Difficult Intubation in Patients Undergone Oromaxillary Surgery: Retrospective Study (전신마취를 시행한 구강외과 환자에서 어려운 기관내삽관: 후향적 연구)

  • Background: This retrospective study aims to describe the airway management and to search predictive parameter for difficult intubation in 700 patients undergoing oromaxillary surgery. Methods: The medical records of 700 patients undergone oromaxillary surgery were reviewed for airway management during perioperative period. The cases of difficult intubation were selected and those radiologic findings were reviewed. The mandibular depth (MD), mandibular length (ML), thyromental distance (TMD) were measured. Results: In 41 cases difficult intubation were recorded in anesthetic record. The grade of Cormack and Lehane was III in 36 patients and IV in 5 cases. The MD of difficult intubation cases was $4.2{\pm}3.2\;cm$. The ML of difficult intubation cases was $10.1{\pm}3.8\;cm$. The TMD of difficult intubation cases was $5.9{\pm}4.3\;cm$. Under the fiberoptic guided awake intubation was undertaken in 75 patient. In none of the cases was failed nasotracheal intubation. Conclusions: The patients undergoing oromaxillar surgery have a potentially difficult airway but, if managed properly during perioperative preiod, morbidity and mortality can be reduced or avoided. The radiologic findings were poor predict for difficult intubation. The fiberoptic guided awake intubation is a safe alternative to direct laryngoscopic intubation.

Comparison of Diets of Urban American Indian and Non-Hispanic Whites: Populations with a Disparity for Biliary Tract Cancer Rates

  • Aim: The incidence of biliary tract cancer (BTC) is many-fold higher for American Indians (AI) relative to non-Hispanic whites (NHW). Neither gallstones nor genetics can account for this difference. There is speculation that certain fatty acids in bile may play a role in preventing BTC. Since diet may influence composition of bile, we compared the dietary intakes of urban AI and NHW adult women in New Mexico. Methods: Design, a cross-sectional study of the diets of lactating AI and NHW women was conducted. Setting, the University of New Mexico Hospital. Participants, healthy lactating women 18 to 39 years of age were recruited. Main outcome measures, a three-day diet record for each participant was analyzed. Results: The AI women consumed less calcium (p = 0.04) and significantly less short and intermediate chain-length fatty acids (C4-C12), but nearly twice as much proinflammatory arachidonic acid as the NHWs (p <0.01). The intake of dairy products by AI women was less than NHW women (p = 0.01) while the intake of processed meat products was higher (p <0.01). Conclusion: Dietary factors may account for the difference in the risk of BTC between AI and NHW women.

A Determination of Magnitude and Frequency of River Floods (하천 홍수량의 크기 및 빈도 결정)

  • In this study, six gaging stations(T/M bureau) in the Han River basin were selected for flood frequency analysis and was carried out frequency analysis by POT(peaks Over a Threshold) model where existing flood data of short record length are available. Frequency and magnitudes of each station floods in the river basins were estimated by POT model based on statistical method, and also were compared with standard errors to verify applicability of the estimates by POT model. Furthermore, in order to evaluate for the adequate design flood which is needed for the design of the hydrologic structures in the ungaged watersheds, it is considered to be possible to develop the statistical regionalized model by regional frequency analysis.

A Cephalometric Study on the Inheritance of Craniofacial Complex (두개안면골격의 유전성에 관한 측모두부방사선 계측학적 연구)

  • This study was undertaken to investigate the inheritance in craniofacial complex among Korean familial members. The subjects were 160 lateral cephalometric radiographs from 45 families. Standard product-moment correlation coefficients(r) were calculated for the measurements between the familial pairings. Further, coefficients of $determination(r^2)$ were calculated and multiple regression analyses were performed to assess the use of parents' record for predicting an offspring's craniofacial features. The following results were obtained. 1. First-degree relatives had a high level of significant correlations which were compatible with a polygenic theory of inheritance than those of non-relatives. 2. Mother-daughter pair had the highest significant correlations, then mother-son and father-son pairs, and, finally, father-daughter pair had the lowest significant correlations. 3. The genetic influence was higher in anterior cranial base length and genial angle. In contrast, the environmental influence was higher in posterior border of ramus, maxillary ant. teeth, maxillary & mandibular apical bases and Gla-P. occ. 4. The predictability of offspring's cranio-facial growth could be improved by using multiple measurements from both parents than those from father or mother only.

The first record of Orthopsyllus species (Copepoda: Harpacticoida: Orthopsyllidae) from Korean waters

  • During a study of harpacticoid copepods from Korea, one species of the family Orthopsyllidae was found by rinsing macroalgae collected from the intertidal and subtidal zones at Hangaechang, Munseum Islet in Jeju Island of Korea. The Orthopsyllus is closely related to Orthopsyllus linearis curvaspinata Mielke, 1993, however it is distinguished from the original description by the combined characters of slightly slender caudal seta V in the female, relatively short and ovoid caudal ramus, the seta formula of P3, and the length of P2 and P4 endopod in the male. Since Orthopsyllus linearis (Claus, 1866) is notorious for its incomplete previous descriptions and therefore its polymorphic status, it is premature to fix the status of present Orthopsyllus species from Korea, without the detailed comparative study among the congeners. However due to the urgent need for the report of the genus in the region, we report this species as Orthopsyllus linearis (Claus, 1866) like but different one within the genus: Orthopsyllus cf. linearis (Claus,1866). This is the first report of the genus Orthopsyllus for the first time in Korea.

Study on the Medical Cost of Patients Visited by Referral Center: Focusing on the Patient in Gastroenterology (진료의뢰센터 경유환자의 진료비 영향요인에 관한 연구: 소화기내과 환자를 중심으로)

  • This study purposed to analyze the differences of cost, length of stay, and number of visits between patients who referred from clinics to a general hospital and patients who directly visit a general hospital. Study sample included 402 patients (177 patients who were not referred from clinics, 225 patients who referred from clinics) who visited the Dept. of Gastroenterology in a university hospital in Daejeon from January to June in 2007. Cost and patients' information were collected from Hospital Information System and medical record. SPSS v.12.0 was used for the statistical analysis. Multiple regression analysis found that for inpatients, location variables and malignant tumors of digestive organs had a significant influence on cost variable. For outpatients, a referring hospital type and visiting month had significant influences on total cost, and sex and visiting month (February) had significant relationship with number of visit. The study results help to understand the differences of patient care depending on whether they were referred from clinics or not. Hospital managements could use the results for marketing purposes, and it could provide valuable information for increasing the competitiveness of hospital in a given market.

A Study on the Safety Evaluation of Bridge Footings using GPR (GPR 시험을 이용한 교량기초의 안전성 조사방법에 관한 연구)

  • The footing of a bridge is a very essential part that support the whole load induced by the bridge itself and the traffic as well. However, once a bridge is built, the footing is buried under soil so the footing is invisible from outside. Therefore, the safety or condition of the footing is very difficult to estimate. Not only the length of the imbedded part of the footings but also the type of footings are unknown once the design record is gone. Some nondestructive techniques can be used to evaluate invisible part of the footings but the results have not been successful yet. Using GPR (Ground Penetrating Radar), which has been used for the nondestructive evaluation in military purposes, the condition assessment of the footings have been successfully conducted in this research. The field evaluation and laboratory tests have been conducted to find effective factors in the condition assessment of the footings. The equipment and basic theory of the GPR has been presented. The field test results show that the GPR can be successfully used for the safety evaluation of the footings. More test results and field data are needed for more precise evaluation of the footings.

Implant overdenture impressions using a dynamic impression concept

  • A dynamic impression is a functional impression that records the functional movement of the patient's own muscle and muscle attachment. This process reduces the number of random factors. This article describes a method for making a special tray using a dynamic impression concept that was made from provisional dentures used for implant healing. The individual tray is used to make a wash-impression to record the features of the mucosa in detail. The main advantage of this technique is that it provides a functional relationship of the implant components to the supporting tissues without overextension because provisional denture had been used for 2 months and the border length of individual tray was nearly the same as that of provisional denture. The delivery of the prosthesis constructed using this impression technique is time-saving because there is no need for border molding and there are fewer post-insertion appliance adjustments.

  • This paper aims to extract characteristics of the spectrum of fingerprint image and to apply them to image enhancement techniques in spatial frequency domain. Based on 1$\times$64 window as a processing unit and the different record lengths(32, 16, 8), the estimate of power spectrum density for each length was made. Each acquired spectrum characteristics was applied to the re-synthesis process of the fingerprint image, an improved gray scale image was obtained. In order to select an optimal predictor and the Huffman table for the fingerprint iamge, the lossless JPEG algorithm was used. Experiments were performed for extracting distribution characteristics for the each of 7 predictors from the fingerprint image and modeling processes, and the result was applied to the data compression algorithm and the selection of the optimal predictor.

  • The effect of mixture component makes a nelay time and a long total combustion period $\tau_{p\;max}$. The flame propagation delay $\tau_{df}$ was determined by the record of current ion. The pressure release delay $\tau_{dp}$ and $\tau_{p\;max}$ were determined by the indicated pressure diagram in constant volume of the combustion chamber. The results are as follows: 1) The ignition delay $\tau_t$ time takes the minimum value around $\Phi=1.15$. 2) $\tau_{df}$ and $\tau_t$ time increased according to the increases of the concentrated dilution gases, because the adiabatic flame temperature decreased due to the increases of the heat capacity. But dilution gases have little effect on flame nucleus formation delay 3) The relation between $\tau_t$ time and reciprocal laminar burning velocity is almost linear. 4) The increase of the propagation length is accompanied with increased ratio of the $\tau_{df},\;\tau_{dp},\;\tau_{t},\;\tau_{p\;max}$.
