한 대학병원 부속 건강검진센터 이용자의 만족도와 재이용 의사에 미치는 요인 (Factors Affecting the Satisfaction and Revisit Intension of Health Promotion Center In A University Hospital)

  • The purpose of this study was to serve as a basis for providing quality medical service and mapping out consumer-centered marketing strategies to successfully cope with the rapidly changing medical environment and meet consumer needs, by examining what affected the satisfaction and revisit of health Promotion center Client. The subjects in this study were 186 of visitor to health Promotion center in a university hospital in the city of Busan. A survey was conducted with structured questionnaire from March 2 to 30, 2001. The collected data were analyzed with SPSS for Windows (ver10.0). For more statistical analysis, frequency analysis, component analysis, t-test, ANOVA and correlation analysis procedures were utilized. Results of the study can be summarized as follows: 1. Regarding demographic characteristics, 51.6% of the Client investigated were male, and 48.4% were female. The greatest number of them were in their 40s(38.9%). 86.5% were married, and 34.2% were self-employed. 44% were a high-school graduate, and the monthly mean income of 59.4% was one to three million Won. And, the residential area of 46.8%, the largest percentage, was a half-an-hour distance from health Promotion center. 2. The most common motivation of their selection of the health Promotion center was a recommendation by Staff and neighborhood(59.7%), followed by excellent facilities and services(17.2%), the tradition and reputation of the hospital(7.5%), and its publicity pamphlets(7.5%). 3. 45.9%, the largest percentage, acquired health-related information from their friends or acquaintances. 43.8%, the greatest percentage, visited there because they felt there's something wrong with their body. 53.4% worried about the possibility of being attacked by cancer, and 57.5% wanted to take a precise cancer examination. For health maintenance, 50.1% got regular exercise. Regular exercise was considered most crucial for health maintenance or promotion. 4. The largest reason they used that examination center again was the kindness of employees(52.7%), followed by the tradition and reputation of the hospital(21%) and excellent examination setting and equipment(10.8%). 5. By demographic factor, there was no significant difference between the man and women in satisfaction level with examination and expenses. The women expressed more satisfaction at facilities, and the high-school graduate group were more contented with expenses. 6. As a result of investigating their satisfaction level according to the motivation of selecting that examination center, the group that chose the center due to excellent facilities and services were more satisfied with examination and expenses. 7. As for the relationship of total examination satisfaction to revisit. intention, there was a higher correlational relationship between total health examination satisfaction and expense satisfaction. Especially, the more they were satisfied with examination, the more they were willing to revisit there for another examination.

농촌지역 보건지소 건축의 특성에 관한 연구 - 익산시 보건소 관할 지소를 중심으로 - (A Study on the Architectural Characteristics of the Health Care Center Branch in Rural Area Ik-san city)

  • There are an increasing number of healthcare facilaties, especially branch offices, in rural areas to serve the aging population living there. However, there has been a gradual decline in the ratios of recognition, satisfaction and utilization by people who live in the regions. A significant reason of declining the ratios should be the population decline, but the most of population hierarchy shows the groups of elderly people over 60. This result appears to be limited to visit the public health centers. According to the result of population hierarchy, a branch office of public health center has been re-established as a complex welfare facility which can be fulfilled in the functions of basic medical supports and cultural supports. This research is focused on collecting the meaningful information of the status of physical facilities and utilization with 15branch offices of public health care centers in the rural regions near the city of Ik-San city. In addition, this research has a purpose of getting fundamental data for future architectural plans of the branch offices in rural regions with the results about the status of facility operation systems and users' needs.

한의학 교의사업 시범시행 후 교사들의 교의만족도에 관한 연구 - 성남시 중고등학교를 대상으로 (A Study of Teacher's Satisfaction Regarding Oriental Medical Doctor's Student Health and Wellness Program)

  • Objectives This study aims to build the baseline data for school health care program by analyzing the questionnaires about satisfaction regarding Oriental Medical Doctor's (Korean Medicine) student health and wellness program. Methods The association of Korean medicine doctor in Sung-nam city conducted Oriental Medical Doctor's student health and wellness program for 12 middle schools and 8 high schools in Sung-nam from August to December, 2015. The participating Oriental Medical Doctor visited each school for 8 times during 5 months period, and conducted health consultations, health education classes and Korean medicine treatment for the school students and school employees. Teachers and administrators from the participating schools answered the self-reported satisfaction questionnaires and the satisfaction questionnaires results were analyzed. Results 85 people responded the program satisfaction questionnaires; 69% responders were very satisfied and 28% were satisfied about the program specifically about the student's health check-ups, informatory brochures for the parents, good participation enrollment process. Responders were pleased about doctor's consultations regarding 'Advising students about their general health', 'Ways to cure sick students fast', 'Providing students with useful information about health', and 'Teaching students how to manage their health in a good shape'. 92.94% of participants agreed to the needs of the school health and wellness program. Conclusions One of the fundamental national health policies is to provide students opportunities to maintain their good health. We have developed a successful pilot program called, "Oriental Medical Doctor's student health and wellness program". We got positive feedback from the participating school teachers regarding our program. Further studies based on this study are needed to show the benefit of the program to broaden its use.

취약계층 노인의 구강건강상태에 영향을 미치는 요인 (Factors Affecting the Oral Health Status in Vulnerable Elders)

  • Propose: This study was to identify the factors affecting the oral health status in vulnerable elders receiving home visiting health care service. Methods: A total of 444 elderly people over the age of 65 at 33 public health centers in Daegu and Gyungbuk areas were included in this survey. The data were collected by personal interview with nurses using oral health knowledge, behavior, and status questionnaires from the $10^{th}$ to $22^{nd}$ of July, 2012. The data were analyzed with t-test, one-way ANOVA, $Scheff{\acute{e}}$ test, and stepwise multiple regression, using SPSS program. Results: Factors affecting the oral health status were chewing satisfaction with denture 14.5% (${\beta}=.391$, p<.001), denture care (wash with water) 3.2% (${\beta}=-.187$, p<.01), and types of health insurance (medical aid type1, 2) 1% (${\beta}=-.111$, p<.05). The factors explained 18.7% of variance in the oral health status of vulnerable elderly subjects. Conclusion: The results of this study indicate that in order to improve the oral health status of vulnerable elders, it is necessary to provide oral health education that includes the proper usage of denture to elders who receive home-visiting care.

보건기관 이용 현황 및 만족도와 관련 요인 -'2010지역사회건강조사' 자료를 이용하여- (Related Factors on Health Service Utilization and Satisfaction of Health Center Clients -Using '2010 Community Health Survey'-)

  • The objective of this study is to assess the utilization and satisfactions about public health centers in Korea. For the analysis, the study utilize the nationwide data of 229,229 person on '2010 Community Health Survey' of Ministry of Health & Welfare. The statistical methodology used in the study is ${\chi}^2$, ANOVA, logistic regression model and multiple regression model. This study have four major findings. First, the significant affecting socio-demographic factors in utilizing public health center were gender, age, region, national basic living secured, married, income, education, job, state of health, chronic disease, unmet medical needs and utilization reason. Second, the most serviced category of health center user was vaccination both city and rural area and the next was certificate, primary care, health screening, other use, the mother and child in city area, primary care, health screening, certificate, home visiting health in rural area. Third, the significant affecting socio-demographic factors in satisfaction degree on health center service were age, region, national basic living secured, income, education, job, state of health, utilization degree and reason. Fourth, the most satisfied service of health service center was home visiting health in city area and mental health service in rural area and the next was nutritive control and the lowest satisfied service was user of certificate. The utilization and satisfaction on health center service were identified as different with residental area and user's characteristics. The politic effort are needed to support socially disadvantaged class and to narrow regional gap.

국내 전자간호기록 개발 및 실무적용 현황 조사 (The Adoptions and Use of Electronic Nursing Records in Korean Hospitals: Findings of a Nationwide Survey)

  • Purpose: To provide clear estimates of the adoption and use of electronic nursing records (ENRs) with standard terminology in Korea and identification of the scope and use as well as perceived or potential benefits of ENRs. Methods: A survey was done of 733 hospitals at three levels: tertiary advanced hospitals, general hospitals, and community hospitals. After performing a literature review a modified version of an existing survey tool was used for 2 months in 2012. The collected information related to EHR functionality and coverage of nursing documentation and nursing process, application of standard terminology, and perceived satisfaction and benefits of ENRs. Results: The response rate was 39.4% (289/733), and 202 hospitals (70.1%, 95% CI64.8~75.5%) of the respondents had ENR systems (82.5% of tertiary hospitals, 66.7% of general hospitals, and 70.1% of community hospitals). Out of these hospitals less than 10% had ENRs fully covering nursing documentation. The adoption rate of standard terminology was 55%, and hospital satisfaction with ENRs was 70%. But personalized care was identified as needing improvement in ENRs. Conclusion: The ENR adoption rate was high but there are many potential opportunities for improving ENR systems in terms of the data standardization and personalized care.

상급종합병원의 간호·간병통합서비스 병동에 근무하는 간호사의 직무스트레스, 직무만족 및 이직의도 (Exploring Job Stress, Job Satisfaction, and Turnover Intention of Nurses in the Comprehensive Nursing Service)

  • 본 연구는 상급종합병원의 간호 간병통합서비스 병동에서 근무하는 간호사의 직무스트레스, 직무만족도, 이직의도를 파악하기 위한 서술적조사연구로서 상급종합병원 두 곳을 편의추출하여 이 두 의료기관의 간호 간병통합서비스 병동에 근무하고 있는 간호사를 대상으로 설문조사를 실시하였다. 자료 분석은 대상자의 일반적 특성, 직무스트레스, 직무만족, 이직의도는 서술통계로, 대상자의 일반적 특성에 따른 직무스트레스, 직무만족, 이직의도의 비교는 Mann-Whitney U test를 이용하였다. 본 조사연구가 실시된 상급종합병원 두 곳의 간호 간병통합서비스 병동에서 근무하는 간호사는 모두 여성으로 30세 미만이며 미혼이 대부분이었다. 학력은 학사가 대부분이었다. 간호사 월수입은 93.8%가 150이상 300만원 미만으로 응답하였다. 직무스트레스가 높게 나타난 하위영역은 '의사와갈등', '보호자와 환자', '대인관계갈등'이었고, 직무만족이 가장 낮은 영역은 '보수및승진' '필수적직무'였다. 높은 이직의도를 보인 경우는 '본인이 원하지 않는 부서이동이 되었을때 이직하고 싶다', '부서이동을 인해 업무수행에 어려움이 심할 때 이직하고 싶다', '업무에 비해 월급이 적다고 느낄 때 이직하고 싶다' 등이었다. 간호 간병통합서비스 본래의 취지에 부합하도록 이 서비스를 정착하고 확대하기 위해서는 이 서비스를 제공하는 주요 의료인력인 간호사의 근무환경에 대한 체계적인 평가가 필요하다. 간호 간병통합서비스를 제공하는 간호사의 직무스트레스를 낮추고, 직무만족을 높이기 위해서는 적절한 보상, 명확한 업무에 대한 규정, 일반병동과 간호 간병통합서비스 병동 중에 선호하는 곳에서 근무할 수 있는 여건 등이 고려되어야 하며 이를 위해 정책적인 지원이 필요하다.

임종돌봄에 대한 교육의 필요성: 2개 대학병원의 인턴을 대상으로 예비 분석 (The Necessity for End-of-Life Care Education: A Preliminary Analysis with Interns at Two University Hospitals)

  • 목적: 본 연구는 2개 대학병원의 인턴을 대상으로 임종돌봄에 대한 교육 현황을 파악하고자 하였다. 방법: 2개 대학병원의 64명의 인턴들에게 구조화된 설문지를 배포하여 임종돌봄 교과과정의 강의습득 경험과 임상견학 경험 여부를 확인하였다. 또한 임종돌봄 관련 교육이나 진료에 대한 태도를 평가할 수 있는 7개의 자가평가질문을 파악하였다. 결과: 임종돌봄 교과과정 강의 습득과 임상실습 견학 개수의 평균은 각각 5개(총 영역 개수: 9)와 2개(총 영역 개수: 7)였다. 가장 많이 교육받은 항목은 나쁜 소식 전하기(96.9%)였고 임상실습 동안 가장 많이 견학한 항목은 신체증상 조절(56.5%)였다. 그러나 사전연명의료의향서를 포함한 의사소통 영역에 대한 경험은 20% 미만이었다. 임종돌봄 단독 교과목 교육군은 그렇지 않은 군과 비교하여 임종돌봄 교과과정에 대한 만족도, 관심도, 준비됨의 7가지 항목에서 모두 유의하게 긍정적인 답변을 하였다. 결론: 본 연구는 임종돌봄이 필요한 환자를 진료해야 하는 인턴들이 임종돌봄에 대한 교과과정 습득과 임상경험이 매우 부족함을 관찰하였다. 추후 국내 임종돌봄 교육에 대한 의과대학교육의 전체적인 상황을 파악하여 관련된 적절한 지식과 기술을 연마하는 교육체계를 세울 필요가 있다.

척추·관절 전문병원의 환자 만족도와 추천 의향 (Patient satisfaction and recommendation intention at spine and joint hospital)

  • 본 연구는 척추·관절질환 환자 만족도와 병원 추천 의향을 분석하고 만족도에 영향을 미치는 요인을 확인하기 위해 수행되었다. 이에 2019년 서울지역 소재 척추·관절 전문병원에서 환자 만족도와 추천 의향을 조사하였고, 인구·사회적 특성과 환자 만족도와의 관련성, 만족도 영향요인 및 추천 의향 등을 분석하였다. 분석 결과 외래의 경우 직원 용모·복장의 단정함과 청결도, 직원 친절도, 시설환경 쾌적성에서 환자 만족도가 높게 나타났고, 대기시간 관련 문항에서 환자 만족도가 낮게 나타났다. 입원의 경우 간호 영역과 병원환경 영역에서 여성의 만족도가 높았다. 하위 영역별로는 간호 영역 중 고객 존중 및 예의, 고객 경청, 담당 간호사에서 환자 만족도가 높게 나타났고, 병원환경 영역은 전반적인 병원 청결도와 안전 문항에서 만족도가 높았다. 환자 만족도와 주변에 해당 병원을 추천할 의향이 있는지를 묻는 문항 간 상관관계 분석에서는 환자 만족도와 병원 추천 의향은 양의 상관관계가 있는 것으로 나타났다. 척추·관절 질환 환자 만족도와 추천 의향 평가 결과는 척추·관절 전문병원에서 의료서비스 질 향상과 병원경영 전략 수립을 위한 기초 자료로 활용될 수 있을 것이다.

시판 유방 확대 수술 환자용 브래지어의 착의 평가 (A Study on the Brassiere Wearing Evaluation for Augmentation of Mammaplasty Patients)

  • The frequency of breast augmentation surgery continues to increase annually; however, the method of follow-up care varies from hospital to hospital. In particular, many different types of post-operative bras are available in the market. This study evaluated the wearing comfort of various commercial bras that were worn immediately after breast enlargement surgery prior to the manufacture of the bra. According to interviews of medical professionals and market research, five types of brassiere were selected and evaluated by wearing satisfaction, functional performance, and an external appearance test for 6 subjects with breast augmentation surgery. The evaluation questionnaires were based on a 5 point Likert scale with data analyzed using SPSS 20.0. The study results revealed that the bra with the highest degree of satisfaction was CNB (without bra cup) type. However, the use of CNB type showed dissatisfaction in functional evaluation questions regarding breast shaking and material & tactile sensation. In the future, it is necessary to develop a new post-operative brassiere based on a CNB type bra that showed the best evaluation. However, it is also necessary to identify the merits of the other four experimental bras and reflect these advantages.
