Conservative Management and Postoperative Rehabilitation of Chronic Lateral Ankle Instability (만성 외측 족관절 불안정성의 보존적 치료와 수술적 치료 후의 재활운동)

  • As a result of the increased participation in recreational and competitive sports activity, the incidence of chronic lateral instability has risen. Because chronic ankle instability can cause significant comorbidity and increase the social cost, the interest in this issue is growing. Although there are well-documented and effective surgical treatments for managing this condition, conservative treatments are a viable first option in selected patients. Through conservative treatment, surgical treatments can be reduced or delayed without necessarily affecting the overall outcome, but the failure of conservative treatment results in the need for surgical treatment to reduce the long term complications. Appropriate postoperative rehabilitation is essential for adequate functional recovery, even when surgical treatment is required.

Forecasting and Prolonging Method of Optimum Economic Life for Office Buildings using Life Cycle Cost (LCC분석에 의한 오피스건축물의 최적경제수명추정분석 및 장수명화에 관한 연구)

  • The purpose of this study is to estimate and analyze the optimum economic life of office buildings in consideration of their LCC (Life Cycle Cost), and thereby, explore the ways to manage the outlived office buildings economically. In estimating the economic life, initial investment cost and maintenance cost are taken into due consideration. For this study, those office buildings in Yeouido and Mapo region were sampled. The surveyed details were reduced to unit area to calculate a unit value, and then, their optimum economic life was estimated using LCC. Five alternatives for management of outlived office buildings were compared in terms of reconstruction or rehabilitation cost.

  • The demand for light-rail construction projects has recently been increasing, and they are mostly supervised by private construction companies. Therefore, a private construction company that aim to raise gains from the operation of the facilities during the contract period greater than what they invested should b able to accurately calculate the costs from the aspect of Life Cycle Cost (LCC). In particular, a light-rail transit bridge that has a heavier portion from the aspect of the cost of light-rail transit construction requires a more accurate calculation method than the conventional LCC calculation method. For this, an LCC analysis model was developed and a cost breakdown structure was suggested based on literature review. The construction costs by shape of the upper part of a light-rail transit were calculated based on the cost breakdown system presented in this paper, and the cost generation cycle and cost unit price were collected and analyzed based on records on maintenance costs, rehabilitation and replacement. In addition, after forming some hypotheses in order to perform the LCC analysis, economic evaluation was conducted from the aspect of the LCC by using performance data by item.

Parents' Perceptions of Cognitive Rehabilitation for Children With Developmental Disabilities: A Mixed-Method Approach of Phenomenological Methodology and Word Cloud Analysis (발달장애 아동 부모의 인지재활 경험에 대한 질적 연구: 워드 클라우드 분석과 현상학적 연구 방법 혼합설계)

  • Objective : The purpose of this study was to investigate parental perspectives on cognitive rehabilitation using a combination of phenomenological research methodology and word cloud analysis. Methods : Interviews were conducted with five parents of children with developmental disabilities. Word cloud analysis was conducted using Python, and five researchers analyzed the meaning units and themes using phenomenological methods. Words with high frequency were considered as a heuristic tool. Results : A total of 43 meaning units and nine components related to the phenomenon of cognitive rehabilitation were derived, and three themes were finalized. The main themes encompassed the definition of cognitive rehabilitation, challenges associated with cognitive rehabilitation, and factors influencing the selection of a cognitive rehabilitation institute. Cognitive rehabilitation emerged as a treatment focused on improving learning, daily functioning, and cognitive abilities in children with developmental disabilities. The perceived issues with cognitive rehabilitation pertained to treatment methods, therapist expertise, and associated costs. In addition, parents highlighted the importance of therapist expertise, humane personality, and affordability of cost and schedule when choosing a cognitive rehabilitation institute. Conclusion : Parents expressed expectations for substantial improvements in their children's daily functioning through cognitive rehabilitation. However, challenges were identified in clinical practices. Going forward, we expect that cognitive rehabilitation will evolve into a better therapeutic support service addressing the concerns raised by parents.

  • Based on related studies on 'disability sports,' 'sports for all,' and 'rehabilitation sports,' this study collected the results of a survey on the disabled, who are the actual users of rehabilitation sports services. It analyzed the need for rehabilitation sports services. The requirements of persons with disabilities were extracted, and the extracted contents were presented as inductive content analysis. The researcher's preconceptions and subjective judgments were to be excluded as much as possible. Based on studies related to "Disability sports," "Sports for all for disability," and "Rehabilitation sports," conducted in Korea, a study that included the results of the survey on disabled people was selected. This study's results were categorized into eight categories (perception of sports activities, programs, instructors, facilities, accessibility, cost/support/financial resources, and others) for people with disabilities.

Current Status and Policies for Improvement of Korean Water works Based on the Analyis of Revenue and Operation Costs (최근 세입·세출 분석을 통한 우리나라 수도사업의 현황 및 개선방안)

  • To understand the financial situation of waterworks, the revenue and operation cost from 1996 have been analyzed. The budget of waterworks was 3,775 billion won in 1996 and grew up to 5,774 billion won in 2007. The budget has grown about 1.53 times during 11 years. Based on 2007, total cost for supplying water, rehabilitating old facilities and returning debt and its interest was 4146 billion won. The cost was recovered by 2,825 billion won of water revenue, 827 billion won of various fees revenue, 571 billion won of financial grant and 89 billion won of loan. Until 2007, the 15.3% of operation cost for waterworks was covered by government grant and loan. The renovation cost for old non-efficient facilities was 1,356 billion won in 1996 and was 1,486 billion won in 2007. The renovation cost was not increased remarkably during 10 years. Since the total operation cost was increased about 1.5 times, the percentage for renovating old facilities was decreased from 36% to 25.7%. This trend need to be alerted because the renovation has been slow down when the facilities for water supply need more rehabilitation.

Effects of Long-Term Harbor Shutdown and Temporal Operational Stoppage upon Optimal Design of Vertical Breakwater Caisson (장기간의 항만 폐쇄와 일시적 운영 중단이 직립 방파제 케이슨의 최적 설계에 미치는 영향)

  • In this study, a model to calculate the expected total construction cost is developed that simultaneously considers the rehabilitation cost related to the sliding of the caisson, the economic damage cost due to harbor shutdown in the event of excessive caisson sliding, and the economic damage cost due to temporal operational stoppage by excessive wave overtopping. A discount rate is used to convert the damage costs occurred at different times to the present value. The optimal cross-section of a caisson is defined as the cross-section that requires a minimum expected total construction cost within the allowable limit for the expected sliding distance of the caisson during the lifetime of the breakwater. Two values are used for the allowable limit: 0.3 and 0.1 m. It was found that the economic damage cost due to harbor shutdown by excessive caisson sliding is more critical than the rehabilitation cost of the caisson or the economic damage cost by excessive wave overtopping in the decision of the optimal cross-section. In addition, the optimal cross-section of the caisson was shown to be determined by the allowable limit for the expected sliding distance rather than the minimum expected total construction cost as a larger value is used for the threshold sliding distance of the caisson for harbor shutdown.

A Literature Study on Usage of and Satisfaction Levels with Combined Treatment Including Oriental and Western Medicine

  • Objective: This study aimed to summarize and analyze the usage of and the satisfaction levels with combined treatment including Oriental and Western medicine. Methods: We searched studies on the usage of and the satisfaction levels with combined treatment including Oriental and Western medicine over the past 10 yrs (2001-2011) from 3 Korean databases (National Assembly Library, Research Information Service System, and National Discovery for Science Leaders). The reviewers also conducted a summarizing analysis by sampling the literature according to the type of study, study period, region, study subjects, sample size, type of sampling, research method, data analysis, study instruments, main results, etc. Results: When the main results of six studies on combined treatment usage and satisfaction levels were considered together, the most important decisive factor in determining the usage of combined treatment was the illness of the patient, followed by the patient's occupation, sex, age, education, marital status, religion, treatment cost, and treatment results. In addition, the most important factor that determined satisfaction levels with combined treatment was age, followed by education, religion, income, health status, treatment procedures, staff attitude, and cleanliness. Conclusions: Elderly patients with musculoskeletal, cerebro-vascular, and circulatory system illnesses are more likely to prefer combined treatment over independent Oriental or Western treatment and are more likely to request specialized, adjusted medical care.

  • In this study, we propose a non-contact circular eddy current load brake using permanent magnet that can be applied to the rehabilitation exercise equipment. The circular eddy current load brake is manufactured and is evaluated for performance. This has the torque value of 49% compared to a "ㄷ" type eddy current load brake having a fixed load. And we confirmed that load is regulated due to the conductivity and air gap. Proposed load brake is adjustable according to the user's condition and can be applied to the equipment having advantages such as miniaturization and cost reduction.

The Effects of Making Horcitultural Products and Selling Activities on Vocational Rehabilitation of Persons with Mental Disabilities

  • This study carried out a horticultural program for people with mental disabilities to determine suitability of the horticultural activity program and to actually apply to vocational rehabilitation. The program was carried out in 16 sessions from April 12 to June 7, 2018 with nine people with a strong will to participate in vocational rehabilitation at a health center for persons with mental disabilities. The tools used were the Horticultural Activity Performance Assessment (psychological behavior) and Purdue Pegboard Test that examines functions of hands and arms. The subjects sold the products they made during class time and obtained the profits. After the program, the scores increased with a significant difference (p = .000) in the psychological behavior area of the Horticultural Activity Performance Assessment. Hand and arm movement also showed continuous improvement along the program with a significant difference (p = .000), as well as finger dexterity also with a significant difference (p = .018). The net profit was 314,000 KRW (subtracting cost of goods 314,000 KRW from total sales 628,000 KRW). The profit was shared equally among the nine subjects, each receiving 34,900 KRW. Therefore this study proved horticultural activity class is suitable for vocational rehabilitation of people with mental disabilities proved by the positive effects. We also expect that the subjects will be able to make more profits if they learn more skills, since they are interested the process of making such profits.
