Evaluation of the Effectiveness and the Level of Self-Management Support on the Metabolic Syndrome Management Program at Public Health Centers in Seoul Metropolitan City (서울시 보건소 대사증후군관리사업의 효과와 자가관리 지원수준 평가)

  • Background: The purpose of this study was to evaluate the effectiveness and the level of self-management support on the Metabolic Syndrome Management Program at public health centers in Seoul metropolitan city. Methods: The effectiveness on the Metabolic Syndrome Management Program were analyzed using secondary data from 1,312 community residents who were receiving program. The level of self-management support on Metabolic Syndrome Management Program was evaluated using an 'Assessment of Primary Care Resources and Supports for Chronic Disease Self-Management' from four public health centers. Results: The effectiveness on the Metabolic Syndrome Management Program was showed that decreased smoking (p= 0.044) and drinking (p< 0.001), and increased healthy dietary habit (p< 0.001) in health behaviors. It was showed that decreased triglyceride (p= 0.002) and increased high-density lipoprotein cholesterol (p< 0.001) in clinical indicators. The level of self-management support on Metabolic Syndrome Management Program was 98.1 points and it meaned that implementation is done in an organized and consistent manner using a team approach. There was difference in the level of self-management support by public health centers (p= 0.003). Conclusion: The Metabolic Syndrome Management Program in public health centers was effective, and level of self-management support was done as organizational level, but patient input and mental health were insufficient.

Effects of Menstrual Self-management Education Program on Knowledge and Behavior of Menstrual Self-management in High School Girls (여자고등학생의 월경기 자가관리 교육 프로그램의 효과)

  • Purpose: To examine the effect of menstrual self-management education program on knowledge and behavior of menstrual self-management in high school girls. Methods: With a randomized controlled trial, 100 high school girl students were randomly assigned into experimental group and control group. Menstruation self-management program was consisted of a total of four sessions of education program (1 hour of education per session). Control group only received the first class of education. Measurement tools were 20-item menstrual knowledge scale and 12-item women's genito-urinary hygiene scale. Pretest and post-test questionnaires were administered. Results: There were significant differences in menstrual knowledge as well as menstrual self-management behavior between the two groups. The experimental group showed significant improvement in both menstrual knowledge score (t=13.37, p<.001) and menstrual self-management behavior score (t=8.38, p<.001) than the control group. Conclusion: Results of this study suggest that the monthly self-management education program is effective in increasing the knowledge of menstruation of female high school students and enhancing their performance. This program needs to be implemented as a part of high school's standard sex education. Further study is needed to analyze follow-up effects of the program on self-management behavior of students after at least one menstrual cycle.

Effects of Self-management Program applying Dongsasub Training on Self-efficacy, Self-esteem, Self-management Behavior and Blood Pressure in Older Adults with Hypertension (동사섭 훈련 적용 자기관리프로그램이 고혈압 노인의 자기효능감, 자아존중감, 자기관리행위 및 혈압에 미치는 효과)

  • Purpose: The purpose of this study was to develop a self-management program applying Dongsasub training based on self-efficacy theory, and to verify the program effectiveness on self-esteem as well as self-efficacy, self-management behaviors, and blood pressure. Methods: The study design was a non-equivalent, pre-post controlled quasi-experiment study. Thirty-eight patients aged 65 and older from a senior welfare center in Seoul participated in this study (20 patients in the experimental group and 18 patients in the control group). The self-management program applying Dongsasub training consisted of eight sessions. After development was complete the program was used with the experimental group. Outcome variables included self-efficacy, self-esteem, self-management behaviors measured by questionnaires, and blood pressure measured by electronic manometer. Results: Self-efficacy (t=2.42, p=.021), self-esteem (t=2.57, p=.014) and self-management behaviors (t=2.21, p=.034) were significantly higher and systolic blood pressure (t=-2.14, p=.040) was significantly lower in the experimental group compared to the control group. However, diastolic blood pressure (t=-.85, p=.400) was not significantly different between the two groups. Conclusion: The results indicate that the self-management program applying Dongsasub training can be used as a nursing intervention in community settings for improving self-management behaviors for older adults with hypertension.

Development and Evaluation of a Joint Health Self-management Program for the Elderly with Knee Osteoarthritis in Communities: Applying the IMB Model (재가 무릎 골관절염 환자의 관절건강 자가관리 프로그램 개발 및 효과: IMB 모델 적용)

  • Purpose: The purpose of this study is to develop and evaluate a joint health self-management program for knee osteoarthritis elders in communities. Methods: This program was developed based on the IMB (information-motivationbehavioral skills) model. The program methods include education, setting and achieving goals, sharing experiences, telephone counseling, and self-monitoring. The topics of the program include joint assessment, exercise, massage, joint protection, medication, depression management, diet, and healthcare approach. The research was conducted following the principles of the nonequivalent control group pretest-posttest design. Participants were 26 subjects in the experimental group, and 27 subjects in the control group. The experimental group participated in the self-management program, and the control group received general education. Results: Both post-test scores of personal motivation, social motivation, behavior skill, self-management behavior, joint pain, joint stiffness, physical function disability, right knee extension, left knee extension, and depression were significantly different between the experimental group and the control group. Conclusion: The results of the study can be used to develop and standardize a systematic joint health self-management program. Further research is highly recommended to develop a strategy to continuously facilitate self-management of osteoarthritis patients' procedures.

Designing Intervention Arthritis Self-Management Program with Tai Chi for Older Adults with Osteoarthritis in Rural Korea

  • Osteoarthritis is one of the most common chronic degenerative disease and prevalent among women in rural area. A variety of self-management programs for arthritis patients have been developed and administered, however the effectiveness and adherence to the program including arthritis exercise are found to be low. The purpose of this study is to design intervention Arthritis Self-Management Program with Tai Chi (ASMP-TC) through identifying and analyzing attributes influencing adherence exercise behavior in Korean older adult women with arthritis in rural area. For this, the existing and relevant evidence on arthritis self-management including exercise intervention were investigated, and then this study describes well-designed arthritis self-management program to provide the credibility and validity necessary for its interventions. In addition, this study try to propose a self-management program model of Tai Chi exercise for rural older adults to improve adherence based on the primary health care facility, which is vulnerable area in Korea.

Development of a Comprehensive Self-Management Program Promoting Self Efficacy for Type 2 Diabetic Patients (제 2형 당뇨병 환자를 위한 포괄적 자기효능증진 자가관리 프로그램 개발)

  • Purpose: The purpose of this study was to develop a comprehensive self-management program promoting self efficacy for Type 2 diabetic patients. Methods: The study was a methodological research design in which previous related research was reviewed to develop the comprehensive self-management program promoting self efficacy using self efficacy theory. Results: Comprehensive self-management programs promoting self efficacy included the whole range of eight self-management domains: diet, exercise, medication, self-testing of blood glucose, complication or foot, stress, time, and general health, and consisted of four sources of self efficacy: enactive mastery experience, vicarious experience, verbal persuasion, and physiological and affective states, as strategies to promote self efficacy. Developmental methods included, in addition to large and small group education, individual education or counseling, and telephone counseling. Conclusion: Further studies are needed in community health centers or hospitals to establish the effects on self-management compliance and glycemic control of the comprehensive self-management program promoting self efficacy.

Development and Evaluation of a Program to Promote Self Management in Patients with Chronic Hepatitis B (만성 B형간염 환자를 위한 자가관리 프로그램의 개발 및 평가)

  • Purpose: The purpose of this study was to identify the effects of the program to promote self management for patients with chronic hepatitis B. Methods: The research was a quasi-experimental design using a non-equivalent control group pre-post test. The participants were 61 patients, 29 in the experimental group and 32 in the control group. A pretest and 2 posttests were conducted to measure main variables. For the experimental group, the self-management program, consisting of counseling- centered activities in small groups, was given for 6 weeks. Data were analyzed using $X^2$, t-test, and repeated measures ANOVA with PASW statistics program. Results: There were statistically significant increases in knowledge, self-efficacy, active ways of coping, and self-management compliance but not in passive ways of coping in the experimental group compared to the control group over two different times. Conclusion: The results of this study indicate that the self-management program is effective in increasing knowledge, self-efficacy, active ways of coping, and self-management compliance among patients with chronic hepatitis B. Therefore, it can be usefully utilized in the field of nursing for patients with chronic disease as a nursing intervention for people with chronic hepatitis B.

The Effects of Self-help Management Program for Post Stroke Patients at Home (재가뇌졸중 환자를 위한 자조관리 프로그램의 효과 연구)

  • Purpose: The purpose of this study was to determine the effects of 5 weeks self-help management program developed by Kim et al. (2000-b) and reinforced with music therapy for post stroke patients. Method: This was pre-experimental study and the subjects were 14 post stroke hemiplegic patients at home who were registered at the Gunsan Health Center. The program was applied for five weeks and two hours for each session composed of the preparation step, the main step and the finishing step. The contents of the program were ROM exercise, daily activity training, risk factors and aftereffects, nutrition management and stress management. To collect data, all subjects were questioned before and after the application of the program. Collected data were analyzed through frequencies, percentages, Wilcoxen signed rank test and Cronbach's alpha using SPSS-WIN program. Result: After the application of the self-help management program, a statistically significant increase was observed in the subjects' ability to perform activities of daily living (p=.039), ability to perform instrumental activities of daily living (p=.005), self-efficacy (p=.001), self-care behavior (p=.001) and quality of life (p=.001), and a statistically significant decrease was observed in depression (p=.012). Conclusion: The present self-help management program was found to be helpful in improving the subjects' physical and psychological functions after they were attacked by stroke, so to be an effective nursing intervention strategy for post stoke patients. Future researches need to reinforce and materialize music therapy and to develop and apply a self-help management program that includes not only post stroke patients but also their families. In addition, it is necessary to expand the scope of subjects and apply follow-up management in order to continue self-help meetings.

The Effect of Self-Help Management Program by Strengthen Self-Efficacy on Self-Efficacy and the Activities of Daily Living in Stroke Patients (자기효능증진 자조관리 프로그램이 뇌졸중 환자의 자기효능과 일상생활동작 수행능력에 미치는 효과)

  • Purpose: This study examined the effect of a self-help management program which strengthens self-efficacy on the self-efficacy and the activities of daily living(ADL) in stroke patients. Methods: A quasi-experimental nonequivalent control group, pre and post design was used. The subjects consisted of 47 stroke patients. Twenty-six patients were assigned to the experimental group and 21 patients were assigned to the control group. The self-help management program by strengthen self-efficacy and general hospital care were carried out on the experimental group. General hospital care was performed on the control group. The data was collected before and 5 weeks after the program. All subjects were questioned about the self-efficacy and ADL before and after the program. Results: The scores of the self-efficacy and ADL in the experimental group were higher than those in the control group. Conclusion: The self-help management program by strengthen self-efficacy is effective in increasing the self-efficacy and ADL. Therefore this program may be a useful intervention to increase the ADL in stroke patients.

Development and Application of a Self-management Program based on Prothrombin INR Monitoring for Patients with Cardiac Valve Replacement (심장판막수술 후 프로트롬빈 INR 모니터링형 자가관리프로그램 개발 및 적용)

  • Purpose: The purpose of this study was to develop and evaluate a self-management program based on INR monitoring for patients with cardiac valve replacement. Methods: This program was comprised of five weekly sessions based on Sousa's Enhance-Behavior Performance Model. The first session included individual teaching, and the other four sessions included Prothrombin Time International Normalized Ratios (PT INR) self-monitoring, telephone counseling and self-management checklist recording. Participants were patients who had cardiac valve replacement. They were randomly assigned to the experimental or control group. Sixteen in the experimental group participated in the self-management program and seventeen in the control group participated in general care. Self-management knowledge, self-efficacy, self-management behavior and PT INR were measured as dependent variables. Data were analyzed using Mann Whitney U-test, t-test and ANCOVA. Results: The experimental group showed significantly higher post-test scores in self-management knowledge (t=5.86, p<.001), self-efficacy (F=18.32, p<.001), and self-management behavior (t=3.44, p=.002) compared to the control group. Also, the experimental group showed significantly higher frequency in maintaining the treatment range of PT INR compared to the control group (${\chi}^2=4.80$, p=.028). Conclusion: The results of the research on the self-management program based on PT INR monitoring showed that it is effective in improving self-management knowledge, self-efficacy, and self-management behavior as well as maintaining treatment range of PT INR of patients with cardiac valve replacement.
