체강액의 세포학 (The Cytopathology of Body Cavity Fluid)

  • Cytologic examination of the body cavity fluid is very important because the specimens represent a significant percentage of nongynecologic samples and this cytologic examination may be the first, best or only chance for making the diagnosis of an underlying malignancy. The purposes of body cavity fluid examination are to correctly identify cancer cells and if possible, to identify the tumor types and primary sites when presented with unknown primary tumor sites. The most important basic differential diagnosis is that of benign and reactive disease vs malignant disease. Reactive mesothelial cells are a consistent population in body cavity fluid, and these are the most versatile cells in the body. Due to the specific environment of the body cavity, the exfoliated reactive mesothelial cells may show significant morphologic overlap with the morphology of cancer cells. With a focus on the differential points between reactive mesothelial cells and metastatic adenocarcinoma cells, the practical diagnostic approaches, the diagnostic clues and the pitfalls to achieve a correct diagnosis are presented in this review.

  • 에너지 물질과 같이 연소 반응을 하는 압축성 물질을 해석하기 위하여 Hydro-SCCM (Shock Compression of Condensed Matter)이라는 에너지 물질과 비반응 물질을 포함한다중 물질 해석툴을 개발하였다. 고에너지 물질은 강한 충격파와 고온과 고압을 가진 물질경계면에서 높은 변형률을 발생시킨다. 이러한 큰 구배를 가진 현상을 해석하기 위하여 새로운 오일러리안 기법을 사용하였다. 본 논문에서는 현상을 해석하기 위한 수학적 방법과 해석결과를 소개하였다.

무균성수막염과 뇌염환자 사이의 인구학과 검사소견의 비교: 나이와 C-반응단백질의 중요성 (Comparison of the Demographic and Laboratory Profiles of Patients with Aseptic Meningitis and Encephalitis: Significance of Age and C-reactive Protein)

  • Background: Viruses can cause either meningitis or encephalitis. It is unclear why some people suffer from aseptic meningitis, and others acquire aseptic encephalitis when infected with the same viral pathogens. The aim of this study was to compare demographic and laboratory factors between patients with aseptic meningitis and encephalitis. Methods: The demographic and laboratory differences were analyzed according to age, sex, diabetes, hypertension, C-reactive protein in the blood, white blood cell and protein in the cerebrospinal fluid, and glucose ratio (cerebrospinal fluid/blood). Additionally, we analyzed the nation-wide differencesin age between the patients with aseptic meningitis and those with encephalitis in Korea. Results: The patients with aseptic encephalitis were older, more likely to have hypertension, and had higher levels of C-reactive protein than did the patients with aseptic meningitis. However, the numbers of white blood cells in the cerebrospinal fluid were significantly higher in the patients with meningitis than in the patients with encephalitis. Multivariable analysis revealed that age >49 years, hypertension and a C-reactive protein level >5.81 mg/dL were independent and significant variables in the prediction of aseptic encephalitis. Additionally, the patients with aseptic encephalitis were older than those with aseptic meningitis in the nation-wide Korean database. Conclusions: Older age, hypertension, and higher levels of C-reactive protein are useful factors for the prediction of aseptic encephalitis.

CpG-DNA induces bacteria-reactive IgM enhancing phagocytic activity against Staphylococcus aureus infection

  • CpG-DNA triggers the proliferation and differentiation of B cells which results in the increased production of antibodies. The presence of bacteria-reactive IgM in normal serum was reported; however, the relevance of CpG-DNA with the production of bacteria-reactive IgM has not been investigated. Here, we proved the function of CpG-DNA for the production of bacteria-reactive IgM. CpG-DNA administration led to increased production of bacteria-reactive IgM both in the peritoneal fluid and serum through TLR9 signaling pathway. When we stimulated B cells with CpG-DNA, production of bacteria-reactive IgM was reproduced in vitro. We established a bacteria-reactive monoclonal IgM antibody using CpG-DNA stimulated-peritoneal B cells. The monoclonal IgM antibody enhanced the phagocytic activity of RAW 264.7 cells against S. aureus MW2 infection. Therefore, we suggest that CpG-DNA enhances the antibacterial activity of the immune system by triggering the production of bacteria-reactive IgM. We also suggest the possible application of the antibodies for the treatment of antibiotics-resistant bacterial infections.

체액도말에서의 AgNOR수의 유의성 - 반응성 중피세포와 악성세포의 감별 - (The Significance of AgNOR Count in Body Fluid - Differential between reactive mesothelial cells & malignant cells -)

  • To distinguish reactive mesothelial cells from malignant cells in body fluid, we applied silver staining of nucleolar organizer regions(AgNORs) to ethanol fixed cytologic preparations. Fifty aspirated samples of benign(22 cases) and malignant(26 cases) body fluids were studied using the one step silver staining method. Two cytologically atypical samples were also included in the study. In malignant cases the mean AgNOR count was $3.56{\pm}0.81$, while in benign cases the mean AgNOR count was $2.02{\pm}0.33$. The difference of AgNOR counts between these two groups were statistically significant(p<0.001). The mean of atypical cases was 2.91. Both were diagnosed as malignant in follow-up cytology. In malignant effusions, there is statistically significant difference in AgNOR counts between cells forming complex papillae or clusters and singly scattered cells(p<0.05), $3.29{\pm}0.95\;and\;3.83{\pm}0.55$, respectively. We concluded that AgNOR count appears to be useful as a diagnostic tool especially when the cytologic differentiation is difficult.

체강 삼출액의 진단에 있어서 $PLC-{\gamma}1$ 면역 염색의 유용성 ([ $PLC-{\gamma}1$ ] for Differentiating Adenocarcinoma from Reactive Mesothelial Cells in Effusions)

  • Cytologic diagnosis of reactive or malignant effusion is sometimes difficult. Especially, differentiation of benign reactive mesothelial cells from malignant cells in body effusion is more difficult. Recently, immunohistochemistry has been used to diagnose difficult cases. Phospholipase $C(PLC)-{\gamma}1$ is one of the isoenzyme of the PLC which plays central role in signal transduction involving cellular growth, differentiation and transformation by phosphorylating many protein component. Increased expression of $PLC-{\gamma}1$ in human breast carcinoma, colorectal carcinoma and stomach cancers are reported. To evaluate the efficacy of positive $PLC-{\gamma}1$ immunostaining in the diagnosis of malignancy in effusions, paraffin-embedded cell blocks of pleural fluid and ascites from 10 patients(5 metastatic adenocarcinomas, and 5 reactive mesothelial cells) were immunostained with a monoclonal antibody to $PLC-{\gamma}1$. $PLC-{\gamma}1$ immuostained all the adenocarcinomas in cell block(5/5) with intense membrane pattern, however, none of the reactive mesothelial proliferations stained with the diagnostic membrane pattern. Thus, our study strongly supports the conclusion that $PLC-{\gamma}1$ immunopositivity is likely to become a useful adjunct for the diagnosis of malignancy in effusions.

류마티스 관절염 병인에서 제2형 콜라겐에 대한 면역반응의 역할 (Role of Immune Response to Type II Collagen in the Pathogenesis of Rheumatoid Arthritis)

  • Type II collagen (CII), major component of hyaline cartilage, has been considered as an auto-antigen in rheumatoid arthritis (RA). However, the clinical and biological significances with regard to the CII autoimmunity need to be clarified in human RA. The presence of antibodies to CII has been identified in sera, synovial fluid, and cartilage of patients with RA. In our study, the increased titer of IgG anti-CII in sera was well correlated with C-reactive protein, suggesting that this antibody may reflect the inflammatory status of RA. The titer of anti-CII antibodies (anti-CII Abs) tended to be higher in early stages of diseases. In our extending study, among 997 patients with RA, 269 (27.0%) were positive for circulatory IgG antibody to CII, those levels were fluctuated over time. It is hard to assess the significant amount of T cell responses to CII and CII (255~274) in RA. By using a sensitive method of antigen specific mixed lymphocyte culture, we can detect the presence of CII-reactive T cells in peripheral blood mononuclear cells of RA patients. Sixty seven (46.9%) of 143 patients showed positive CII reactive T cell responses to CII or CII (255~274). The frequencies of CII reactive T cells were more prominent in inflamed synovial fluid (SF) than in peripheral blood. These T cells could be clonally expanded after consecutive stimulation of CII with feeding of autologous irradiated antigen presenting cells (APC). Moreover, the production of Th1-related cytokine, such as IFN-${\gamma}$, was strongly up-regulated by CII reactive T cells. These data suggest that T cells responding to CII, which are probably presenting the IFN-${\gamma}$ producing cells, may play an important role in the perpetuation of inflammatory process in RA. To evaluate the effector function of CII reactive T cells, we investigated the effect of CII reactive T cells and fibroblasts-like synoviocytes (FLS) interaction on the production of pro-inflammatory cytokines. When the CII reactive T cells were co-cultured with FLS, the production of IL-15 and TNF-${\alpha}$ from FLS were significantly increased (2 to 3 fold increase) and this increase was clearly presented in accord to the expansion of CII reactive T cells. In addition, the production of IFN-${\gamma}$ and IL-17, T cell derived cytokines, were also increased by the co-incubation of CII reactive T cells with FLS. We also examined the impact of CII reactive T cells on chemokines production. When FLS were co-cultured with CII stimulated T cells, the production of IL-8, MCP-1, and MIP-1${\alpha}$ were significantly enhanced. The increased production of these chemokines was strongly correlated with increase the frequency of CII reactive T cells. Conclusively, immune response to CII was frequently found in RA. Activated T cells in response to CII contributed to increase the production of proinflammatory cytokines and chemokines, which were critical for inflammatory responses in RA. The interaction of CII-reactive T cells with FLS further augmented this phenomenon. Taken together, our recent studies have suggested that autoimmunity to CII could play a crucial role not only in the initiation but amplification/perpetuation of inflammatory process in human RA.

터보 차져 디젤 엔진에서의 기류음 감소를 위한 연구 (A study on the reduction of the flow-induced noise in turbo-charger diesel engines)

  • Turbocharger has been widely used in many passenger cars in application with diesel engines because of high power and fuel efficiency. However, flow-induced noise (whoosh or hissing noise) which is generated within the compressor during its operation at marginal surge line can deteriorate noise characteristics. Hissing noise excitation was associated with the generation of turbulence within the turbocharger compressor and radiated through the transmission path in turbocharger system. In this study, a sharp-edged reactive-type muffler was devised and installed in the transmission path to reduce the hissing noise. Acoustic and fluid dynamic characteristics for the muffler were investigated which is related to the unsteadiness of turbulence and pressure in turbocharger system. A transfer matrix method was used to analyze the transmission loss of the muffler. Simple expansion muffler with extended tube of the reactive type is proposed for the reduction of high frequency component noise. Turbulence computation was carried out by a standard ${\kappa}-{\varepsilon}$ model. An optimal design condition of the muffler was obtained by extensive acoustic and fluid dynamic analysis on the engine dynamometer with anechoic chamber. A significant reduction of the hissing noise was achieved at the optimal design of the muffler as compared with the conventional turbocharger system.

채널형상과 마하수가 천음속 연소에 미치는 영향에 대한 수치해석적 연구 (A Numerical Study of Channel Shape and Mach Number Effects on Transonic Combustion)

  • 천음속 미교란 모델과 1단계 1차 Arrhenius 화학반응식을 이용하여 반응유체의 압축성 유동에 대하여 연구하였다. 유체 유동은 적은 열방출을 수반하는 희박 예혼합 반응에 국한시켰다. 천음속 연소에 끼치는 채널형상과 채널입구 마하수의 영향 등을 수치해석을 이용하여 조사하였다. 수치결과에서 채널확대는 주어진 채널길이 내에서 화학반응을 증가시키고 있음에 반하여 채널수축은 출구 근처에서 화학반응을 억제시키고 있음을 보여주고 있다. 확대형 채널 내에서 입구유동 마하수 증가는 고정된 반응속도에서 유동을 가속시켰으며 불활성 유체 경우에는 나타나지 않는 약한 충격파가 나타났다. 또한 확대형 채널 출구 근처의 압력과 온도를 증가시키고 주어진 채널길이 내에서 반응체의 소비를 도와준다.
