Development of Personalized Respiratory Training Device with Real-time Feedback for Respiratory Muscle Strengthening

  • Objective: The practice of breathing exercises involves altering the depth and frequency of respiration. Strengthening respiratory muscles plays a crucial role in maintaining overall health and well-being. The efficiency of the respiratory system affects not only physical activity but also various physiological processes including cardiovascular health, lung function, and cognitive abilities. The study evaluated the reliability of the developed device for inspiratory/expiratory training using pressure sensors and Bluetooth connectivity with a smartphone application. Design: Design & development research Methods: The research methodology involved connecting a custom-made respiratory sensor to an IMT-PEP BIC Breath device. Various pressure conditions were measured, and statistical analyses were performed to assess reliability and consistency. Results showed high Intraclass Coefficient Correlation (ICC) values for both inspiratory and expiratory pressures, indicating strong test-retest reliability. The device was designed for ease of use and wireless monitoring through a smartphone app. Results: This study conducted at expiratory pressure confirmed the proper operation of the IMT/PEP breathing trainer at the specified pressure setting in the product. The pressure sensor demonstrated high test-retest reliability with an ICC value of 0.999 for both expiratory and inspiratory pressure measurements. Conclusions: The developed respiratory training device measured and monitored inspiratory and expiratory pressures, demonstrating its reliability for respiratory training. The system could be utilized to record training frequency and intensity, providing potential benefits for patients requiring respiratory interventions. Further research is needed to assess the full potential of the device in diverse populations and applications.

기침에 대한 비내시경을 이용한 한열변증(寒熱辨證)의 임상적 가치평가 (Clinical Values of Cold-Heat Pattern Diagnosis by the Nasal Endoscopy for Patients with Cough)

  • Objectives: This study was aimed to figure out an agreement between the diagnosis of nasal endoscopy and a preexisting questionnaire focusing on Cold-Heat pattern. Methods: 52 patients with cough who met the criteria filled out a pattern questionnaire and the examiner looked at their nasal cavities through nasal endoscopy. According to the checked questionnaire results, the subjects were identified by 6 patterns. After examining subject's mucous membrane of oropharynx and nasal cavity through nasal endoscopy, we classified each to the Cold or Heat group. Correlation between questionnaire and nasal endoscopy results was analyzed. Results: In diagnosing Cold-Heat, there was no significant difference by McNemar test (p=0.227) between nasal endoscopy and the questionnaire, and the two methods agreed moderately (${\kappa}=0.428$). The color of mucous membrane of oropharynx and the Cold-Heat pattern on questionnaire agreed slightly (${\kappa}=0.133$). The color of mucous membrane of nasal cavity and the Cold-Heat pattern on questionnaire agreed fairly (${\kappa}=0.384$). In the patients with cough related to upper respiratory tract, they got higher diagnosis accuracy than the patients with cough related to lower respiratory tract did. Similarly, external cough patients got higher diagnosis accuracy than internal cough patients did. Conclusions: To identify Cold or Heat, examining oropharynx and nasal cavity using nasal endoscopy is a meaningful method in patients with cough, showing that two diagnosis methods which use nasal endoscopy and questionnaire agreed moderately. Especially, it is more useful diagnosing patients with cough related to the upper respiratory tract than diagnosing the patients with cough related to the lower respiratory tract.

맥문동탕(麥門冬湯)에 대한 연구 논문 분석 (Analysis of studies on Maekmundong-tang)

  • Objectives: This study analyzed the contents of the research papers of Maekmundong-tang (MMDT). This study was conducted to help development of new clinical application and clinical studies for treating COPD with Oriental medicine. Materials and Methods: We inspected 26 theses and scrutinized their classification, objective diseases, study design, participants, methodological quality of clinical trial. Results: The following results were obtained in this study. 1. The studies of MMDT started in 1989 and have continuously increased, but it decreased recently. 2. The studies were mainly focused on experimental models rather than clinical studies. The topics of studies were mainly relaxation of airway contraction and anti-asthmatic effect. 3. MMDT was showed to have effects on chronic cough and asthma in these papers. Conclusion: MMDT is being used in respiratory disease. However, mechanism study should be conducted at experimental study and more clinical studies on the efficacy of MMDT for chronic respiratory disease are needed.

만성폐쇄성 폐질환 노인의 호흡기 자가간호 실천정도와 간호요구 (A Study on Nursing Needs and Respiratory Self-care Practice Degree in Elderly Chronic Obstructive Pulmonary Disease)

  • This study investigated the degree of self-care practice to maintain the stable respiratory function and the required respiratory nursing needs upon the elderly with chronic obstructive pulmonary diseases. The research subjects were 115 elderly people over 60 years old, who were diagnosed to have chronic obstructive pulmonary diseases (the mixed type, chronic bronchitis, bronchial asthma, emphysema) and were hospitalized in 3 university hospitals in Busan or treated as outpatient, and the data was collected over the period between June 1, 2002 and September 30, 2002. In order to measure the respiratory self-care practice degree and nursing needs of the subjects, the instrument on the self-care practice and the instrument on nursing needs were developed by the researcher. The research results are as follows: 1. The mean of respiratory self-care practice degree by the subjects was 2.41 out of 4 for each clause, and the degree of practicing general management was the highest(2.70), and nutrition control(2.66), respiratory exercise(2.65), expectoration of sputum (2.63), oxygen therapy(2.60), environment control(2.50), and medication control(2.36) succeeded. 2. The degree of self-care practice had a significant difference depending on age(F=2.82, P=0.02), frequency of hospitalization(F=3.11, P=0.01), and diagnosed disease(F=15.66, P=0.00). 3. The subjects nursing needs of respiratory system were 3.07 on the average out of 4.00, and 'I want to know how to prevent the infection of respiratory system such as cold scored the highest 3.83, while the clauses like 'want to know how to face the respiratory disorder properly'(3.77) and 'want the specific explanation of the symptoms of my disease'(3.66) also had scored high points. 4. The nursing needs of the respiratory system had a significant difference depending on diagnosed disease such as mixed type of chronic obstructive pulmonary diseases, emphysema, bronchitis, and asthma (F=6.70, P=0.00). The research showed that the subjects degree of self-care practice of respiratory system was low on the whole, while the nursing needs for managing respiratory organ were relatively high. Therefore, specific education concerning the nursing of the disease and self-care upon the elderly having chronic obstructive pulmonary disease is necessary.

Characteristics of Vibration Response Imaging in Healthy Koreans

  • Background: Vibration response imaging (VRI) is a new technology that records energy generated by airflow during the respiration cycle. Analysis of lung sound using VRI may overcome the limitations of auscultation. Objectives: To set a VRI standard for healthy Koreans, we conducted a clinical assessment to evaluate breath sound images and quantification in healthy subjects and compared the findings with reported breath sound characteristics. Methods: Recordings were performed using the VRIxp. Eighty subjects took a deep breath four times during a 12-second interval while sitting upright. The quantitative aspect was analyzed using the VRI quantitative lung data (QLD) for total left lung, total right lung and for six lung regions: left upper lung (LUL), left middle lung (LML), left lower lung (LLL), right upper lung (RUL), right middle lung (RML), right lower lung (RLL). The qualitative aspect was provided through image assessments by three reviewers. Results: In all regions the left lung had significantly higher QLD than the right lung (P<0.005, paired t-test). The inter-rater agreement was 0.78. 84% of the images were found normal by the final assessment. Among the 16% (n=13) of images with abnormal final assessment, the most common flawed features were dynamic image (77%, n=10) and maximum energy frame (MEF) shape (77%, n=10). No significant differences were found between males and females for QLD but there were significant differences in qualitative aspects including dynamic images, MEF shape, and missing LLL. Conclusion: The characteristics of healthy Koreans are similar to those of Western subjects reported previously. VRI is easy to use and objective, and so is helpful to diagnose patients with respiratory diseases and to monitor the progress of diseases after medical treatments.

Lung Cancer의 Stereotactic Radiosurgery시 Respiratory Gating system의 유용성에 대한 연구 (Effectiveness of the Respiratory Gating System for Stereotectic Radiosurgery of Lung Cancer)

  • Lung cancer 환자 중 호흡에 의한 종양의 움직임이 큰 lung lower lobe에서 stetreotactic radiosurgery(SRS)시 호흡 주기 중 종양의 움직임이 적은 호흡 주기에서만 방사선을 조사하기 위해 respiratory gating system을 사용함으로써 그 유용성에 대하여 알아보고자 한다. lung lower lobe의 SRS 환자 2명을 대강으로 하였으며, 환자의 복부에 maker block(sensor)을 부착하고 tracking camera와 real time position management(RPM)를 이용하여 호흡 주기를 측정하면서 1회 호흡주기에서 10 phases의 4D-CT를 촬영 하였다. 종양의 위치 변화가 급격하지 않은 percent($\%$)의 phases를 치료 phases로 결정하였고, 치료 phases의 CT image를 maximum intensity projection(MIP) 기법으로 재구성 후 volume contouring을 하였다. set up 의 재현성 및 GTV의 위치 변화를 확인하기 위해 치료 전과 치료 중 2회의 4D-CT 촬영을 하였다. GTV의 움직임이 가장 큰 Y(longitudinal)축에서 A환자 full($0\%\~90\%$) phases의 9.4mm가 치료 ($30\%\~60\%$) phases 에서는 2.6mm로, B환자 full($0\%\~90\%$) phases의 11.7mm가 치료($30\%\~70\%$) phases 에서는 2.3mm로 줄었다. 2회의 4D-CT 비교 결과 Set up의 X, Y, Z축 오차는 모두 3mm 이내였다. 호흡에 의한 종양의 움직임이 큰 lung lower lobe에서 SRS 시행 시 respiratory gating system의 사용은 종양의 움직임을 5mm 이내로 감소시킬 수 있어 유용하였다.

  • In cardiac magnetic resonance imaging (CMRI), heart and respiratory motions are one of main obstacles in obtaining diagnostic quality of images. To synchronize CMRI to the physiological motions, ECG and respiratory gatings are commonly used. In this paper multi-biological signal (ECG, respiratory, and SPO2) based smart trigger system is proposed. By using multi-biological signal, the proposed system is robust to the induced noise such as eddy current when gradient pulsing is continuously applied during the examination. Digital conversion of the multi-biological signal makes the system flexible in implementing smart and intelligent algorithm to detect cardiac and respiratory motion and to reject arrhythmia of the heart. The digital data is used for real-time trigger, as well as signal display, and data storage which may be used for retrospective signal processing.

천식(喘息)환자의 유지치료를 위한 보중익기탕(補中益氣湯)의 효과 (Usage of Bojungiki-tang to Maintain and Treat Asthmatic Patients)

  • Objectives : To assess the effect of Bojungiki-tang for symptomatic management in stable asthmatic patients after initial treatment. Methods : Outcome measures of asthmatic patients were recorded using patient's charts and inquiry. Compared with the patient's state right after they got their treatment, "Excellent" was if it was almost the same state, "Good" was if the patient's state was maintained for almost 70%, "Fair" was for 50%, "Poor" was for under 50% or if the patient required another treatment. Results : Of 25 patients, 21 used Bojungiki-tang for more than one month and 20 maintained a stable state, that is, feeling more than 70% of symptom control compared with previous best status. Conclusions : Bojungiki-tang has a good effect for maintaining asthma after treatment.

두한증 및 수족다한증 환자의 비내시경, 체성분 검사를 통한 특성비교분석 (Analysis of Characteristics of Craniofacial Hyperhidrosis and Palmar/Plantar Hyperhidrosis by Nasal Endoscopy and Body Composition Test)

  • Objectives This study was designed to analyze the characteristics of craniofacial hyperhidrosis and palmar/plantar hyperhidrosis by nasal endoscopy and body composition test. Methods The study sample consisted of 20 and 22 patients with craniofacial hyperhidrosis and palmar/plantar hyperhidrosis, respectively, who answered questionnaires and underwent nasal endoscopy and body composition test. The questionnaires estimated the quality of life by Dermatology Life Quality Index score (DLQI), and the degree of obesity was evaluated using body mass index (BMI), percent body fat (PBF), and waist-hip ratio (WHR). The state of nasal cavity was evaluated by color, humidity, and swelling of the mucous membranes, and runny nose. Results BMI, PBF, and WHR were higher in patients with craniofacial hyperhidrosis than in patients with palmar/plantar hyperhidrosis. Rhinitis score was not significantly different between craniofacial hyperhidrosis and palmar/plantar hyperhidrosis. There was a positive correlation between rhinitis score and DLQI. Conclusions The degree of obesity was higher in patients with craniofacial hyperhidrosis than in those with palmar/plantar hyperhidrosis. The state of nasal cavity was not significantly different between craniofacial hyperhidrosis and palmar/plantar hyperhidrosis, but was associated with quality of life of hyperhidrosis patients.

은교산과 Rufloxacin 병용이 호기성 Gram(+) 세균주에 대한 시험관내 항균력에 미치는 영향 (Effects of In Vitro Synergism of Eunkyo-san and Rufloxacin against 9 Strains of Aerobic Gram(+) Bacteria)

  • In order to evaluate the in vitro synergic effect of Eunkyo-san, with quinolone antibiotics, rufloxacin (RUFX), the minimal inhibitory concentration (MIC), MIC50 and MIC90 of single use of quinolones and concomitant treatment with Eunkyo-san against 9 strains of aerobic gram positive bacteria. The obtained results were as follows : In the case of aerobic gram positive bacteria, the MIC, MIC50 and MIC90 against Staphylococcus aureus, Staphylococcus aureus smith, Staphylococcus epidermidis, Staphylococcus pyogens, Streptococcus pneumoniae Type Ⅰ, Type Ⅱ and Type Ⅲ was significantly decreased in concomitant treated groups with Eunkyo-san compared to those of single treated groups of RUFX, respectively. However, no significant changes were demonstrated against Bacillus subtilis and Enterococcus faecalis. In conclusion, the in vitro antibacterial activity of RUFX were increased against some strains of aerobic gram positive strains, especially, pneumococcus such as Staphylococcus and Streptococcus by concomitant use of Eunkyo-san.
