약물에 노출되지 않은 공황장애 환자들에서 인지행동치료 후 뇌혈류 변화에 대한 예비연구 (Preliminary Study for Changes in Brain Perfusion in the Drug-Naive Patients with Panic Disorder with SPECT Following Cognitive-Behavioral Therapy)

  • Objective : Although cognitive-behavior therapy (CBT) is effective in patients with panic disorder, its the-rapeutic mechanism of action in the brain remains unclear. This study was performed to investigate regional blood flow changes associated with successful completion of CBT in drug-naive patients with panic disorder. Method : The regional blood flow in 4 patients with panic disorder was compared to that in 11 healthy controls before and after a 12-week group CBT using $^{99m}Tc$-ECD SPECT imaging. Psychopathology was assessed using Panic Disorder Severity Scale. Data were analyzed using software for statistical parametric mapping (SPM2). Results : Before CBT, significantly decreased blood flow was found in the parietal and occipital area in panic patients than normal volunteers. In all the patients who showed remission after CBT, increased blood flow was detected in the right cingulate gyrus, left lingual gyrus, and left superior parietal lobule, whereas decreased blood flow was seen in the left inferior temporal gyrus. Conclusion : These results suggested that CBT is effective for panic disorder and change the activity of cingulate gyrus and left temporal gyrus, a part of the brain areas associated with fear in panic disorder.

익모초(益母草) 추출물이 뇌허혈 흰쥐의 뇌혈류량 및 사이토카인에 미치는 효과 (Effects of LEONURI HERBA Extract on the Cerebral Blood Flow and Cytokines in Cerebral Ischemic Rats)

  • The study was designed to investigate the effects of Leonuri herba extract (LHE) on the change of regional cerebral blood flow (rCBT and cytokines production (IL-1${\beta}$, TNF-${\alpha}$, IL-10, TGF-${\beta}$) in ischemic rats. The results were as follows : The rCBF was significantly and stably increased by LHE (10 mg/kg, i.p.) during the period of cerebral reperfusion, which contrasted with the findings of rapid and marked increase in control group. In cytokine production of serum by drawing from femoral arterial blood at 1 hr after middle cerebral arterial occlusion, experimental group (LHE 10 mg/kg treated group) was significantly decreased IL-l${\beta}$ production, and increased TGF-${\beta}$ production compared with control group. In cytokine production of serum by drawing from femoral arterial blood at 1 hr after reperfusion, experimental group was significantly decreased IL-1${\beta}$ production compared with control group. IL-10 and TGF-${\beta}$ production of sample group were significantly increased compared with control group. These results suggested that LHE was significantly and stably increased regional cerebral blood flow by inhibited IL-1${\beta}$ production, and accelerated IL-10 and TGF-${\beta}$ production.

삼소음(蔘蘇飮)이 백서(白鼠)의 국소뇌혈류량(局所腦血流量)과 혈압(血壓)에 미치는 영향(影響) (The Effects of Samsoeum Extracts on Regional Cerebral Blood Flow and Blood Pressure in Mice)

  • Objective : The objective of this study is to measure the changes of regional cerebral blood flow(rCBF) and blood pressure(BP) in rats with the following injection. Methods : The measurement was continually monitored by laser-doppler flowmeter-Transonic Instrument, USA- and pressure tranducer(Grass, USA) in anesthetized adult Sprague-Dawley rats about for two hour to two hour and a half hours through the data acquisition system composed of Maclab and Macintosh computer. Result : This experiment with Samso-eum increased the changes of rCBF in rats, significantly, but did not change the blood pressure. The rCBF of Samso-eum decreased by pretreated propranolol, and was not changed by pretreated L-NNA and ODQ. Conclusion : It is considered that the dosage-dependent increase on rCBF is mediated by adrenegic ${\beta}$-receptor.

한약처방을 활용한 Medicinal Food의 개발에 관한 연구 - 생맥산이 수축동맥의 이완과 국소뇌혈류량에 미치는 효과 및 기전 - (Study on Oriental Prescription for Medicinal Food Applications -Effects and Mechanism of Saengmaegsan on the Relaxation of Contracted Artery and Regional Cerebral Blood Flow-)

  • This study was performed to provide basic data that predict the application of Saengmaegsan(SMS) as medicinal food. SMS has been used in oriental medicine for many years as a therapeutic agent for cerebral disease. We examined the effects of SMS on physiological function in isolated abdominal aorta and femoral artery from rabbit and measured the changes of regional cerebral hood flow(rCBF), which was continually monitored by laser-doppler flowmeter and pressure transducer in anesthetized adult Spargue-Dawley rats through the data acquisition system composed of MacLab and Macintosh computer. The contraction forces by injection of norephinephrine in isolated abdominal aorta and femoral artery were significantly decreased in each concentration of SMS treatment compared with control. rCBF was increased by SMS in a dose-dependent manner. These results suggest that SMS causes a diverse response of rCBF and arterial diameter. These mechanism in rCBF increase may be mediated by prostaglandis, cyclic GMP and adrenergic $\beta-receptor$. Also mechanism in artery contraction decrease is also mediated by prostaglandis and cyclic GMP. These results indicate that SMS can be nsed as a safe and clinically applicable as a supplementation of diet therapy for cerebral cardiovascular disease patients.

심적환(心適丸)이 산화적 손상에 따른 신경교세포 보호효과 및 국소 뇌혈류량 변화에 미치는 영향 (Protective Effects of Cardiotonic Pills(CP) on Neuroglia Cells Against Oxidative Stress and the Effects on Regional Cerebral Blood Flow in Normal Rats)

  • Objectives: SimJeok-Hwan(CP, Cardiotonic Pills) was made to treat patients with coronary arteriosclerosis, angina pectoris and hyperlipidemia. This study was designed to investigate the effects of CP on Proliferation rates neuroglia cells and protective effect of CP against oxidative stress, and also investigate the effects on regional Cerebral Blood Flow(rCBF) in normal rats. Methods: In this experiment, effects of CP on proliferation rates of neuroglia cells were measured using modified MTT methods. Oxidative stress was induced by treatment with 200 mM of hydrogen peroxide for 2 hr. rCBF and MABP were measured using Laser doppler flowmeter. Results: Treatment with CP elevated proliferation rates in C6 cells. In addition, CP protected cell death of C6 cells induced by oxidative stress. In results, rCBF was elevated by treatment with CP in normal rats. But, Mean Arterial Blood Pressure(MABP) did not affected by CP. In addition, the elevation of rCBF was blacked by pre-treatment with 1 mg/kg of indomethacin effectively. On the other hand, pre-treatment with 0.01 mg/kg of methylene blue did not affect rCBF level in normal rats. Conclusions: In conclusion, these results suggest that CP can act as anti-oxidant to protect neuroglia cells and also suggest that CP can elevate rCBF, which are involved in cyclooxygenase pathway.

$^{99m}Tc-HMPAO$ 뇌 SPECT에서 시각자극에 의한 국소 뇌 혈류변화의 정량적 검증 (Quantitative Evaluation of Regional Cerebral Blood Flow by Visual Stimulation in $^{99m}Tc-HMPAO$ Brain SPECT)

  • 목적: 이 연구의 목적은 시각 활성 자극에 의한 뇌 활성영역을 확인하고 국소 뇌 혈류량을 정량적으로 분석하는데 그 목적이 있다. $^8F-fluoro-2-deoxy\;glucose\;(FDG)\;PET$$^{15}O-water\;PET$에서 시각 자극을 가하는 동안 시각피질에서의 기전이 확인되었으며 PET에 비해 적은 비용으로 널리 사용되고 있는 $^{99m}Tc-HMPAO$ Brain SPECT를 이용하여 시각자극을 주었을 때 대뇌 후두엽의 일차시각피질과 시각 연합 피질에서의 국소 뇌 혈류 변화율을 정량적으로 분석하고자 하였다. 대상 및 방법: 오른손을 주로 사용하는 25세에서 33세의 나이 분포를 갖는 남자 3, 여자 3명(평균나이 26.7세)의 정상인을 대상으로 $^{99m}Tc-HMPAO$ Brain SPECT (925 MBq)를 시행하였고 8 Hz의 발광다이오드를 이용하여 $4{\sim}5$분 동안 시각활성 자극을 주었을 때와 대조군으로 자극을 가하지 않은 비 활성상태에서의 경우로 나누어 스캔하고 해부학적 기준정보를 제공하기 위해 MR 영상과 융합하였으며 관심영역을 설정하여 혈류량을 정량적으로 분석하였다. 결과: 정량적인 분석을 위해 대뇌의 전체 용적과 계수율을 계산하고 관심 영역을 설정하여 관심영역에 대한 복셀 당 평균 계수율을 측정하여 각 대상의 시각 활성자극을 가한 영상에서 정상 상태의 영상을 픽셀 당 픽셀로 감산하여 통계적 파라미터를 이용한 뇌 지도를 구성하였으며 뇌 활성이 Brodmann 영역의 시각피질(Ba 17)과 시각연합피질(Ba 18,19)에서 활성화되어 있음을 Talairach 좌표를 통해 확인하였고 regional index를 계산하여 국소 뇌혈류 변화율을 측정하였다. 결론: $^{99m}Tc-HMPAO$ Brain SPECT에서 8 Hz의 발광 다이오드로 시각 자극을 주었을 때 후두엽의 일차 시각피질과 시각 연합 피질에서 뇌 활성화를 확인하였고 통계적 파라미터를 이용한 SPM99에서 뇌 지도를 작성하여 시각자극에 의한 활성영역을 Talairach 좌표와 Brodmann 분류에 의해 확인하여 T1 강조 MR 영상에 융합하였다. 후두엽의 일차시각피질에서 시각 활성자극에 의해 국소 뇌 혈류량이 $32.50{\pm}5.67%$ 증가하고 있음을 정량적으로 분석하였다.

뇌혈유와 혈압에 미치는 오미자의 효능에 대한 연구 (Study on the Effects of Fructus Schizandrae on the Cerebral Blood Flow and Blood Pressure)

The BP of Fructus Schizandrae did nol change by pretreated I-NNA. 11. The BP of Fructus Schizandrae was decreased by pretreated methylene blue. 12. The BP of Fructus Schizandrae did not change by pretreated indomethacin. These results indicate that Fructus Schizandrae can increase the rCBF and decrease the BP, that related to guanylyl cyclase activity.

위축성 비염 환자에서 성상신경절 블록의 치료효과 -증례 보고- (Stellate Ganglion Blocks in Atrophic Rhinitis)

  • Atrophic rhinitis is characterized by mucosal atrophy, bony absorption, persistent fetid odor and resistance to medical and surgical treatment. Stellate ganglion block (SGB) can be used as a therapeutic modality by improving the regional blood flow through sympathetic blockade. We present a case of a 57 year-old male patient who had been treated unsuccessully for atrophic rhinitis for several years by surgical and conservative measures. The patient presented at our pain clinic with shoulder pain and received stellate ganglion block once or twice a week. He received more than 75 SGBs in addition to the routine conservative treatment for atrophic rhinitis. As the number of blocks performed increased, the patient demonstrated subjective symptom relief. We measured regional mucosal blood flow using a laser doppler flowmeter after the 28th, 63rd and 75th blocks. Nasal mucosal blood flow was improved by 4.9%, 28.8% and 36.3% respectively. We also were able to observe the recovery of mucosal atrophy to an almost normal level by nasal endoscopy. The patient is currently free of symptoms and is being followed up on an outpatient bases.

Changes in blood flow at the mandibular angle and Horner syndrome in a rat model of superior cervical ganglion block

  • Background: A stellate ganglion block (SGB) causes increased blood flow in the maxillofacial region, exhibiting the potential for regenerative effects in damaged tissue. The focus of this study was to understand the efficacy of SGB for regenerative effects against nerve damage. A rat model of the superior cervical ganglion block (SCGB) was created instead of SGB, and facial blood flow, as well as sympathetic nervous system function, were measured. Methods: A vertical incision was made on the left side of the neck of a Wistar rat, and a 5-mm resection of the superior cervical ganglion was performed at the back of the bifurcation of the internal and external branches of the left common carotid artery. Blood flow in the skin at the mandibular angle and mean facial temperature were measured using a laser-Doppler blood flow meter and a thermographic camera, respectively, over a 5-week period after the block. In addition, the degree of ptosis and miosis were assessed over a period of 6 months. Results: The SCGB rat showed significantly higher blood flow at the mandibular angle on the block side (P < 0.05) for 3 weeks, and significantly higher skin temperature (P < 0.05) for 1 week after the block. In the SCGB rat, ptosis and miosis occurred immediately after the block, and persisted even 6 months later. Conclusions: SCGB in rats can cause an increase in the blood flow that persists over 3 weeks.

구인(蚯蚓)이 흰쥐의 뇌혈류역학 변화에 미치는 효과 (Effects of Lumbricus on the Change of Cerebral Hemodynamics in Rats)

  • Objectives : This experimental study was designed to investigate the effects of Lumbricus extract (LE) on the changes in regional cerebral blood flow (rCBF) and mean arterial blood pressure (MABP) in normal rats, and further to determine the mechanism of action of LE. Methods : The changes of rCBF were observed by Laser-Doppler flowmetry (LDF) and the changes of MABP were recorded by a data acquisition system assembled with MacLab and Macintosh. Results : LE significantly increased rCBF in a high dosage(10.0 mg/kg, i.p), but MABP was somewhat increased as compared with baseline. This result suggests that LE significantly increased rCBF by dilating pial arterial diameter. Increase of LE-induced rCBF was significantly inhibited by pretreatment with indomethacin (1 mg/kg, i.p), an inhibitor of cyclooxygenase, but was not significantly inhibited by pretreatment with methylene blue ($10{\mu}g/kg$, i.p), an inhibitor of guanylate cyclase. Conclusions : LE increased rCBF by dilating pial arterial diameter, and the action of this response was mediated by cyclooxygenase.
