  • The traditional safety management method starts with the misconception that similar accidents will recur if the inappropriate behavior of the person who caused the accident is investigated and punishment is not judged. However, in modern safety management, incidents or situations occur when negative conditions latent in the system are mutually influenced and triggered. The precedent for revoking the disposition of suspension of first officer A of Eastar Jet, which won a legal lawsuit against the administrative regulatory authority, is a representative example that will serve as an opportunity for the administrative regulatory authority to break away from the punishment-oriented safety management method of the past. On the other hand, airmans and air carriers also need to have a clear understanding of flight regulations, and when judicial judgment is required, predictable and effective legal effects can be obtained by preparing clear standards for flight regulations. In addition, administrative regulatory authorities expect a change from the punishment-oriented safety management policy of the past to a systematic safety management policy.

Transdermal scopolamine for the treatment of recurrent parotid sialocele: a case report

  • Recurrent parotid sialocele is rare and challenging to treat. Treatment options are limited for cases of parotid sialocele that recur despite ductal ligation. This case study presents a patient who underwent wide excision of the right buccal mucosa due to squamous cell carcinoma. During the wide excision, a segment of the parotid duct was excised, and ductal ligation was performed to prevent the occurrence of a sialocele, followed by reconstruction using a folded anterolateral thigh free flap. Twenty-two days after surgery, parotid sialocele occurred despite the initial ductal ligation and subsequent ductal ligation was performed; however, the sialocele recurred. As an alternative therapeutic option, a transdermal scopolamine patch was applied for 3 weeks, with one patch used every 3 days. The results were encouraging, with complete resolution of the sialocele. A transdermal scopolamine offers a noninvasive, convenient method of treating parotid sialocele with minimal side effects. The successful outcome of this case suggests that a transdermal scopolamine can be an effective therapeutic option for recurrent parotid sialocele in conjunction with surgical treatment.

An unusual arrangement between the highest denticulate ligament and posterior inferior cerebellar artery

  • The posterior inferior cerebellar artery (PICA) is often involved in pathologies of the posterior cranial fossa. Therefore, a good understanding of the vessel's normal and variant courses is important to the neurosurgeon or neurointerventionalist. During the routine microdissection of the craniocervical junction, an unusual arrangement between the highest denticulate ligament and PICA was observed. On the right side, the PICA was given rise to by the V4 segment of the vertebral artery 9 mm after the artery entered the dura mater of the posterior cranial fossa. The artery made an acute turn around the lateral edge of the highest denticulate ligament to then recur 180 degrees and travel medially toward the brainstem. Invasive procedures that target the PICA should be aware of the variant as described herein.

Oxidative hemolytic crises in a dog due to fragrance products: clinical insights and treatment approaches

  • Importance: This is the first reported case of fragrance products-induced recurrent oxidative hemolytic anemia in a dog, detailing the successful therapeutic approach employed. Case Presentation: A 4-year-old intact female Pomeranian dog presented with brown tongue, pigmenturia, peripheral edema, and vomiting. Blood smears revealed a high count of eccentrocytes and Heinz bodies, along with a precipitous decline in packed cell volume and an increase in blood methemoglobin levels, suggesting an oxidative hemolytic crisis. This clinicopathological pattern recurred several times after the patient returned home. Antioxidants, methylene blue, hyperbaric oxygen (HBO) therapy, and blood transfusion were successfully employed to address recurrent hemolytic anemia; however, oxidative hemolytic crises recurred. After the owner removed exposure to various home remedies and fragrances, the clinical signs and hemolytic crises did not recur. Conclusions and Relevance: Recurring oxidative hemolytic crises should raise suspicions of environmental toxicity, which, although harmless in small quantities to humans, can be devastating to small-breed dogs. In addition to removing the causative agents, methylene blue and other antioxidants, along with HBO, may be beneficial in the acute management of oxidative hemolytic anemia.

$CO_2$ Laser Treatment of Adult-onset Laryngeal Papillomatosis ($CO_2$ 레이저를 이용한 성인 후두유두종의 치료)

  • Background and Objective : Laryngeal Papillomatosis (LP) is the most common benign neoplasm of the larynx, but it tend to recur and it makes eradicating difficult. Meticulous $CO_2$ laser excision has been the most effective treatment to date. This article analyzes the clinical feature and therapeutic results of 42 LP patients who were undergone $CO_2$ laser excision. Methods : Forty two patients with recurrent LP were treated with $CO_2$ laser. And their medical records were reviewed retrospectively. Demographics, chief complaints at onset, initial distribution of papillomas, number of operations performed on each patient, and current results were evaluated. Results : Male in their twenties and forties are dominant in number in patient number. Most common site was anterior one thirds (69%) of glottis area (86%). LP recurred in 17 cases (40%), and in 4 cases, the lesion extended over the original margin. Patients were undergone surgery $1.62{\pm}0.87$ times, $2.53{\pm}0.72$ in recurred cases. Mean relapsing time was 6 momths (from 1momth to 8years). Ant. laryngeal web occurred in 2 cases (4.8%) and 1 case was combined with squamous cell carcinoma. Conclusion : Meticulously performed $CO_2$ laser excision can achieve significant voice and airway improvement and clinical cures. The $CO_2$ laser through microdirect laryngoscopy allows more precise and bloodless removal of papillomas.

  • Recently, the robot manipulators are needed more slim size and longer reach and more accurate movement for increasing productivity. So, in this paper, the simulation modeling method and the efficient modeling method for new slim & long reach robot has been investigated for forecasting the slim robot performance before making prototype. To do this investigation, the major parts of robot driving system such as motor, belt and reducer devices and parts assembly method have been investigated mainly. And then, using this developed modeling method the new designed robot will be forecasted about the dynamic performance of new designed robot.

  • To identify the dynamic characteristics of the Auto-leveling system applied to the Tractor-Trailer type Transporter for mounting a large scale precision equipment, Dynamics Modeling & Simulation were performed using general Dynamics Analysis Program - RecurDyn(V9R2). The axial load data, transverse load data and pad trace data of leveling actuators were obtained from M&S. And they were analyzed and compared with each other by parameters, i.e. friction coefficients on the ground, landing ram speed of actuators, and direction & quantity of ground slope. It was observed that ground contact friction coefficients affected to transverse load and pad trace; the landing ram speed of actuators to both amplitude of axial & transverse load, and this phenomena was able to explain from the frequency analysis of the axial load data; the direction of ground slope to driving sequence of landing ram of actuators. But the dynamic behaviors on the two-directional slope were very different from them on the one-directional slope and more complex.

A Case Report of Extensive Limb Swelling in Newborn After Vaccination: Unusual Thigh Swelling After Hepatitis B vaccination (신생아 B형 간염 예방 접종 후 하지 부종이 발생한 환아 치험 1례)

  • Objectives The purpose of this study is to report the clinical effects of oriental medical treatment on Extensive limb swelling which is rare adverse reaction of vaccination. Methods A 11 months old infant who appealed unilateral thigh swelling after immunization and signs of local inflammation was administered a complex herbal medicine consisting of five decoctions; Gwakhyangjunggisan, Binsosan, Danggwisusan, Samchulgunbitang and Gwibitang. The length of the constant regions of the thigh circumference was measured every visit. Each process was recorded by photograph. After one year from the first visit, phone call was conducted to follow-up. Results The length of the thigh after administration of herbal medicine did not change much, but the part where swelled hard became little bit softer. A year later, patient's growth and walking was normal but the swollen part not changed. Since then the similar adverse effects did not recur on booster doses vaccines. Conclusions The adverse reactions of vaccination are rarely reported and there is no specific treatment for swelling from Hepatitis B vaccine. It will be more helpful to study if more cases reported about any adverse drug reactions or any side effects from the immunization.

  • Odontomas, hamartomas of odontogenic origin, are composed of all the structures that make up teeth. The WHO distinguishes odontoma into two types. The complex odontoma is defined as "a malformation in which all the dental tissues are represented, individual tissues being mainly well-formed but occurring in a more or less disorderly pattern." The compound odontoma is defined as "a malformation in which all the dental tissues are represented in a more orderly pattern than in the complex odontoma, so that the lesion consists of many toothlike structures. Most of these structures do not resemble morphologically the teeth of normal dentition, but in each one enamel, dentine, cementum, and pulp are arranged as in the normal tooth." Almost all odontomas are located intraosseously, but they have occasionally been reported in extrabony location. Peripheral or soft tissue odontomas, those arising outside of the alveolar bone, are very rare. Peripheral or soft tissue odontoma are defined as tumors that demonstrate the histologic characteristics of their intraosseous counterparts but occur solely in the soft tissue covering the tooth-bearing portion of the mandible and maxilla. When they mature, they appear as a radiopaque mass without the peripheral halo. The final diagnosis should be confirmed by biopsy. The origin of peripheral odontoma is probably related to remnants of the dental lamina in the gingiva. The treatment of choice is complete surgical excision, similarly to intraosseous odontoma and it does not tend to recur. This report presents a case of 5-year-old boy with swelling on labial gingiva of primary central incisor. And it was diagnosed as peripheral odontoma by excisional biopsy.

Intralesional Cidofovir Therapy for Recurrent Repiratory Papillomatosis (재발성 호흡기 유두종증의 병변내 시도포비어 치료)

  • Background and Objectives: Recurrent respiratory papillomatosis (RRP) is difficult to treat because of its tendency to recur and spread throughout the aerodigestive tract. We aimed to estimate the effect of intralesional injections of cidofovir in patients with RRP. Materials and Method: Within the period from January 2003 to July 2007, 13 patients aged 2 to 61 years were treated with intralesional injections of cidofovir combined with surgical excision of RRP. Cidofovir was injected intralesionaly at a concentration of 5 mg/cc after complete removal of the papilloma with $CO_2$ laser or microdebrider. We evaluated the effect of intralesional cidofovir therapy by comparing pre-treatment mean interval of recurrence with post-treatment interval of recurrence. Results: Of 13 patients, two patients showed complete response during follow up period and four patients showed partial response. Seven patients did not respond to cidofovir at all. Mean pre-treatment mean interval of recurrence was 9 months and mean post-treatment interval of recurrence was 13.1 months (p=0.039). There was a statistical significance between the injected dose of cidofovir and post-treatment interval of recurrence (p=0.009). There were no local or systemic side effects caused by cidofovir. Conculsion : Intralesional injection of cidofovir seems to have a potential of a safe and effective adjuvant therapy of RRP. There was a positive correlation between the injected dose of cidofovir and patient clinical outcomes so that administration of higher doses and more frequency of injections should be needed to reduce recurrence. Further study regarding injection therapy regimen for RRP is required.
