공주, 연기지역 젖소의 Neospora caninum 항체 양성율 및 갓 태어난 송아지의 감염양상 조사 (Estimation of Neospora caninum Seroprevalence in Dairy Cattle in Gongju and Yeongi and Transmission Pattern to Newborn Calves)

  • To examine the seroprevalence of Neospora caninum (N. caninum) infections in cattle raised at farms where an abortion had previously occurred in Gongju city and Yeongi-gun of Chungnam Province, 280 head of cattle from six farms were examined by ELISA. Thirty-two pregnant cattle from four farms were examined to determine the abortion rate in N. caninum-infected pregnant cattle and the infection pattern in newborn calves. The six farms where the abortions had previously occurred demonstrated positive reactions to N. caninum. Although the mean seropositive rate of the six farms was 17.5%, the range of seropisitivity was 3.4-57.1% due to the difference in seroprevalence between farms. The general pattern of seropositivity in cows was higher than in heifers. In addition, the abortion rate of N. caninumpositive in 18 out of 32 pregnant dams raised in four farms was 16.7%. The seroprevalence of newborn calves from N. caninum-positive pregnant animals differed according to the farm but was 86.7% overall. In contrast, 14 newborn calves from the N. caninum-negative pregnant cattle showed negative responses. This suggests that N. caninum is endemic and occurs through vertical transmission from chronically-infected dams. Therefore, aborted dams should be discarded.

사람 혈청내 B형과 C형 간염바이러스에 대한 항체분포 (Distribution of Antibodies Against Hepatitis B virus and Hepatitis C virus in Human Sera)

  • Serum samples from 123 males and 123 females collected by age in 1996 were analyzed for antibodies against surface antigen of Hepatitis B virus and C22-3, C200 antigens of Hepatitis C virus. Sera from the children under the age of 10 showed 30% seropositivity to the surface antigen of Hepatitis B virus, 33.3% in $10{\sim}19$ year group, 20% in $20{\sim}29$ year group, 17.6% in $30{\sim}39$ year group, 3.3% in $40{\sim}49$ year group, 5.9% in $50{\sim}59$ year group, 8,3% in $60{\sim}69$ year group, 2.9% in $70{\sim}79$ year group, but antibody could not found in $80{\sim}86$ year group. 12 out of 123 male sera were positive, 19 out of 123 female sera were positive and overall rate of positivity of antibody against surface antigen of Hepatitis B virus was 12.6%. Serum samples from peoples under the age of 30 had not antibody against C22-3, C200 antigens of Hepatitis C virus. The positivity rate was 2.9% in $30{\sim}39$ year group. 5 out of 30 sera from $40{\sim}49$ year age group were positive, and 3 positive sera showed extremely high titer (1:524,288) but the titers of two remaining sera were 1:32, 1:8,192 respectively. 5.9% was positive in $50{\sim}59$ year group, 8.3% in $60{\sim}69$ year group, 11.8% in $70{\sim}79$ year group but all negative in $80{\sim}86$ year group 6 out of 123 male sera were positive (4.9%), 9 out of 123 female sera were positive (7.3%). Overall rate of positivity of antibody against C22-3, C200 antigen of Hepatitis C virus was 6.1 %. None out of 246 sera had both antibodies against Hepatitis B virus and Hepatitis C virus.

Relationship between Antibody-Positive Rate against Plasmodium vivax Circumsporozoite Protein and Incidence of Malaria

  • The relationship between anti-Plasmodium vivax circumsporozoite protein (CSP) antibody levels and the prevalence of malaria in epidemic areas of South Korea was evaluated. Blood samples were collected from inhabitants of Gimpo-si (city), Paju-si, and Yeoncheon-gun (county) in Gyeonggi-do (province), as well as Cheorwon-gun in Gangwon-do from November to December 2004. Microscopic examinations were used to identify malaria parasites. ELISA was used to quantitate anti-circumsporozoite protein (CSP) antibodies against P. vivax. A total of 1,774 blood samples were collected. The overall CSP-ELISA-positive rate was 7.7% (n=139). The annual parasite incidences (APIs) in these areas gradually decreased from 2004 to 2005 (1.09 and 0.80, respectively). The positive rate in Gimpo (10.4%, 44/425) was the highest identified by CSP-ELISA. The highest API was found in Yeoncheon, followed by Cheorwon, Paju, and Gimpo in both years. The positive rates of CSP-ELISA were closely related to the APIs in the study areas. These results suggest that seroepidemiological studies based on CSP may be helpful in estimating the malaria prevalence in certain areas. In addition, this assay can be used to establish and evaluate malaria control and eradication programs in affected areas.

산모의 HBsAg 보유 여부에 따른 만삭아의 B형간염 기본접종 및 무반응자에서 재접종 후 항체 양전율에 대한 연구 (Comparison of the seroconversion rate after primary hepatitis B vaccination and after revaccination of non-responders in full-term infants according to mother's HBsAg seropositivity)

  • 목 적: 현재 우리나라에서는 B형 간염 백신접종 후 일률적으로 항체검사를 권장하지는 않으나, 아직도 우리나라의 성인 및 소아의 보유율이 외국에 비해 매우 높고, 영아기의 어린이집 등 단체생활 증가와 이로 인한 수평감염의 위험성이 높다고 생각되므로 영아기의 면역획득여부를 확인하는 것이 중요하다고 생각된다. 이에 본 연구에서는 출생 후 B형 간염 백신을 접종한 만삭아들을 대상으로 항체검사를 실시하여 산모가 보유자인 경우와 그렇지 않은 경우의 두 군으로 나누어 기본접종 후 항체 양전율을 평가하였고, 무반응자에서의 재접종 시 효과를 비교 분석하였다. 방 법: 2004년 10월부터 2007년 6월까지 문화병원에서 출생한 716명의 만삭아들을 대상으로 산모의 B형 간염 보유여부에 따라 두 군으로 나누어 현재 추천되고 있는 일정으로 기본접종 후 생후 7-12개월(산모가 보유자인 경우는 생후 9-15개월)에 항체가를 측정하여 기본접종의 효과를 비교하였다. 또한 각 군의 무반응자에게 3회 재접종을 실시하고 1-3개월 후 항체가를 측정하여 재접종의 효과를 비교하였다. 또한 보유자인 산모로부터 출생한 영아에서 산모의 HBeAg 양성 여부가 주산기 예방조치의 실패와 관련이 있는지 알아보았다. 결 과: HBsAg이 음성인 산모에서 태어난 총 662명의 건강한 만삭아에서 B형 간염 기본접종 후 623명(94.1%)에서 항체 양전되었고, HBsAg이 양성인 산모에서 태어난 만삭아중 감염된 4명의 영아를 제외한 50명에서는 39명(78%)이 기본접종 후 항체 양전되어 산모가 보유자가 아닌 경우가 더 높은 항체 양전율을 나타냈다(P<0.001). 또한 건강한 만삭아의 무반응자 39명중 32명에서 재접종 후 31명(96.9%)에서 항체가 양전되었고, 산모가 보유자인 무반응자 11명 중 8명에서 재접종 후 7명(87.5%)이 항체 양전되어 두 군의 무반응자에서의 재접종은 매우 효과적이었다. 또한 두 군에서 마지막 기본접종 후 항체검사시기에 따른 항체 양전율은 거의 차이가 없었다(P>0.05). HBeAg이 음성이고 HBsAg만 양성인 산모로부터 출생한 영아 40명 모두 예방이 되었고, HBeAg과 HBsAg 모두 양성인 산모로부터 출생한 14명중에서는 4명(28.6%)이 예방조치가 실패하였다. 이와 같이 산모의 HBeAg 양성여부는 주산기 예방조치의 실패와 밀접한 관련이 있었다(P<0.001). 결 론: 보유자가 아닌 산모로부터 출생한 건강한 만삭아에서 기본접종 후 무반응자의 재접종의 효과는 매우 좋았기 때문에, 가족 내에 보유자가 있거나 보유자가 없다 하더라도 확실한 효과를 위해서 기본접종 후 항체검사 및 재접종을 고려해야 할 것으로 생각되나, 이를 위해서는 현재 시행되지 않고 있는 일률적인 항체검사의 비용효과적인 측면에 대한 연구가 뒷받침되어야 할 것으로 사료되며, 산모가 보유자인 경우에는 항체 양전율이 감소하므로 수직감염이 되지 않았다 하더라도 산모로부터 수평감염의 위험이 높으므로 현재 추천되는 방법으로 반드시 항체검사를 실시하여 항체 양전여부를 확인해야 할 것이다.

국내 서식 한우에서 큐열 항체 양성율 조사 (Seroprevalence of Q-fever in Korean native cattle)

  • Q-fever is a vector-borne (Coxiella [C.] burnetii) zoonotic disease that is an increasing public health concern. To date, some research about Q-fever prevalence in dairy herds and human patients has been reported in Korea, but information about Korean native cattle is scarce. To measure the prevalence rates of C. burnetii in Korean native cattle, a total of 1,095 bovine serum samples collected during 2010~2013 were analyzed with an enzyme-linked immunosorbent assay. Sixty-eight heads of cattle were diagnosed as positive and while 19 heads were suspected (positive rate = 6.2%). Interestingly, Jeju province had a seropositivity rate six times greater than that of other provinces (18.9% vs. 3.2%). High seroprevalence might be caused by wide distribution of ticks in Jeju province compared to other regions. Based on these data, extensive monitoring of C. burnetii infection in cattle, tick distribution, and climate changes is required.

1998년 한국인 성인에서 혈청 HBsAg 양성률 추정을 위한 조사연구 (A Study on the Seropositivity of HBsAg among Biennial Health Examinees ; A Nation-wide Multicenter Survey)

  • 목적 : 1998년 한국인 성인에서의 혈청 HBsAg의 양성률을 추정하는 것을 일차적 목적으로 하며, HBsAg 양성률의 연령별로 분포, 지역적 차이, 과거 간질환력, 만성간질환 가족력 및 예방접종과의 관련성을 파악하며 또한 양성자를 6개월간 추적후 재검사 하여 B형 간염의 만성보균율을 파악하고자 하는 것을 이차적 목적으로 하였다. 방법 : 1998년 전국 10개 지역에서 의료보험관리공단의 정기건강검진 수진자를 대상으로 하여 HBsAg의 혈청유병률을 조사하였다. HBsAg 양성인 사람들을 대상으로 6개월 이상 추적조사하여 B형간염 만성보균율을 파악하였다. 총 1,816명에 대한 혈청과 설문서가 수집되었다. HBsAg는 RIA로 측정하였다. 결과 : HBsAg의 혈청유병률은 5.5%(95% CI-4.5-6.6)였으며 남자에서 7.4%(95% CI=5.8-9.4), 여자에서 3.6%(95% CI=2.5-5.0)로 나타났다. 급성간질환 과거력과 만성간질환 가족력을 HBsAg 혈청검사결과와 비교한 결과 유의한 관련성이 있는 것으로 나타났다. HBsAg 양성자에서 6개월 후에 음성으로 전환한 사람은 3.2%(95% CI=0.1-16.7)였으며 따라서 B형간염의 만성보균율은 5.3%(95% CI=3.7-6.6)로 추정되었다. 결론 : 본 연구결과에서는 HBsAg의 양성률이 1980년대의 연구결과들에 비하여 비교적 낮게 추정되었으며 이는 특히 여성과 젊은 연령층에서 두드러지게 나타났다. 그러나 우리나라에서의 간암 및 만성간질환의 공중보건학적 중요성을 고려하면 지속적인 간염발생의 예방대책이 필요하다고 할 수 있다.

Seroepidemiological study of Toxoplasma gondii infection in the rural area Okcheon-gun, Korea

  • There have been some reports about the prevalence of anti-Toxoplasma gondii antibody among Koreans, and most of all data were taken from patients visiting hospitals. However, the epidemiological data of the community-based study in Korea are rare. This study was performed to evaluate the seroprevalence of toxoplasmosis among the inhabitants of the rural area Okcheon-gun, Korea. A total of 1,109 serum samples (499 males, 610 females) were examined for the IgG antibodies by ELISA. To set up the cut-off point for ELISA, we used a commercial latex agglutination (LA) kit. The sensitivity and specificity of ELISA against LA test were 89.5%, and 98.6% respectively. Among 1,109 sera, 6.9% showed seropositivity by ELISA. The positive rates of males and females were 6.0% and 7.2%, respectively. However, there were no significant differences between sexes. Comparing the age groups, the highest seropositive rate showed in the seventies or higher, and their rates had a tendency to increase with age (0.05 < p < 0.3). These results revealed that the seroprevalence of toxoplasmosis in rural inhabitants is similar to previous reports in Korea; however we need further investigation to clarify the prevalence of toxoplasmosis in the general population.

제주도 고등학교 학생의 톡소포자충 항체 양성률 및 감염 위험요인 (Seroprevalence and Risk Factors of Toxoplasma gondii Infection in High School Students in Cheju Province)

  • Objectives : To assess the seroprevalence and risk factors of toxoplasmosis in high school students in Cheju Province, Korea. Methods : A total of 4,570 high school students from 18 schools in Cheju Province were investigated for Toxoplasma gondii antibodies(IgG) by enzyme linked immunosorbent assay (ELISA). Risk factors for toxoplasmosis, such as place of residence, type of house, contact with cats and other pets, and rare meat consumption, were examined by questionnaire. Results . The overall antibody positive rate was 5.5% and ranged from 2.6 to 11.5% by school. There was no significant difference between males and females. Statistical analyses of the questionnaire data indicated that the risk factors for seropositivity were: (1) birth place (Cheju/others), (2) place of residence (rural/urban), (3) dietary habits (vegetarian/non vegetarian), (4) eating rare meat, (5) exposure to pets and (6) hepatitis B. Conclusion : We confirmed that the prevalence of the anti-Toxoplasma gondii antibody in a population of high school students in Cheju Province was to the previously reported prevalence.

현미경적 다발혈관염을 동반한 폐섬유증 1예 (A Case of Pulmonary Fibrosis with Microscopic Polyangiitis)

  • A 65-year-old woman was admitted due to poor oral intake and a dry cough over the previous 3 months. The physical examination was remarkable for bibasilar crackles, and plain chest radiography showed reticulation in both lower lung fields. A pulmonary function test demonstrated a restrictive pattern with a reduced diffusing capacity of the lung for carbon monoxide. High resolution computed tomography showed reticulation and honey-combing in both peripheral lung zones, which was consistent with usual interstitial pneumonia pattern. Her skin showed livedo reticularis. The erythrocyte sedimentation rate and C-reactive protein level were elevated, and hematuria was noted on urinary analysis. A serologic test for auto-antibodies showed seropositivity for Myeloperoxidase-Anti-neutrophil cytoplasmic antibody (MPO-ANCA). A kidney biopsy was performed and showed focal segmental glomerulosclerosis. She was diagnosed as having pulmonary fibrosis with microscopic polyangiitis (MPA) and treated with high dose steroids. Here we report a case of pulmonary fibrosis coexistent with microscopic polyangiitis.

Serological Detection of Antibodies against Anaplasma spp. in Cattle Reared in the Gyeongsangbuk-do, Korea

  • Anaplasmosis is a tick-borne, non-contagious, zoonotic disease caused by Anaplasma spp., which include Anaplasma marginale, A. centrale, A. phagocytophilum, A. platys, A. ovis, and A. bovis. Recently, in Korea, the prevalence of Anaplasma spp. has been investigated in some animals, such as dogs, horses, goats, cats, and Korean water deer. In cattle, A. marginale is the most virulent species and regarded as the typical type of species. However, data on the seroprevalence of Anaplasma spp. in cattle in Korea during the last decade is limited. This study was designed to investigate the seroprevalence of bovine anaplasmosis in Korea. From 2010 to 2013, blood samples were collected from 568 cattle. Forty animals (7.0%) tested seropositive for Anaplasma spp. by cELISA. Despite that current bovine anaplasmosis seropositivity rate in the Gyeongsangbuk-do is lower than those in tropical countries, anaplasmosis needs to be regarded as a concerning disease. The identification of the specific Anaplasma species infecting cattle in this province requires additional molecular studies. Moreover, further monitoring and control programs for bovine anaplasmosis is required, and the information from this study will be beneficial to develop these programs.
