Status of Infection Control in Jeju-area General Hospitals (제주특별자치도 종합병원의 감염관리 현황)

  • The aim of this study was to examine and analyze the infection management status of Jeju-area general hospitals, and in order to convey the importance of infection management, and to determine role plans of medical technologists as infection management staff, the infection management status was examined through surveys targeting 7 general hospitals located in Jeju Special Self-Governing Province. The infection management of Jeju-area medical institutions showed excellence in that all institutions operated an infection management room, there was dedicated manpower, and hospital infection management guidelines were established and executed. However most institutions were operating their infection management room with only 1 nurse, reporting many difficulties in securing dedicated manpower, microbe-related culture deciphering, frequency of multiple-resistance bacteria, infection statistics, and details on microbe testing. Accordingly, it is believed that the medical technologist who can perform the practical tasks of infection management has sufficient qualification and experience in infection management as per the medical law enforcement regulations, and in operation of an infection management room. If medical technologists (infection control microbiological technologist) with expert knowledge on microbes and infection control nurses can execute the tasks as dedicated personnel, the operation of the infection management room will be more advanced. In addition, for proper infection management in the future, the introduction of a medical system specialized in infection management and full support for infection management of vulnerable small/medium hospitals in addition to general hospitals across the country is considered important.

Methods and Evaluation of Korean Population Census (인구총조사의 방법과 평가)

  • It is generally known that the quality of Korean population census results is very high. The completeness of the census has been over 97% since 1960. The age errors are very low and they belong to the lowest group in the world. However, Korean census has several problems to be solved, which are suggested as the followings: 1) Since most Koreans are basically educated, we will be able to adopt the self-reporting survey method for the next census instead of the interview method. Then, we can save a lot of time and cost in a census. 2) We have to simplify, the items of age since the effects of the four age items (Korean ago, zodiac year at birth, birth date in solar or lunar calendar) are not large enough to compare to the investments of manpower and costs. 3) It should be helpful to improve the quality of census data if the post enumeration survey is conducted more successfully and the results evaluated for more items not only age are published. 4) For the reduction of the error rates for the age groups of infants, young children and the young labours who are moving frequently, we need some special items for the compliteness of the population census by age group. 5) We have to prepare and release the 1995 2% sample data file to the public users for the improvement of the usefuleness in time. Furtheremore, it is also very important for the research in the field of social sciences that the back series of 2% sample files from 1985 census data are reconstructed.

The Efficiency on the Oral Health Training of Elderly Adults at Welfare Institutions (노인복지센터 이용노인의 구강보건교육 효율성)

  • The purpose of this survey was to evaluate the efficiency on the oral health training of elderly adults who have experienced the oral health training program at welfare institutions in An-yang city, Korea. A total of 451 adults (aged 65 or more) were selected and surveyed by the self-reporting questionnaire that consist of 4 items and socio-economic general characteristics (gender, age). Oral health indices were produced by using the SPSS. The percentage of perceived oral health status that felt poor or very poor was decreased about 8.5% than before and the frequency of perceived oral symptom was decreased too and the practice rate and satisfaction with oral health training was significantly increase than before. Oral health promotion of the elderly adults at welfare institutions in An-yang city should focused on the constant attention the means of the preventive oral health training program.

  • Background: The Bronchiectasis Health Questionnaire (BHQ) is a simple and repeatable, self-reporting health status questionnaire for bronchiectasis. We have translated the original version of the BHQ into Korean using a standardized methodology. The purpose of this study was to assess the validity of the Korean version of the BHQ (K-BHQ) with Korean patients. Methods: Stable state patients with bronchiectasis from two academic hospitals were enrolled in this study. The validity was assessed by investigating the relationship between the K-BHQ scores and the Korean version of the Chronic Obstructive Pulmonary Disease Assessment Test (K-CAT) scores. We also investigated the relationship between the K-BHQ scores and other variables of the modified Medical Research Council's (mMRC) dyspnea scale, lung function, and exacerbations. Results: A total of 126 patients with bronchiectasis were enrolled. The mean age was 64.3 (standard deviation [SD], 9.7). Women comprised 53.2% of the patients. The mean forced expiratory volume in one second (FEV1) was 60% of the predicted value (SD, 18.9%); the mean K-CAT score was 17.6 (SD, 9.1). The K-BHQ scores correlated strongly with the K-CAT scores (r=-0.656, p<0.001). There was significant correlation between the K-BHQ scores and the mMRC dyspnea scale (ρ=-0.409, p<0.001), FEV1 (r=0.406, p<0.001), and number of exacerbations requiring hospitalization (ρ=-0.303, p=0.001). Conclusion: The K-BHQ is valid for assessing the health-related quality of life or health status of Korean bronchiectasis patients.

A Study on the Correlation of Hormonal Status with Depression - Anxiety Traits in Menopausal Women (폐경기 여성에서 호르몬 상태와 우울 및 불안 성향간의 상관관계 연구)

  • Objects : There has been the controversy that menopause may or may not influence the psychological distresses. The purpose of this study was to investigate the correlation between the hormonal status and traits of depression & anxiety in menopausal women. Method : Among the women attending menopausal clinic. menopausal women, defined as who having a amenorrhea for more than 12 months, were selected as a study group(n=83). The control group(n=73), who visited to screen the cervix cancer with regular menstruation, hod no history of hormone replacement therapy. Individual data were collected by self-reporting questionnaires. Depression state was evaluated by Beck Depression Inventory(BDI) and anxiety state by Stale Trail Anxiety Inventory(STAI), and the female hormones such as E2, FSH, and LH were obtained by blood sampling at visting clinic. Statistically the data were processed by t-test and pearson correlation analysis(p<0.05). Results : 1) The mean age of menopausal was 45.3 years. 2) In menopausal women the scores of BDI & STAI were significantly higher than in control group(p<0.05). 3) There were significant differences between menopausal and control group by E2 & FSH. 4) There was no correlation between female hormonal status and the STAI & BDI scores in both groups. Conclusion : Although menopausal women had more traits of depression & anxiety than control group, it was difficult to insist that the lack of estrogen was the only etiological factor for the traits of depression & anxiety in menopausal period. In further studies, we must consider another factors, including biological, sociocultural, psychological factor, as the cause of psychological symptoms during menopausal period.

Penile Blood Change after Oral Medication of Korean Red Ginseng in Erectile Dysfunction Patients (발기부전환자에서 홍삼 복용후의 음경혈류와 발기력 변화)

  • The clinical efficacies of Korean Red Ginseng for improving sexual function has been reported only by self-reporting questionnaires.AVS-Penogram is a method evaluating natural penile blood flow changes by Gamma-camera after intravenous injection of Techmetium 99$\^$m/-RBC and also stimulating by erotic video tape. To more objectively evaluate the efficacies of Korean Red Ginseng for erectile function, we applied AVS-Penogram as a objective diagnostic tool. AVS-Penogram studies were done in 28 patients with erectile dysfunction after administration of Red Ginseng(19) or placebo(9) group. The pattern of AVS-Penogram before medication were normal type in 4, impossible in 2, unstable in 20, delayed in 1, and mixed in 1. The unstable type were the most prevalent observed in 71.4% of all patients. The patterns of AVS-penogram were 2.2. 13.1 in Red Ginseng group, while in placebo group normal 2, unstable 7. After 4 weeks in Red Ginseng group, there were 3 cases of improved penogram findings, while only 1 in placebo group. There were no statistical significancies in the type of penogram changes on both group. But the maximum radioactivity were improved in 11 patients(57.9%) in Red Ginseng group, while 2(22.2%) in placebo group. There were statistical significancies in comparing the maximal radioactivity of penogram on both group. These AVS-Penogram findings shows, encouraging results of by increased penile blood flow Korean Red Ginseng our medication. Further study is more needed to reconfirm these results by more concentrated active formulations of sapanonin St other active ginsenosides. These promising results will lead to develop a new remedy for erectile dysfunction by Korean Red Ginseng

Impact of 12-hour Shifts on Job Satisfaction, Quality of Life, Hospital Incident Reporting, and Overtime Hours in a Pediatric Intensive Care Unit (일 소아 중환자실에서의 12시간 교대근무가 간호사의 직무만족도, 삶의 질, 안전사건 보고 및 시간 외 근무에 미치는 영향)

  • Purpose: The aim of this study was to compare job satisfaction, quality of life (QOL), incident report rate and overtime hours for 12-hour shifts and for 8-hour shifts in a pediatric intensive care unit (PICU). Methods: A descriptive survey was conducted with a convenience sample of 36 staff nurses from a PICU in a regional hospital in Korea. Data were collected using self-administrated questionnaires regarding job satisfaction and QOL at 6 months before and after the beginning of 12-hour shifts. Incident report rate and overtime hours for both 12-hour and 8-hour shifts were compared. Comparisons were made using $x^2$-test, paired t-test and Mann-Whitney U test. Results: After 12-hour shifts were initiated, job satisfaction significantly increased (t=3.93, p<.001) and QOL was higher for nurses on 12-hour shifts compared to 8-hour (t=7.83, p<.001). There was no statistically significant change in incident report rate ($x^2=0.15$, p=.720). The overtimes decreased from $36.3{\pm}34.7$ to $17.3{\pm}34.9$ minutes (Z=-8.91, p<.001). Conclusion: These results provide evidence that 12-hour shifts can be an effective ways of scheduling for staff nurses to increase job satisfaction and quality of life without increasing patient safety incidents or prolonged overtime work hours.

Effects of Total Sleep Deprivation on Visual Discrimination (전수면박탈이 시각변별력에 미치는 영향)

  • Objectives: The purpose of this study is to investigate the effects of the 37-hour sleep deprivation on visual discrimination using the Cybernetic Flicker Fusion Analyzer (FLIMK) and to see the correlation between this finding and various subjective scale scores. Methods: Twenty six subjects (20 men 6 women, $24.50{\pm}1.45$ years of age) participated in this study. Subjects remained awake for 37 hours under continuous surveillance. In the morning and evening of two study days, the FLIMK and the self-reporting scale of sleepiness, fatigue, anxiety and mood states were instituted. Results: In FLIMK, the flicker fusion threshold was significantly decreased after sleep deprivation (F=7.66, p=0.01). The number of trials (responses) before reaching fusion frequency threshold was significantly increased after sleep deprivation (F=13.16, p=0.001). The reduction of fusion frequency was correlated with the increase of sleepiness and fatigue (p<0.05), and the number of the trials was correlated with the scores of negative mood, anxiety, and fatigue (p<0.05). Conclusions: These results suggest that 37 hour total sleep deprivation cause the decrement in visual discrimination. The decrease of the fusion frequency after sleep deprivation was correlated with sleepiness and fatigue. Further study is needed to enlighten the biological mechanism of the decrement in visual function after sleep deprivation.

Convergence study on Relationship among Verbal violence experience, Verbal violence impact and Burnout in Operating Room Nurses (수술실 간호사의 언어폭력경험, 언어폭력충격, 및 소진간의 상관성에 대한 융합연구)

  • The purpose of this study was to evaluate the relationship among verbal violence experience, verbal violence impact and burnout of operating room nurses. The data were collected by structured self-reporting questionnaires from 202 operating room nurses and were analyzed with descriptive statistics, t-test, ANOVA, Scheffe test, Pearson's correlation coefficients and multiple regression. The level of verbal violence experience and verbal violence impact was 1.96 and 1.67. The level of burnout was 3.08. Verbal violence impact and burnout have a significant positive association with verbal violence experience(r=.39, p<.001; r=.41, p<.001). Verbal violence impact was positively associated with burnout(r=.29, p<.001). Factors influencing burnout were verbal violence experience, verbal violence impact and position(staff nurse) which explained 30% of the variance(F=9.15, p<.001). These findings indicate that verbal violence experiences of operating room nurses have influence on stability and productivity in personal, social aspect and suggest developing the verbal violence prevention program in hospital.

A Qualitative Content Analysis of Journals for Cadaver Practice Experiences in Nursing Students (간호대학생의 카데바 실습일지에 대한 내용분석)

  • The purpose of this study was to identify the experiences of nursing students who used journals for cadaver practice experience. The data were collected between October 26 and October 28 2016. Participants were 20 2-year nursing students with cadaver practice experience at D College in D City. The data were collected from daily sheets by self-reporting their experiences during the cadaver practice program. The participants wrote their experiences anonymously on sheets on paper. Data were categorized and coded by using content analysis. Eight categories and 21 statements were drawn from three domains. The study revealed that participants experienced anxiety, flow, and reflection of cadaver practice. The analyzed domains were 'anxiety', 'flow', and 'reflection'. 'Anxiety' was categorized into 'worry', 'fear', and 'surprise'. 'Flow' was categorized into 'interesting', 'focusing', and 'knowing'. 'Reflection' was categorized into 'dignity' and 'appreciation'. Therefore, It is necessary to provide a group counseling program for seniors who have experienced counseling and cadaver practice for psychological support and anxiety relief of nursing students before cadaver practice. In addition, this study is expected to contribute to the development of a cadaver practice program for nursing college students.
