Influence of the Nursing Practice Environment on Job Satisfaction and Turnover Intention

  • Objectives: To examine whether the nursing practice environment at the hospital-level affects the job satisfaction and turnover intention of hospital nurses. Methods: Among the 11 731 nurses who participated in the Korea Health and Medical Workers' Union's educational program, 5654 responded to our survey. Data from 3096 nurses working in 185 general inpatient wards at 60 hospitals were analyzed using multilevel logistic regression modeling. Results: Having a standardized nursing process (odds ratio [OR], 4.21; p<0.001), adequate nurse staffing (OR, 4.21; p<0.01), and good doctor-nurse relationship (OR, 4.15; p<0.01), which are hospital-level variables based on the Korean General Inpatients Unit Nursing Work Index (KGU-NWI), were significantly related to nurses' job satisfaction. However, no hospital-level variable from the KGU-NWI was significantly related to nurses' turnover intention. Conclusions: Favorable nursing practice environments are associated with job satisfaction among nurses. In particular, having a standardized nursing process, adequate nurse staffing, and good doctor-nurse relationship were found to positively influence nurses' job satisfaction. However, the nursing practice environment was not related to nurses' turnover intention.

보건교사의 소명의식이 학교조직몰입에 미치는 영향 (The Influence of Calling on School Organizational Commitment of School Health Nurses)

  • 최근 신종감염병의 등장 및 교육환경의 변화 등으로 학령기 학생들의 건강문제가 다양해지면서 보건교사의 역할은 더욱 확대되고 중요해지고 있다. 본 연구는 보건교사의 소명의식이 학교조직몰입에 미치는 영향을 파악함으로서 보건교사의 학교조직몰입을 증진시키기 위한 기초자료를 제공하기 위함이다. 연구의 대상자는 경기지역에 근무하고 있는 초등학교, 중학교, 고등학교 보건교사로서 본 연구의 목적을 이해하고 연구참여에 자발적으로 동의한 143명이다. 자료수집은 보건교사 대상 직무연수를 진행하는 한국교총 종합연수원에 연구자가 직접 방문하여 수집하였다. 수집된 자료는 SPSS/WIN 23.0 통계프로그램을 통해 백분율, 평균 및 표준편차, independent t-test, ANOVA, Pearson correlation coefficient와 simple regression analysis로 분석하였다. 분석결과 보건교사의 직무몰입에 영향을 미치는 요인은 소명의식의 하위요인 중 목적·의미(β=.28, p=.011)가 긍정적 영향을 미치는 것으로 나타났으며, 학급수(β=-.20, p=.006)의 경우 부정적 영향을 미치는 것으로 나타났다. 이 요인들의 설명력은 30.7%였다. 이에 보건교사의 학교조직몰입을 증진시키기 위해서는 목적·의미 증진시키고 학급수를 낮추는 중재방안이 마련되어야 할 것이다.

양호교사 자격연수교과과정 개발에 관한 연구 (Curriculum Development for Inservice Training of School Health Nurses)

  • School health promotion is related to reinforce on various roles of school health teachers including health instruction, health services, healthful school living. In this aspects, there would be necessory for qualified continous education proceed to activate roles of school health teachers. So the objectives of this study are first, to develop curriculum framework for inservice training of health school nurses, second, to descrive of curriculum objectives, third, to state of curriculum objectives and learning methods. There were used study methods as literature review, expert conferencing and workshop. Major results are as follows; 1. Curriculum framework for inservice training of health school nurses was constitute of major variables of health care model and direct services role of school health teacher such as health service, health instruction and healthful environment area. 2. The curriculum for health instruction were different from elementary school and middle school. 3. Time distribution for curriculum was composed of that health service is 12hours, health instruction is 96 hours and healthful environment is 12 hours. 4. The learning methods for inservice training were suggested to intensify positive attitude through the discussion and practical exercise. 5. The curriculum objectives were classified by total, area and a course of study and time summary table was suggested. In conclusion, the developed curriculum would be contribute to improve of practical ability for school health teachers. And to increase of inservice training effect, there should be operate in order to participate trainees activately and to be educate concurrently. Also, there are necessary for execute in series of the program such as standized text developing by course, evaluation index developing and instructor education etc.

재미한인간호사의 문화적응과 사회적 지지가 정신건강에 미치는 영향 (The Impact of Acculturation and Social Support on Mental Health among Korean-American Registered Nurses)

  • Purpose: This study was to examine the impact of acculturation and social support on the mental health among Korean-American registered nurses. Methods: Data were collected with a convenience sample of 203 Korean-American registered nurses living in New York state and New Jersey state from 8th May to 25th August in 2012. The structured questionnaires measured the degree of acculturation, social support, and mental health. Data analysis was conducted using t-test, ANOVA, Pearson correlation efficient, and Stepwise multiple regression. Results: Lower level of integration and higher level of marginalization were associated with the lower mental health. Social support was also positively related to mental health and had mediating effect on the relationship between acculturation and mental health. The five variables, including participants' educational background, work shift, integration, marginalization, and social support were significant to predict the level of mental health among Korean-American registered nurses, accounting for 41% of the variability. Conclusion: The results imply that higher level of social support and strong identity of both home and host culture will enable Korean-American registered nurses to maintain sound mental health.

일지역 보건교사의 지각된 직무스트레스와 우울 (Perceived Occupational Stress and Depression of School Nurses)

  • Objectives: The purpose of this study was to identify occupational stress and to determine level of depression of school nurses and to identify factors that influence depression. Methods: A cross-sectional design was employed in this study. Participants included 146 school nurses from D city. Data collection was conducted on January 17-21, 2011, using self-administered questionnaires. The SPSS/WIN 11.5 was used in performance of data analysis. Results: Significant differences in occupational stress were observed according to age, occupation and perceived mental health status, and in depression according to marital status, perceived mental health status and physical disease. Depression showed significant positive correlation with occupational stress (r=.35, p<.001). In regression analysis, occupational stress and physical health state were found to be the cause of depression in 17% of subject. Conclusion: Findings of this study allow for a comprehensive understanding of occupational stress and level of depression of school nurses in Korea. Further study using a larger random sample and various variables is needed.

양호교사의 성교육에 대한 태도 유형분석 : Q방법론적 접근 (An Analysis of School Health Nurses' Attitude Toward Sex Education: A Q-methodological Approach)

  • This study was designed to identify school health nurses' attitudes toward sex education through a Q-methodological approach. Research was done from Apr. 3, 1995 to Oct. 15, 1995. A final Q-sample was selected to 37 statements out of initial 128 statements after consultation from counselors, educators and writers related to sex education. The P -sample was consisted with 32 school health nurses in Chonbuk province. The collected data were analyzed by Quanal program on PC. The results of the study were as follows: School health nurses are categorized into 6 types. The first type, receiving type with cognition deficiency about general learning objectives of sex education were consisted with 4 subjects. The second type, valuing type with cognition deficiency about general learning objectives of sex education were consisted with 6 subjects. The third type, adopting behavior type with cognition deficiency about specific learning objectives of sex education were consisted with 5 subjects. The fourth type, receiving type with cognition deficiency about specific learning objectives of sex education were consisted with 5 subjects. The fifth type, making sense of information type with cognition deficiency about specific learning objectives of sex education were consisted with 5 subjects. The sixth type, adopting behavior type with cognition deficiency about general learning objectives of sex education were consisted with 7 subjects. As a result of this study, we may realize necessity of prepared sex educators. Sex the educators must be fully cognitive and affective toward sex education before practicing sex education.

종합병원 간호사들의 직무스트레스와 건강수준(SF-36)과의 관련성 (Relationship between Job Stress and Health Status(SF-36) Among General Hospital Nurses)

  • 본 연구는 종합병원에 근무하는 간호사를 대상으로 이들의 건강수준(SF-36)을 파악하고, 건강수준과 인구사회학적 특성, 직업관련 특성 및 직무스트레스 내용 등의 제 요인들과의 관련성을 파악하고자 실시하였다. 조사대상은 250병상 이상의 6개 종합병원에 근무하고 있는 간호사 572명으로 하였으며, 자료 수집은 2014년 5월 1일부터 5월 31일까지의 기간 동안에 구조화된 무기명 자기기입식 설문지를 이용한 설문조사에 의하였다. 연구결과, 조사대상 간호사들의 건강수준(SF-36)은 개인의 인구사회학적 특성, 건강관련행위 특성 및 직업관련 특성뿐만 아니라 직무스트레스 내용과도 유의한 관련성이 있음을 시사하고 있다. 특히 건강수준(SF-36)은 직무스트레스 내용에 있어서 업무 요구도가 낮고, 업무의 자율성, 상사 및 동료의 지지도가 높을 때 높아지는 것을 알 수 있다. 따라서 간호사의 직무스트레스를 통제할 수 있는 조직 내 시스템 개발과 분위기 조성이 우선되어져야 할 것이며, 또한, 간호사들의 건강수준(SF-36)을 증대시키기 위한 프로그램 개발이 필요하다고 사료된다.

보건교사의 응급간호 경험과 지원요구 (The Emergency Care Experience and Demand for Support of School Nurse)

  • Purpose: This study explores school nurses' emergency care experiences and their needs for systemic institutional support. Methods: Data were collected in 2016 from the interviews with five focus groups comprising thirty school nurses. Qualitative content analysis was then performed using the collected data. Results: The study found that school nurses were vulnerable to over-reaction in uncertain situations as the school's sole health service provider. The study's findings are divided into ten categories. 1) Major obstacles to overcome as the sole health service provider, 2) Assessing an uncertain situation and making appropriate decisions, 3) Providing limited first aid while maintaining control over the situation, 4) Referring or transferring a student to a hospital that creates tensions and raises cost, 5) Becoming an advocate for information disclosure and treatment, 6) Ensuring follow-up actions and proper transfer of responsibility, 7) Making preparations for future emergency, 8) Responding to conflicts arising from over-reaction as a safeguard and professional expertise, 9) Need for the development of standardized manual for school emergency care, 10) Need for practical case-based training. Conclusion: The findings of this study should contribute to the development of the programs aimed at improving school emergency care and the professional competence of school nurse.

일제강점기 ‘간호부규칙(看護婦規則)’에 관한 연구 (The 'Nurses Ordinance' of Korea under Japanese Rule)

  • The Japanese colonial authorities promulgated the 'Nurses Ordinance(Kanhoboo Kyuchick), in 1914. It was the first act that regulated nurses' licensure in Korea. The gendarme did the administrational work of the ordinance. After the Nurses Ordinance of 1914, nurses without licenses could no longer work with the name of nurse, and Korean nursing gained a more professional status. After the March 1st Movement of 1919, Japan realized that its iron rule had to be more sophisticated. The gendarme gave way to an ordinary constabulary force. The Nurses Ordinance was amended to set the nurses quality as good as that of Japanese nurses, and the nurses licensure of Korea could also be used in Japan. In 1931 the Japanese war against China began, and the Japanese imposed military rule once again. The Nurses Ordinance was amended to 'The Korea Nurses Ordinance'. After the outbreak of the Sino-Japanese War(1937) and of World War II in the Pacific(1941), the Japanese desperately needed additional manpower to re plenish the dwindling ranks of their military and labour forces. To produce more nurses, the colonial authorities amended the 'Korea Nurses Ordinance' and lowered the age and educational status of nurses to produce more numbers. Until the Japanese surrender in August 1945, Korea was under Japanese rule. Koreans had no say in the passing of these acts, and the colonial authority could make and pass any act at will.
