Traumatic injuries in the young peranent dentition are common, but root fractures, defined as fractures in volving dentin, cementum and pulp, are relatively uncommon. Case 1 is a 9-year-old boy who had a horizontal root fracture of his maxillary right central incisor in the apical third. Root canal therapy was performed in coronal segment and calcium hydroxide therapy was initiated. Six months after treatment, a periapical radiograph showed calcific tissue formation and normal root development. 1 year and 3 months later, the canal was permanently obturated with gutta-percha. Case 2 is a 7-year-old girl who had a vertical root fracture of her maxillary right central incisor. Fractured tooth was intentionally extracted atraumatically, and then the separated fragments are bonded with resin cement. the restored tooth was replanted into the original socket. Recalls up to 8 months showed normal mobility and no periapical pathosis. In these cases, we performed conservative treatment. Clinical and radiographic examination showed no pathosis or abnormality of the teeth and periodontal tissue.

Deciduous teeth can be extracted for two reasons, one due to the physiologic resorption and the other by the inflammation at the apex after traumatic injury. Physiologic resorption may be different from pathologic resorption in timing and mechanism. Therefore we resumed the different features of physiologic and pathologic resorption root surfaces. Many previous studies showed micromorphology of resorbed surface of roots of deciduous teeth. But, few studies compared physiological and pathological root resorption surfaces. In this study, we carefully observed microscopic morphologies of those two different root surfaces by scanning electron microscope and histologic features by light microscope. The resultant differences between physiologic and pathologic resorption surfaces of deciduous teeth were as follows: 1. The morphology of pathologic resorption lacunae due to inflammation varied in size and shape with irregular boundaries compared with the physiologic areas from scanning electron microscope observations. 2. From light microscope observations, several large resorption fossae containing numerous resorption lacunae were found, whereas the resorption lacunae were irregular in shape with pathologic resorption surface. 3. Numerous multinucleated giant cells were closely attached to the physiologic resorption lacunae, whereas several kinds of mesenchymal cells with numerous inflammatory cells were found in the areas adjacent to the pathologic resorption surface. 4. Light microscope findings showed that compensating cementum formation took place along some of the areas of inflammatory dentinal resorption. In conclusion, several morphological differences were present between physiologic and pathologic root resorption surfaces of human deciduous teeth. The future studies should include cytochemistry to clarify the cellular roles in resorption process observations of pulpal surfaces of coronal and radicular dentin to and the changes that occur in each phase of human deciduous tooth resorption.

The purpose of this study was to evaluate the structural change of root surface and the occlusion of dentinal tubule following $CO_2$ laser treatment. Seven extracted healthy human premolar werw curetted, sectioned, and four specimens were randomly assigned to each of 6 different treatment groups : 1) untreated EDTA etched control: 2) root plande only: 3) $CO_2$ laser treated with 2W mode 6(10msec/pulse, 20pps) for 1 minute: 4) $CO_2$ laser treated with 2W mode 6(lOmsec/pulse, 20pps) for 2 minutes: 5) $CO_2$ laser treated with 2W mode 7(20msec/pulse, 20pps) for 1 minute: 6) $CO_2$ laser treated with 2W mode 7(20msec/pulse, 20pps) for 2 minutes. Following the prescribed treatment, the specimens were prepared for SEM evaluation. Results showed that $CO_2$ laser may be effective to occlude dentinal tubules tor dentin sensitivity treatment. The effect of dentinal tubule occlusion was enhanced with increasing the total energy level lased to specimen regardless of lasing mode. The structural changes of root surfaces were restricted to superficies, and these changes included fissuring, charring, crater formation over the smooth lava like texture. The charring and crater formation implying root damage was observed in the case of the longer duration of a pulse. The results of the present study suggests that the pulsed $CO_2$ laser with shorter pulse duration and longer exposure time can be used effectively in order to obtain the optimal dentinal tubule occlusion with minimal root damage.

Tooth development involves bud, cap, bell and hard tissue formation stages, each of which is tightly controlled by regulatory molecules. The aim of this study was to identify genes that are differentially expressed during dental hard tissue differentiation. Sprague-Dawley rats at postnatal days 3, 6 and 9 were used in the analysis. Differential display RT-PCR (DD-PCR) was used to screen differentially expressed genes between the 2nd (root formation stage, during mineralization) and 3rd (cap stage, before mineralization) molar germs at postnatal day 9. The DNA detected in the 2nd molar germs showed homology to osteonectin only (GenBank accession no. NM_012656.1). The level of osteonectin mRNA expression was much higher in the 2nd molar germs than in the 3rd molar germs and was found to increase in a time-dependent manner from the early bell stage to the root formation stage in the 2nd molar germs. The pattern of osteonectin protein expression was consistent with these RT-PCR results. Osteonectin protein was found by immunofluorescent analysis to localize in odontoblasts and preodontoblasts rather than the dentin matrix itself. Further studies are needed to validate the involvement of osteonectin in mineralization and root formation.

Fusion is defined as union of two separate tooth buds at some stage in their development with confluence of dentin and characterized by separate root canal and large single crown, while gemination is defined as an attempt of the single tooth bud to incompletely divide and usually result in a single root with one root canal and two completely or incompletely separated crowns. It is sometimes difficult to decide whether an abnormally large tooth is the result of fusion of a normal and a supernumerary tooth, or of gemination; use of the term 'Double tooth' may make the clinicians avoid this difficulty(Brook & Winter). Commonly there are no symptoms, but the problems associated with these anomalies include esthetics, possible loss of arch length and delayed or ectopic eruption of the permanent teeth, caries along the line of demarcation, and periodontal disease. Commonly, it dose not need to be treated in primary dentition but in case of permanent dentition, it may be requested to be treated due to esthetics and other problems. In our case, a 8 years old girl showed a Double tooth, we attained the favorable results by performing hemisection with apexification.

Objectives: The evaluation of iatrogenic dentinal defects in extracted teeth may be influenced by extraction forces and prolonged dry times. The purpose of this study was to compare the presence of dentinal defects in freshly extracted, periodontally compromised teeth with those in a group of teeth with uncontrolled extraction forces and storage time. Materials and Methods: The experimental group consisted of eighteen roots obtained from teeth extracted due to periodontal reasons with class II or III mobility. They were kept in saline and sectioned within 1 hour following extraction. The control group consisted of matched root types obtained from an anonymous tooth collection, consistent with previous dentinal defect studies. The slices were obtained at 3, 6, and 9 mm from the apex. The imaging process exposed all specimens to no more than 60 seconds of dry time. The ${\times}12.8$ magnification was used for the 9 mm slices and ${\times}19.2$ magnification for the 3 mm and 6 mm slices under light-emitting diode (LED) transillumination. The root canal spaces and periodontal tissues were masked to minimize extraneous factors that might influence the evaluators. Chi-square test was used for statistical analysis. Results: Dentinal defects were detected in 17% of the experimental group teeth, compared to 61% of control teeth (p = 0.015). Conclusions: LED transillumination assessment of freshly extracted roots with class II or III mobility showed smaller number of dentinal defects than roots with uncontrolled storage time and extraction forces. The use of freshly extracted roots with mobility should be considered for future dental defect assessment studies.

The purpose of this study was to propose the utility which was evaluated the digital image processing and clinical application of the videodensitomery. The experiments were performed with IBM-PC/16bit-AT compatible, video camera(CCdtr55, Sony Co., Japan), an color monitor(MultiSync 3D, NEC, Japan) providing the resolution of 512×480 and 64 levels of gray. Sylvia Image Capture Board for the ADC(analog to digital converter) was used, composed of digitized image from digital signal and the radiographic density was measured by 256 level of gray. The periapical radiograph(Ektaspeed EP-21, Kodak Co., U. S. A) which was radiographed dried human mandible by exposure condition of 70 kVp and 48 impulses, was used for primary X-ray detector. And them evaluated for digitzed image by low and high pass filtering, correlations between aluminum equivalent values and the thickness of aluminum step wedge, aluminum equivalent values of sound enamel, dentin, and alveolar bone, the range of diffuse density for gray level ranging from 0 to 255. The obtained results were as follows: 1. The edge between aluminum steps of digitized image were somewhat blurred by low pass filtering, but edge enhancement could be resulted by high pass filtering. Expecially, edge enhancement between distal root of lower left 2nd molar and alveolar lamina dura was observed. 2. The correlation between aluminum equivalent values and the thickness of aluminum step wedge was intimated, yielding the coefficient of correlation r=0.9997(p<0.00l), the regression line was described by Y=0.9699X+0.456, and coefficient of variation amounting to 1.5%. 3. The aluminum equivalent values of sound enamel, dentin, and alvolar bone were 15.41㎜, 12.48㎜, 10.35㎜, respectively. 4. The range of diffuse density for gray level ranging from 0 to 255 was wider enough than that of photodenstiometer to be within the range of 1-4.9.

Purpose: Extensive research is actively ongoing for development of an ideal bone substitute that meets the gold standard. Tooth was selected as a donor site for evaluation of potentials in bone substitutes based on its similar chemical compositions to alveolar bone. Previous studies have evaluated inorganic components of autogenous tooth bone graft material (AutoBT) and osteoconductivity. In continuation from the previous studies, the current study was conducted for analysis of organic components and evaluation of osteoinductivity of AutoBT. Methods: Forty-six extracted teeth were collected from actual patients (Korea Tooth Bank, R&D Institute). Extracted teeth were processed into AutoBT and implanted in dorsal subcutaneous muscular tissues of 15 athymic mice. Biopsy samples were harvested at two, five, and eight weeks. The Bradford assay, sodium dodecyl sulphate polyacrylamide gradient gel, and western blotting were performed for investigation of organic contents of AutoBT. Results: Histology analyses showed signs of new bone formation as early as two weeks. Results of the Bradford assay indicated the existence of noncollagenous proteins (NCP). 0.29% (2.89 mg/g) of proteins were extracted by weight in the root portion of AutoBT; 0.02% (0.029 mg/g) and 1.79% (17.93 mg/g) of proteins were measured by weight in crown and block-form of AutoBT, respectively. However, recombinant human bone morphogenetic protein-2 was not observed in AutoBT. Conclusion: Within the limitation of the current study, AutoBT induced new bone formation by NCP embedded in dentin.
The restorations of the severely damaged teeth by post core have been increased with the developement of endodontic procedures. But high failure rates of these procedures being reported, various restorative modifications were induced for successful treatments. Cast post-core and prefabricated post with core buildups are choice of treatment. The main causes of failure of the restorations are the fracture of post and core, root fracture, and recurrent caries due to microleakage. Recently, the acid etching technique and the use of dentin bonding agent at tooth surface to reduce the possible microleakage at the tooth-restoration interfaces were introduced. The object of this study was to measure and compare the microleakage by the types and cementation methods of post-core. For this study, forty extracted human anterior teeth were selected for specimen. After cleansing and routine endodontic procedures, anatomic crowns of each specimen were removed at the level of 2mm above the cementoenamel junction. Canals were preparated for post insertion and specimens were divided into four groups randomly. Post-cores were fabricated according to method for each group. Microleakage was measured by length of dye penetration at the tooth-restoration interfaces with measuring microscope at 50 magnification. Oneway ANOVA and t-test were performed for statistical analysis of resulting data. The following results were obtained from this study. 1. There wert significant statistical differences in degree of microleakage between each group (p<0.01). 2. Cast post-core cemented with ZPC (Group I) showed the most severe microleakage pattern$(1.5547{\pm}0.0872mm)$, and cast post-core cemented with adhesive resin cement after tooth surface treatment with dentin bonding agent (Group II) showed the least microleakage $(0.1497{\pm}0.0872mm)$. 3. Group IV revealed less dye penetrations than group III, but no statistical significance was seen between two groups.
Dentinal hypersensitivity is a type of dental pain that occurs when various stimuli are applied to exposed dentin lesions. If the symptoms of dentinal hypersensitivity continue, the dentin is exposed and the hypersensitivity may become a periodontal disease or root caries due to bacterial infection. Therefore, the clinical goal of the present study is to reduce the pain of the patient suffering from dentinal hypersensitivity by developing antimicrobial hypersensitivity treatments and to improve oral hygiene. We prepared chlorhexidine, tetracycline, cetylpyridinium chloride, gallic acid loaded desensitizer by adding 0.1%, 0.5%, 1.0%, and 2.0% chlorhexidine, tetracycline, cetylpyridinium chloride, gallic acid to desensitizer (Micro Prime; Denville and Hurri Seal; Beutlich), and antibacterial effect, and cytotoxicity. The antibacterial test by using Staphylococcus aureus (SA. ATCC 6538, FDA 209) showed that the antibacterial effect of all experimental groups was significantly higher than that of control group (p<0.05). Cytotoxicity test by using agar diffusion assay indicated that Micro Prime showed mild toxicity, Hurri Seal showed severe toxicity and this cytotoxicity is supposed to be caused by one of desensitizer components showing weak antibacterial effect.
