A Study on the Factors of the school health Teachers' Self-confidence Affecting the School Nursing Activities in Jeonbuk Province (전북지역 양호교사의 업무수행과 자신감과 그에 영향하는 요인에 관한 연구)

  • The purpose of this study is to analyze the factors affecting the school health teachers' self-confidence. This study was conducted with 87 school health teachers working in Jeonbuk province, from September to December, 1986, The results are follows; 1. Demographic characteristics of school health teachers 1) Age mean ; 34, range; 23-54 2) School Nursing experience mean; 11 years, range; 0-24 3) Clinical experience mean; 1.5 years, range: 0-13 2. Status of school health resources & nursing activities 1) Personnel resource school health teacher: pupils : 1 : 1,436 'classes=1:31 'general teacher=1:39 2) Budget Total school operating budget: School health budget : 100 : 4.2 (52.2 thousand Won) Half of the school health budget expend on medicine. 3) Clinic 80% of all schools have health clinic seperately. 71.32 of all schools have less than $35m^2$, 23.9%, $36-66m^2$. 4) Only 20% of all schools have organization for health 5) Average of clinic visitor for 1 year; 2,084 Major problem is on digestive system. And other problem: respiratory, skin, musculo - skeletal system, dental problem, etc... 6) Literal message for 1 year; 12 times. Need for use the health clinic seperately, adequate facilities and free utilization by visitors.

The Longitudinal Relationship Between Emotionality at Age Three and Media Overdependence in Early School Age: The Mediating Role of Internalizing Behavior Problems (3세 유아의 정서성 기질과 학령 초기 아동의 미디어 과몰입 간 종단적 관계: 내재화 문제행동의 매개역할)

  • Objective: The purpose of this study is to investigate whether emotionality at age three predicts early school-age media overdependence and to examine the potential mediating role of internalizing behavior problems in children. Methods: This study employed data from the 4th and 10th waves of the Panel Study of Korea Children, comprising a sample of 1,270 children. Assessments of emotionality (4th wave), media overdependence (10th wave), and internalizing behavior problems (10th wave) were conducted. Data analysis utilized SPSS and the Process Macro. Results: At age 3, heightened emotionality was found to directly contribute to an increased tendency for media overdependence in early school age. Internalizing behavior problems played a significant mediating role in the relationship between emotionality and media overdependence. Conclusion/Implications: This study validated a significant longitudinal connection between emotionality and the inclination to excessively rely on media, highlighting the role of internalization behavior problems in this relationship. It is essential to provide focused attention, especially to children displaying heightened emotionality in early childhood, in order to proactively prevent internalization behavior problems and the undue dependence on media.

Clinical features and prognostic factors of early-onset sepsis: a 7.5-year experience in one neonatal intensive care unit

  • Purpose: In this study, we investigated the clinical features and prognostic factors of early-onset sepsis (EOS) in neonatal intensive care unit (NICU) patients. Methods: A retrospective analysis was conducted on medical records from January 2010 to June 2017 (7.5 years) of a university hospital NICU. Results: There were 45 cases of EOS (1.2%) in 3,862 infants. The most common pathogen responsible for EOS was group B Streptococcus (GBS), implicated in 10 cases (22.2%), followed by Escherichia coli, implicated in 9 cases (20%). The frequency of gram-positive sepsis was higher in term than in preterm infants, whereas the rate of gram-negative infection was higher in preterm than in term infants (P<0.05). The overall mortality was 37.8% (17 of 45), and 47% of deaths occurred within the first 3 days of infection. There were significant differences in terms of gestational age (26.8 weeks vs. 35.1 weeks) and birth weight (957 g vs. 2,520 g) between the death and survival groups. After adjustments based on the difference in gestational age and birth weight between the 2 groups, gram-negative pathogens (odds ratio [OR], 42; 95% confidence interval [CI], 1.4-1,281.8) and some clinical findings, such as neutropenia (OR, 46; 95% CI, 1.3-1,628.7) and decreased activity (OR, 34; 95% CI, 1.8-633.4), were found to be associated with fatality. Conclusion: The common pathogens found to be responsible for EOS in NICU patients are GBS and E. coli. Gram-negative bacterial infections, decreased activity in the early phase of infection, and neutropenia were associated with poor outcomes.

  • The purpose of this study was to examine the differences in the service quality evaluation on the skill and expert, the shop facilities and atmosphere, the time and access, and the price and benefit of a nail care shop by age, income, spendings of appearance and a nail care, and a regular nail care. The survey was performed with questionnaires and the subjects were was 240 consumers who used the nail care. The data were analyzed by frequencies, F-test, T-test, Factor Analysis, one way ANOVA. The results of the study were as follows: Service quality evaluation were classified four factors- the price and benefits, the skill and expertness, the facilities and atmosphere, the time and access. There were no statistically significant differences in the price and benefits by age, income, spendings of appearance and a nail care, and a regular nail care, but there were statistically significant differences in the skill and expertness, the facilities and atmosphere, and the time and access. The service quality on the time and access was evaluated as high by the consumer in the 30s, with more income, and with high appearance and nail care spendings. The service quality on the shop facilities and atmosphere was evaluated as high by the consumers with high nail care spending. The consumers with a regular nail care evaluated the service quality on the skill and expertness, the time and access, the shop facilities and atmosphere as high.

Estimation and adequacy of dentist supply through the dental care productivity (생산성을 고려한 치과의사의 공급 추계와 적정성)

  • Running title: Adequacy of dentist supply Objectives: The proper provision of dentists is very important for delivering the dental care that people need. The purpose of this study is to evaluate the adequacy of dentist supply and to predict the proper supply of dentists in Korea for the next 10 years. Methods and materials: Using the data from 2004 to 2017 in the Statictics Korea, the dental needs were measured as days of admission and the annual total dental care hours in 2020, 2023, 2026 and 2030 were predticted using regression analysis. The dental care productivity (average in-office hours) of dentists was analyzed using regression analysis including age, gender, education level and region as a confounder. The annual care hours were predicted according to seven scenarios based on the percentage of women among dentists, post-dentist education levels, the percentage of dentists in the metropolitan area, aging and retirement age. Results: The needed dentist based on the data in the Statictics Korea will be 27, 288 ~ 27, 311 in 2020, 28,104 ~ 28,1785 in 2023, 28,977 ~ 29,124 in 2026, and 30,174 ~ 30,413 in 2030. On the other hand, the number of dentists in Korea will be 26,945 in 2020, 28,813 in 2023, 30,682 in 2026 and 33,173 in 2030. The adequacy index of dentist supply will be -0.01 in 2020 to +0.10 in 2030. Conclusions: Our data showed that dentists will be adequately supplied in Korea for the next 10 years. Overall, the ratio between dentist versus 1,000 Koreans will be increased from 0.5 in 2019 to 0.7 in 2020, which will meet the mean of OECD countries.

Out-of-pocket Health Expenditures by Non-elderly and Elderly Persons in Korea (우리나라 성인과 노인의 개인부담 의료비용 지출의 관련요인)

  • Objectives : The purpose of this study was to determine the impact of the sociodemographic and health characteristics on the out-of-pocket health spending of the individuals aged 20 and older in Korea. Methods : We used the data from the 2001 National Public Health and Nutrition Survey. The final sample size was 26,154 persons. Multiple linear regression models were used according to the age groups, that is, one model was used for those people under the age of sixty-five and the other was used for those people aged sixty-five and older. In these analyses, the expenditures were transformed to a logarithmic scale to reduce the skewness of the results. Results : Out-of-pocket health expenditures for those people under the age of 65 averaged 14,800 won per month, whereas expenditures for those people aged 65 and older averaged 27,200 won per month. In the regression analysis, the insurance type, resident area, self-reported health status, acute or chronic condition and bed-disability days were the statistically significant determinants for both age groups. Gender and age were statistically significant determinants only for the non-elderly. Conclusions : The findings from this study show that the mean out-of-pocket health expenditures varied according to the age groups and also several diverse characteristics. Thus, policymakers should consider the out-of-pocket health expenditure differential between the elderly and non-elderly persons. Improvement of the insurance coverage for the economically vulnerable subgroups that were identified in this study should be carefully considered. In addition, it is necessary to assess the impact of out-of-pocket spending on the peoples' health care utilization.

Disparities in Health Care Utilization Among Urban Homeless in South Korea: A Cross-Sectional Study

  • Objectives: We examined health care disparities in Korean urban homeless people and individual characteristics associated with the utilization of health care. Methods: We selected a sample of 203 homeless individuals at streets, shelters, and drop-in centers in Seoul and Daejeon by a quota sampling method. We surveyed demographic information, information related to using health care, and health status with a questionnaire. Logistic regression analysis was adopted to identify factors associated with using health care and to reveal health care disparities within the Korean urban homeless population. Results: Among 203 respondents, 89 reported that they had visited health care providers at least once in the past 6 months. Twenty persons (22.5%) in the group that used health care (n = 89) reported feeling discriminated against. After adjustment for age, sex, marital status, educational level, monthly income, perceived health status, Beck Depression Inventory score, homeless period, and other covariates, three factors were significantly associated with medical utilization: female sex (adjusted odds ratio [aOR, 15.95; 95% CI, 3.97 to 64.04], having three or more diseases (aOR, 24.58; 95% CI, 4.23 to 142.78), and non-street residency (aOR, 11.39; 95% CI, 3.58 to 36.24). Conclusions: Health care disparities in Seoul and Daejeon homeless exist in terms of the main place to stay, physical illnesses, and gender. Under the current homeless support system in South Korea, street homeless have poorer accessibility to health care versus non-street homeless. To provide equitable medical aid for homeless people, strategies to overcome barriers against health care for the street homeless are needed.

Effects of Nursing College Students' Health Consciousness, Health Self-efficacy and Self-care Agency on Health Promoting Behavior (간호대학생의 건강관심도, 건강관리 자기효능감, 자가간호역량이 건강증진행위에 미치는 영향)

  • Objectives: This study aimed to confirm the degree of health consciousness, health self-efficacy, self-care agency of nursing students and the factors that affect health promoting behaviors. Methods: From June 7 to June 20 in 2022, the structured questionnaire was used for 255 nursing students. Data were analyzed by descriptive analysis, mean comparison(t-test, ANOVA), correlation analysis(Pearson's correlation coefficient) and multiple regression using SPSS/WIN 25.0. Results: The health promoting behaviors had positive correlations with health consciousness, health self-efficacy, and self-care agency. And the prediction factors influencing health promoting behaviors were health self-efficacy(𝛽=.40), health consciousness(𝛽=.19), age over 40 (𝛽=-.23, p<.001), exercise six times a week or more(𝛽=.22, p=<.001), and having breakfast six times a week or more(𝛽=.11, p=.025). The explanatory power on health promoting behaviors from these variables was 53%(F=20.72, p<.001). Conclusions: In order to improve the health promoting behaviors of nursing students, various ways to increase the active changes should be sought, and program should be prepared to improve their health promotion behaviors in the future.

Content Analysis of Korean-American Women's School-aged Child Rearing on Internet Community (재미 한인 여성의 인터넷 커뮤니티에 나타난 학령기 아동양육에 관한 내용 분석)

  • The purpose of this study was to explore the ideas about Korean-American mother's school-age child rearing that are represented on internet community. The method used for this study was content analysis and the data consisted of articles about Korean-American mother's school-age child rearing on a internet community bulletin board during 1 year in 2008. It was found that these articles dealt with problems of practice in interfacing between home and social institutions(Especially school), teaching, counseling, nurturing and disciplining. For example, 831 articles put great emphasis on interfacing between home and social institutions, 339 on teaching, 268 on counseling with children. From these results, we might conclude that many Korean-American mothers gave their children school related care.

  • Purposes: The purpose of this study is to analyze the factor of affecting on job satisfaction by mediating attitude of care service. Methodology: (1)Identify socio-demographic characteristics, work characteristics, the elderly support period, the presence of education related to the elderly, the presence of service associated with the elderly, and significant factors of the research subjects. (2)Identify changes in the number of education and services before and after obtaining a caregiver qualification. (3)Identify the attitude of providing care services and job satisfaction according to the characteristics of the research subjects. (4)Identify the direct and indirect factors influencing job satisfaction by mediating the service attitudes of the research subjects. Findings: First, age factors have positive effects and the volunteering experience have negative effects on job satisfaction by mediating attitude of care service. Although the number of education class has increased after qualification, educational factors had no significant effect. Second, the longer elderly support period had a positive effect on job satisfaction. Practical Implications: This study is meaningful in that it identifies the direct and indirect impacts on job satisfaction through care attitudes. In the future, it will be necessary to pay attention to improving the quality of long-term care services by analyzing the influence factors more.
