Dynamic Reconstruction of Anal Sphincter with Camera Shutter Style Double-Opposing Gracilis Flaps

  • Fournier's gangrene is a life-threatening infection which requires prompt recognition, early surgical debridement of unhealthy tissue, and initiation of broad-spectrum antibiotics. Relook debridement are usually performed until all the devitalized tissue has been removed. Involvement of the anal sphincter may result in significant morbidity such as permanent incontinence. Dynamic reconstruction of the anal sphincter has always been one of the holy grails in the field of pelvic reconstruction. We demonstrate a new method of camera shutter style double-opposing gracilis muscle flaps that allows dynamic sphincteric function without the need for electrostimulation. The bilateral gracilis muscles are inset in a fashion that allows orthograde contraction of the muscle to narrow and collapse the neoanal opening. With biofeedback training, the patient is able to regain dynamic continence and return to function without a stoma. There was also no need for neurotization or microsurgery techniques to restore sphincteric function to the anus. The patient was able to reverse his stoma 14 months after the initial insult and reconstruction with biofeedback training without the use of electrostimulation.

A 2×2 MIMO Spatial Multiplexing 5G Signal Reception in a 500 km/h High-Speed Vehicle using an Augmented Channel Matrix Generated by a Delay and Doppler Profiler

  • This paper proposes a method to extend Inter-Carrier Interference (ICI) canceling Orthogonal Frequency Division Multiplexing (OFDM) receivers for 5G mobile systems to spatial multiplexing 2×2 MIMO (Multiple Input Multiple Output) systems to support high-speed ground transportation services by linear motor cars traveling at 500 km/h. In Japan, linear-motor high-speed ground transportation service is scheduled to begin in 2027. To expand the coverage area of base stations, 5G mobile systems in high-speed moving trains will have multiple base station antennas transmitting the same downlink (DL) signal, forming an expanded cell size along the train rails. 5G terminals in a fast-moving train can cause the forward and backward antenna signals to be Doppler-shifted in opposite directions, so the receiver in the train may have trouble estimating the exact channel transfer function (CTF) for demodulation. A receiver in such high-speed train sees the transmission channel which is composed of multiple Doppler-shifted propagation paths. Then, a loss of sub-carrier orthogonality due to Doppler-spread channels causes ICI. The ICI Canceller is realized by the following three steps. First, using the Demodulation Reference Symbol (DMRS) pilot signals, it analyzes three parameters such as attenuation, relative delay, and Doppler-shift of each multi-path component. Secondly, based on the sets of three parameters, Channel Transfer Function (CTF) of sender sub-carrier number n to receiver sub-carrier number l is generated. In case of n≠l, the CTF corresponds to ICI factor. Thirdly, since ICI factor is obtained, by applying ICI reverse operation by Multi-Tap Equalizer, ICI canceling can be realized. ICI canceling performance has been simulated assuming severe channel condition such as 500 km/h, 8 path reverse Doppler Shift for QPSK, 16QAM, 64QAM and 256QAM modulations. In particular, 2×2MIMO QPSK and 16QAM modulation schemes, BER (Bit Error Rate) improvement was observed when the number of taps in the multi-tap equalizer was set to 31 or more taps, at a moving speed of 500 km/h and in an 8-pass reverse doppler shift environment.

Cancer survivor's dietary safety management awareness and competency type (암 생존자의 식생활 안전관리 인식과 역량 유형)

  • Purpose: Although the number of cancer survivors is increasing because of early diagnosis and better treatment, they are worried about relapses and metastasis. This study examined the dietary safety management awareness, capacity, and behavior of cancer survivors. Methods: Data were collected from 233 cancer survivors in Daegu and Gyeongbuk areas using a self-administered 5-Likert questionnaire from March to May 2018. Frequency analysis, one-way analysis of variance, factor analysis, reliability analysis, and correlation analysis were analyzed using the SPSS. Results: The average score for each factor was as follows: importance of weight management (3.83), education requirement (3.79), unhealthy diet (reverse, 3.64), food safety anxiety (3.34), lack of awareness of over and malnutrition (reverse, 3.26), obsession (3.23), cooking capacity (3.16), health-function pursuit (3.04), balanced diet (2.93), and chronic disease anxiety (2.64). The average dietary safety competency factors were significantly lower among the male survivors, aged 60-80 years, lower economic condition, and less nutritional supplement intake. Female survivors comprised the highest proportion with high health management and cooking capacity, while men had the lowest of all the factors (p < 0.001). The group that was low in all three capacity factors had a significantly lower average score of difficulty in acquiring knowledge (reverse), lack of awareness of over- and malnutrition (reverse), importance of weight management, education requirement, knowledge, balanced diet, and health-functional pursuit compared to the other groups (p < 0.001). Conclusion: To effectively manage health, dietary safety management education should be differentiated and actively conducted according to sex, age, cancer diagnosis time, economic status, nutritional supplement intake, and the capacity cluster of cancer survivors.

Analysis of Carbon Emission from a Forward Osmosis and Reverse Osmosis Hybrid System for Water Reuse and Seawater Desalination (하수재이용 및 해수담수화를 위한 정삼투-역삼투 융합공정의 탄소배출량 분석)

  • A conventional seawater reverse osmosis (SWRO) and a forward osmosis (FO) and reverse osmosis (RO) hybrid process to produce 1,000 m3/d of fresh water, were designed and compared in terms of carbon emission. When FO was adapted for the osmotic dilution, the required pressure for RO decreases, and thus energy consumption decreases. The decrease in carbon emission by decreased energy consumption (up to -0.73 kgCO2/m3 using coal as the energy source) was compared with the increase in carbon emission by the FO system (+0.16 kgCO2/m3), which is a function of various factors such as the number of FO modules and energy consumption. The comparison revealed that the FO-RO process causes less carbon emission compared with the SWRO process when the energy sources are coal and oil. However, if energy sources with low carbon emission such as solar, wind, and nuclear energy are selected, the carbon emission of the FO-RO process becomes higher than that of the SWRO process. This implies that the type of energy source is a key factor to determine the necessity of the FO-RO process from the aspect of carbon emission.

  • Our society witnesses the rapid progress in the areas of politics, economy, society and culture in the process of national modernization since 1960s, which in turn as a reverse function gets to contract a societic pathology, totally lowering the security level of citizens' lives owing to various violent crimes like hostage commotions and murders with rifles and deadly weapons. what is the main reason for that? That may be partly because the chief police force concentrates on the current situation resulting in the vacuum of the public peace. However, the main reason is that the police fall short of man-power and equipments even if the whole police power were put to use in preventing and quelling the crimes. That is true not only of Korea but also of the advanced countries like the U.S.A., England and Japan. We realize that these advanced countries have higher level of security in every individual's life and property than Korea because their progress of the private guard systems can fill in a vacuum of the shortage of the police power, Therefore, we should without delay internationalize our private guard systems expecting the widely opening of the guard service markets in the age of Uruguay Round. To do this, we need to change our ideas for fostering the policy of the private guard from passive defense ideas into positive aggressive ones. Our police should urgently set up a plan to pursue the orientation of vision that we should dispatch our private guards overseas before foreign guards rush into our markets. Accordingly it goes without saying that the private guard group should distinguish their services from the public services initiating their own theory and strategy of private guard services and also readjust themselves between the public duties and the private services with the study of minimizing the reverse function of the private guard systems. The history criminal justice has always shown that the criminal system progressed at the initiative of the civil factor in case its demand and supply do not make both ends meet. Nevertheless, in the process the power of the government never weakens, rather it is built up in general. In conclusion, the necessity of the build-up of the private guard services must duly be acknowledged by the police as well as by the business which has its unique sphere within the criminal justice instead of as the suplemtary services of the simple the police power on the long-term basis. The purpose of the private guard services can be largely classified into the two categories; first it means the function to prevent the crimes against the citizens and secondly to enhance the national interest as an increasing mammoth business with a worldly competition capacity. The police has an absolute responsibility that they should protect the modem public in general from feeling the crisis of the personal threat, tension, anxiety and nervousness. In short, if we develop the complete private guard system to guarantee the societic atmosphere for all citizens, keep the public peace, and protect all citizens' lives and properties, we will sure enjoy a beautiful land, a wholesome society and a happy life in goodharmony of law and order.

A Study of Periodic Solutions of Typical Relay Servo System (릴레이 제어기구 조기해법에 관한 연구)

  • A relay servo, one of the nonlinear sytsems, is inherently compact compared to a linear system for an equivalent control problem. The power element or actuator is not adjusted proportionally in accordance with an error signals but rather is switched abruptly between several discrete conditions. Usually switched conditions are off, full, forward or full reverse. The relay system is a particularly simple and compact one, but probably more effort has been expended on its analysis and design than on all other systems together. Early studies in the art were made by Goldfarb, austin, Oppelt and Kochenburger on the describing function method, which can be used as an approximate check on the stability of the system. The describing function method is based on the assumption that any periodic wave could be approximated as a fundamental one in wide ranges of practical applications. A relay servo system usually operates on a limit cycle condition as the loop gain increases. The stability analysis compensation or any improvement effort based on the describing function method sometimes may present considerable discrepancies on physically realized practical systems. An approach to exact periodic solutions of a relay servo system is much important for the analysis, design and system improvement. This paper dells with periodic solutions of a relay servo system on the basis of describing function and generalized chopper wave form which is composed of infinite number of harmonic series. Various ways of graphical representation were attempted to get periodic solutions, some of which have shown its validity in rapid approach to exact solutions and also in judgement of system behavior.

Intra-Spinal Bone Marrow Mononuclear Cells Transplantation Inhibits the Expression of Nuclear Factor-${\kappa}B$ in Acute Transection Spinal Cord Injury in Rats

  • Objective : To assess the effect of bone marrow mononuclear cells (BMMNCs) transplantation in the expression of nuclear factor-${\kappa}B$ (NF-${\kappa}B$) in spinal cord injury (SCI) in rats. Methods : BMMNCs were isolated from tibia and femur by a density gradient centrifugation. After establishment of acute transection SCI, rats were divided into experiment (BMMNCs), experiment control (0.1 M PBS infused) and sham surgery groups (laminectomy without any SCI). Locomotor function was assessed weekly for 5 weeks post-injury using BBB locomotor score and urinary bladder function daily for 4 weeks post-injury. Activity of NF-${\kappa}B$ in spinal cord was assessed by immunohistochemistry and reverse transcriptase polymerase chain reaction. Results : At each time point post-injury, sham surgery group had significantly higher Basso, Beattie, Bresnahan locomotor and urinary bladder function scores than experiment and experiment control group (p<0.05). At subsequent time interval there were gradual improvement in both experiment and experiment control group, but experiment group had higher score in comparison to experiment control group (p<0.05). Comparisons were also made for expression of activated NF-${\kappa}B$ positive cells and level of NF-${\kappa}B$ messenger RNA in spinal cord at various time points between the groups. Activated NF-${\kappa}B$ immunoreactivity and level of NF-${\kappa}B$ mRNA expression were significantly higher in control group in comparison to experiment and sham surgery group (p<0.05). Conclusion : BMMNCs transplantation attenuates the expression of NF-${\kappa}B$ in injured spinal cord tissue and thus helps in recovery of neurological function in rat models with SCI.

The effect of coenzyme Q10 on endothelial function in a young population

  • Objective: Coenzyme (CoQ10) is an enzymatic co factor used in normal cellular metabolism. Recent evidence shows that in people with heart disease it can reverse endothelial cell damage in the blood vessels. It is also a potent antioxidant. Design: One group pretest-posttest design. Methods: In the present study, endothelial function was evaluated using the response to occlusion and heat before and 2 weeks after administration of CoQ10, 300 mg/day. Thirty Eight subjects, who are physical therapy students, participated in a series of experiments to see if taking 300 mg of CoQ10 daily for 2 weeks would impact resting blood flow in the forearm skin and the blood flow response to 4 minutes of vascular occlusion and the response to local heat ($42^{\circ}C$) for 6 minutes. Results: The results showed that, for this population, there was no difference in the response to heat. However, the response to occlusion was improved after administration of CoQ10. Conclusions: It would appear that in a young population CoQ10 has no effect on the nitric oxide vasodilator pathway in skin but does influence other vasodilator pathways.

Role of Sphingolipids and Metabolizing Enzymes in Hematological Malignancies

  • Sphingolipids such as ceramide, sphingosine-1-phosphate and sphingomyelin have been emerging as bioactive lipids since ceramide was reported to play a role in human leukemia HL-60 cell differentiation and death. Recently, it is well-known that ceramide acts as an inducer of cell death, that sphingomyelin works as a regulator for microdomain function of the cell membrane, and that sphingosine-1-phosphate plays a role in cell survival/proliferation. The lipids are metabolized by the specific enzymes, and each metabolite could be again returned to the original form by the reverse action of the different enzyme or after a long journey of many metabolizing/synthesizing pathways. In addition, the metabolites may serve as reciprocal biomodulators like the rheostat between ceramide and sphingosine-1-phosphate. Therefore, the change of lipid amount in the cells, the subcellular localization and the downstream signal in a specific subcellular organelle should be clarified to understand the pathobiological significance of sphingolipids when extracellular stimulation induces a diverse of cell functions such as cell death, proliferation and migration. In this review, we focus on how sphingolipids and their metabolizing enzymes cooperatively exert their function in proliferation, migration, autophagy and death of hematopoetic cells, and discuss the way developing a novel therapeutic device through the regulation of sphingolipids for effectively inhibiting cell proliferation and inducing cell death in hematological malignancies such as leukemia, malignant lymphoma and multiple myeloma.

Identification of Differentially Expressed Genes between Neonatal and Peripubertal Rat Thymi Using $GeneFishing^{TM}$ Polymerase Chain Reaction

  • Aging causes thymus involution, and genes in thymus play an important role in the development of the immune system. In this study, we compared genes expressed in thymus of neonatal and peripubertal rats using annealing control primers (ACPs)-based GeneFishing polymerase chain reaction (PCR) and semiquantitative reverse transcription (RT)-PCR. We identified 10 differentially expressed genes (DEGs) with 20 ACPs. Of 10 DEGs, bystin-like, collagen type V alpha 1 (COL5A1), and T-cell receptor beta-chain segment 2 (TCRB2) that are related to immune-function were detected in rat thymus. Bystin-like and TCRB2 were up-regulated, while COL5A1 was down-regulated in peripubertal thymus. Semiquantitative RT-PCR confirmed postnatal changes in expression of bystin-like, COL5A1, and TCRB2. These results suggest that bystin-like, COL5A1, and TCRB2 could regulate immune function controlled in thymus as age increases.
