A case of breast cancer Patient Treated with Allergen Removed Rhus Verniciflua Stokes(ARV) (알러젠 제거(除去) 옻나무 추출물(抽出物) 투여(投與)로 호전(好轉)된 유방암(乳房癌) 환자 1례)

  • Breast cancer is common in West and Incidence of breast cancer has increased in Korea. According as conventional western medical treatment, breast cancer patient received MRM(modified radical mastectomy) and anti cancer chemotherapy. In this case report, We introduce a case of breast cancer patient who showed lung metastasis(metastatec adenocarcinoma) after MRM(modified radical mastectomy) 4 years before the diagnosis of relapse. After lung metastasis of breast cancer, the patient received 4th chemotherapy and refused to get more conventional western medical treatment including chemotherapy. After 24 month of traditional oriental medical treatment using allegen removed Rhus Verniciflua Stokes(ARV), however, the size of cancer mass decreased and the patient showed improved condition. Further case study will be needed in order to determine the effect of ARV on breast cancer patient.

Objective and Subjective Voice Examination in Korean Medicine

  • Objectives: When a person speaks, voice problems usually include pain or discomfort and/or difficulties in terms of the pitch, the loudness and the quality of the voice. When patients with voice problems induced by stroke, Parkinson's disease, and systemic diseases involving the voice are examined, generally, of the Four Diagnoses (四診), a Diagnosis of Hearing can be used in current Korean medicine. The effects of acupuncture and herb medicine on voice problems have been reported for over 20 years. However, when it comes to improvements, objective and subjective evaluation methods need to be explained. Methods: Subjective methods for evaluating voice were studied through a literature search of old medicinal books containing Korean medicine diagnostics, and an objective evaluation method using Praat software is presented. Results: Korean medicine doctors analyze the patient's voice in clinical settings unconsciously on a daily basis. However, most voice diagnoses depend on the doctor's subjective evaluation. Voice qualities can be evaluated by using the Eight Principles (八綱), including Yin-Yang; the Five Elements (Phases); the Grade, Roughness, Breathy, Asthenic, Strained (GRBAS) score, and the Visual Analogue Scale (VAS) as subjective methods, and an acoustic analysis using the Praat program can be used as an objective method. Conclusion: A more complete voice examination can be achieved by using subjective and objective methods at the same time. For an objective explanation and management of patient's voice problems or systemic disorders, an objective method should be used in Korean medicine, which already has many subjective diagnostic methods. More research needs to be conducted, and more clinical evidence needs to be collected in the future.

A Clinical Case of Liver Injury Induced by Chungsim Yeonja-tang (청심연자탕 투여후 발생한 약물 유인성 간손상 치험 1례)

  • Herbal medicine has been used in the Eastern world for 2,000 years, and is beneficial for numerous diseases. There have been reports pertaining to the safety of herbal medicine, but there have been few reports about herbal medicine induced liver injury in Eastern or Western medicine. Most are descriptions of hepatotoxicity of certain toxic herbs. We experienced one case of drug induced liver injury in the treatment of cerebral infarction with Chungsim Yeonja-tang. Although the patient had not used medication and Chungsim Yeonja-tang has no toxic herbs in it. patient's ALP, AST, ALT, GGT was twice elevated after 20 days on medication. This was diagnosed as drug induced liver injury, possibly due to incorrect diagnosis of Sasang constitution. So no herbal medicine was given for seven days. Then Gagam Saeng Gan-tang was given. Gagam Saeng Gan-tang have been used to treat hepatic disease and have been known to have beneficial effects. After 25 days on medication, the clinical symptoms and liver function improved. So, this case is presented to bring more attention to the toxicity of herbal medicines.

Yearly Report on CVA Patients(VI) (뇌졸중환자에 대한 연례보고(VI))

  • Objectives : This study was prepared for investigating the clinical features of stroke patients. Methods : We retrospectively reviewed the medical records of 177 patients who were admitted to the Dept. of Internal Medicine, College of Oriental Medicine, Kyungwon University with a diagnosis of stroke from Jan. I, 1999 to Dec. 31, 1999. Results : Ischemic stroke (74.0%) was more common than hemorrhagic stroke (19.8%). The proportion of males was 53.7%, of females was 46.3%, and the most prevalent age group is those in their sixties. Cerebral infarction was most frequently noticed in MCA territory, hemorrhage in putamen. Hypertension was the most common preceding disease, followed by diabetes mellitus. The rate of recurrence was high in cerebral infarction. The onset mode of cerebral infarction was sleeping and rising and of cerebral hemorrhage was acting. Most patients visited the hospital within 24 hours. The most common symptoms at admission were motor weakness and speech disorder. The most common complication was urinary tract infection. Admission period was 29.5 days. Physical treatment from onset in cerebral infarction and in cerebral hemorrhage was 17.4 and 22.0 days, respectively. Conclusions : Our study of CVA patients was similar to previous studies from 1994 to 1998. In most cases, western and oriental treatment and medicine were given synthetically. Prescription of sasang constitutional medicine had wide application.

Study on Association of DSOM Items for Uterine Myoma in Oriental Medicine -Control Group: Outpatient and Clinical Trials Data - (자궁근종 여부에 대한 DSOM 항목의 연관성분석 - 대조군 : 한방부인과 외래환자와 임상시험 피시험자 -)

  • Uterine myoma is a benign tumor of smooth muscle in the uterine wall. Recently, in Oriental medicine, concerns about uterine myoma patients have increased. We analyzed the medical records for 944 patients, including 257 uterine myoma patients, who visited Dongeui University Oriental Medical Center from May 2001 to June 2006. We investigated the DSOM (Diagnosis System of Oriental Medicine) symptom scores which effect uterine myoma patients using stepwise logistic regression model. Logistic regression analysis indicated as follows: In the control group composed of 558 outpatients, 18 items of DSOM were associated with myoma, 9 positively and 9 negatively, and the results showed that the correct rate was equal to 81.1%, sensitivity 72.8%, and specificity 84.9%. In 129 clinical trials data, 33 items of DSOM were associated with myoma, 18 positively and 15 negatively, and the results showed that the correct rate was equal to 85.8%, sensitivity 84.8%, and specificity 87.6%. In 687 outpatient and clinical trials data, 18 items of DSOM were associated with myoma, 10 positively and 8 negatively, and the results showed that the correct rate was equal to 82.8%, sensitivity 70.8%, and specificity 87.3%.

  • It is different from the DongEuiBoGam(東醫寶鑑) of Korea traditional medicine that China BeunJungLonChi(中醫辨證論治) was made for the scientific and systematic assortment of China medicine. Symptom(症)is more important than symptom complex(證)in china BeunJungLonChi at DonEuBoGam. And unusual symptoms generally are diseases in DongEuiBoGam. DongEuiBoGam is emphasis on body form, temper, body color, pulse(形氣色脈) and the functional difference of organs by body form and body color. there are many problems in order to unify the theory of Korea traditional medicine by china BeunJungLonChi. I suggest that a new assortment that can combine Sasang constitutional medicine(四象醫學), Constitution-Acupuncture(體質針), and Hyungsang medicine(四象醫學) will be necessary. BeunJungLonChi : diagnosis and treatment based on overall analysis of symptoms and signs, the cause, nature and location of the illness and the patient's physical condition according to the basic theories of traditional Chinese medicine

A case of Intracerebral Hemorhage with Alcoholic liver disease (알코올성 간질환을 동반한 뇌내출혈환자 치험 1례)

  • Recently there has been an increase in annual per capita consumption of alcohol beverage and the incidence of Alcoholic liver disease is steadily and significantly increasing. Alcoholic liver disease includes alcoholic fatty liver, alcoholic hepatitis, alcoholic cirrhosis and it may lead to systemic influence, in a case of CVA. This report is about one case's treatment for ICH with alcoholic liver disease. In this case, we administrated Taeumjowetang and did acupuncture treatment to a patient suffering from ICH with alcoholic liver disease and its withdrawal symptoms. After administration of Taeumjowetang medication, clinical symptoms and liver function were prominently improved. This report showed that Taeumjowetang might be useful for alcoholic liver disease.

  • This study aims to construct Constitution Information Bank to objectification of Sasang Constitution Diagnosis and Treatment. 422 cases clinically confirmed their Constitution by experts are collected retrospectively and 219 cases for constitution-family study are also collected in Constitution Information Bank from Nov. 2006 to Sept. 2007. The information include their physique, appearance, character, temper, symptom and treatment records. Their DNA cell lines also included in Constitution Information Bank for constitution-genome study. Constitution Information Bank is composed of constitution-clinical information -physique, appearance, character, temper, symptom and treatment records- and biological information -DNA genotyping data-. Total of 550 cases are collected in Constitution Information Bank from Nov. 2006 to Sept. 2007. Among these, 219 cases are for constitution-family study, especially, 74 cases are collected from Han's large family.

Facial Impression Classification for Sasang Constitution Diagnosis (사상체질 진단을 위한 얼굴인상 분류)

  • In this paper, we propose an efficient method to classify human facial impression using frontal face image. The features that represent the shape of eye, jaw and face are used. The proposed method employs PCA, LDA and SVM in series. PCA is used to project the feature space to a low dimensional subspace. LDA produces well separated classes in a low dimensional subspace even under severe variation. This results in good discriminating power for classification. SVM is used to classify the data. Human face has been classified for 8 facial impressions. The experiments have been performed for many face images, and show encouraging result.

A study on the Development of Traditional Korean Medicine Clinical Practice Guideline for Atopic Dermatitis (아토피피부염의 한의학 진료 지침 개발을 위한 사전 연구)

  • Background : The atopic dermatitis patient and their families are looking for safer herb-medicine treatments that possess therapeutic effects, but without the recurrence of symptoms and long-term harmful consequences that can result from other treatment. However, for many reasons, including methodological difficulties and lack of high quality study like randomized clinical study and systematic review, there are no consistent clinical guide line for atopic dermatitis in Traditional Korean Medicine. Objectives : Evidence-based clinical practice guidelines support clinical decision-making by making recommendations to guide clinical practice. The purpose of this study was to develop Traditional Korean Medicine clinical practice guideline for atopic dermatitis Result : The future guide should be based on scientific evidence and include the followings: (1) diagnosis of atopic dermatitis (2) the pattern identification of atopic dermatitis (3) classification of Sasang Constitution (4) efficacy assessment (5) treatment guideline (6) education for patients and care givers Conclusion : The development of Traditional Korean Medicine clinical practice guideline for atopic dermatitis is needed.
