임상가를 위한 특집 1 - 니켈티타늄 전동파일의 파절과 임상적 예후 (Fracture of Nickel-Titanium Rotary Instruments and its Clinical Prognosis)

  • Nickel-Titanium(NiTi) rotary instruments have brought a big step toward "efficient" practice of endodontic procedure. The rotary files help clinicians to reduce their working time and also increase the clinical success rate with minimal procedural errors by stainless steel instruments. In spite of these advantages, NiTi instruments still have a few drawbacks including unpredictable fatigue fracture. Clinicians may reduce the potential risk of instruments fracture by following some clinical guidelines for rotary instruments. In some clinical cases of instruments fracture, we may try to remove the instruments' fragments or bypass the fragment to reach the apical canal. In some limited cases, the fractured instruments' fragments would not jeopardize the clinical prognosis of root canal treatment. However, it is impossible to be overemphasized that the fragment removal is more difficult than the prevention of fracture. Clinicians need to understand the fracture mechanisms and, in clinic, need to discard the used instruments timely.

Preference of undergraduate students after first experience on nickel-titanium endodontic instruments

  • Objectives: This study aimed to compare two nickel-titanium systems (rotary vs. reciprocating) for their acceptance by undergraduate students who experienced nickel-titanium (NiTi) instruments for the first time. Materials and Methods: Eighty-one sophomore dental students were first taught on manual root canal preparation with stainless-steel files. After that, they were instructed on the use of ProTaper Universal system (PTU, Dentsply Maillefer), then the WaveOne (WO, Dentsply Maillefer). They practiced with each system on 2 extracted molars, before using those files to shape the buccal or mesial canals of additional first molars. A questionnaire was completed after using each file system, seeking students' perception about 'Ease of use', 'Flexibility', 'Cutting-efficiency', 'Screwing-effect', 'Feeling-safety', and 'Instrumentation-time' of the NiTi files, relative to stainless-steel instrumentation, on a 5-point Likert-type scale. They were also requested to indicate their preference between the two systems. Data was compared between groups using t-test, and with Chi-square test for correlation of each perception value with the preferred choice (p = 0.05). Results: Among the 81 students, 55 indicated their preferred file system as WO and 22 as PTU. All scores were greater than 4 (better) for both systems, compared with stainless-steel files, except for 'Screwing-effect' for PTU. The scores for WO in the categories of 'Flexibility', 'Screwing-effect', and 'Feeling-safety' were significantly higher scores than those of PTU. A significant association between the 'Screwing-effect' and students' preference for WO was observed. Conclusions: Novice operators preferred nickel-titanium instruments to stainless-steel, and majority of them opted for reciprocating file instead of continuous rotating system.

Effect of glide path preparation with PathFile and ProGlider on the cyclic fatigue resistance of WaveOne nickel-titanium files

  • Objectives: The aim of this study was to investigate the effect of glide path preparation with PathFile and ProGlider nickel-titanium (NiTi) files on the cyclic fatigue resistance of WaveOne NiTi files. Materials and Methods: Forty-four WaveOne Primary files were used and divided into four groups (n = 11). In the first group (0 WaveOne), the WaveOne Primary files served as a control group and were not used on acrylic blocks. In the 1 WaveOne Group, acrylic blocks were prepared using only WaveOne Primary files, and in the PF+WaveOne group and PG+WaveOne groups, acrylic blocks were first prepared with PathFile or ProGlider NiTi files, respectively, followed by the use of WaveOne Primary files. All the WaveOne Primary files were then subjected to cyclic fatigue testing. The number of cycles to failure was calculated and the data were statistically analyzed using one-way analysis of variance (ANOVA) and the Tukey honest significant difference multiple-comparison test at a 5% significance level. Results: The highest number of cycles to failure was found in the control group, and the lowest numbers were found in the 1 WaveOne group and the PF+WaveOne group. Significant differences were found among the 1 WaveOne, PF+WaveOne, and control groups (p < 0.05). No statistically significant differences were found between the PG+WaveOne group and the other three groups (p > 0.05). Conclusion: Glide path preparation with NiTi rotary files did not affect the cyclic fatigue resistance of WaveOne Primary files used on acrylic blocks.

Buckling resistance, bending stiffness, and torsional resistance of various instruments for canal exploration and glide path preparation

  • Objectives: This study compared the mechanical properties of various instruments for canal exploration and glide-path preparations. Materials and Methods: The buckling resistance, bending stiffness, ultimate torsional strength, and fracture angle under torsional load were compared for C+ file (CP, Dentsply Maillefer), M access K-file (MA, Dentsply Maillefer), Mani K-file (MN, Mani), and NiTiFlex K-file (NT, Dentsply Maillefer). The files of ISO size #15 and a shaft length of 25 mm were selected. For measuring buckling resistance (n = 10), the files were loaded in the axial direction of the shaft, and the maximum load was measured during the files' deflection. The files (n = 10) were fixed at 3 mm from the tip and then bent $45^{\circ}$ with respect to their long axis, while the bending force was recorded by a load cell. For measuring the torsional properties, the files (n = 10) were also fixed at 3 mm, and clockwise rotations (2 rpm) were applied to the files in a straight state. The torsional load and the distortion angle were recorded until the files succumbed to the torque. Results: The CP was shown to require the highest load to buckle and bend the files, and the NT showed the least. While MA and MN showed similar buckling resistances, MN showed higher bending stiffness than MA. The NT had the lowest bending stiffness and ultimate torsional strength (p < 0.05). Conclusions: The tested instruments showed different mechanical properties depending on the evaluated parameters. CP and NT files were revealed to be the stiffest and the most flexible instruments, respectively.

니켈티타늄 파일의 부식에 영향을 미치는 다양한 환경 조건 비교 (Comparative analysis of various corrosive environmental conditions for NiTi rotary files)

  • 이 연구에서는 다양한 부식 환경하에서 니켈티타늄 파일에 대한 부식 경향을 전기화학적 검사와 시각적 검사를 이용해 비교 평가하였다. 각 45개의 21 mm ProTaper Universal 52와 21 mm #25/0.06의 Hero642 file을 기계적 하중 여부, 저장 용액(증류수 및 차아염소산나트륨), 저장 온도(22$^{\circ}C$ 및 55$^{\circ}C$), 파일 종류에 따라 18개의 그룹(n = 5)으로 나누었다. 40개의 발거 치아 근관을 성형한 후 18시간 동안 저장 용액에 조건에 따라 보관하였다. 부식 산물의 생성 여부나 변색 등을 관찰하고 주사전자현미경을 이용해 각 시편을 다양한 배율에서 관찰하였으며 에너지 분산형 X선 분광기로 성분 분석을 하였다 전기화학적분극시험을 통하여 새 파일과 근관 성형에 사용한 파일의 부식저항성을 검사하였다. 본 실험 조건하에서는 저장 용액의 종류와 보관 온도가 니켈 티타늄 파일의 부식에 영향을 주는 것으로 나타났다. 또한 이종 금속이 존재하는 경우 갈바닉 부식의 영향으로 인해니켈 티타늄 파일의 부식 경향이 증가할 수도 있음을 보였다.

수종의 NiTi 전동 파일 단면 형태 비교 및 pecking motion의 사용방법이 피로 파절에 미치는 영향 (COMPARATIVE STUDY ON MORPHOLOGY OF CROSS-SECTION AND CYCLIC FATIGUE TEST WITH DIFFERENT ROTARY NITI FILES AND HANDLING METHODS)

  • 본 연구는 현재 시판중인 $Profile^{(R)},\;ProTaper^{(R)},\;K3^{(R)},\;Hero\;642^{(R)},\;Hero\;Shaper^{(R)}$를 단면을 비교하고 이들 5가지 NiTi 파일과 사용방법에 따른 피로 파절 소요 시간을 비교하고자 시행되었다. 각 NiTi전동파일의 치근단 1/3을 temporary resin에 매몰시키고 다이아몬드 표면처리된 버로 잘라내어, 초음파를 이용하여 세척 후, 주사전자현미경으로 단면을 관찰하였다. 주기적인 피로 실험을 수행하기 위해, 회전속도, pecking 거리를 자동적으로 조절할 수 있는 장치를 제작하고 파일을 만곡된 상태를 재현하기 위하여 15도 경사진 금속 토막에 위치시키고 분당 300회전의 속도로 회전시켜 파일이 파절될 때까지의 시간을 측정하였다. 사용방법에 따른 실험군은 동적군과 정적군으로 나누고 pecking 거리는 3 mm, 6 mm로 하였다. 실험결과 동적군 보다는 정적군의 피로파절 소요시간이 통계학적으로 유의차 있게 짧았으며 3 mm, 6 mm의 pecking distance에 따른 영향은 나타나지 않았다. 결론적으로 NiTi전동파일은 pecking motion으로 사용하는 것이 피로파절을 줄이는데 도움이 될 것으로 사료된다.

만곡 근관에서 삼각 혹은 장방형 단면 구조의 니켈-티타늄 파일 응력 분포에 관한 3차원 유한요소 연구 (STRESS DISTRIBUTION FOR NITI FILES OF TRIANGULAR BASED AND RECTANGULAR BASED CROSS-SECTIONS USING 3-DIMENSIONAL FINITE ELEMENT ANALYSIS)

  • 이 연구의 목적은 3차원 유한요소 모형 분석을 이용하여 삼각 대칭형과 5형태의 단면 및 변형된 사각 형태인 장방형 단면으로 분류되는 네 종류의 니켈티타늄 파일이 만곡 근관 적용 시의 응력 분포를 비교하는 것이다. 삼각 대칭형 단면 구조의 ProFile #30 / .06과 Heroshaper #30 / .06, 장방형 단면구조의 Mtwo #30 / .05와 NRT #30/ .06 파일을 마이크로컴퓨터 단층촬영을 하고 reverse engineering을 통하여 파일의 구조를 얻고 삼차원 유한요소모형을 제작하였다. 모형 근관 내에서 파일이 근관장끝까지 진입하여 회전할 때 발생하는von Mises 응력 분포 및 파일의 제거후의 잔류 음력의 분포양상을 ABAQUS 프로그램을 이용하여 비교하였다. 근관 내 회전 시에 발생하는 응력을 관찰하였을 때 NRT 파일에서 가장 큰 응력을 나타냈으며, 각 파일에서의 최고 응력과 최저 응력을 비교하였을 때 Mtwo파일에서 가장 큰 차이가 나타났다. 응력의 내부 분포 경향을 보았을 때 장방형 구조의 단면을 가진 Mtwo 및 NRT 파일에서 불연속적인 응력의 집중 부위가 관찰되었으며, 근관 외부로 파일을 제거하여 탄성 회복이 일어난 후의 잔류 응력도 NRT 파일에서 가장 높게 나타났다. 이상의 결과로 유추할 때, 삼각대칭 단면 구조를 가진 파일보다 S-형태의 단면과 변형된 사각 형태의 단면 등의 장방형 단면 구조의 파일이 파절의 위험성이 더 클 것으로 사료된다.
