Effect of School-Based Fluoride Mouth Rinsing Program in a Socially Deprived Community

  • Purpose: The objective of this study was to evaluate the caries prevention effect of school-based fluoride mouth rinsing (FMR) program implemented in the Hanuul district of Mongolia, which has a very low socioeconomic status and extremely poor infrastructure for oral health. Materials and Methods: One hundred and seventy children aged from 6 to 8 years of the FMR school and 187 children aged from 6 to 8 years of the control school completed the baseline survey. Children from the FMR school rinsed with 0.05% sodium fluoride everyday under supervision, while those from the control school did not. Adjusted caries preventive fraction (CPF) for 2 years were calculated to evaluate the effect of the FMR program. Result: After 2 years, 288 schoolchildren remained in the study. Decayed, missing or filled permanent teeth (DMFT) and index of the FMR and the control schools at baseline were 0.11 and 0.12, respectively, and the average DMFT increment of the FMR and the control schools after 2 years were 0.35 and 0.65, respectively. The adjusted CPF of DMFT was 48.5%. Conclusion: These findings show that a school-based FMR is an effective caries preventive program in a socially deprived community with poor infrastructure for oral health.

학교구강보건사업이 초등학교 아동들의 유치 및 영구치 우식실태에 미치는 영향 (Effects of a School - Based Oral Health Care Program on the Prevalence of Dental Caries in Primary School Children)

  • Purpose: The aim of this study was to evaluate the effects of oral health care programs in 3 school-based oral health care center among primary schoolchildren. Methods: School-based oral health care programs included fluoride mouth rinsing, pit and fissure sealing for permanent premolars and molars, fluoride gel application and chewing of xylitol candy. All of the programs were carried out by one dental hygienist among 'D' primary schoolchildren in Daegu city under the supervision of a dentist. Baseline dental examinations were completed and preventive care was implemented for 544 children during one year. All of the children visited a school-based oral health care center every three months for a regular check-up. The final oral examination was conducted from March 15 to April 1, 2004. The data analysis data was made on the basis of SAS 8.01. Mean differences between 2003 and 2004 data were compared by paired t-test. Corresponding p-values were considered significant at values less than 0.05. Results: The DMF rate and DFT index were reduced to 8.0% and 8.4% during one year respectively, but there were no statistically significant differences. The DMF rate was significantly reduced (16.3%) after a one year program of school-based oral health care practice. The DMFT(Decay Missing Filling Tooth) index was also reduced compared to 2003 throughout the entire grade. Conclusion: School-based oral health care programs can reduce the prevalence of dental caries prevalence among schoolchildren during one year. This program also improved the oral health capacity of schoolchildren. It is recommend that the school-based oral health care program should be extended to every primary school in Korea.

Korean Dental Hygienists' Job Description Based on Systematic Review

  • Background: Korean dental hygienists perform various tasks under the supervision of dentists in addition to the tasks listed in the law. Many meaningful studies have been conducted to determine the actual tasks of dental hygienists, but these studies did not show common results due to the differences in research methods or designs. Hence, this study aimed to review the reported data on the tasks of dental hygienists in Korea and to clarify them based on a systematic literature review. Methods: For the literature search, the COre, Standard, and Ideal model presented by the National Library of Medicine was referenced. Seven databases were searched for literatures published in Korea, including PubMed, and Google Scholar. Of the 352 studies found using key words, titles, and abstracts, 46 were finally extracted based on the first and second exclusion criteria. After confirming the tasks of Korean dental hygienists in 46 literatures, 136 tasks were listed and calculated as appearance rate in the literature. Results: The most common tasks in 46 studies were fluoride application (67.2%), radiography (65.4%), scaling (65.4%), sealant (60.7%), patient management and counseling (56.7%), tooth-brushing education (52.2%), impression taking with alginate (50.1%), and making temporary crowns (47.9%). The most mentioned tasks of dental hygienists in public health centers were fluoride application (100%), sealant (100%), oral health education (71.4%), public oral health program evaluation (71.4%), school fluoride mouth-rinsing program (71.4%), water fluoridation (57.1%), tooth-brushing education (57.1%), school oral health programs (57.1%), and public elderly oral health programs (57.1%). Conclusion: This study showed that Korean dental hygienists had 136 tasks by reviewing 46 related studies and that the main job of Korean dental hygienists was oral disease prevention including scaling, sealant, and fluoride application.

안양시 B초등학교 학교구강보건실 운영사업이 학생구강건강에 미치는 효과 (The Effect of an Oral Health Program by B School Dental Clinic in Anyang City)

  • 본 연구는 초등학교 구강보건사업에 대한 유용성을 제시하고자 안양시에 소재한 B초등학교의 학교구강보건사업의 사업군과 대조군을 대상으로 구강건강상태 및 구강보건지식, 태도, 행동 그리고 이와 관련된 변수들의 연관성을 조사 분석한 바 다음과 같은 결론을 얻었다. 1. 조사 대상자의 일반적 특성에 따른 성별, 학년, 부모의 직업, 부모의 교육수준과 영구치우식경험도의 차이를 보면 성별과 학년에서 두 집단 간에 유의한 차이가 있었다(p < 0.05). 2. 학교구강보건실 구강보건사업의 사업군과 대조군의 구강보건 지식, 태도, 행동 수준을 비교해 본 결과 행동과 태도에서 두 집단 사이에 유의한 차이를 나타냈으며(p < 0.05), 지식에서는 유의한 차이가 없었다. 3. 학교구강보건실 구강보건사업의 사업군과 대조군의 구강건강인지도, 구강건강상태만족도, 구강치료필요도에 따른 차이를 비교해 본 결과 두 집단 사이에 유의한 차이가 없었다(p > 0.05). 4. 주관적 구강건강인지도, 건강상태만족도, 치료필요도, 구강보건지식, 태도, 행동, 영구치우식경험도간의 상호연관성에 대한 분석결과 구강보건에 대한 지식이 높을수록 구강보건에 대한 태도와 행동이 적극적이었으며(P < 0.01), 구강보건태도가 좋을수록 행동에 적극적이었다(p < 0.05). 5. 구강보건에 대한 태도가 양호할수록 영구치우식경험도는 낮게 조사되었으나 통계적으로 유의한 차이는 없었고(p > 0.05), 구강보건행동과 영구치우식경험도와는 약간의 음의 연관성이 있었다(p < 0.05). 6. 주관적 구강건강인지도와 구강건강상태만족도는 강한 양의 상관관계가 있었고, 영구치우식경험도가 높을수록 구강건강인지도가 낮아지는 것으로 나타났다(p < 0.05). 또한 영구치우식경험도가 높을수록 구강병치료가 필요하다고 느끼는 것으로 나타났으며, 구강보건지식이 높고 태도가 좋을수록 구강치료가 필요없다고 느끼는 것으로 나타났다(p < 0.05).

대구광역시 일부초등학교 구강보건실의 계속구강건강관리 평가 (The Appraisal for Effectiveness of School-based Oral Health Program in Daegu, Korea)

  • 본 연구에서는 학교구강보건실 운영이 초등학교 아동들의 치아우식증 예방에 기여하는 효과를 평가할 목적으로 대구광역시 남구보건소에서 보건복지부의 지원으로 학교구강보건실이 설치된 초등학교를 관리군으로 선정한 후 구강검진 자료를 이용하여 우식경험영구치면지수, 열구전색영구치수 및 열구전색 영구치 보유자율 등의 여러 가지 구강건강지표를 산출하였다. 또한 사업효과 평가를 위해 대조군으로 선정된 인근의 초등학교의 구강건강실태조사 자료와 비교하여 다음과 같은 결론을 얻었다. 1. 조사대상 아동의 우식경험영구치면지수는 관리군 초등학교 1-6학년 아동이 0.07면, 0.39면, 0.55면, 0.67면, 0.81면 및 1.79면이었고, 대조군 초등학교 1-6학년 아동이 0.31면, 0.86면, 1.07면, 1.04면, 2.71면 및 3.08면으로 고학년인 5, 6학년에서 유의하게 낮았다(p<0.05). 2. 조사대상 아동의 열구전색영구치수는 관리군 초등학교 1-6학년 아동이 0.44개, 3.34개, 3.35개, 3.80개, 4.31개 및 4.97개였고, 대조군 초등학교 1-6학년 아동들은 0.98개, 1.45개, 2.26개, 2.51개, 1.73개 및 2.68개로 관리군의 1학년을 제외한 모든 아동에서 보유하고 있는 치면열구전색영구치수는 대조군보다 유의하게 많았다(p<0.001). 3. 조사대상 아동의 열구전색영구치 보유자율은 관리군 초등학교 1-6학년 아동이 19.5%, 100.0%, 96.4%, 96.1%, 98.1% 및 97.0%였고, 대조군 초등학교 1-6학년 아동의 33.3%, 52.9%, 73.3%, 72.5%, 53.8% 및 60.0%로 1학년을 제외하고는 모든 학년에서 통계적으로 유의하게 높았다(p<0.05). 이상의 결과를 종합해 보면, 초등학교 아동들에게 학교구강보건실 운영은 치아우식증을 효과적으로 예방할 수 있으며, 효과를 최대화시키기 위해서는 장기간 지속적이며, 포괄적인 구강보건진료의 시행이 필요합니다.
