Network Analysis of Herbs that are Frequently Prescribed for Osteoporosis with a Focus on Oasis Platform Research (골다공증 다빈도 처방과 구성 약물의 네트워크 분석 - 오아시스 검색을 중심으로)

  • Objectives: This study analyzed, through network analysis and data mining analysis, the relationship between herbs used in osteoporosis prescriptions, diversified the analysis of osteoporosis-related prescriptions, and analyzed the combination of herbs used in osteoporosis-related prescriptions. Methods: The prescriptions used in osteoporosis treatment and experiments were established by conducting a full survey of the papers published by the OASIS site. A database for osteoporosis-related prescriptions was established, herbs were extracted, and the frequency of frequent herbs and prescriptions were investigated using Excel (MS offices ver. 2013). Using the freeware R version 4.0.3 (2020-10-10), igraph, and arules package, network analysis was performed in the first second of prescription composition. Results: Among the osteoporosis-related prescriptions, the most studied prescriptions are as follows.: Yukmijihwang-tang (六味地黃湯) and Samul-tang (四物湯). In the osteoporosis prescription network, herbs with connection centrality, proximity centrality, mediation centrality, and eigenvector centrality appeared in the order of Rehmanniae Radix Preparata, Angelicae Gigantis Radix, Poria Sclerotium, Paeoniae Radix, and Glycyrrhizae Radix et Rhizoma. After extracting the herbal combination network, including the corresponding herbs, and clustering it, it can be divided into drugs of the Yukmijihwang-tang (六味地黃湯) series and the Samul-tang (四物湯). Conclusions: This study could assist researchers in diversifyingy formula analysis in future studies. Moreover, the herbal combination used in osteoporosis prescriptions could be used to search for osteoporosis prescriptions in other databases or to create a new prescription.

A Study of 123 prescriptions in high-chepter from Bang-Yak-Hap-Pyun (Mainly Basic-prescriptions) (방약합편(方藥合編) 상통(上統) 처방(處方)에 대한 방제학적(方劑學的) 고찰(考察))

  • This report describes the studies as follows through the research of Basic-prescriptions about 123 prescriptions in high-chepter(上統) from Bang-Yak-Hap-Pyun(方藥合編). There are 24 Basic-prescriptions including Sagunja-tang(四君子湯), Samul-tang(四物湯), Sogeonjung-tang(小建中湯), lijung-tang(理中湯), Dangguibohyul-tang(當歸補血湯), Yijin-tang(二陳湯), Gauwon-chun(擧元煎), Jeongweon-eum(貞元飮), and Yukmichiwhang-won(六味地黃元), etc. about 123 prescriptions in high-chepter(上統) from Bang-Yak-Hap-Pyun(方藥合編). There were many cases compounding the Basic-prescriptions each other. So I investigated those cases. The results were the other typed (blended) Basic-prescriptions including Palmul-tang(八物湯), Sipjeondaebo-tang(十全大補湯), Yukgunja-tang(六君子湯), Bojungyikkee-tang(補中益氣湯), SsangHwa-tang(雙和湯), and Gwibitang(歸脾湯). And I investigated about pathologies in connection with these Basic-prescriptions. Oriental Medical Pathology applied with these Basic-prescriptions were Giheo(氣虛), hyulheo(血虛), Gihyulguheo(氣血俱虛), Biheohan(脾虛寒), Giheo-dameum(氣虛痰飮), Giheohaham(氣虛下陷), Sineumheo(腎陰虛), etc. These results indicated that 123 prescriptions in high-chepter from Bang-Yak-Hap-Pyun are composed of Bojei(補劑) to treatment against many kinds of Infirmity and promote physiological function.

Concentration of Heavy Metals, Residual Pesticides and Sulfur Dioxide before/after a Decoction - In Prescription consist of Sipjeondaebo-tang - (전탕 전과 후의 중금속, 잔류농약 및 잔류이산화황의 농도변화 - 십전대보탕 구성처방을 중심으로 -)

  • Objective: To compare the contents of heavy metals, residual pesticides and sulfur dioxide before/after a decoction. Methods: The heavy metal contents before/after a decoction were measured by inductively-coupled plasma atomic emission spectrometer (ICP-AES) and mercury analyzer. In order to analyze pesticides in 5 samples we used simultaneous multi-residue analysis of pesticides by GC/ECD, which was followed by GC/MSD analysis to confirm the identity of the detected pesticide in each sample. In addition, the contents of sulfur dioxide ($SO_2$) were performed by Monier-Williams distillation method. Results: 1. The mean values of heavy metal contents (mg/kg) for the samples were as follows: Sipjeondaebo-tang (before decoction - Pb; 1.163, Cd; 0.257, As; 0.080 and Hg; 0.016, after decoction - Pb; 0.059, Cd; 0.007, As; 0.006 and Hg; 0.0003), Palmul-tang (before decoction - Pb; 1.181, Cd; 0.242, As; 0.152 and Hg; 0.014, after decoction - Pb; 0.067, Cd; 0.008, As; 0.008 and Hg; 0.0003), Sagunja-tang (before decoction - Pb; 1.285, Cd; 0.283, As; 0.063 and Hg; 0.012, after decoction - Pb; 0.047, Cd; 0.009, As; 0.004 and Hg; not detected) and Samul-tang (before decoction - Pb; 1.025, Cd; 0.169, As; 0.099 and Hg; 0.013, after decoction - Pb; 0.065, Cd; 0.007, As; 0.010 and Hg; 0.001). 2. Contents (mg/kg) of residual pesticides before/after a decoction were not detected in any samples. 3. Contents (mg/kg) of sulfur dioxide ($SO_2$) before a decoction in Sipjeondaebo-tang, Palmul-tang, Sagunja-tang and Samul-tang exhibited 5.0, 6.0, 14.0 and 6.9, respectively. However, contents of sulfur dioxide after a decoction were not detected in any samples. Conclusion: These results will be used to establish a criterion for heavy metals, residual pesticides and sulfur dioxide.

The Effect of Samul-tanggahyangbuja on Depression and Learning on Repeated Stress in Ovariectomized Rats (사물탕가향부자(四物湯加香附子)가 난소적출 흰쥐의 우울 및 학습에 미치는 영향)

  • Objectives: In this research, the effect of samul-tanggahyangbuja on depression and learning in ovariectomized rats subjected to repetitive stress were assessed. Samul-tanggahyangbuja is the prescription consisting of Samul-tang and Cyperi Rhizoma. Methods: Ovariectomized rats were repeatedly stressed over a 2-week period. After being orally medicated with samul-tanggahyangbuja (100 or 400 mg/kg), rats performed the Morris water maze test and forced swimming test, and social exploration was assessed in a behavior test. As well, sucrose intake was measured and measurements of blood serum corticosterone and the change of interleukin-$1{\beta}$ (IL-$1{\beta}$) and tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$) in blood samples were made. Results: 1. In the Morris water maze test, rats medicated with 100 mg samul-tanggahyangbuja mastered the maze in a shorter time on the 4th day in comparison with the control group, while rats medicated with 400 mg samul-tanggahyangbuja mastered the maze more quickly (p<0.05 on the 3rd day ; p<0.01 on the 4th day, as compared to control). 2. Immobility time in the forced swimming test was significantly decreased in rats receiving 400 mg samul-tanggahyangbuja compared with the control group (p<0.05). 3. Sucrose intake and active social behavior of rats receiving 400 mg samul-tanggahyangbuja were markedly increased in comparison with the control group (p<0.01). 4. Blood serum corticosterone measurements revealed decreased blood serum corticosterone level after medicating with samul-tanggahyangbuja. But it was not statistically significant. 5. Treatment with either dose of samul-tanggahyangbuja significantly reduced IL-$1{\beta}$ and TNF-${\alpha}$ (p<0.05). Conclusions: These results suggest that samul-tanggahyangbuja possesses the anti-depressant and cognitive-enhancing activities related to menopause.

Effects of SaengRyo-SaMul-tang on Atopic Dermatitis Induced by DNCB in Mice (생료사물탕(生料四物湯)이 DNCB로 유발된 생쥐의 아토피 피부염에 미치는 영향)

  • Objectives : Atopic Dermatitis (AD) is chronic skin disease characterized by allergic hypersensitivity reactions. Saeng-ryo-Samul-tang (SRSM) can treat skin disease by cooling down blood heat, cealering away congenital fever and detoxicating. This study was conducted to investigate the effects of SRSM on AD induced by 2,4-dinitrochlorobenzene (DNCB) in mice Methods : In this study, the effects of SRSM on changes in body weights, thicknesses of dorsum skin, thicknesses and weights of ear, changes of symptoms on the dorsum skin, histopathological degree of ear and dorsum skin, IL-10 and $TNF-{\alpha}$ in serum were observed. And the effects on the proliferation rates of splenocytes were also investigated in vivo and in vitro study. Results : In SRSM topical application (Topical) group, SRSM oral application (Oral) group and SRSM Combination (Combi) group thickness of dorsum skin decreased significantly. But in TPC, ORL and CBN group, weight of ear didn't show any changes, but thickness of ear decreased significantly. And TPC, ORL and CBN group showed meaningful effectiveness symptoms like desquamation and erythema on AD's clinical espect. In histopathological observation, spongiosis, edema and inflammatory cell infiltration of epidermal were remarkably diminished in TPC, ORL and CBN group. And SRSM diminished the proliferation rates of splenocytes in vivo and vitro study. Conclusions : The present study suggests that SRSM can significantly reduced symptoms of AD, therefore SRSM is effective to treatment of AD.

A study on the frequencies of medicinal herb combinations in the prescriptions of "Bangyakhappyeon(方藥合編)" ("방약합편(方藥合編)" 수록(收錄) 처방(處方) 내(內)의 약물(藥物) 조합 빈도(頻度) 연구)

  • Through analyzing the frequencies of medicinal herb combinations in the prescriptions of "Bangyakhappyeon(方藥合編)", we could understand the characteristics of prescription composition and author's medical thoughts. So we conducted this study that analyze combination of 324 medicinal herbs in 467 prescriptions, and found meaningful combinations of 5,045. Among these, the most meaningful combination is the one of 10 herbs, Glycyrrhiza uralensis Fischer, Angelica gigas Nakai, Ziziphus jujube Mille, Atractylodes japonica Koidzumi, Poria cocas Wolf, Zingiber officinale, Rehmanniae radix preparata, Cinnamomum loureirii, Panax ginseng C.A.Meyer, Astragalus membranaceus. This combination is Sibjeondaebo-tang(Shiquandabu-tang) without Cnidium officinale and Paeonia japonica which compose Samul-tang(Siwu-tang). It means this combination is kind of modified Sibjeondaebo-tang(Shiquandabu-tang), which is more effective in tonifying qi than tonifying blood. Prescriptions of "Bangyakhappyeon(方藥合編)" involve herbs mostly used to improve weak and frail health and the herbs which are more effective in tonifying qi than tonifying blood. Through this study, we know the author' s one of medical view points on tonifying qi and blood, attaching importance to tonifying qi relatively to tonifying blood.

The Effects of Very Low Calorie Diet Using Meal Replacements that Contain the Wild Grass Extracts Based on Samul-tang Prescription on Weight Reduction and Health Promotion in the Obese Premenopausal Women (산야초 추출물을 함유한 식사 대용식을 이용한 초저열량 식사요법이 성인 비만 여성들의 체중감량과 건강 개선에 미치는 영향)

  • This study was performed to investigate the effects of very low calorie diet (VLCD) using meal replacements that contain the wild grass extracts that are consisted of mainly Samul-tang ingredients on weight reduction and health promotion in the obese women (BMI$\geq$25 kg/$m^2$) for four weeks. Fifty six premenopausal women participated in this experiment. Each subject was either classified as part of the control group (C group, n=28) that consumed 3 regular meals a day or the meal replacement group (MR group, n=28) that consumed 1 regular meal and 2 meal replacements within 600 kcal/day. Anthropometric measurements, body composition, health related factors, and psychological factors were assessed before (the initial) and after (the 4$^{th}$ week) the study. Obesity was significantly decreased after the diet intervention. Anthropometric measurements such as weight, waist and hip circumference, and BMI and body composition such as body fat, and fat mass were significantly decreased in all groups after the diet intervention. Anthropometric measurements and body composition of the MR group were significantly decreased more than those of the C group. Serum total cholesterol was significantly decreased in the MR groups but not in the C group and serum triglyceride concentration was significantly decreased in all groups after the diet intervention. All the biochemical measurements were within normal ranges after the diet intervention. Psychological factors such as stress and depression were significantly decreased in two groups after the diet intervention. Therefore, VLCD using meal replacements is very effective for weight reduction in the obese premenopausal women.

Effects of Very Low Calorie Diet using Meal Replacements on Psychological Factors and Quality of Life in the Obese Women Aged Twenties (식사대용식을 이용한 초저열량 식사요법이 20대 비만 여성들의 심리적 요인과 삶의 질에 미치는 효과)

  • This study was performed to investigate the effects of very low calorie diet (VLCD) using meal replacements that contain the wild grass extracts based on Samul-tang ingredients on psychological factors and quality of life in the obese women $(BMI{\geq}25kg/m^2)$ for four weeks. Seventy five women $(20{\leq}age<26)$ participated in this experiment. Subjects were randomly classified three groups: 1) General diet group (GD roup, n=27) consumed 3 regular meals within 600 kcal/day 2) Meal replacements group (MR group, n=27) consumed 1 regular meal and 2 meal replacements within 600 kcal/day 3) Herbal Meal replacements group (HMR group, n=27) consumed 1 regular meal and 2 meal replacements within 600 kcal/day. Physical factors (weight, BMI, fat (%)) of the HMR group significantly decreased more than those of GD and MR groups. Moreover, binge eating habit and environmental factors (surrounding support, emotional reaction, expression of opinion) of the HMR group significantly decreased more than those of GD and MR groups. Psychological factor and quality of life were no significant differences among three groups during the experimental period, because both were significantly decreased in all groups after 4 weeks. Therefore, very low calorie diet using meal replacements that contain the wild grass extracts based on Samul-tang ingredients for 4 weeks was effective on improvement of psychological factor and quality of life as well as weight reduction in the obese premenopausal women.

A Case of Urticaria occurred to a Breastfeeding Patient in Postpartum Period Treated with Korean Medicine (모유 수유 중인 산욕기 환자의 두드러기 치험 1례)

  • Objectives : The aim of this study is to report the effect of herbal medicine and acupuncture on a breastfeeding woman's postpartum urticaria which was unresponsive to a 1st generation antihistamine and a topical steroid. Methods : Wheals and pruritus occurred to a 36-year-old breastfeeding woman 2 days after the parturition. Symptoms were unresponsive to about a week of systemic antihistamine and topical steroid treatments. GamiSamul-tang herbal decoction was administered three times a day for the first 21 days and twice a day for the next 10 days of the treatment. Acupuncture treatment was applied to 12 acupoints (both sides of LI04, LR03, ST36, LI11, SP10, SP6) for 15 minutes once a week, 5 times in total. The patient's symptom was evaluated with Weekly Urticaria Activity Scale (UAS7) and Visual Analogue Scale for Worst Pruritus within 24 hours ($VAS_{Pruritus}$) each time the patient visited out-patient department. Results : The scores of UAS7 and $VAS_{Pruritus}$ at the baseline were 38 and 8.2 respectively. 10 days after the herbal medicine and acupuncture treatment began, both decreased to 0 and wheal, pruritus, erythema didn't appeared again for the rest of the follow up periods. Conclusions : This study shows the possibility of using GamiSamul-tang and acupuncture to treat postpartum urticaria.

The Effects of Samul-tang-ga-dansam for Wound Healing (사물탕(四物湯) 가(加) 단참(丹參)의 상처 치료에 대한 효과)

  • Objectives The purpose of this study was to evaluate the antioxidant, anti-inflammatory and wound healing effects of Samul-tang-ga-dansam water extract (SD) in wound-induced mice. Methods The mice were divided into five groups (n=7): the normal group, the control group, the positive control group, the low-dose SD group and the high-dose SD group. The normal group had no wounds and the other groups were wounded on the back with a leather punch. Distilled water was administered to the control group, 200 mg/kg of vitamin E was administered to the positive control group. In the low-dose SD group and the high-dose SD group, 1.23 g/kg and 2.47 g/kg of SD were administered, respectively. Antioxidant and anti-inflammatory protein levels were evaluated using western blot analysis. Skin tissue was analyzed by H&E, Masson's trichrome staining method. Results Oral administration of the SD significantly reduced the visible skin damage and decreased the reactive oxygen species and ONOO- activity of the serum. It significantly increased heme oxygenase-1, superoxide dismutase, catalase, GPx-1/2, Nrf2 and Keap-1 which are antioxidant-related factors in skin tissue and reduced NF-κB p65, inducible nitric oxide synthase, cyclooxygenase-2, tumor necrosis factor α, interleukin (IL)-1β, IL-6 which are inflammation-related factors. Also, SD significantly decreased NOX2, p22phox and p47phox and increased α-smooth muscle actin and COL1A1 protein expression in fibroblasts involved in connective tissue repair. According to histological examination, the thickened epithelial layer was thinned and collagen fibers were increased to accelerate wound healing. Conclusions It is suggested that Samul-tang-ga-dansam has antioxidant and anti-inflammatory effects and promotes wound tissue repair.
