Causes of Child Mortality (1 to 4 Years of Age) From 1983 to 2012 in the Republic of Korea: National Vital Data

  • Objectives: Child mortality remains a critical problem even in developed countries due to low fertility. To plan effective interventions, investigation into the trends and causes of child mortality is necessary. Therefore, we analyzed these trends and causes of child deaths over the last 30 years in Korea. Methods: Causes of death data were obtained from a nationwide vital registration managed by the Korean Statistical Information Service. The mortality rate among all children aged between one and four years and the causes of deaths were reviewed. Data from 1983-2012 and 1993-2012 were analyzed separately because the proportion of unspecified causes of death during 1983-1992 varied substantially from that during 1993-2012. Results: The child (1-4 years) mortality rates substantially decreased during the past three decades. The trend analysis revealed that all the five major causes of death (infectious, neoplastic, neurologic, congenital, and external origins) have decreased significantly. However, the sex ratio of child mortality (boys to girls) slightly increased during the last 30 years. External causes of death remain the most frequent origin of child mortality, and the proportion of mortality due to child assault has significantly increased (from 1.02 in 1983 to 1.38 in 2012). Conclusions: In Korea, the major causes and rate of child mortality have changed and the sex ratio of child mortality has slightly increased since the early 1980s. Child mortality, especially due to preventable causes, requires public health intervention.

Pathway of stroke patients seeking medical care (뇌졸중 환자의 의료이용 경로에 관한 연구)

  • The purposes of this study were to investigate the pathway which stroke patients take in seeking medical care and to identify factors which influence that pathway. This study was conducted by a survey. The subjects were 130 adults, who were diagnosed with a stroke between January and April of 2000. Data was collected by means of a interview, questionnaires, and an Activity Index. The result of this study are summarized as follows: 1. The mean age of subject was 61.58 years. Forty-nine percent were men and fifty percent were women. The majority of the subjects were married. For educational level, thirty-three percent finished elementary school, and twenty-three percent, high school. Most of the subjects reported 'middle' economic status. Ninety-two percent lived with their spouse and children. The diagnosis for seventy-two percent was ischemic stroke and for twenty-two percent, hemorrhagic stroke. 2 The most frequent early sign of stroke was hemiplegia(43.3%), loss of consciousness (36.7%), dysarthria(33.3%). The type of hospital first used was a herb hospital(40.8%), or a western hospital(59.2%). The factors in their choice were ; distance from the hospital to the place that the stroke occurred(47.5%), desire for a herb hospital(15.8%), and an invitation(12.9%). The ischemic stroke patients preferred herb hospitals, but hemorrhagic stroke patients preferred a western hospitals. 3. The pattern of stroke patients seeking medical care was that forty-three percent of stroke patients pass through step 1, forty-six percent, through step 2, and 8.5% through step 3. The more steps, the higher the use of herb hospitals. 4. The factors influencing the pathway of stroke patients seeking medical care were diagnosis, and level of consciousness. Ischemic stroke patients used herb hospitals, more frequently hemorrhagic stroke patients, who used western hospitals. The alert patient preferred a herb hospital, but stupor patients preferred a western hospital. 5. The Activity Index was not related to the pathway which stroke patients used in seeking medical care.

A retrospective study of deep sedation with concomitant administration of sedative agents in children undergoing surgical removal of a mesiodens

  • Background: Pediatric dentists face challenges when young patients require a mesiodens extraction. General anesthesia may be a burden to the child as well as the parent due to dental fears and costs. The aim of this study was to evaluate oral and intravenous sedation in the outpatient setting as a safe and effective means of managing patients who require a mesiodens extraction. Methods: Records were reviewed retrospectively to find patients who underwent a mesiodens removal procedure from January 2013 to September 2014 in the Department of Pediatric Dentistry at Ajou University Hospital (Suwon, Gyeonggi-do, Republic of Korea). A total of 81 patients (62 male and 19 female) between 4 and 11 years of age (mean [${\pm}SD$] $81.6{\pm}14.1$ months) were studied, with a mean weight of $22.9{\pm}3.3kg$ (16 kg to 30 kg). Vital signs, sedation drug dosage, and sedation time were studied. Results: Mean doses of $63.7{\pm}2.5mg/kg$ chloral hydrate and $1.36{\pm}0.22mg/kg$ hydroxyzine were used for oral sedation. Nitrous oxide/oxygen was administrated for $40.0{\pm}2.1$ min. The mean dose of midazolam administered intravenously was $0.14{\pm}0.06mg/kg$ ($2.38{\pm}0.97$ times). In all cases, the mesiodens was removed successfully. Conclusions: Intravenous sedation combined with oral sedation and nitrous oxide/oxygen inhalation can be an alternative to general anesthesia when administrated and monitored properly.

Knowledge, Cognition and Behaviour of the Parents and the State of Dental caries of Preschoolers living in Pusan Province (부산지역 유치원 아동의 우식치 실태와 부모의 구강보건지식에 관한 연구)

  • This study was performed to evaluate the importance of parents, knowledge, cognition and behaviour about dental health care on the development of dental caries in preschoolers. Subjects were comprised of 170 preschoolers collected from three preschoolers placed in Pusan and Kimhae area and study period were 6 months from January, 2000. Examination on the preschoolers' dental health and questionnarie survey on parents' knowledge, cognition and behaviour about dental health care were conducted. Collected data were analyzed by chi-square test and t-test using SAS program. Dental caries was so prevalent as the rate of 73.5%. And, the rate was different by the area(p=0.001), and increased with preschooler's age(P=0.004). The parents gain the knowledge about dental health care mainly from dentist or dental hygienist(32.9%), newspaper, magazine booklets(26.5%), TV and VTR tape(20.0%), etc, but rarely from school health education(4.7%). Caries prevalence was significantly lower in case of regular brushing than that of insufficient brushing(p=0.002) and in case of consistently informing brushing time to their children(p=0.014). This results showed that the dental health care for preschoolers is insufficient and the most important factor for caries prevention is regular brushing. And it seemed that the parent's concern and knowledge play an important role for the prevention of preschooler's dental caries. Dental health education would be focused on the parents, especially for the practice of preventive behavior by preschoolers themselves.

Food Safety Perceptions on Pesticide Contamination among Koreans -Based on the 2008 Social Survey- (한국인의 농산물 농약 오염 불안도 연구 -2008년도 사회조사를 바탕으로-)

  • Objective: This study aimed to examine the level of risk perception related to pesticide contamination of food products and associated sociodemographic factors, as well as investigate the association with environmental pollution preventive behaviors. Methods: Based on data from Social Survey conducted in 2008 among a nationally representative sample of the population 15 years or older, we estimated proportions and 95% confidence intervals of the risk perception of pesticide contamination of food products. Logistic regression analyses were used to analyze the effects of sociodemographic status on risk perception. Results: Among respondents, those who feel anxious about the risks of pesticide contamination of domestic food products accounted for 40.4%, whereas the proportion rose to 87.0% with imported food products. These numbers showed that levels of anxiety about the pesticide contamination of imported food products were ranked at the top, and those for domestic products were at the bottom of the list of all environmental pollutants examined in the survey. Significantly related factors to anxiety about pesticide contamination of food products included women, the 40-50 age group, university graduates, married, non-manual employment, having school-aged children, and owning one's own home. The population attempting to prevent environmental pollution showed higher levels of anxiety about pesticide contamination of food products than did those who were not taking such steps. Conclusion: Compared to other forms of environmental pollution, Koreans show the highest levels of anxiety about pesticide contamination of imported food products. Therefore, it is necessary to establish appropriate regulations and transparent risk communication with the public about the safety of imported food.

The Purchasing Behavior of Fashion Goods According to Life Style and Role Model of Preteen Generation (프리틴세대의 라이프스타일과 역할모델에 따른 패션상품 구매행동)

  • The purpose of this study is to investigate, analyze the purchasing behavior of fashion goods according to life style and role model of preteen generation, and provide manager or marketing planner for the reference data so that they can understand preteen generation and make proper strategy efficiently. So called, preteen market focusing on 1014 generation (from ten to fourteen years old) is highlighted. This generation created between the year 1989 to 1993 after Seoul Olympic monopolize parent's love in abundant economic environment and rise to the core of consumption subject. Products aiming at this preteen generation continuously though consumption mind was shrunk greatly due to recession. Only 2~3 years before preteen market was regarded as grey zone which doesn't belong to not only children (between six and nine years old) but also teenagers (between fifteen to eighteen years old). But in recent day their purchasing powers have increased rapidly and age group is divided on details, so that preteen market has become a niche market. Subjects were 333 persons consisting of students in the 4th~6th grade of primary school and the 1st~2nd grade of middle school in Daegu city. Measuring instruments are as follows: 5questions to differentiate preteen generation, 22 questions to measure life style, 17questions (which have six sub-factors such as purchase motive, factor of product selection, utilization of informant, purchase time, purchase place, and purchase method) to measure the purchase behavior of fashion goods measurement, and 16 questions (which have four sub-factors such as parent, entertainer & sports stars, brothers and sisters, friends) to measure model of role. Statistical data were processed by SPSS 10.0 programs. Frequencies, Factor analysis, Cluster analysis, ANOVA, Cross analysis, Multiplex regression analysis, and Duncan's multiple range test were carried out.

A Novel Mutation in the MUT Gene in an Asymptomatic Newborn with Isolated Methylmalonic Acidemia (메틸말론산혈증 신생아의 MUT 유전자에서 발견된 새로운 돌연변이)

  • Isolated methylmalonic acidemia (MMA) is a group of autosomal recessive inborn errors of metabolism caused by impaired activity of methylmalonyl-coenzyme A mutase (MCM). Mutations in the gene encoding MCM (MUT ) is the most common cause of isolated MMA. In this report, we identify an asymptomatic 15 days old female who had elevated C3-acylcarnitine (C3) in the newborn screening. Her serum homocysteine was normal and urine methylmalonic acid was increased that suggested isolated MMA. She was maintained on a low-protein diet and carnitine supplementation. At 3 months of age, she was still asymptomatic and had normal growth. We analyzed MUT gene mutations. Two heterozygote mutations in the MUT gene were identified including c.323G>A and c.1672+2T>C (IVS8(+2)T>C. Among these, c.1672+2T>C (IVS8(+2)T>C) have not been described previously.

  • This research was designed to investigate the possibility to teach function concept and graph representation of functions in explicit manner toward at elementary level. Eight class-hours instruction was given to four Grade 5(age 11) students, and dynamic geometry software GSP was partially used in the class. Results indicate that the students could conceptualize the function relation, interpret linear function graphs, recognize the meaning of their slopes, and discuss the relationships among linear graphs and real life situation. Results also indicate that GSP helped students to recognize the relation between dots and the linear graph clearly and that GSP-line graph did decisive role for children to understand the meaning of graph representation of function.

A Study on the Work and Life Conditions of Single, Young Parents : The Effect of Vocational Education and Public nanny service Support on Full-time Employment (청소년 한부모의 일과 삶의 실태와 현실 : 정규직 취업에 미치는 직업교육 경험과 아이돌봄 서비스 지원의 효과)

  • This study aimed to examine the impact of academic background on full-time employment and on other important issues concerning job security and care faced by single-parent youths. The study alse aimed to verify the effect of related support on full-time employment. The analysis targeted 262 single parents under 24 years of age who were raising their children, and the main results were as follows: First, the ratio of adulthood was high for those who were employed, and the academic achievement of full-time workers was also significantly high. However, the rate of discontinuities in school was also high among those who were employed, especially those seeking full-time employment, whose desire to stay in school was higher than that of other difficulties of single, teenage parents. Second, the vocational education experience of single, teenage parents was 32 percent overall, and the rate of those seeking full-time employment was also relatively high, reaching 44 percent. However, the results of this study did not verify the direct effect of vocational education experience on full-time employment nor the interaction effect of the academic background. Therefore, a thorough review of the effectiveness of a vocational education program for young, single parents wishing to gain employment is necessary. Third, the overall rate of those who received a public nanny service was 31 percent, while the rate of full-time workers who experienced this service was significantly higher reaching 59 percent. In the research model verification, single parents who received a public nanny service were 3.8 times more likely to get a full-time job than those who did not, and receiving a public nanny service showed a regulatory effect between academic studies and full-time employment. The higher the academic level of the parents, the higher their possibility of full-time employment.

Dietary Behavior of Infertile Women In Korea (불임여성의 식생활 특성)

  • Objective: The purpose of this study was to identify dietary factors related to infertility in Korean women through a case-control study. Methods: The case group was composed of 236 women who had been diagnosed as infertility in hospital. The control group of 181 healthy women with children were recruited from local immunization centers. Socio-economic status, medical history, dietary intakes using food frequency questionnaire and stress were surveyed by interview. Anthropometric measurements were made and the causes of infertility were identified through medical records. Fasting blood samples were taken from subgroup of the subjects. Results: The mean age of infertile and control groups was 31.1 and 32.4 years, respectively and the difference was statistically significant. The mean Body Mass Index of infertile women was not significantly different from control women, however, Waist/ Hip Ratio and Triceps Skinfolds Thickness were significantly lower in infertile women than in control women. The dietary intake status was generally satisfactory in both groups. The intakes of energy, protein, fat, carbohydrate, retinol, vitamin B2 and niacin were lower in infertile women than in control women. The infertile women also showed lower intakes of animal foods. No differences were found between two groups in serum concentrations of albumin, hemoglobin, Fe, TIBC, total cholesterol, HDL-cholesterol, LDL-cholesterol, triglyceride, C3, IgA, IL-2, however, infertile women showed higher levels of Zn and IgG. The stress score was higher in infertile women. Conclusions: From the results of this study, dietary factors and nutritional status do not seem to be directly related to infertility. However, the intertile women have lower nutrient intake and lower body fat content than control women. Further researches are needed according to the causes of infertility for long term to establish the relationship between dietary factors and infertility.
