  • How to compare the environmental damage caused by vehicles with different foe]s and drive trains\ulcorner This paper describes a methodology to assess the environmental impact of vehicles, using different approaches, and evaluating their benefits and limitations. Rating systems are analysed as tools to compare the environmental impact of vehicles, allowing decision makers to dedicate their financial and non-financial policies and support measures in function of the ecological damage. The paper is based on the "Clean Vehicles" research project, commissioned by the Brussels Capital Region via the BIM-IBGE (Brussels Institute for the Conservation of the Environment) (Van Mierlo et at., 2001). The VriJe Universiteit Brussel (ETEC) and the universite Libre do Bruxelles (CEESE) have jointly carried out the workprogramme. The most important results of this project are illustrated in this paper. First an overview of environmental, economical and technical characteristics of the different alternative fuels and drive trains is given. Afterward the basic principles to identify the environmental impact of cars are described. An outline of the considered emissions and their environmental impact leads to the definition of the calculation method, named Ecoscore. A rather simple and pragmatic approach would be stating that all alternative fuelled vehicles (LPG, CNG, EV, HEV, etc.) can be considered as ′clean′. Another basic approach is considering as ′clean′ all vehicles satisfying a stringent omission regulation like EURO IV or EEV. Such approaches however don′t tell anything about the real environmental damage of the vehicles. In the paper we describe "how should the environmental impact of vehicles be defined\ulcorner", including parameters affecting the emissions of vehicles and their influence on human beings and on the environment and "how could it be defined \ulcorner", taking into account the availability of accurate and reliable data. We take into account different damages (acid rain, photochemical air pollution, global warming. noise, etc.) and their impacts on several receptors like human beings (e.g., cancer, respiratory diseases, etc), ecosystems, or buildings. The presented methodology is based on a kind of Life Cycle Assessment (LCA) in which the contribution of all emissions to a certain damage are considered (e.g. using Exposure-Response damage function). The emissions will include oil extraction, transportation refinery, electricity production, distribution, (Well-to-Wheel approach), as well as the emission due to the production, use and dismantling of the vehicle (Cradle-to-Grave approach). The different damages will be normalized to be able to make a comparison. Hence a reference value (determined by the reference vehicle chosen) will be defined as a target value (the normalized value will thus measure a kind of Distance to Target). The contribution of the different normalized damages to a single value "Ecoscore" will be based on a panel weighting method. Some examples of the calculation of the Ecoscore for different alternative fuels and drive trains will be calculated as an illustration of the methodology.

치위생과 학생의 흡연에 관한 지식 및 실태 연구 (A study on the knowledge of dental hygiene students on smoking and the state of smoking)

  • The purpose of this study was to examine the knowledge of dental hygiene students on smoking and their state of smoking in an attempt to assist dental hygienists to develop good anti-smoking education programs, as dental hygienists should play a critical role in conducting an anti-smoking campaign and education. The subjects in this study were dental hygiene students in six different colleges located in Seoul and Gyeonggi province. After a survey was conducted from April to June, 2007, the answer sheets from 1,322 students were gathered and analyzed. The findings of the study were as follows: 1. As for the reality of smoking, 82.7% were nonsmokers, and 12.2% had been smokers in the past, 5.1% were smokers, which accounted for 5%. Regarding when they started smoking, 47.1% and 35.3% started it in middle school and high school days respectively. So 80% or more began smoking as middle and high schoolers. As to a daily amount of smoking, 36.8% smoked five or less cigarettes, and 22.1% smoked six to 10 cigarettes, 14.7 % smoked 11 to 15 cigarettes. Concerning motivation of smoking, 51.5% did it out of curiosity, and 26.6% intended to get rid of stress. Among the smokers, 80.9% had ever tried to quit smoking, and as for the reason, 51.0 % wanted not to develop a disease in the future, and 25.0% weren't in good health. In regard to what made them fail to give up smoking, 49.0% answered they were increasingly stressed out, and 24.0 % were tempted by others. 2. As to the impact of smoking on the body and illness, the largest group believed it affected unborn babies, and the second largest group was aware of the harmfulness of secondhand smoking. The third largest group thought it exercised an influence on the lung and respiratory ailments, and the fourth largest group believed it was detrimental to health. The fifth greatest group thought tar was harmful to the body. They didn't know well about whether smoking deteriorated the sense of taste, but were well cognizant of the harmfulness of smoking. 3, Their knowledge on the impact of smoking on the body and diseases was evaluated by academic year and in consideration of whether they were smokers or not. And the largest number of them knew the best about smoking's impact on unborn babies and the harmfulness of secondhand smoking, regardless of academic year and whether they were smokers or not. They weren't well aware of its impact on weakening the sense of taste.

  • 전염병 발생의 잠재적 위험은 지구촌 사회의 시대를 맞이하여 심각하게 증가하였다. 그러나 여러 국가들의 다양한 정부기관에서 전염병에 대한 지방자치단체의 대응계획은 적절한 수준으로 준비되어 있지 않은 실정이다. 인류는 의료 및 공중보건에서 괄목할만한 성장을 이룩해왔다. 그러나 SARS는 아직도 막대한 인명손실을 야기시킬 수 있는 잠재력을 가진 매우 위험한 전염병으로 남아있다. 2002년 11월부터 2003년 7월까지 SARS는 전세계적으로 8,096건의 사례가 발생하였으며 774명의 사망자와 9.6%의 사망률을 결과하였다(WHO, 2004). 이는 대표적인 보건재난 중 하나이다. 중국이 전세계에서 발생한 총 SARS사례의 약 66%를 점유한 결과 이웃국가인 한국의 관광산업은 심각한 영향을 받게 되었다. 본 연구의 목적은 한국의 입국자 자료의 주요요소를 파악하고, SARS발생기간 동안 한국에 입국한 일본과 대만 관광객의 수요변화가 한국의 관광산업에 미친 경제적 영향을 분석하는 것이다. 본 연구의 결과로서 일본, 미국, 중국, 대만의 4개 국가 입국자들의 계절적, 장기적 추세를 파악할 수 있었으며 한국 관광산업에 대한 SARS의 직 간접적 영향을 분석할 수 있었다.

Impact of antimicrobial resistance in the $21^{st}$ century

  • Antimicrobial resistance has been a well-recognized problem ever since the introduction of penicillin into clinical use. History of antimicrobial development can be categorized based on the major antibiotics that had been developed against emerging resistant $pathogens^1$. In the first period from 1940 to 1960, penicillin was a dominating antibiotic called as a "magic bullet", although S.aureus armed with penicillinase led antimicrobial era to the second period in 1960s and 1970s. The second stage was characterized by broad-spectrum penicillins and early generation cephalosporins. During this period, nosocomial infections due to gram-negative bacilli became more prevalent, while those caused by S.aureus declined. A variety of new antimicrobial agents with distinct mechanism of action including new generation cephalosporins, monobactams, carbapenems, ${\beta}$-lactamase inhibitors, and quinolones characterized the third period from 1980s to 1990s. However, extensive use of wide variety of antibiotics in the community and hospitals has fueled the crisis in emerging antimicrobial resistance. Newly appeared drug-resistant Streptococcus pneumoniae (DRSP), vancomycin-resistant enterococci (VRE), extended-spectrum ${\beta}$-lactamase-producing Klebsiella, and VRSA have posed a serious threat in many parts of the world. Given the recent epidemiology of antimicrobial resistance and its clinical impact, there is no greater challenge related to emerging infections than the emergence of antibiotic resistance. Problems of antimicrobial resistance can be amplified by the fact that resistant clones or genes can spread within or between the species as well as to geographically distant areas which leads to a global concern$^2$. Antimicrobial resistance is primarily generated and promoted by increased use of antimicrobial agents. Unfortunately, as many as 50 % of prescriptions for antibiotics are reported to be inappropriate$^3$. Injudicious use of antibiotics even for viral upper respiratory infections is a universal phenomenon in every part of the world. The use of large quantities of antibiotics in the animal health industry and farming is another major factor contributing to selection of antibiotic resistance. In addition to these background factors, the tremendous increase in the immunocompromised hosts, popular use of invasive medical interventions, and increase in travel and mixing of human populations are contributing to the resurgence and spread of antimicrobial resistance$^4$. Antimicrobial resistance has critical impact on modem medicine both in clinical and economic aspect. Patients with previously treatable infections may have fatal outcome due to therapeutic failure that is unusual event no more. The potential economic impact of antimicrobial resistance is actually uncountable. With the increase in the problems of resistant organisms in the 21st century, however, additional health care costs for this problem must be enormously increasing.

기관절개술을 시행한 뇌졸중 환자들에서의 기도미생물 탐색 연구 (Airway Microbiota in Stroke Patients with Tracheostomy: A Pilot Study)

  • Objectives: We investigated differences between the tracheostomized and the non-tracheostomized stroke patients through microbiological analysis for the purpose of preliminary explorations of full-scale clinical research in the future. Methods: We collected tracheal aspirates samples from 5 stroke patients with tracheostomy and expectorated sputum samples from 5 stroke patients without tracheostomy. Genomic DNA from sputum samples was isolated using QIAamp DNA mini kit. The sequences were processed using Quantitative Insights into Microbial Ecology 1.9.0. Alpha-diversity was calculated using the Chao1 estimator. Beta-diversity was analyzed by UniFrac-based principal coordinates analysis (PCoA). To confirm taxa with different abundance among the groups, linear discriminant analysis effect size analysis was performed. Results: Although alpha-diversity value of the tracheostomized group was higher than that of the non-tracheostomized group, there was no statistically significant difference. In PCoA, clear separation was seen between clusters of the tracheostomized group and that of the non-tracheostomized group. In both groups, Bacteroidetes, Proteobacteria, Fusobacteria, Firmicutes, Actinobacteria were identified as dominant in phylum level. In particular, relative richness of Proteobacteria was found to be 31% more in the tracheotomized group (36.6%) than the non-tracheostomized group (5.6%)(P<0.05). In genus level, Neisseria (24%), Prevotella (17%), Streptococcus (13%), Fusobacteria (11%), Porphyromonas (7%) were identified as dominant in the tracheostomized group. In the non-tracheostomized group, Prevotella (38%), Veillonella (20%), Neisseria (9%) were genera that found to be dominant. Conclusions: It is meaningful in that the tracheostomized group has been identified a higher rate of microbiotas known as pathogenic in respiratory diseases compared to the non-tracheostomized group, confirming the possibility that the risk of opportunity infection may be higher.

반하(半夏) 약침액(藥鍼液)이 사람 기관지 상피세포의 TARC 분비에 미치는 효과 (Effect of Pinelliae Rhizoma Herbal Acupuncture on the Release of Thymus and Activation-Regulated Chemokine(TARC) in Human Bronchial Epithelial Cell)

  • 사람 기관지 상피세포에 알러지 환경을 유발 하고자 사이토카인을 처리하여 TARC의 분비를 유도하고, 이 케모카인 분비에 반하(半夏) 약쇄액(藥鎖液)이 미치는 효과를 실험한 결과 다음과 같은 결론을 얻었다. 1. 사람의 기관지 상피 세포주에 각각 IL-4, TNF-${\alpha}$, IFN-${\gamma}$ 및 IL-$1{\beta}$를 처리하는 경우와 IL-4와 TNF-${\alpha}$, INF-${\alpha}$와 IFN-${\gamma}$, IFN-${\gamma}$와 IL-$1{\beta}$를 병용 처리할 경우 TARC의 분비량를 측정한 결과 IL-4와 TNF-${\alpha}$와 TNF-${\alpha}$와 IFN-${\gamma}$를 병용 처리하였을 경우 TARC의 분비량이 유의하게 증가하였다. 2. 반하(半夏) 약쇄액(藥鎖液) 처리군의 24시간 및 48시간에서 통계적으로 유의한 감소를 관찰할 수 있었다. 3. 반하(半夏) 약쇄액(藥鎖液)에 의한 TARC 분비억제를 관찰 한 결과 농도의존적인 분비 감소 효과를 관찰 할 수 있었다. 4. MTT assay법을 이용한 세포 독성 측정에선 대조군과 반하(半夏) 약침액(藥鍼液) 처리군간에 유의성이 없었으므로 50, 100 및 200${\mu}g/m{\ell}$의 농도에선 세포독성이 없었음을 관찰할 수 있었다. 이에 반하(半夏) 약광액(藥鑛液)은 TARC 케모카인 억제를 통해 천식에 대한 치료효과를 나타낼 수 있을 것으로 사려된다.

노인의 건강상태와 복약실태 (A Study on the Status of Health and Medication in the Elderly)

  • In general the prevalence of old people is high and frequently have multiple diseases and symtoms requiring treatment. The nature of illness in the elderly has to be faced, and drugs do have an important part in the treatment of that illness. The purpose of this study were to describe health status and medication, and to provide some basic data for elderly's health education, especially for the right medication. Body mass index, self perceived health status, activities of daily living, disease and experience of past operation were surveyed to recognize the 249 elderly's actual health status. The degree of the elderly's understanding the way of medication, experience of side effects, experience of drug combination and incidence of drug adverse reaction along with drug combination were examined for medication. The aged $women(BMI; 10.7\pm13.3\%)$ overweighed the aged $men(BMI; 4.0\pm10.4\%)$. $69.0\%$ of them recognized their health good. Their activities of daily living were diminished following by the age group(p=0.0068) and relationship with self perceived health status were very significant(p=0.0005). They(192 elderly) suffered from multiple disease such as $osteoarthritis(49.5\%)$, $hypertension(32.0\%)$, gastric $disorder(16.1%)$, $diabetes(14.6\%)$, $osteomalacia(10.9\%)$, cardiovascular $disease(9.9\%)$, senile $cataract(5.7\%)$, skin $rash(5.2\%)$, liver $disease(4.2\%)$, kidney $disease(3.6\%)$, spinal cord $problem(3.6\%)$, respiratory $disease(2.1\%)$ $tuberculosis(1.0\%)\;etc(1.0\%).$ $28.3\%$ of them replied that they had an operation for appendictis senile cataract, peptic ulcer, spinal cord problem, pleurisy, hemorrhoid and the rest. Most of $them(87.4\%)$ knew the way very well how to use drugs, and $21.6\%$ of the replied 171 elderly experienced adverse drug reaction. Drug compliance rate were $high(83.6\%)$. In our study 56.9% of the 167 elderly took several medicine together. And $18.9\%$ of the 95 elderly who did drug combination had an experience of drug interaction. One person kept average 5.5 kinds of drugs at home among 243 elderly. They kept $digestives(79.4\%)$, $ointments(68.7\%)$, $vitamins(59.7\%)$, $analgesics(59.7\%)$, cold $medicines(45.3\%)$ antiarthritic $drugs(33.3\%)$, health $foods(27.7\%)$, antihypertensive $drugs(25.1\%)$, anti peptic ulcer $drugs(24.7\%)$, $laxatives(19.8\%)$, $antacids(16.5\%),\;antibiotics(l6.5\%)$, hypoglycemic $agents(10.3\%)$, cardiac $stimulants(7.0\%)$, $diuretics(4.5\%)$, antiarrhythmic $drugs(4.9\%)$, anti anginal $drugs(4.1\%)$, $hypnotics(3.3\%)$, $etc(38.3\%)$. With this result, we ascertain that polypharmacy in the elderly caused by multiple disease is common, which lead to drug interaction. So our task is to educate elderly how to use drugs in order to maximize their efficiency and to minimize their adverse effects.

Latex of Ficus carica L. Induces Apoptosis Through Caspase and Bcl-2 Family in FaDu Human Hypopharynx Squamous Carcinoma Cells

  • Ficus carica L. (common fig), one of the first plants cultivated by humans, originated in the Mediterranean basin and currently grows worldwide, including southwest Asia and South Korea. It has been used as a traditional medicine for treatment of metabolic, cardiovascular, and respiratory diseases as well as hemorrhoids and skin infections. Its pharmacological properties have recently been studied in detail, but research on the anti-cancer effect of its latex has been only been studied on a limited basis on several cell lines, such prostate cancer, breast cancer, and leukemia. In this study, we investigated the anti-cancer activity of the latex of Ficus carica L.and its underlying mechanism in FaDu human hypopharynx squamous carcinoma cells. (See Ed. note above) We confirmed through SDS-PAGE analysis and gelatinolytic activity analysis that the latex of Ficus carica contains cysteine protease ficin. Our data showed that the latex inhibited cell growth in a dose-dependent manner. In addition, the latex treatment markedly induced apoptosis in FaDu cells as determined by FACS analysis, elevated expression level of cleaved caspase-9, -3 and PARP (poly (ADP-ribose) polymerase), and. increased the expression of Bax (pro-apoptotic factor) while decreasing the expression of Bcl-2 (anti-apoptotic factor). Taken together, these results suggested that latex containing the ficin inhibited cell growth and induced apoptosis by caspase and the Bcl-2 family signaling pathway in FaDu human hypopharynx squamous carcinoma cells. These findings point to the potential of latex of Ficus carica to provide a novel chemotherapeutic drug due to its growth inhibition effects and induction of apoptosis in human oral cancer cells.

수술후성 기관협착증에 관한 임사적 고찰 (The clinical study for the postoperative tracheal stenosis)

  • 문명의 발달로 나날히 복잡해지는 사회 환경속에서 우리는 생명을 위협하는 수 많은 사고를 당하게 되며, 이럭 경우에 상기도를 유지하기 위한 기관절재술을 많이 시행하게 되는데, 때로는 기관협착등 이에 따른 여러가지 합병증을 유발하게 되는 경우가 있으며, 임상가들에게도 곤란한 문제를 갖어다 주고있다. 그러나 이러한 합병증은 수술방법의 개선과 항생제의 출현등으로 그 빈도가 많이 감소되기는 하였으나, 1969년 Lindholm은 술후성 기관협착증의 빈도를 1.5%내지 10%라고 했으며, 1969년 Donnelly, Mulder와 Rubush, 그리고 1971년 Andrew와 Pearson 등은 이들 원인의 대부분이 cuff 가 있는 기관삽입관이나, 기관케뉼(tracheal canula)의 부적합한 사용이나, 또는 감염에 의한 압박괴사(pressure necrosis)로 형성된다고 하였고, 협착부위로서 1972년 Bryce는 절개공(stoma)주위가 가장 많으며, 응급기관절개술시에 의사의 부정요법(mal-practice)도 원인이 될 수 있다고도 주의한 바 있다. 기관협착증의 치료로서 협착이 경미한 경우에는 일차적으로 세심한 관찰을 실시하면서 실리콘관(silicon tube)이나 스텐트(stent)를 사용하거나, 비강을 통한 삽관법(naso tracheal intubation)으로 기계적인 확장을 시도하며 (Schmiegelow, 1929, Montgomery, 1965), 육아조직이 형성되었을 경우에는 기관경검사하에서 이를 제거한 후에 steroid를 병용하는 편이 좋은데(Birck, 1970) 그밖에도 기관개찰술(Fenestration method, 백·홍 1974)이나 재수술(Revision)을 하기도 한다. 이러한 방법으로서도 치료가 불가능한 경우에는 그 협착 부분을 절제한 후에 단단문합술(End-to-End Anasto-mosis)을 시행하는 수도 있다. 저자들은 1967년 10월부터 1977년 3월까지 10연년간 세브란스병원에서 기관절개술을 받았던 1514례를 대상으로 일련의 조사를 실시하여 이들 중에서 기관협착증을 유발한 23례를 치료하였으며, 여기서 몇가지 지견을 얻었기에 보고하는 바이다.

Midline Granuloma의 방사선 치료 및 Steroid병용요법 (Combination Radiation and Steroid Therapy for Midline Granuloma)

  • 상기도에 오는 Midline Granuloma는 비강, 부비동 및 인후두부에 발생하여 국소성 괴저성 염증 및 육아종이 진행성으로 주위조직을 침범하여 결국은 사망을 초래케되는 불치의 병으로 알려져 왔다. 이 질환은 1897년 McBride에 의하여 처음으로 기술되었고, 1933년 Stewart이래 많은 임상적 및 조직학적 연구가 있었다. 그러나 현재까지 그 원인 및 확실한 치료방법이 알려져 있지 않고 있다. 본원에서는 1964년 1월부터 1980년 12월까지 17년간 Midline Granuloma로 진단받고 치료받은 15례에 대한 임상적 관찰과 방사선치료 및 Steroid병용요법으로 치료받은 7례를 분석하여 다음과 같은 결과를 얻었다. 1) 성별 및 연령 : 30세부터 39세사이가 6례 (40%)로 가장 많았으며, 남자 13례 (87%) 여자 2례 (13%)였다. 2) 초발 병변 부위는 비강 및 비중격 부위가 8례 (53%), 인두편도 3례 (20%) 및 인두부 2례 (13%)의 순이었다. 3) 증상으로는 비폐색과 인두통이 각각 6례 (40%)로 가장 많았고, 두통5례(33%), 비루 및 안면 부종이 각각 3례(20%)의 순이었다. 4) 4례에서 균을 검출하였으며 포도상구균 2례, 녹농군 1례, 연쇄상구균이 1례였다. 5) 방사선 치료와 Steroid병용요법을 실시한 7례에서는 전례에서 현재까지 추적 관찰한 결과 재발이나 악화가 없었다. 6) Steroid와 항생제 치료를 받았던 5례중 2례는 사망하였고 3례는 일시적인 증상의 호전을 보였으나 그 후 악화를 보였다.
