Applying Clinical Judgment Rubric for Evaluation of Simulation Practice for Nursing Students : A Non-Randomized Controlled Trial

  • The purpose of this study is to investigate the effects of debriefing using Lasater's Clinical Judgment Rubric to study nursing students' academic self-efficacy, clinical performance, and clinical judgment. The experiment group was subjected to debriefing by applying the Clinical Judgment Rubric, while general debriefing was applied to the control group. The results of the study are as follows: Clinical judgment scores were improved after debriefing for both groups, significantly higher for students in the experimental group compared to the control group. However, there was no significant difference between the two groups in academic self-efficacy or clinical performance. In conclusion, the debriefing based on the Clinical Judgment Rubric used in this study proved to be effective in improving the clinical judgment of nursing students.

정당방위 유형, 신문기사의 정당방위 인정비율, 판단자 개인 특성이 정당방위 판단에 미치는 영향 (The Effects of Self-Defense Categories, Rate of Self-Defense recognition in News Article, and the Individual Characteristics of Mock Jurors on the Self-Defense Judgment)

  • 본 연구는 일반인이 정당방위를 어떻게 판단하고 있으며 판단시 영향을 줄 수 있는 요인들은 어떤 것이 있는지 실증적으로 고찰하고자 하였다. 정당방위 판단에 영향을 주는 요인을 정당방위 유형, 정당방위에 대한 기사, 판단자 개인 특성인 폭력 허용도, 법적 태도로 나누어 만 20세 이상의 성인 남녀 총 651명을 대상으로 연구자료를 수집 및 분석하였다. 참가자들은 정당방위가 주장되는 상황을 유형화하여 작성된 세 유형 중 하나의 유형에 할당되고, 각 유형에 해당하는 정당방위 관련 기사와 시나리오를 제공받은 후 정당방위 판단을 하였다. 또한, 개인적 요소인 법에 대해 가지는 태도, 폭력 허용도가 측정된 후 정당방위 판단에 미치는 영향을 분석하였다. 연구 결과, 자신을 위한 정당방위 유형에서는 정당방위를 인정하는 비율이 가장 높았으나, 국가기관에 대항하는 정당방위 유형에서는 정당방위를 불인정하는 비율이 훨씬 높은 반대 양상을 나타내었다. 또한, 정당방위에 대한 기사 중 정당방위가 잘 인정되지 않는다는 부정적인 기사가 정당방위 판단에 영향을 미치는 것으로 나타났다. 더불어 참가자 개인의 폭력 허용도, 법적 태도가 정당방위 판단에 영향을 미칠 수 있음을 확인하였다. 본 연구를 통해 확인한 일반인들의 정당방위 판단 과정과 정당방위 판단에 영향을 줄 수 있는 제반 요소들은 실제 배심 재판에서의 편향적인 판단을 방지하기 위한 고려 요소가 될 수 있을 것이다. 마지막으로 본 연구의 의의와 한계점, 후속 연구를 제언하였다.

시뮬레이션 기반한 심근경색증 상황 교육 적용에 따른 임상판단력과 간호수행자신감 (Clinical Judgment Force and Nursing Performance Satisfaction by Application of Simulation-based Myocardial Infarction Education)

  • The purpose of this study is to investigate the relationship between clinical judgment and nursing performance based on myocardial infarction simulation applied to nursing students. Method: 39 fourth grade nursing students participated in this explanatory study with sampling of accommodations. The Lasater Clinical Judgment Rubric has been used in self-reported clinical trials and satisfaction with judging practices. Data were analyzed by descriptive statistics, t-test and Pearson's correlation coefficient using SPSS WIN 19. Results: Scenarios with myocardial infarction were scored by self-reported clinical judgment. Total mean score of clinical judgment and total mean value of clinical judgment is 4.17 ± 0.80. Correlation Between Variables After Simulation Practice is highly significant for providing positive clinical judgment and satisfaction with nursing performance. Conclusion: The simulation-based practice associated with adult nursing in myocardial infarction was useful for clinical judgment and nursing performance satisfaction, which improved the core basic skills of nursing students.

간호대학생의 시뮬레이션실습 기반 임상판단력과 흡인간호수행능력 (Simulation-based Clinical Judgment and Performance Ability for Tracheal Suction in Nursing Students)

  • Purpose: This study was conducted to explore the relationship between simulation-based clinical judgment and performance ability for tracheal suction in nursing students. Methods: With a convenience sampling, 207 nursing students participated in this descriptive study. Lasater clinical judgment rubric was used for self-reported clinical judgment in addition to observe the skill of tracheal suction using a checklist. Data were analyzed by descriptive statistics, t-test and Pearson's correlation coefficients using the SPSS/WIN 22. Results: A scenario with pneumonia patient was developed to observe the skill of tracheal suction during simulation-based practices. Then self-reported clinical judgment was scored. The mean score of total sum of clinical judgment, total mean of clinical judgment, and performance skill were $36.44{\pm}4.82$, $13.44{\pm}1.71$, and $42.32{\pm}5.05$, respectively. Statistically, students having good skills in suction showed significant differences in clinical judgment of interpreting (p=.031) compared to students having fair skills. Conclusion: The results of this study show that a structured debriefing method utilizing Lasater clinical judgment rubric is helpful. Also, simulation-based practice related to adult nursing in the respiratory system was useful for increasing the core basic skills among nursing students.

  • 본 연구의 목적은 시뮬레이션 실습교육이 간호대학생의 임상판단력, 자신감, 임상수행능력에 영향을 미치는 효과를 파악하고자 실시되었으며 단일군 사전-사후 실험연구이다. 연구 대상자는 S지역에 소재한 간호학과 2학년 시뮬레이션 실습교과목을 수강한 45명의 학생으로 자료는 설문지를 이용하여 시뮬레이션 실습교육 전 후에 수집되었다. 연구결과 임상판단력, 자신감, 임상수행능력 모두가 통계적으로 유의미하게 향상되었다. 그러므로 시뮬레이션 실습교육이 간호대학생의 임상판단력과 자신감, 임상수행능력에 미치는 긍정적인 효과를 확인하였다. 추후 임상판단력, 자신감, 임상수행능력을 향상시킬 수 있는 시뮬레이션 실습교육의 모듈 개발 및 효과를 검증할 수 있는 연구가 필요하다고 사료된다.

  • Purpose: The purpose of this study was to evaluate the effects of mechanical ventilation simulation on the clinical judgment and self-confidence of nursing students. Methods: This study was a quasi-experimental study. From one university, 118 undergraduate nursing students participated in this study. Sixty students were in the intervention group, and fifty-eight students were in the control group. A simulation scenario utilizing a high-fidelity human simulator focusing on nursing care for patients with a mechanical ventilator was developed for this study. Data were collected with a self-report survey method before the intervention, right after intervention, and two weeks later. Results: Students in the intervention group showed significantly higher increases in clinical judgment and self-confidence than those in the control group at the immediate posttest. Moreover, 2 weeks later, the increase in clinical judgment and self-confidence from the pretest among the intervention group was significantly larger than those in the control group. Conclusion: Utilizing simulation education focusing on patients with a mechanical ventilator may contribute to training more competent nurses in the area of critical care nursing. It may also serve to provide a better critical care environment for the safety and health of patients.

아동의 행복 상황 및 판단에 대한 조사 연구 : 문화적 특수성 접근 (A Study on the Situations of Happiness and Self-Judgment of Happiness Level among Children : A Culture-Specific Approach)

  • This study examined children's happy situations and their self-judgment of happiness level. Subjects were 831 children (7-13 years of age). Research findings were that : (1) There were three categories of happy situations : rewards, daily routine activities, non-daily activities/incidents. These categories were subcategorized into personal and relational experiences. Personal experiences gave them greater happiness than relational experiences. (2) The younger children felt happier through relational experiences than older children. (3) Sixty-two percent of the children reported they were happy most of the time. (4) Preschool children answered "I'm happy" more than older children. Sixth grade children answered "I'm happy" least among the entire sample. (5) Academics correlated positively with children's judgment of happiness level; free time correlated negatively with happiness.

  • Purpose: This study was conducted to identify effects of debriefing based on the clinical judgment model for nursing students on their knowledge, skill performance, clinical judgment, self-confidence and learner satisfaction during simulation based end-of-life care (ELC) education. Methods: Simulation based ELC education was developed in six steps as follows: selection of learning subjects and objects, development of learning tools, a trial run of simulation-based education, students' skill training, and evaluators' training. Forty-eight senior nursing students (25 in the experimental group and 23 in the control group) participated in the simulation-based ELC education using a high-fidelity simulator. Debriefing based on the clinical judgment was compared with the usual debriefing. Results: ANCOVA showed that there were differences in knowledge (F=4.81, p=.034), skill performance (F=68.33, p<.001), clinical judgment (F=18.33, p<.001) and self-confidence (F=4.85, p=.033), but no difference in satisfaction (t=-0.38, p=.704) between the experimental and control groups. Conclusion: This study found that debriefing based on the clinical judgement model is effective for supporting nursing students for reflecting on clinical judgment and improving their diverse competencies in complex clinical settings such as ELC.

프리셉터의 핵심역량과 지도유형에 따른 프리셉터 자기효능감 (Effects of Core Competency and Teaching Style on Preceptor Self-efficacy Among Preceptors)

  • Purpose: This study was aimed to find out the effect of core competency and teaching style on preceptor self-efficacy among preceptors. Methods: One hundred twelve nurses working at four university hospitals with previous preceptor experience participated in the survey. The data were analyzed by t-test, ANOVA, Pearson correlation coefficients and multiple regression. Results: The preceptors used 'judgment-initiative' teaching style most frequently, and reported the highest scores in the role model dimension of core competency. There were significant positive relations between age (r=.266, p=.005), clinical experience (r=.274, p=.004), preceptorship experience (r=.204, p=.032), core competency (r=.593, p<.001) and preceptor self- efficacy. But preceptor self-efficacy was not significantly different depending on the teaching style (F=0.72, p=.54). The most predictive factors of the preceptor self-efficacy were core competency and teaching style (judgment)(F=31.849, p<.001). The explained variance for preceptor self-efficacy was 35.9% in the model. Conclusion: The preceptor self-efficacy is essential for the preceptors' successful teaching experience and the clinical competency improvement of the entry level nurses. Management for an effective preceptor training program needs to focus on the improvement of core competency of preceptors, which will lead to strengthen their self-efficacy.

형사사건에서의 고의성 판단: 도덕적 특성의 역할 (Intentionality Judgement in the Criminal Case: The Role of Moral Character)

  • 형사사건에서 고의성 판단은 사건 가해자에게 적용되는 혐의의 유·무죄 판단은 물론 처벌의 크기를 판단하는 근간이 되는 사실인정의 핵심적인 영역이다. 그러나 고의성이라는 개념은 행위자의 주관적 측면, 즉 그 행위자의 심리적 측면을 반영하고 있기 때문에 제 삼자가 이를 판단하는 것이 매우 어렵다. 따라서 고의성이 어떻게 판단되어지는가에 대한 물음은 형사사법체계는 물론 학계 내에서도 매우 중요한 문제로 다뤄지고 있다. 기존의 대표적 고의성 판단 모델들 중, '심적-상태 모델'은 행위 당시 행위자의 '예견가능성(믿음)' 및 '욕망' 요인이 고의성을 판단하는 중요한 요인이라고 주장한다. 이는 한국과 영미의 형법에서 고의성 판단 요소로 '지(知)적 요소' 및 '의(意)적 요소'를 중요하게 고려하고 있는 것과 상통하는 것이다. 그러나 '도덕적-평가 모델'에서는 행위의 결과적 측면을 반영한 요인들, 즉 그러한 결과를 발생시킨 '행위자에 대한 비난가능성' 및 '결과의 나쁨 정도' 요인이 고의성 판단을 하는데 중요한 요인으로 작용한다고 주장한다. 최근에는 행위자의 행위 당시 심적-상태 요인이나 행위의 결과적 측면에 대한 도덕적-평가 요인보다 행위자의 '핵심-자기' 요인이 고의성을 판단하는 데 보다 중요한 역할을 한다고 주장하는 '핵심-자기 일치 모델'이 등장했다. 그러나 이러한 모델들은 형사사건의 중요한 특징들, 즉, 사건의 결과는 항상 부정적일 수밖에 없다는 것과 그렇기 때문에 법적 처벌의 당사자인 행위자가 행위 당시 자신의 심적 상태를 진실하게 표명하는 일은 거의 없다는 것을 고려하지 못했다는 한계가 있다. 이에 본 연구에서는 기존의 고의성 판단 연구들과 형사사건의 특징들을 종합적으로 고려했을 때, 형사사건에서는 그 행위자가 본래 어떤 사람이었는가에 대한 추론이 고의성을 판단하는 데 가장 중요한 역할을 할 것이라는, 이른바 '도덕적-특성 모델'을 제안한다. 나아가 이와 관련하여 언론 및 형사사법기관에서 유념해야할 점과 추후 연구 방향에 대해 논하였다.
