and thickness measurements of the surrounding fibrous capsule. Results: The volume swelling ratio of all STEs did not exceed 5.2. In tensile mode, the highest mean strain was registered for the L group ($98.03{\pm}0.3g/cm$), whereas the lowest mean value was obtained in the S group ($81.3{\pm}0.1g/cm$), which was a statistically significant difference (P<0.05). In addition, the S and L groups were significantly different in terms of tensile strength ($1.5{\pm}0.1g/cm$ for the S group and $2.2{\pm}0.1g/cm$ for the L group, P<0.05). Clinical monitoring showed successful dilatation of the soft tissues without signs of inflammation up to 28 days. The STEs remained volumetrically stable, with a mean diameter in vivo of 6.98 mm, close to the in vitro post-expansion findings (6.69 mm). Significant histological effects included highly vascularized collagen-rich fibrous encapsulation of the STEs, with a mean thickness of $0.67{\pm}0.12mm$. The bone reaction consisted of resorption underneath the STEs, while apposition was observed at their edges. Conclusions: The swelling and mechanical properties of the STEs enabled clinically successful soft tissue expansion. A tissue reaction consisting of fibrous capsule formation and bone loss were the main histological events.

Amoebicidal Effect of Nephrite-containing Contact Lens Storage Case (연옥이 첨가된 콘택트렌즈 보관용기의 항아메바 효과)

  • Purpose: To compare the amoebicidal effects of nephrite containing contact lens (CL) storage cases with conventional CL storage cases. Methods: Acanthamoeba lugdunensis were inoculated onto 5% nephrite containing CL storage cases as well as conventional CL storage cases both with and without silicone hydrogel contact lenses (SHCLs). Then the amount of Acanthamoeba proliferation on CL storage cases and the number of adherent Acanthamoeba on SHCLs were determined and compared. The effects of multipurpose solution (MPS) with and without 1% or 5% nephrite solution on Acanthamoeba adhesion were analyzed. Results: Nephrite containing CL storage cases showed more inhibitory effects on Acanthamoeba proliferation (p = 0.02) and significantly reduced the number of adherent Acanthamoeba on SHCLs compared with conventional CL storage cases, regardless of SHCLs generation (p = 0.001, p = 0.001 and p < 0.001, respectively). The number of adherent Acanthamoeba on the first generation of SHCLs was significantly reduced by MPS with 1% and 5% nephrite solutions (p = 0.03 and p = 0.004, respectively), but the numbers for the second and third generation SHCLs were not. Conclusions: Nephrite could be used as a new additive component for CL storage cases and multipurpose solutions to improve the disinfection effects on Acanthamoeba.

Contact Lens Prescribing Pattern in Korean During 2010 to 2013 (대한민국의 최근 4년간(2010-2013) 콘택트렌즈 처방 추세)

  • Purpose: To understand the pattern of prescribing contact lenses in Korea from 2010 to 2013. Methods: Four hundred survey questionnaires were randomly posted to Korean Optometric clinics for 4 years. The questionnaire was the same form as a form used for International Contact Lens Prescribing Trend. It was also designed to collect information about the contact lenses prescribed to the first 10 patients after its receipt. Results: 1937 fits were used for the analysis among the four hundred survey questionnaires. The average patient age was $26.2{\pm}7.0$ in 2010, $26.1{\pm}7.6$ in 2011, $24.7{\pm}6.6$ in 2012 and $24.8{\pm}7.2$ in 2013. The patients were mainly women (75%). Of all the soft contact lenses, silicon hydrogel lenses were 18% of fits (17% in 2010, 10% in 2011, 20% in 2012, 24% in 2013). By degine, the percentage of toric design lenses accounted for 17% of soft lenses fits and had increased during 4 years (10% in 2010, 18% in 2011, 16% in 2012, 22% in 2013), and cosmetic contact lens made up for 29% (15% in 2010, 20% in 2011, 40% in 2012, 41% in 2013). Conclusions: The contact lenses wearers were mostly women and the average patient age becomes slightly younger. In relation to the increase in the mumber of younger female contact lens wearers,the percentage of cosmetic lenses prescription was much higher (29%) than the global average (7%), and it has gradually increased for last 4 years. This trend could be related to the growth of beauty market in Korea. By emphasizing on toric lens marketing in manufactures, the prescription rate has been gradually increased.
