Recovery of the ascending reticular activating system and consciousness following comprehensive management in a patient with traumatic brain injury: a case report

  • We report on changes in the ascending reticular activating system (ARAS) concurrent with the recovery of impaired consciousness following rehabilitation and cranioplasty in a patient with traumatic brain injury (TBI), which were demonstrated on diffusion tensor tractography (DTT). A 34-year-old male patient was diagnosed with a traumatic intracerebral hemorrhage after falling from a height of approximately 7 m and underwent a right frontoparietotemporal decompressive craniectomy and hematoma removal. At 5 months after onset, when starting rehabilitation, the patient showed impaired consciousness, with a Glasgow Coma Scale (GCS) score of 4. Comprehensive rehabilitative therapy was provided until 14 months after onset, and his GCS score improved to 8. Cranioplasty was performed using auto-bone at 14 months after onset. One month after cranioplasty, his GCS score improved to 12. On the 15-month DTT, the deviated lower dorsal ARAS was restored on both sides, and the right side had become thicker. The right lower ventral ARAS was reconstructed, and increased neural connectivity of the upper ARAS was detected in both the prefrontal cortices. Thus, changes in the ARAS were demonstrated in a patient with TBI during recovery of consciousness following rehabilitation and cranioplasty.

  • The people who is returning to the community after hospital treatment are faced with the choosing where to proceed with rehabilitation exercise with an appropriate exercise specialists based on the current health condition of them. It is required to supprot a personalized exercise rehabilitation service (hereinafter referred to as ARES system) in the community based on individual disease information and health conditions based on hospital clinical data. In this study, we designed a resource management system for online-to-offline (O2O) based coach matching to improve accessibility with facilities and coaches for exercise rehabilitation in the community.

  • Objectives The purpose of this study is analyzing internal research trends of oriental obesity pattern identification in korean literature based on obesity pattern identification questionnaires, development and application of Pattern Identification System to help solve problems of future researches. Methods 6 Korean databases were searched for articles of oriental obesity pattern identification, irrespective of publication year and 13 studies were reviewed. An analytical method such as descriptive statistics and an actual number and percentage was used. Results We collected 13 studies. 4 studies were published in 2012 and 2008, the highest number of studies. 7 Clinical studies were the major research method. The Studies were classified according to the characteristics and design. 5 studies were about research of analysis and improvement of oriental obesity pattern identification questionnaire, the most number of studies. 4 studies were about research of observational studies in clinical on obesity pattern identification. 3 studies were about research of intervention studies in clinical on obesity pattern identification and 1 study was about clinical practice recommendation. Conclusions Establishment of obesity pattern identification system and its clinical application could lead to standardizing obesity pattern identification and clinical practice guideline. Applied on other diseases, obesity pattern identification system could also lead to improving treatment rate, contributing to the development of clinical practice guidelines and academic field of research.

  • KARES (KAIST Rehabilitation Engineering System) is the rehabilitation robot system in the type of the 6 degrees of freedom robot arm mounted on the wheelchair, in order to assist the independent livelihood of the disabled and the elderly. The interface device for programming and controlling of the robot arm is essential in the rehabilitation robotic system. Specially, in the case of the manual operation of the robot arm, the user has the burden of cognition and the difficulty for the operation of the robot arm. As a remedy, color vision system for the autonomous performance of jobs is proposed, and four basic desired jobs are specified. By mounting the camera in eye-in-hand type, color vision system for KARES is set up. The desired jobs for picking up the target and moving it to the user's face for drinking are successfully performed in real-time at the indoor environment.

  • This study was conducted to investigate the nursing diagnosis applying to rehabilitation unit. The subject of this study was consisted of 101 nurses who have been working over 1 year in rehabilitation unit. The clssification of nursing diagnosis used NANDA's system and analysis of the validity was based on Fehring's model. In summury of this study. some dirrerences were found in related factors in nursing diagnosis between rehabilitation and general unit. Major related factors of nursing diagnosis were physical factors associated with neuromuscular disorder. Valid related factors in altered nutrition more than body requirements was not found for rehabilitation unit. It is helpful for the nurses who work at rehabilitation unit to apply the nursing diagnosis validated in this study. This finding can be used as the database for accomplished nursing diagnosis appropriate for improving the rehabilitation nursing practice.

  • This paper presents upgraded equipment to assist a patient in rehabilitating of their knee-joint by themselves. A new automatic mechanism is suggested and developed in order to add functions for enforcing the leg muscles, which were absent in previous rehabilitation machines. Using the Pro-engineer software, we analyze the displacements and speeds of several moving points of the equipment during various planned exercises. In addition, an appropriate control panel for operation is developed. Three motors andthree motor drivers are used in a tilting part for the ankle joint, sliding guide part for the knee joint, and up-down moving part for the whole leg. Finally, various newly upgraded motions can be generated by controlling the three motors simultaneously. we show that by using this equipment, we can adjust the proper length of the equipment according to the user's height and the intensity of the rehabilitation exercise.

  • Balance measurement is widely used in fields of physiotherapy, neurology, sport medicine and rehabilitation medicine. This paper presents the development of balance measurement and rehabilitation training system. To this end, we have developed a prototype system using an 3-axis accelerometer sensor attached to the under surface of balance board. As a results, the system is stable and shows a good degree of balance function. Also, the various patterns for rehabilitation training can be added easily. This system can be used for daily balance monitoring and will contribute to increasing the effectiveness of training.

Reviewing Research on Fracture Animal Experimental Study with Herbal Medicine Intervention in Korea (한약을 중재로 한 골절 동물실험연구에 대한 국내 문헌 분석)

  • Objectives The aim of this research is to analyze research trends on fracture animal experimental study in Korea. Methods For reviewing research, we constructed a key question as the effect of herbal medicine on fracture healing and selected fracture animal experimental research. We searched the following 6 databases: KoreaMed, Kmbase, Koreanstudies Information Service System (KISS), National Digital Science Library (NDSL), Korea Institute of Science and Technology Information (KISTI), Oriental Medicine Advanced Searching Integrated System (OASIS). Results A total of 283 potentially relevant studies were identified; only 12 studies were selected for reviewing research. Almost studies showed that herbal medicine had significant effect on fracture healing. Conclusions The research showed that herbal medicine could significantly stimulate fracture healing and reduced inflammation. In the future, more fracture animal experimental study should be actively conducted.

Research Trends of Herbal Medicines for Obesity: Mainly since 2015 to 2019 (한국에서 비만 치료에 쓰이는 한약에 대한 연구 동향 보고: 2015년부터 2019년까지의 국내외 논문을 중심으로)

  • Objectives This study has been conducted to verify frequently used herbs and herbal formulas to treat obesity in Korea during 2015 to 2019. Methods Publications related to treatment of obesity using herbal medicines from 2015 to 2019 were retrieved from 12 databases (PubMed, EMBASE, Scopus, China National Knowledge Infrastructure [CNKI], Koreanstudies Information Service System [KISS], National Digital Science Library [NDSL], Research Information Sharing Service [RISS], DBPIA, KMBASE, KoreaMed, KoreanTK, Oriental Medicine Advanced Searching Integrated System [OASIS]). Extracted articles were analyzed in terms of publication year, journal, and type of herbs. Results A total of 136 studies have been included in this study. 33 herbal formulaes and 129 herbs have been analyzed. Taeyeumjowui-tang was the most frequently studied herbal formulae and Ephedrae Herba was the most frequently studied herb. Ginseng Radix was the most frequently studied single herb in isolation from herbal formulae. Conclusions These results suggest that various herbs and herbal formulae can be used to treat obesity. Since this study includes every type of study, additional studies based on clinic are thought to be necessary.

  • Objectives This study was conducted to evaluate the therapeutic effect of acupuncture after rotator cuff surgery. Methods Key words such as rotator cuff surgery and acupuncture were searched in 10 databases (Ovid-Medline, Ovid-EMBASE, Ovid-AMED, Cochrane Library, China National Knowledge Infrastructure [CNKI], National Digital Science Library [NDSL], Koreanstudies Information Service System [KISS], Oriental medicine Advanced Searching Integrated System [OASIS], KoreaMed, KMBASE), and only suitable randomized controlled trials (RCTs) were selected. Results 6 RCTs were finally selected, and acupuncture showed positive results for pain relief and functional recovery after rotator cuff surgery. However, the analyzed 6 RCTs showed a high overall risk of bias. Conclusions In the future, RCTs with a higher level of evidence on the effect of acupuncture after rotator cuff surgery need to be continuously conducted.
