Strategic Planning for the Contract-Managed Hospital Foodservice Through QFD Methodology (QFD 기법을 이용한 병원 위탁급식 운영전략 수립)

  • At present, health care industries throughout the world are struggling with the challenges to set up financial structures as cost-effective ways and means of satisfying customer needs for health care services. Many hospitals consign foodservice management to foodservice companies for the purpose of efficiency. The companies taking charge of hospital foodservice are also striving to gain an advantage over keen competitions. This study applied Quality Function Deployment(QFD) to one hospital (which will be shown as $\ulcorner$A hospital$\lrcorner$ below) managed by a contract foodservice company for the purpose of strategy planning to provide sustainable competitive advantage. First of all, this study scanned internal and external environment of $\ulcorner$A hospital$\lrcorner$ by means of a Quality Measurement Tool and a fieldwork study. With the result of environment scanning, this study elicited 20 strategies through SWOT analysis, which were categorized by 4 perspectives such as financial, customer, internal process, learning and growth perspectives. Finally, the priorities of 20 strategies were extracted from QFD methodology. According to the results obtained by applying QFD to $\ulcorner$A hospital$\lrcorner$'s foodservice, the strategies which $\ulcorner$A hospital$\lrcorner$ foodservice was obliged to introduce and implement were : the specialization of Children's hospital foodservice, scientific foodservice management through the standardization of foodservice operations, the maintenance of sanitary quality through sanitary system, the remodeling of facilities, the introduction of new equipment, the prompt and accurate response to customer needs, the development of appropriate patient menus, the provision of competitively priced meals for patient selection, the development of a demand forecast model by considering the characteristics of a children's hospital, improvement of productivity and the reduction of labor costs through the employment of experienced employees based on their seniority.

A Proposal of Cohousing for Dual Income Families in Korea - on the Basis of Interview and Mimic Test - (맞벌이 가족을 위한 코하우징 제안 - 설문지를 이용한 면접조사와 모의실험을 기초로 -)

  • The purpose of this research was to reduce the burden of household management and the difficulty of rearing children, which are two most important obstacles for a dual-income family. For this, we suggested a cohousing project, as an alternative concept to conventional housing, where people can socially interact with each other and share mutual cooperation for a better quality of life. We surveyed the lifestyle of dual-income families in Korea, how they understood the cohousing project and their opinions on it. We carried out a survey with the wives from dual income families who were living in the metropolitan areas including Seoul. We contacted 30 wives for the survey to determine the reality of the double income couple family and interviewed them to make a detailed plan for cohousing based on their understanding and needs. In the results, many women who responded to the interview agreed that cohousing was an ideal housing arrangement for the dual-income family. However, they were anxious that it would take too much time and effort to develop their own housing estate by themselves. Therefore, we looked for a way to save time, efforts and cost in developing a cohousing project and came up with the remodeling as a solution.

Mutational Analysis of Extranodal NK/T-Cell Lymphoma Using Targeted Sequencing with a Comprehensive Cancer Panel

  • Extranodal natural killer (NK)/T-cell lymphoma, nasal type (NKTCL), is a malignant disorder of cytotoxic lymphocytes of NK or T cells. It is an aggressive neoplasm with a very poor prognosis. Although extranodal NKTCL reportedly has a strong association with Epstein-Barr virus, the molecular pathogenesis of NKTCL has been unexplored. The recent technological advancements in next-generation sequencing (NGS) have made DNA sequencing cost- and time-effective, with more reliable results. Using the Ion Proton Comprehensive Cancer Panel, we sequenced 409 cancer-related genes to identify somatic mutations in five NKTCL tissue samples. The sequencing analysis detected 25 mutations in 21 genes. Among them, KMT2D, a histone modification-related gene, was the most frequently mutated gene (four of the five cases). This result was consistent with recent NGS studies that have suggested KMT2D as a novel driver gene in NKTCL. Mutations were also found in ARID1A, a chromatin remodeling gene, and TP53, which also recurred in recent NGS studies. We also found mutations in 18 novel candidate genes, with molecular functions that were potentially implicated in cancer development. We suggest that these genes may result in multiple oncogenic events and may be used as potential bio-markers of NKTCL in the future.

In-situ Production Analysis of Composite Precast Concrete Members of Green Frame

  • Recently, there have been many cases in which the difficulty of repair and replacement of principal elements in the bearing wall structure for apartment buildings, which is a major part of apartment buildings in Korea, has led to the reconstruction of buildings rather than their remodeling. To address this problem, the Korea government now allows a floor area ratio of up to 20 %, and has relaxed the building height limits to encourage the use of a rahmen structure instead of a bearing wall structure. However, since reinforced concrete rahmen structures have many problems, including higher floor height and greater construction cost, a great deal of research into rahmen composite precast concrete structures have been conducted. Green Frame, one of the developed prototypes, is expected to provide economic benefits through in-situ production for precast concrete column and beam. For in-situ production of composite precast concrete members, a detailed plan for production, curing, and installation is needed. However, it needs to be confirmed that the space is sufficient to produce the precast concrete members on-site before planning those activities. Therefore, this study proposes in-situ production analysis of composite precast concrete members of Green Frame with the evaluation of structural safety and available area on the parking structure. The result of this study shows that the in-situ production of precast concrete members is possible through a case study.

A Simple Evaluation Method Using a "Sun-Wukong's Headband" Wire for Positional Plagiocephaly Correction after Cranial Remodeling Helmet Therapy (손오공 머리띠를 이용한 간편한 사두증 치료결과 평가방법)

  • Purpose: A cranioremodeling helmet for correcting plagiocephaly was recently developed. However, no discrete objective methods to evaluate how the deformity is being corrected have been developed. We have established an easy and cost-effective method that can be used not only to show the correction process, but can also be used by physicians to assess the degree of plagiocephaly two-dimensionally. Methods: For two-dimensional evaluation, a length of malleable memory wire (2 mm in diameter) resembling "Sun-Wukong's headband" was placed on the patient's head. The wire around the patient's head was positioned on a plane including points 1 cm above the eyebrow and 1 cm above the auricle. The wire was placed on a sheet of paper and the outline was marked using pens of various colors during each visit. The degree of plagiocephaly correction could then be shown to the patient's parents at every consultation. Results: The method established by the present study easily shows the horizontal cross-section transformation of the head, illustrates plagiocephaly correction by the helmet, and shows the degree of correction in a two-dimensional manner. Conclusion: A soft-shell helmet is widely used for correcting plagiocephaly. However, evaluating the effectiveness of the helmet has been determined in a subjective manner, and a more objective method is now in demanded. Our study found that a "Sun-Wukong's headband" wire can accurately measure two-dimensional changes. Future studies will be required to identify landmarks needed for assessing plagiocephaly correction.

Automatic Algorithms of Rebar Quantity Take-Off of Green Frame by Composite Precast Concrete Members (합성 PC부재에 의한 그린 프레임의 철근물량 산출 자동화 알고리즘)

  • As the bearing wall structure, which has been widely applied to domestic apartment buildings since the 1980s, cause many problems during remodeling of buildings, the government encourages constructors to adopt flat plate or rahmen structure through legal incentives. In line with such a trend, the green frame, an eco-friendly rahmen structure that has removed the shortcomings of previous structures, was developed to enhance structural safety, constructability, and eco-friendliness. The construction of green frame can reduce the labor cost and facilitate the composition of iron bars to reduce rebar loss through calculating the quality and establishing the bar bending schedule automatically on the precast concrete member data collected over the design phase. Therefore, the purpose of this study is to develop the algorithm to automate the calculation of iron bar volume for the green frame designed on composite precast concrete members. Automated algorithm to calculate concrete structural design information and design information. Practices through the application site should prove efficacy. The database established by the developed algorithm will automate the establishment of iron bar processing map and bar cutting list and the calculation of optimal composition and order volume to minimize the rebar loss. This will also reduce the expenses on management staff and overall construction cost through the minimization of rebar loss.

Selecting Decision Variable for a Plant-wide Optimization (석유화학공장 규모 최적화를 위한 변수 선정)

  • Chemical plants which consume lots of energy are not operating in the best conditions due to their own peculiar nonlinearity, instability, and diverse disturbances. In order to improve this, the plant wide optimization was performed. It is important to select the most appropriate number of decision variables which strongly affect the operating cost because there are too many decision variables which economically have an effect on plant wide. For instance, if all decision variables which can economically affect are applied in optimization and then the result of the optimization is applied to operation, a lot of operating conditions should be going to be changed. As a result of changing a plenty of operating conditions, the cost of the change will absolutely increase. Thus, in this study, the method of selecting the most appropriate decision variables which can influence on saving operation costs was presented in order to optimize plant wide. TPA (Terephthalic-acid) plant is considered as a case study. In other word, after modeling, the most proper decision variables was selected by examining the degree which decision variables influence on operating costs through sensitivity analysis. In TPA process, the three decision variables were selected by the presented method in this study. Then the plant was optimized by selected the decision variables. Consequently, it was seen that the plant are expected to save the 350 million won of energy annually without additional investment for facilities or remodeling of the plant.

Comparative Study on the Job Satisfaction of Part-time Nurses and Internship Nurses (시간제 간호사 및 인턴간호사의 직무만족도에 관한 비교연구)

  • Recently, Korean health care industry managers are paying more attention to customer oriented service, the rationalization of business administration, and quality control of service to adjust their business to outer environment since IMF bailout program. They are focusing on the cost reduction through remodeling the size and structure of man power, viz. labor flexibility. Nurses are not free from this juncture and contingent nurses are increasing fast. This article intends to verify the actual condition of two types of contingent nurses -part-time nurses and internship nurses-and to compare their job satisfaction to provide basic resource for efficient management of nurse man power. The concrete goals of this article are; Firstly, to verify the actual condition of their employment. Secondly, to compare their job satisfaction. And Thirdly, to the relation between employment condition and job satisfaction. To accomplish these research goals, a statistical survey was executed, in which 384 questionnaires - 66 for manager nurses, 318 for contingent nurses-were given to nurses working at 66 general hospitals-which have at least 100 beds-in Seoul. Among them, 121 questionnaires-of 28 general hospitals-were returned. Then, the data coded and submitted to mean, standard deviation, T-test, variance analysis (ANOVA), correlation analysis, multiple regression analysis with SAS program. The research results of the contingent nurses are followings: 1. Two types of contingent nurses shows similar age spans: they are mostly 21 - 30 years old and unmarried. But internship nurses have high level of educational career. Part - time nurses spread at general beds, out -patient part, intensive care part, operation part, etc, but internship nurses work mostly at general beds. 2. Two groups shows difference in actual employment condition: average employment career of part -time nurses is 7.0 months but internship nurses' is 2.0 months: average duty-on days per month of part - time nurses are 23.7 days but internship nurses' are 24.8 days. But there are little difference in average working time per day: 7.7 hours for part -time nurses and 0 hours for internship nurses. 3. The average wage per month for part -time nurses is 836,026 won but for internship nurses is 557,428 won-66.7% of part-time nurses'. Both groups are enjoying little additional pay. 4. Both groups are getting job not so much through advertisement of newspaper or hospitals as through acquainted person or college. 5. Both groups show very high level of job satisfaction: 3.2195 for part -time nurses and 3.2881 for internship nurses. But they show very low satisfaction on payment level compared with other categories and two groups show meaningful difference(P<.0001). 6. The multiple regression test reveals the factors related with job satisfaction: wage level, working part(OR or ICU), age, job career, and motive of contingent job-taking('because I can take care of family duties at the same time') influence positively: motive of contingent job-taking('because I can work regularly without alternation') influences negatively.

Economic Evaluation System for Deteriorated Military Facilities (노후 군시설물의 경제성 평가 시스템)

  • Given both structural safety and economic benefits of aging facilities, remodelling of the existing facilities is preferable to reconstruction. This recognition provides an opportunity to reduce the commitment of resources and national budget. However, when a subordinate troop asks for remodeling or reconstruction of a deteriorated facility, it is difficult to ensure the consistency and objectivity in the process of decision making for the alternatives due to the absence of systematic and quantitative rating methodology. Their economic evaluation methodology only exists in a manual format. Thus, further research is required for converting the methodology into an automated system in view of practicality such as rapid and accurate data processing. The contributions of this study are as follows: 1) Literature review found out a representative economic evaluation model focused on military facilities, and comparative analysis with a similar study identified the strength and weakness. 2) this study presented how to convert the theoretical framework which enables to solve a specific subject matter into an automated system. 3) it developed a user friendly interfaces which consist of four functional modules by considering the usability and accessibility of the system user. 4) the developed system was verified by a case study in terms of four kinds of performance indicators.

  • The modular construction method has been getting more attention followed by global eco-friendly trend as the domestic construction industry has focused more on remodeling and extension work. The modular construction method is an industrialized construction system which is not likely as the existing construction method it manufactures more than 70% modules at the factory then assembling can be completed in a short amount of time on site. The modular construction method has various strengths; shortening of construction period by on-site work decrease, weight pressure reduction by usage of light steel frames and cost saving by repetitive manufacturing. However, it is currently not expanded due to the existing commercialized construction method. Therefore, this research is in order to help the related authorities make decisions to select the construction method and motivate expansion of modular construction method which can be utilized effectively in the extension works. The intention of this research is to stress differentiation from other construction methods in construction period, construction expenses, labor and forces by comparing and analyzing actual cases, to inform competitiveness of modular construction method by concrete effect analysis and to support adoption of the modular construction method into the domestic industry.
