중·소병의원 건물매력도와 교통인프라가 고객만족도에 미치는 영향 : 의료집적성의 매개효과를 중심으로 (A Study on the Effects of the Building's Attractiveness and the Transportation Infrastructure on the Customer's Satisfaction : Centered with the Effects of Medical Integration)

  • 본 연구는 급변하는 의료 환경 속에서 중소병의원의 개원 및 운영과 관련하여 입지 및 건물의 매력도와 의료집적성이 고객만족도에 미치는 영향을 의료집적성이 매개효과를 중심으로 분석한 실증연구이다. 주요 연구결과는 다음과 같다. 첫째, 건물매력도가 고객만족도에 유의적인 정(+)의 영향을 미칠 것이라는 가설은 채택되었다. 둘째, 교통인프라가 고객만족도에 유의적인 정(+)의 영향을 미칠 것이라는 가설은 기각되었다. 셋째, 건물매력도가 의료집적성에 유의적인 정(+)의 영향을 미칠 것이라는 가설은 채택되었다. 넷째, 교통인프라가 의료집적성에 유의적인 정(+)의 영향을 미칠 것이라는 가설은 채택되었다. 다섯째, 의료집적성이 고객만족도에 유의적인 정(+)의 영향을 미칠 것이라는 가설은 채택되었다. 여섯째, 건물매력도와 고객만족도 간의 관계를 의료집적성이 매개할 것이라는 가설은 부분매개역할이 있음이 확인되어 채택되었다. 일곱째, 교통인프라와 고객만족도 간의 관계를 의료집적성이 매개할 것이라는 가설은 완전매개역할이 있음이 확인되어 채택되었다. 본 연구의 결과는 향후 의료서비스 시설의 입지선정을 위한 기초를 제공하는 데 응용할 수 있을 것이다.

보건의료기기 박람회 참관객의 추구편익이 만족도 및 행동의도에 미치는 영향 (Impact of Pursuit Sought on Satisfaction Level and Behavioral Intention among Visitors in a Health Care and Medical Equipment Fair)

  • 본 연구의 목적은 보건의료기기 박람회 참관객들의 추구편익을 바탕으로 방문자 만족도와 행동의도를 분석하는데 있다. 본 연구에서는 방문자의 만족도와 추구편익에 대한 만족도 및 행동의도를 파악할 수 있는 각 변인들 간의 관계를 규명함으로써 보건의료기기 박람회를 주최자 및 관련자에게 필요한 기초 자료를 제시하고자 하였다. 따라서 본 연구는 원주 보건의료기기 박람회 이벤트에 참가하는 참관자들을 대상으로 250명을 표집하여 설문조사를 실시 하였다. 수집된 자료는 SPSS 14.0과 AMOS 16.0 통계프로그램을 이용하여 기술통계분석, 요인분석(Factor analysis), 신뢰도 분석(reliability analysis), 구조방정식모형 분석을 실시하였고, 다음과 같은 결론을 얻었다. 추구편익에 따른 방문객 만족도, 추구편익에 대한 행동의도, 방문객 만족과 행동의도 간에 각각 유의한 영향을 미치는 것으로 나타나 가설이 모두 채택되었다. 따라서 보건의료기기 박람회를 주최자 및 담당 관련자들은 박람회 참여자들의 특성과 욕구가 일치하는 박람회 또는 이벤트 상품, 서비스 등이 차후 행사시 만족도 및 행동의도로 이어 질수 있도록 전시주체들은 전시기획을 하여야 할 것으로 판단된다.

Nursing Home Employee and Resident Satisfaction and Resident Care Outcomes

  • Background: Nursing home resident care is an ongoing topic of public discussion, and there is great interest in improving the quality of resident care. This study investigated the association between nursing home employees' job satisfaction and residents' satisfaction with care and medical outcomes. Methods: Employee and resident satisfaction were measured by questionnaire in 175 skilled nursing facilities in the eastern United States from 2005 to 2009. Facility-level data on residents' pressure ulcers, medically unexplained weight loss, and falls were obtained from the Centers for Medicare and Medicaid Services Long-Term Care Minimum Data Set. The association between employee satisfaction and resident satisfaction was examined with multiple and multilevel linear regression. Associations between employee satisfaction and the rates of pressure ulcers, weight loss, and falls were examined with simple and multilevel Poisson regression. Results: A 1-point increase in overall employee satisfaction was associated with an increase of 17.4 points (scale 0-100) in the satisfaction of residents and family members (p < 0.0001) and a 19% decrease in the incidence of resident falls, weight loss, and pressure ulcers combined (p < 0.0001), after adjusting for staffing ratio and percentage of resident-days paid by Medicaid. Conclusion: Job satisfaction of nursing home employees is associated with lower rates of resident injuries and higher resident satisfaction with care. A supportive work environment may help increase quality of care in the nation's nursing homes.

지역주민의 한방의료 이용성향과 진료만족도 (Trends in the Use of Oriental Medical Care Service and Treatment Satisfaction)

  • The purpose of this study was to identify a tendency in patients who seek oriental medical service and factors influencing patient satisfaction. The study was conducted with 1,520 residents of a community during the period from February 5, 2005 through June 30, 2005 using a questionnaire. This study results are summarized as follows : 1. Of subjects who sought oriental medical service, 66.2% had musculoskeletal disorder and connective tissue disease, 18.9% had digestive tract disease, 16.4% had respiratory disease, 8.2% had endocrinemetabolic disease, 7.5% had circulatory disease and the remaining subjects had other diseases(p<0.001). 2. Of subjects who sought oriental medical service for the treatment of musculoskeletal disorder, 84.9% preferred acupuncture. Of those who had digestive tract diseases, 47.0% preferred packaged herbal medicine. Of those who had respiratory disease, 63.0% preferred packaged herbal medicine. 3. Acupuncture was the most often sought by subjects with musculoskeletal disorder. Packaged herbal medicine was sought by subjects with respiratory disease, digestive tract disease, endocrine-metabolic disease or circulatory disease. Tablet-type herbal medicine was sought by subjects with musculoskeletal disorder or digestive tract disease. Combined therapy was sought by subjects with musculoskeletal disorder, digestive tract disease, hematopoietic disease or immune disorder. 4. The level of satisfaction with oriental medical service was higher in subjects with circulatory disease, subjects with digestive tract disease, subjects with neurological disorder and subjects with musculoskeletal disorder in descending order. Of total subjects, 39.4% experienced side effects of oriental medical care, 38.1% experienced side effects of herbal medicine. About 51.9% considered the price of herbal medicine costly while 23.2% considered it reasonable. 5. Subjects' knowledge of herbal medicine was measured as $29.2{\pm}3.83$ out of 42 scores or 69 out of 100 points, indicating a low knowledge level. Subjects' knowledge was influenced by occupation, religion, side effects, sex, age, residence area, the type of insurance. These variables explained 15.2% of the variance. 7. Of total subjects, 56.8% were satisfied with oriental medical service. Patient satisfaction varied with occupation, religion, the type of insurance, health state and treatment outcomes. These variables explained 37.3% of the variance. Conclusion : The majority of subjects were satisfied with oriental medical service. However, oriental medical care are not widely used to treat all kinds of diseases while its use skews to a small categories of diseases. It is therefore necessary for the government and oriental medical service providers to develop new therapy approaches for the treatment of a broader range of diseases.

Medical Interpreters in Korea: A Qualitative Study of Medical Tourists' Satisfaction and the Role of Interpreters

  • Background: As a result of the South Korean government's medical tourism strategies, the number of UAE inbound medical tourists have the highest annual increase rate among the Middle Eastern countries. The need for medical interpreters in such cases is inevitable, especially considering the languages spoken in both countries differs. The work of a medical interpreter affects patient satisfaction, quality of care, and the UAE patients' revisit intention. However, there is a lack of literature that focuses on the role of medical interpreters for the Arabic-speaking medical tourists in South Korea. The purpose of this study is to assess the satisfaction and the experience of UAE medical tourists in South Korea who have used medical interpretation services. Method: A descriptive exploratory design, including semi-structured interviews, was used in this study to examine factors influencing the satisfaction of UAE patients seeking medical attention in South Korea, who considered travelling exclusively for medical purposes. Eleven UAE medical tourists were selected for the interview using convenience sampling. Results: Two types of interpreters were revealed by the analysis: interpreters with Arabic as their mother tongue and interpreters with Korean as their mother tongue. Patients emphasized the origin of the interpreters and differentiated between the two types of interpreters' professionalism, abilities, and personal skills. However, due to the huge gap between the two cultures, languages, and religions, the UAE patients mainly preferred the Arabic medical interpreters. Further, each type had certain traits that impacted the medical care process and patients' satisfaction. Conclusion: This study shows that the South Korean government needs to focus on the training and assessment system for Arabic language interpreters. Additionally, cultural differences training for the interpreters may help to increase UEA patients' satisfaction.

Observational Study on Patient's Satisfactions and Quality of Life (QoL) Among Cancer Patients Receiving Treatment with Palliative Care Intent in a Tertiary Hospital in Malaysia

  • The main objective of palliative treatment for cancer patients has been to maintain, if not improve, the quality of life (QoL). There is a lack of local data on satisfaction and QoL among cancer patients receiving palliative treatment in Malaysia. This study covers patients with incurable, progressive cancer disease receiving palliative treatment in a teaching hospital in Kuala Lumpur, comparing the different components of QoL and correlations with patient satisfaction. A cross-sectional survey using Malay validated SF36 QoL and PSQ-18 (Short Form) tools was carried out between July 2012 -January 2013 with 120 cancer patients receiving palliative treatment, recruited into the study after informed consent using convenient sampling. Results showed that highest satisfaction were observed in Communication Aspect ($50.6{\pm}9.07$) and the least in General Satisfaction ($26.4{\pm}5.90$). The Mental Component Summary ($44.9{\pm}6.84$) scored higher when compared with the Physical Component Summary ($42.2{\pm}7.91$). In this study, we found that patient satisfaction was strongly associated with good quality of life among cancer patients from a general satisfaction aspect (r=0.232). A poor significant negative correlation was found in Physical Component (technical quality, r=-0.312). The Mental Component showed there was a poor negative correlation between time spent with doctor (r=-0.192) and accessibility, (r=-0.279). We found that feeling at peace and having a sense of meaning in life were more important to patients than being active or achieving good physical comfort. More studyis needed to investigate patients who score poorly on physical and mental component aspects to understand their needs in order to achieve better cancer care.

Perceived Service Quality among Outpatients Visiting Hospitals and Clinics and Their Willingness to Re-utilize the Same Medical Institutions

  • Objectives : This study was to determine how the perception and the satisfaction of outpatients who utilized clinics and hospitals are structurally related with their willingness to utilize the same institution in the future. Methods : Three hundred and ten responses (via convenient sampling) were collected from 5 hospitals and 20 clinics located in Seoul listed in the "Korea National Hospital Directory 2005". Service quality was utilized as the satisfaction measurement tool. For analysis, we used a structural equation modeling method. Results : The determining factors for general satisfaction with medical services are as follows: medical staff, reasonability of payment, comfort and accessibility. Such results may involve increased competition in the medical market and increased demands for quality medical services, which drive the patients to visit hospitals on their own on the basis of changed determining factors for satisfaction. Conclusions : The structural equation model showed that the satisfaction of outpatients with the quality of medical services is influenced by a few sub-dimensional satisfaction factors. Among these sub-dimensional satisfaction factors, the satisfaction with medical staff and payment were determined to exert a significant effect on overall satisfaction with the quality of medical services. The structural relationship in which overall satisfaction perceived by patients significantly influences their willingness to use the same institution in the future was also verified.

노인요양병원 환자보호자의 병원 선택속성의 중요도와 만족도 차이 분석 및 재이용 의도에 관한 연구 (An Analysis of the Difference between Importance and Satisfaction of Selection Attributes and Reuse Intention in Long Term Care Hospital for Elderly Patient Caregivers)

  • Advances in healthcare technology and rapid economic growth lead to the increased life expectancy and consequently the size of elderly population. Korea is one of the countries that are rapidly aging. Thus, it is particularly important to prepare for the aging society. Recently, the number of healthcare institutions for the elderly citizens has increased. The purpose of selecting a hospital for the elderly is, in general, maintenance of health rather than improvement of health receiving proper treatment. Unlike choosing a hospital for treatment, customers of a long term care hospital have a different set of factors to consider. Especially, when choosing a long term care hospital, the influence of patient's family is greater than the patient. This study examines the factors they consider for long term care hospital. A total of 198 questionnaires were collected from the families of actual patients of long term care hospitals. Twelve questionnaires were found to be non-usable because of missing and unsatisfactory responses. Consequently, 186 questionnaires were used for the analyses. Findings of this study are as follows. First, seven factors have been identified to consider when choosing a long term care hospital for the elderly. They include convenience of facilities, costs variety of facility programs, service hours, reputation, accessibility, quality of medical staff, medical facilities, and facility size. Second, This study measured both importance and satisfaction with these attributes and analyzed the difference between them. Satisfaction was lower than importance in the categories of convenience of facilities, costs, and programs, and accessibility. On the other hand, satisfaction was higher in terms of service hours, reputation, and quality of medical staff. Finally, the current study found positive impact of accessibility and quality of medical staff on reuse intention of a long term care hospital.

일 대학병원의 가정간호서비스 내용 및 만족도 조사 (A Study on Home Care Service and the Level of Client Satisfaction)

  • This study was done to examine the home care service provided by home care nurses and the level of client satisfaction. Data were collected from 110 clients who received the home care service at the P-hospital in Pusan from 23th April, 2001 to 30th November, 2002. The tool for measurement of satisfaction was composed of 16 items and was 4 score scale. Data was analyzed by using SPSS/WIN 10.5 program, the results of this study are summarized as follows; 1) 77.3% among 110 clients were over sixty years old. By the disease pattern. subjects were distributed into cancer(52.7%), cerebro-vascular disease(22.7%) and the others. 36.4% of clients were received home visits from 10 to 20 times by home care nurse. 2) The total number of home care services was 20,828. And most of the clients received the health education and training. 3) The mean score of satisfaction on provided home care services was $3.36\pm0.45$, out of 4. among 16 items. 'The home care nurses were kind enough' was highest ($3.59\pm0.49$), the total level of satisfaction of home care services was very high. with total mean score $53.84\pm7.16$. As mentioned above. the satisfaction level of home care services was very high. so we can except that the prospect of home care services is very challengeable. Therefore. we should try to expand the service recipients with promotional education to the home care clients under the cooperation with medical staff and make more efforts to develop the management system about clients' information. to improve the quality of the service, to assure close ties with the public health centers for the continuous home care service linking.

Perioperative Comprehensive Supportive Care Interventions for Chinese Patients with Esophageal Carcinoma: a Prospective Study

  • Objective: To assess the effects of perioperative comprehensive supportive care interventions on outcome of Chinese esophageal cancer patients in a prospective study. Methods: 60 patients with primary esophageal carcinoma were randomized into an intervention group (IG, n=31) and a control group (CG, n=29). The Chinese version of symptom checklist-90 (SCL-90) was adopted to assess their psychological status. The interventions, including health education, psychological support, stress management, coping strategies and behavior training, were carried out in 3 phases (preoperative, postoperative I and postoperative II), and psychological effects were thereafter evaluated accordingly before surgery, and 1 week, 4 weeks and 24 weeks post-surgery. Medical costs were estimated at discharge. Survival of patients was estimated each year post-surgery. General health status and satisfaction-with-hospital were surveyed by a follow-up questionnaire 4 years post-surgery. Results: All the subjects demonstrated higher scores in the preoperative phase than the normal range of Chinese population concerning 7 psychological domains including somatization, obsessive-compulsive, depression, anxiety, hostility, phobic anxiety and paranoid ideation. Although no significant difference was observed between the two groups at admission, the scores of IG, which tended to decrease at a faster rate, were generally lower than those of CG at weeks 1, 4 and 24 post-surgery. The length of hospital stay and medical costs of IG were significantly less than those of CG and satisfaction-with-hospital was better. However, there was no significant difference in 4-year survival or health status between two groups. Conclusions: Appropriate perioperative comprehensive supportive care interventions help to improve the psychological state of Chinese patients with esophageal carcinoma, to reduce health care costs and to promote satisfaction of patients and their families with hospital.
