Intraperitoneal Paclitaxel Combined with S-1 Plus Oxaliplatin for Advanced Gastric Cancer with Peritoneal Metastasis: a Phase I Study

  • Purpose: We designed a new regimen by combining intraperitoneal (IP) paclitaxel (PTX) with systemic S-1 plus oxaliplatin (SOX) for the treatment of advanced gastric cancer with peritoneal metastasis. This dose-escalation study aimed to determine the maximum tolerated dose (MTD) and recommended dose (RD) of IP PTX administered weekly to patients. Materials and Methods: Eight cycles of IP PTX plus SOX regimen were administered to the patients. S-1 was administered orally twice daily at a dose of 80 mg/m2/day for 14 consecutive days, followed by 7 days of rest. Intravenous oxaliplatin was administered at a fixed dose of 100 mg/m2 on day 1, while IP PTX was administered on days 1 and 8. The initial dose of IP PTX was 40 mg/m2, and the dose escalation was set in units of 20 mg/m2 up to 80 mg/m2. Dose-limiting toxicities (DLTs) were defined as grade 3 non-hematologic toxicities, grade 4 leukopenia, grade 3 febrile neutropenia, and grade 3 thrombocytopenia. Results: Nine patients were included in the study. No DLTs were observed in any of the enrolled patients. Therefore, the MTD was not reached, and the RD of IP PTX was determined to be 80 mg/m2. Four patients (44%) showed a decreased peritoneal cancer index score on second-look laparoscopic examination. Conclusions: The present study determined the dose for further clinical trials of IP PTX to be 80 mg/m2, when combined with a systemic SOX regimen.

The Evaluation of the Effect of Herbal Extract on Osteoarthritis: In Vitro and In Vivo Study

  • In this study, the anti-osteoarthritis effects of Cynanchum wilfordii, Phlomis umbrosa, and Angelica gigas extract (CPAE), observed and confirmed in previously clinical studies were further investigated by in vitro and in vivo studies. Anabolic biomarkers related to healthy cartilage maintenance, such as aggrecan, type II collagen ${\alpha}$-1 (Col2a1), sex determining region Y-box-9 (Sox-9), and catabolic biomarkers related to osteoarthritis, such as cyclooxygenase-2 (Cox-2), matrix metalloproteinase-13 (Mmp13), and nuclear factor kappa-light-chain-enhancer of activated B cells ($Nf{\kappa}b$), were evaluated by quantitative reverse transcriptase polymerase chain reaction and reporter gene assay. In vitro study results showed significant changes in both anabolic and catabolic biomarkers. For anabolic factors, significant changes in the level of aggrecan (P<0.05), Col2a1 (P<0.05), and Sox-9 (P<0.01) activation were shown after treatment of cartilage cells with CPAE (50 ng/mL) with similar efficacy compared to insulin growth factor, the positive control (100 ng/mL). For catabolic factors, significant changes in the inhibition activity of Cox-2 (P<0.05), Mmp13 (P<0.01), and $Nf{\kappa}b$ (P<0.05) were shown for CPAE (50 ng/mL) with similar efficacy compared to Celecoxib, the positive control ($10{\mu}M$). In the in vivo carrageenan-induced paw edema model study results showed that CPAE-treated groups (100 mg/kg) and Celecoxib-treated groups (60 mg/kg) showed comparably significant efficacy of inhibition by 37.1% and 52.1%, respectively. Furthermore, CPAE (200 mg/kg) showed similar effect to Celecoxib (60 mg/kg) with an inhibition rate of 54.3%. This result confirms that CPAE effectively inhibited the inflammation-induced osteoarthritis symptoms.

  • IMO has enacted a convention that air pollution due to emissions of ships and sulfur oxides emissions should be significantly reduced by 2020. Based on the current support policies, this work intended to draw up the government support plans required by the shipping companies. Analytic Hierarchy Process analysis was done with factors derived from brainstorming and literature studies. The analysis results showed that the cost factor was generally the most important criterion and the Financial Aid was relatively more important within this cost factor. The policy implications for the regulation of sulfur oxides emissions was provided.

선박에 기인한 대기오염물질 배출량 산정 연구 -광양항과 울산항을 중심으로 (A Study on Estimating Ship Emission - Focusing on Gwangyang Port and Ulsan Port)

  • 최근 항만에서 기인한 대기오염물질에 대한 심각성이 고조되고 있다. 한국은 무역의존도가 매우 높고 수 출입 교역량의 99.7%가 해상을 통해 운송되고 있어, 항만 대기오염의 정도를 파악하고 이에 대비하기 위한 친환경 정책이 필요하다. 이에 본 연구에서는 많은 물동량을 취급함에도 불구하고 관련 연구가 적었던 광양항과 울산항을 대상으로 선박 접안시 발생하는 대기오염물질 배출량을 산정하였다. 이를 위해 2017년 기준으로 선박의 활동 및 제원자료를 수집하고, 해외 환경기관인 EEA와 EPA 방법론 및 계수를 적용하였다. 그 결과, 광양항은 CO 253.09톤, NOx 1986.61톤, SOx 684.01톤, $PM_{10}$ 47.88톤, $PM_{2.5}$는 44.69톤, VOC 61.56톤, NH3 0.24톤의 대기오염물질을 배출하였다. 울산항의 경우 CO 212.28톤, NOx 1712.54톤, SOx 573.72톤, $PM_{10}$ 40.16톤, $PM_{2.5}$는 37.48톤, VOC 51.63톤, $NH_3$ 0.20톤을 배출하였다. 이어서 선박 접안시 적용 가능한 친환경 정책인 AMP의 단계적 도입 방안을 제시하였다. 본 연구는 광양항과 울산항을 대상으로 대기오염 정도를 파악한 것으로, 이를 통해 현황을 진단하고, 향후 관련 정책 수립을 위한 참고자료로 활용 가능하다.

Efficacy of Different Number of XELOX or SOX Chemotherapy Cycles After D2 Resection for Stage III Gastric Cancer

  • Purpose: We aimed to explore whether the prognosis of patients treated with capecitabine and oxaliplatin (XELOX) or S-1 and oxaliplatin (SOX) regimens who received fewer cycles of chemotherapy after D2 radical resection for gastric cancer (GC) would be non-inferior to that of patients who received the standard number of cycles of chemotherapy. Materials and Methods: Data on patients who received XELOX or SOX chemotherapy after undergoing D2 radical resection at Harbin Medical University Cancer Hospital between January 2011 and May 2016 were collected. Results: In patients who received 4, 6, and 8 cycles of chemotherapy, the 5-year overall survival (OS) rates were 59.4%, 64.8%, and 62.7%, respectively. Compared to patients who received 4 cycles of chemotherapy, those who received 6 cycles (hazard ratio [HR], 0.882; 95% confidence interval [CI], 0.599-1.299; P=0.52) or 8 cycles (HR, 0.882; 95% CI, 0.533-1.458; P=0.62) of chemotherapy did not exhibit significantly prolonged OS. The 3-year disease-free survival (DFS) rate of patients who received 4, 6, and 8 cycles of chemotherapy was 62.1%, 67.2%, and 60.8%, respectively. Compared to patients who received 4 cycles of chemotherapy, those who received 6 cycles (HR, 0.835; 95% CI, 0.572-1.221; P=0.35) or 8 cycles (HR, 0.972; 95% CI, 0.606-1.558; P=0.91) of chemotherapy did not show significantly prolonged DFS. However, the 3-year DFS and 5-year OS rates of patients who received 6 cycles of chemotherapy appeared to be superior to those of patients who received 4 and 8 cycles of chemotherapy. Conclusions: For patients with stage III GC, 4 to 6 cycles of XELOX or SOX chemotherapy may be a favorable option. This study provides a rationale for further randomized clinical trials.

국내 주요 항구도시의 선박 배기가스 배출량 산정 및 사회적 비용 추정 (Estimation of Atmospheric Pollutant Emissions from Vessels in Major Harbor Cities in Korea and related Social Cost)

  • 본 연구는 국내 항구도시 인천, 목포, 광양, 부산, 울산의 대기오염물질 배출특성, 대형선박 배기가스 배출량 및 사회적 비용을 산정하여 배출가스 저감방안의 필요성을 제시하였다. 부산은 선박 입항수가 많고 목포는 선박 입항수는 적지만 선박 배기가스에 의한 배출기여도는 두 항구도시 모두 높았다. 울산은 세계적 수준의 중화학공업, 광양은 제철소, 인천은 제조업과 선박 입항수가 목포보다 많았지만 배출기여도는 낮았다. 선박 배기가스 배출량 산정 결과, CO2의 배출량이 가장 많고 다음으로는 NOx, SOx으로 배출량이 많았다. 선종별로는 부산, 울산, 인천은 유조선, 광양은 일반화물선, 목포는 여객선에서 각각 많았다. 사회적 비용 결과, 부산은 배출량이 많은 영향으로 사회적 비용이 높았으며, 항목별로는 PM이 높았다. 저황유 사용으로 PM, SOx 배출량을 직접적으로 줄이고, NOx의 배출량을 간접적으로 줄 일수 있다. 하지만 선박 배기가스 중 가장 많은 배출량을 보인 CO2를 저감하기 위해서는 저황유의 보급만으로는 부족하다. CO2 배출량 감축을 위해서는 육상전원공급장치(Alternative Maritime Power)를 사용하는 등 화석연료를 사용하지 않는 저감방안 수립의 필요성을 제기하였다.

The Expression Patterns of Cdc25A, Cdc25B, Sox2 and Mnb in Central Nervous System in Early Chicken Embryos

  • The sense and antisense digoxigenin-labeled RNA probes of four genes, Cdc25A, Cdc25B, Sox2 and Mnb, were produced by using SP6 and T7 RNA polymerases, respectively, and in vitro transcription. Expression patterns of the four genes were detected by in situ hybridization in HH (Hamburger and Hamilton) stage 10 chick embryos. In general, expression patterns of the four genes were similar. mRNA of the four genes was mostly restricted to the entire CNS (central nervous system). All were confined to an identical region, neural tube, neural groove and caudal neural plate, corresponding to the notochord or spinal cord, but there was some distinction in specific region or in concentration, for example in somites. The overlap in expression at the same developmental stage in the CNS suggests that the four genes may be functional similar or related in CNS development. Expression patterns of the four genes support specific roles of these regulators in the developing CNS.

A Study on the Enhancement of Inventories for Precursors (NOx, SOx) Released from Open Burning of Agricultural Waste Vinyl Causing the Secondary Generation of Particulate Matters

  • Background and objective: While response measures to particulate matters in rural areas are limited due to poor inventory record keeping in the agricultural sector, it is necessary to control agricultural waste vinyl and the emission of precursors released from open burning and the secondary generation of particulate matters. Currently, the open burning emission calculation method uses the definition prescribed in CAPSS by the National Institute of Environmental Research. Methods: This study presented an open burning emission calculation formula for agricultural waste vinyl, which is included as agricultural waste. As for activity data, the open burning ratio of agricultural waste vinyl, and the annual incineration volume provided in the Status Survey by the Ministry of Agriculture, Food, and Rural Affairs were applied. The emission factor was generated through incineration tests on three agricultural plastic film samples collected by the Korea Environment Corporation. Results: Among precursors, SOx and NOx were selected and their emission features were monitored with incineration experiment infrastructure based on the EPA 5G method. The highest emission concentration by agricultural waste type was concentrated in the first and second quarters. As for emission factor of SO2, it was calculated at 98.25 g/kg for mulching-use LDPE, 52.31 g/kg for greenhouse-use LDPE, and 14.40 g/kg for HDPE. As for NOx, it was calculated at 18.21 g/kg for mulching-use LDPE, 16.49 g/kg for greenhouse-use LDPE, and 10.67 g/kg for HDPE. Conclusion: This test confirmed the incineration features of PE-based plastics, ascertained the SOx emission factor that had not been included in open burning in the past, and established that low NOx emission concentration is interfered by soil mixed with livestock excretions. The findings from this study are expected to contribute to improving the system for controlling air pollutants in rural environments.

국내 산업 및 시도별 대기오염물질 배출량자료를 이용한 미세먼지 형성 가능성 및 인체 호흡기 영향 평가추정 (Assessment and Estimation of Particulate Matter Formation Potential and Respiratory Effects from Air Emission Matters in Industrial Sectors and Cities/Regions)

  • 많은 산업 및 도로이동오염원 등으로부터 발생되는 미세먼지는 인간건강과 호흡기에 큰 영향을 미치고 있으며 이에 대한 중요성이 점차 증가하고 있다. 세계보건기구(WHO)에서도 미세먼지($PM_{10}$) 및 초미세먼지$PM_{2.5}$)를 석면 및 비소 등의 물질과 같은 등급인 1급 발암물질로 지정한 이후로 우리나라에서도 지속적인 관심과 연구를 진행해오고 있다. 현재 환경부와 각 지자체별로 미세먼지 및 대기오염정보를 구축하여 제공하고 있지만, 특히 미세먼지 주의보 및 경보발령에 있어서 미세먼지 $PM_{10}$$PM_{2.5}$ 자료만을 활용하고 있고 미세먼지형성에 영향을 주는NOx, SOx, $NH_3$자료는 활용 및 고려를 하지 않고 있다. 또한 국내 산업별 및 지자체별로 세부적인 미세먼지형성 가능성(particulate matter formation potential) 및 발생되는 미세먼지로 인한 인체호흡기 영향평가(respiratory effects)와 관련된 연구는 많이 진행이 되고 있지 않다. 이에 본 연구의 목적은 국내 11개 산업별 및 시도별로 2001년과 2013년 환경부 및 국립환경과학원에서 제시하고 있는 NOx, SOx, $PM_{10}$, $NH_3$ 자료를 활용하여 미세먼지형성(particulate matter formation potential) 평가와 이로 인한 인체 호흡기 영향을 평가 및 산정하여 비교 제시하고자 하였다. 본 연구결과로는 산업별 및 시도별로 미세먼지형성과 인간건강에 영향을 제시하였으며, 향후 미세먼지 관련 연구에 중요하게 활용할 수 있을 것으로 사료된다.
