Cardiovascular Magnetic Resonance Versus Histopathologic Study for Diagnosis of Benign and Malignant Cardiac Tumours: A Systematic Review and Meta-Analysis

  • BACKGROUND: The gold standard for diagnosis of cardiac tumours is histopathological examination. Cardiovascular magnetic resonance (CMR) is a valuable non-invasive, radiation-free tool for identifying and characterizing cardiac tumours. Our aim is to understand CMR diagnosis of cardiac tumours by distinguishing benign vs. malignant tumours compared to the gold standard. METHODS: A systematic search was performed in the PubMed, Web of Science, and Scopus databases up to December 2022, and the results were reviewed by 2 independent investigators. Studies reporting CMR diagnosis were included in a meta-analysis, and pooled measures were obtained. The risk of bias was assessed using the Quality Assessment Tools from the National Institutes of Health. RESULTS: A total of 2,321 results was obtained; 10 studies were eligible, including one identified by citation search. Eight studies were included in the meta-analysis, which presented a pooled sensitivity of 93% and specificity of 94%, a diagnostic odds ratio of 185, and an area under the curve of 0.98 for CMR diagnosis of benign vs. malignant tumours. Additionally, 4 studies evaluated whether CMR diagnosis of cardiac tumours matched specific histopathological subtypes, with 73.6% achieving the correct diagnosis. CONCLUSIONS: To the best of our knowledge, this is the first published systematic review on CMR diagnosis of cardiac tumours. Compared to histopathological results, the ability to discriminate benign from malignant tumours was good but not outstanding. However, significant heterogeneity may have had an impact on our findings.

Separation of Nanomaterials Using Flow Field-Flow Fractionation (흐름 장-흐름 분획기를 이용한 나노물질의 분리)

  • Recently, the consumption of nanomaterials has been significantly increased in both industrial and commercial sectors, as a result of steady advancement in the nano-technologies. This ubiquitous use of nanomaterials has brought up the concern that their exposure to environments may cause detrimental effects on human health as well as natural ecosystems, and it is required to characterize their behavior in various environmental media and to evaluate their ecotoxicity. For the sake of accomplishing those assessments, the development of methods to effectively separate them from diverse media and to quantify their properties should be requisitely accompanied. Among a number of separation techniques developed so far, this study focuses on Field-Flow Fractionation (FFF) because of its strengths, such as relatively less disturbance of samples and simple pretreatment, and we review overseas and domestic literatures on the separation of nanomaterials using the FFF technique. In particular, researches with Flow Field-Flow Fractionation (FlFFF) are highlighted due to its most frequent application among FFF techniques. The basic principle of the FlFFF is briefly introduced and the studies conducted so far are classified and scrutinized based on the sort of target nanomaterials for the purpose of furnishing practical data and information for the researchers struggling in this field. The literature review suggests that the operational conditions, such as pretreatment, selection of membrane and carrier solution, and rate (velocity) of each flow, should be optimized in order to effectively separate them from various matrices using the FFF technique. Moreover, it seems to be a prerequisite to couple or hyphenate with several detectors and analyzers for quantification of their properties after their separation using the FFF. However, its application has been restricted regarding the types of target nanomaterials and environmental media. Furthermore, domestic literature data on both separation and characterization of nanomaterials are extremely limited. Taking into account the overwhelmingly increasing consumption of nanomaterials, the efforts for the area seem to be greatly urgent.

  • Uniform laws and regulations and reasonable design is necessary for the prevention of possible fire in super high-rise building. To this end, this study focused on super high-rise and massive building-related architectural review performance-based design (PBD) evaluation disaster impact assessment (DIA), and provided fire engineering measures for improving fire prevention on the basis of performance-based design by analyzing the buildings subject to these systems and problems in terms of contents. Above all, in the aspect of law and standard improvement, first, with regard to dual parts of two statutes though significant portion of them has the same contents in performance-based design (PBD) evaluation and disaster impact assessment (DIA), it is necessary to operate the systems after making them conform with each other and consolidating or abolishing them. Second, if it is impossible to consolidate or abolish performance-based design (PBD) evaluation and disaster impact assessment (DIA), the areas of contents of performance-based design (PBD) evaluation and disaster impact assessment (DIA) should be precisely classified and established. Next, engineering improvement measures against fire hazard in super high-rise building are as follows. First, it is necessary to revise the provisions of straight-run stairs in special escape stairs. And in case of installing a mechanical smoke exhaust system instead of smoke vent, sandwich pressurization used in the United Stated should be permitted. Second, with regard to smoke control system for special escape stairs, it was shown that there was necessity for revising the standards in order to enable air to be supplied according to section in case of fire, carrying out performance-based design, and the like from the early design stages to the completion stages. In the future, it is expected that an epoch-making contribution will be made to a decrease in casualties and property damage due to fire in case of super high-rise building where the results can be reflected after carrying out a study on maintenance and carrying out an additional study on other considerations of super high-rise building together with reflecting the improvement measures provided in the above-mentioned study.

Agrometeorological Early Warning System: A Service Infrastructure for Climate-Smart Agriculture (농업기상 조기경보체계: 기후변화-기상이변 대응서비스의 출발점)

  • Increased frequency of climate extremes is another face of climate change confronted by humans, resulting in catastrophic losses in agriculture. While climate extremes take place on many scales, impacts are experienced locally and mitigation tools are a function of local conditions. To address this, agrometeorological early warning systems must be place and location based, incorporating the climate, crop and land attributes at the appropriate scale. Existing services often lack site-specific information on adverse weather and countermeasures relevant to farming activities. Warnings on chronic long term effects of adverse weather or combined effects of two or more weather elements are seldom provided, either. This lecture discusses a field-specific early warning system implemented on a catchment scale agrometeorological service, by which volunteer farmers are provided with face-to-face disaster warnings along with relevant countermeasures. The products are based on core techniques such as scaling down of weather information to a field level and the crop specific risk assessment. Likelihood of a disaster is evaluated by the relative position of current risk on the standardized normal distribution from climatological normal year prepared for 840 catchments in South Korea. A validation study has begun with a 4-year plan for implementing an operational service in Seomjin River Basin, which accommodates over 60,000 farms and orchards. Diverse experiences obtained through this study will certainly be useful in planning and developing the nation-wide disaster early warning system for agricultural sector.

The Application of Fuzzy Logic to Assess the Performance of Participants and Components of Building Information Modeling

  • In the last decade, the use of Building Information Modeling (BIM) as a new technology has been applied with traditional Computer-aided design implementations in an increasing number of architecture, engineering, and construction projects and applications. Its employment alongside construction management, can be a valuable tool in helping move these activities and projects forward in a more efficient and time-effective manner. The traditional stakeholders, i.e., Owner, A/E and the Contractor are involved in this BIM system that is used in almost every activity of construction projects, such as design, cost estimate and scheduling. This article extracts major features of the application of BIM from perspective of participating BIM components, along with the different phrases, and applies to them a logistic analysis using a fuzzy performance tree, quantifying these phrases to judge the effectiveness of the BIM techniques employed. That is to say, these fuzzy performance trees with fuzzy logic concepts can properly translate the linguistic rating into numeric expressions, and are thus employed in evaluating the influence of BIM applications as a mathematical process. The rotational fuzzy models are used to represent the membership functions of the performance values and their corresponding weights. Illustrations of the use of this fuzzy BIM performance tree are presented in the study for the uninitiated users. The results of these processes are an evaluation of BIM project performance as highly positive. The quantification of the performance ratings for the individual factors is a significant contributor to this assessment, capable of parsing vernacular language into numerical data for a more accurate and precise use in performance analysis. It is hoped that fuzzy performance trees and fuzzy set analysis can be used as a tool for the quality and risk analysis for other construction techniques in the future. Baldwin's rotational models are used to represent the membership functions of the fuzzy sets. Three scenarios are presented using fuzzy MEAN, AND and OR gates from the lowest to intermediate levels of the tree, and fuzzy SUM gate to relate the intermediate level to the top component of the tree, i.e., BIM application final performance. The use of fuzzy MEAN for lower levels and fuzzy SUM gates to reach the top level suggests the most realistic and accurate results. The methodology (fuzzy performance tree) described in this paper is appropriate to implement in today's construction industry when limited objective data is presented and it is heavily relied on experts' subjective judgment.

AN EXPERIMENTAL STUDY OF ELECTROPHYSIOLOGICAL AND HISTOLOGICAL ASSESSMENT ON THE INJURY TYPES IN RABBIT INFERIOR ALVEOLAR NERVE (가토의 하치조 신경 손상 형태에 따른 전기생리학적 및 조직학적 변화에 관한 실험적 연구)

  • Inferior alveolar nerve dysfunction may be the result of trauma, disease, or iatrogenic injury. Inferior alveolar nerve injury is inherent risk in endodontic therapy, orthognathic surgery of the mandible, and extraction of mandibular teeth, particularly the third molars. The sensory disturbances of inferior alveolar nerve associated with such injury have been well documented clinical problem that is commonly evaluated by several clinical sensory test including Tinels sign, Von Frey test(static light touch detection), directional discrimination, two-point discrimination, pin pressure nociceptive discrimination, and thermal test. These methods used to detect and assess inferior alveolar nerve injury have been subjective in nature, relying on the cooperation of the patients. In addition, many of these techniques are sensitive to differences in the examiners experience and skill with the particular technique. Data obtained at different times or by different examiners are therefore difficult to compare. Prior experimental studies have used electro diagnostic methods(sensory evoked potential) to objectively evaluate inferior alveolar nerve after nerve injury. This study was designed with inferior alveolar nerve of rabbit. Several types of injury including mind, moderate, severe compression and perforation with 19 gauze, 21 gauze needle and 6mm, 10mm traction were applied for taking the sesory evoked ppterntial. Latency and amplitude of injury rabbit inferior alveolar nerve were investigated with sensory evoked potential using unpaired t-test. The results were as follows : 1. Intensity of threshold (T1) was $128{\pm}16{\mu}A$ : latency, $0.87{\pm}0.07$ microsecond : amplitude, $0.4{\pm}0.1{\mu}V$ : conduction velocity, 23.3 m/s in sensory evoked potential of uninjured rabbit inferior alveolar nerve. 2. Rabbit inferior alveolar nerve consists of type II and III sensory nerve fiber. 3. Latency was increased and amplitude was decreased in compression injury. The more injured, the more changed in latency and amplitude. 4. Findings in perforation injury was similar to compression injury. Waveform for sensory evoked potential improved by increasing postinjured time. 5. Increasing latency was prominent in traction injury rabbit inferior alveolar nerve. 6. In microscopic histopathological findings, significant degeneration and disorganization of the internal architecture were seen in nerve facicle of severe compression and 10mm traction group. From the above findings, electrophysiological assessment(sensory evoked potential) of rabbit injured inferior alveolar nerve is reliable technique in diagnosis and prognosis of nerve injury.

Current Trend of EV (Electric Vehicle) Waste Battery Diagnosis and Dismantling Technologies and a Suggestion for Future R&D Strategy with Environmental Friendliness (전기차 폐배터리 진단/해체 기술 동향 및 향후 친환경적 개발 전략)

  • Owing to the increasing demand for electric vehicles (EVs), appropriate management of their waste batteries is required urgently for scrapped vehicles or for addressing battery aging. With respect to technological developments, data-driven diagnosis of waste EV batteries and management technologies have drawn increasing attention. Moreover, robot-based automatic dismantling technologies, which are seemingly interesting, require industrial verifications and linkages with future battery-related database systems. Among these, it is critical to develop and disseminate various advanced battery diagnosis and assessment techniques to improve the efficiency and safety/environment of the recirculation of waste batteries. Incorporation of lithium-related chemical substances in the public pollutant release and transfer register (PRTR) database as well as in-depth risk assessment of gas emissions in waste EV battery combustion and their relevant fire safety are some of the necessary steps. Further research and development thus are needed for optimizing the lifecycle management of waste batteries from various aspects related to data-based diagnosis/classification/disassembly processes as well as reuse/recycling and final disposal. The idea here is that the data should contribute to clean design and manufacturing to reduce the environmental burden and facilitate reuse/recycling in future production of EV batteries. Such optimization should also consider the future technological and market trends.

Patterns of care for patients with nasopharyngeal carcinoma (KROG 11-06) in South Korea

  • Purpose: To investigate the patterns of care for patients with nasopharyngeal carcinoma (NPC) in South Korea. Materials and Methods: A multi-institutional retrospective study was performed (Korean Radiation Oncology Group [KROG] 11-06) on a total of 1,445 patients from 15 institutions. Results: Of the 1,445 patients, more than half were stages III (39.9%) and IV (35.8%). In addition to patterns of care, we also investigated trends over time with the periods 1988-1993, 1994-2002, and 2003-2011. The frequencies of magnetic resonance imaging and positron emission tomography-computed tomography were markedly increased in the third period compared to previous 2 periods. Concurrent chemoradiation (CCRT) was performed on 894 patients (61.9%), neoadjuvant chemotherapy on 468 patients (32.4%), and adjuvant chemotherapy on 366 patients (25.3%). Of stage II-IV patients, CCRT performed on 78.8% in 2003-2011 compared to 15.0% in 1988-1993. For patients treated with CCRT, cisplatin was the most commonly used agent in 81.3% of patients. Over the periods of time, commonly used radiotherapy (RT) techniques were changed from 2-dimensional RT (1988-1993, 92.5%) to 3-dimensional RT (2003-2011, 35.5%) or intensity-modulated RT (IMRT; 2003-2011, 56.5%). Median RT doses given to primary tumors, high-risk lymphatics, and low-risk lymphatics were 70.0 Gy, 58.1 Gy, and 48.0 Gy, respectively. Adoption of IMRT increased the dose per fraction and escalated total radiation dose. Conclusion: Assessment of the patterns of care for NPC patients in South Korea demonstrated that management for NPC including diagnostic imaging, treatment regimen, RT techniques and dose schedule, advanced in accordance with the international guidelines.

The Effects of Coordinated Upper-limb Postures of Back, Shoulder, and Elbow Flexion Angles on the Subjective Discomfort Rating, Heart Rate, and Muscle Activities

  • Objective: This study was to investigate the effects of coordinated upper-limb body postures on the subjective discomfort rating, heart rate, and muscle activities. Background: Although generally many checklists such as OWAS, RULA, and REBA were applied to evaluate various body postures, the body postures were might be overestimated or underestimated because each body part(i.e., back, shoulder, and elbow etc.) was evaluated separately, and then added all rates of individual body parts to assess an overall risk level for the body posture in these methodologies. Methods: A total of 20 participants maintained 14 postures which were combinations of back, shoulder, and elbow flexion angles and then muscle activities, subjective discomfort, and heart rates were collected every three minute during a sustained 15 minute and 0.5kg weight holding task. Four muscle groups were investigated: erector spine, anterior deltoid, upper trapezius, triceps brachii. Results: Results showed that subjective discomfort was the lowest when the angle of back and shoulder were both $0^{\circ}s$, while the body posture with $45^{\circ}$ of back angle and $45^{\circ}$ shoulder angle was rated as the most subjective discomfort posture. In general, the subjective discomfort ratings increased as back and shoulder flexion angles increased. It was noted that, however, the subjective discomfort of body posture with a $45^{\circ}$ back angle and $45^{\circ}$ shoulder flexion angle was lower than that of body posture with a $0^{\circ}$ back and $45^{\circ}$ shoulder flexion angle. The research findings of heart rates and muscle activities showed similar results for the analyses of subjective discomfort ratings. Conclusions: The possible limitations of the current ergonomics evaluation techniques which assessing a body posture with summing all body part score after individually analyzed in this study. Based on the analyses of subjective discomfort, heart rate, and muscle activities, it was recommended that a use of effects of coordinated upper-limb body postures would be considered when one evaluates work-load for various working postures. Application: These findings can be used for developing a more accurate assessment checklist for working posture as well as preventing musculoskeletal disorders of workers in workplaces.
