FCM과 FSM공법에 의한 강-콘크리트 복합사장교의 신뢰성에 기초한 시공간 구조안전도평가 (Reliability-Based Assessment of Structural Safety of Steel-Concrete Hybrid Cable-Stayed Bridge Erected by the FCM and FSM during Construction)

  • 본 연구에서는 시공간의 구조안전도 확보와 교량붕괴를 방지하기 위해 FCM과 FSM공법으로 가설되는 강-콘크리트 복합사장교의 안전도평가 모형을 제안하였다. 구조물의 저항과 하중효과에 내제된 각종 불확실성을 합리적으로 고려하는 구조신뢰성이론에 의해 강합성거더와 콘크리트거더가 연결된 강-콘크리트 복합사장교의 저항과 하중의 분포특성을 정의하고 가설중 영구구조물과 가설구조물의 강도한계상태방정식을 제안하였다. 케이블, 주탑, 거더, 강-콘크리트 접합부 및 가설벤트의 신뢰성해석을 위해 AFOSM 알고리즘과 MCS 기법을 사용하였다. 또한, 가설단계별 구조시스템에 따라 요소신뢰성해석을 수행하였다. 제안된 평가모형의 타당성과 실용성을 검증하기 위하여 제시된 방법을 실제 교량에 적용하였다. 유사한 교량의 안전도 관리를 위한 중점 항목을 정의하기 위해서 주요 인자에 대한 민감도 분석이 수행되었다. 본 연구를 통해, 제안된 모형은 FCM과 FSM으로 가설되는 강-콘크리트 복합사장교의 시공간 안전도 평가를 위한 합리적이고 실용적인 방법으로 적용될 수 있을 것으로 판단되었다.

통년성 알레르기 비염의 변증 도구 개발을 위한 전문가 집단 설문조사 연구 (An expert survey for developing pattern diagnosis instrument of persistent allergic rhinitis)

  • Background : According to the rising cases of clinical researches with Traditional Korean Medicine for persistent allergic rhinitis patients, the need for developing and applying Pattern diagnosis instrument has been increasing. Objective : This study was done to investigate and pragmatize the Pattern diagnosis instrument for persistent allergic rhinitis patients launched by KFDA in 2008. Methods : Data was collected by structured survey papers from Korean Medicine doctors participate in the Conference of The Korean Oriental Medical Ophthalmology & Otolaryngology & Dermatology Society in October 2012. We investigated their representative Patterns(변증) of every questions and used the mean results for assuming agreement rates. The importance order of 8 symptom domains for considering Pattern diagnostic steps were also asked and analyzed for inducing importance rates. Finally based on above two results, we developed weight points of each questions by multiplying agrement percents with importance rates. Results : 50 Korean medicine doctors described the informed consent and survey papers. Response agreement rates of each questions of Pattern diagnosis instruments were analyzed and described in Table 2. Comparing the survey results with diagnostic references, matched percent of 肺胃熱 was the 87.9%, 肺氣虛寒 was 62.5%, 肺脾氣虛 was 62.5% and 腎元虧虛 was 50%. Mean importance rates of rhinorrhea was 7.28, otoscope diagnosis 5.12, obstruction 5.04, sneeze 4.82, symptom duration 4.63, other body condition 3.54, tongue diagnosis 3.02, nasal pruritus 2.86, accordingly. Final importance rates of each questions were assumed, and the range of them was from 1.60 to 4.72, which were listed in Table 5. Conclusion : These results might provide the rational backgrounds and practical methods for developing and utilizing methods of Pattern diagnosis questionnaire for perennial allergic rhinitis.

대유역 홍수예측을 위한 연속형 강우-유출모형 개발 (Development of Continuous Rainfall-Runoff Model for Flood Forecasting on the Large-Scale Basin)

  • 본 연구에서는 대유역의 홍수모의가 가능한 연속형 강우-유출모형을 개발하고자 하는데 그 목적이 있다. 이를 위해 가변저류변수와 유출곡선지수를 기반으로 하는 시단위 지표유출량 산정방법을 개발하였으며 수문성분을 토양수분에 대한 연속방정식에 적용하여 연속적인 강우에 대한 토양수분 모의가 가능하도록 모형을 구성하였다. 또한 유출수문성분과 유역 저류함수모형을 연계하여 유역에 대한 연속적인 유출모의가 가능하도록 하였으며 하도 저류함수모형을 이용하여 대유역에 대한 유출모의가 가능하도록 모형을 개발하였다. 대상유역은 낙동강 유역을 채택하였으며 2006년(보정기간) 및 2007~2008년(검증기간)의 홍수기간 동안 본류와 지류에 위치한 8개 유량관측지점에 대해서 모형의 정확도 평가를 수행하였다. 모든 평가지점에서 모의유량이 관측유량과 유사한 결과를 보이며 보정기간과 검증기간의 모형효율성계수는 각각 0.81~0.95와 0.70~0.94 범위의 우수한 결과를 보이는 것으로 나타났다. 또한 강우에 대한 토양수분의 거동과 수문 성분 발생량에서도 합리적인 결과를 도출하는 것으로 확인되었다. 이상의 결과로부터 본 연구에서 개발된 연속형 강우-유출모형은 대유역의 홍수예측에 활용이 가능할 것으로 판단된다.

원가계산방식에 의한 다가구임대주택 해체공사비 예측 (Demolition Cost Estimation of Small-size Rental Housing based on the Quantity per Unit Method)

  • 본 연구는 LH의 다가구임대주택 철거 신축사업에 대비하여 노후주택에 대한 해체공사비를 합리적 근거에 의하여 예측하는 것을 목표로 한다. 이를 위하여 소형주택 해체공사 수행 현황과 작업 프로세스를 조사하고 공사비 견적전문가 면담을 통해 해체공사비 산정실태를 파악하였다. 아울러 공공건설공사의 '예정가격 산정기준'과 '건설폐기물 처리비 산정기준', 선행연구 및 문헌을 고찰하였다. 연구 결과의 하나로 표준품셈에 기초한 해체공사 원가계산체계와 내역체계 및 관련 일위대가를 제안하고, 이를 활용하여 지상 2~3층 규모의 다가구임대주택에 대한 해체공사 예정가격을 산출하였다. 산출 결과, 1개 동당 해체공사비는 18,331천원, 바닥면적($m^2$) 기준 104천원으로 예측되었는데, 세부적으로는 직접 철거비가 14,339천원/동, 폐기물 위탁처리비는 3,992천원/동 소요될 것으로 나타났다. 연구 결과는 노후 다가구임대주택 철거 신축사업의 예산수립 단계에서 사업비 추정을 위한 기본 자료로 활용될 뿐 아니라, 연구 제안한 해체공사 원가계산체계 및 내역체계는 공공 건설공사의 발주업무에 유용하게 활용되리라 기대한다.

제1위내용액 이식에 의한 비육우의 농후사료 적응법에 관한 연구 (Adaptation of Feedlot Cattle to a High-energy Ration by Intraruminal Transplantation of Adapted Ruminal Fluid)

  • In feedlot cattle the abrupt change of diet from roughage to a large quantity of grain for the purpose to improve production often results in increased occurrence of rumen acidosis or acute carbohydrate encouragement enterotoxemia, bloats diarrhea liver abscess and laminitis or robot disease. The common management practice to control these problem is to increase the amount of concentrates in the diet in a stepwise manner until the animals are adapted to a high-grain ration. However this practice requires at least about 3 weeks adaptation period and specially prepared adaptation rations which contain various amount of concentrates. Present experiment was undertaken in order to findout the more simple and rapid adaptation method of cattle to a high grain ration. Nineteen Korean calves aging from four to six month were fed artifical hay (Youngchoun Chuk-Hyup, Korea) which contains 10% of concentrates or alfalfa and rye grass hays for two months and randomly alloted to three experimental groups and two control groups. The experimental group-1 was inoculated by stomach tube for two days with li500 ml/day of ruminal fluid fished from Korean beef cattle that had been previously adapted to a high-energy ration. The experimental group-2 was inoculated by trocalization for two days with the same ruminal fluid. The experimental group-3 was inoculated by trocalization with 1,500 ml/day of bacterial culture which contained 2$\times $10$^{9}$/m1 of Gram-negative bacteria derived from adapted luminal fluid. The two control groups were treated with normal saline solution by the same methods. All animals were fed high-energy ration that contained 80% of grain ad libitum for 30-74 days beginning on the third of the treatment. The effect of the inoculation on the adaptation was observed clinicopathologically with the following results; All of the experimental calves inoculated with the ruminal fluid or Gram-negative bacterial culture derived from adapted cattle did not show any signs of rumen acidosis or other related diseases, while most of the control calves did show diarrhea and bloat and a calf laminitis. The average daily weight gain and feed efficiency of experimental calves were slightly improved compared with control calves. Following the feeding of high-grain rational the pH of the ruminal fluid was lowered in both the experimental and control groups. However severe acidosis with the pH of below 5.0 was observed in only a control group-2. The protozoal number in ruminal fluid was markedly decreased during the high-grain feeding in both the experimental and control calves. However the decrease was mere severe in control calves compared with the experimental calves. The activation of the protozoa were completely disappeared within nine hours at the refrigerator temperature (4"C). No significant differences in heamatological and blood chemical values between the experimental and control calves were recognized. However in one control calf which showed clinically laminitis marked elevations of serum glutamic oxaloacetate transaminase and lactic dehydrogenase activities and a decrease of serum glucose level were observed. From these results it would be concluded the intraruminal transplantation of unadapted calves with the adapted ruminal fluid from cattle previously adapted to a high-energy ration prevents disease problem associated with high-grain feeding and improve weight gain and feed efficiency.ency.

비촉지성 정류고환의 진단 방법의 평가 (Evaluation of diagnostic methods in children with nonpalpable undescended testis)

  • 목 적 : 정류고환은 소아비뇨기영역에서 가장 흔한 기형으로 여러 합병증을 일으키며, 그 중 비촉지성 정류고환은 전체 정류고환 중 약 20%를 차지한다. 비촉지성 정류고환의 진단방법은 여러 가지가 있으나 아직 확립되지 않아 임상에서 통상적으로 적용될 수 있는 비촉지성 정류고환의 진단방법을 알아보고자 하였다. 방 법 : 2000년 3월부터 2005년 2월까지 본원에서 수술한 정류고환 129례 중 비촉지성 정류고환 31례를 대상으로 초음파검사, 전신마취하 신체검사, 복강경검사를 실시한 후 각 검사의 효과와 필요성을 분석하였다. 결 과 : 비촉지성 정류고환 31례에서 초음파검사상 13례(41.9%)에서 고환을 발견하였고 18례(58.1%)에서 고환을 발견할 수 없었고, 전신마취하 신체검사상 15례(48.4%)에서 고환을 촉지하였고 16례(51.6%)에서 고환을 촉지할 수 없었다. 초음파 검사와 전신마취하 신체검사에서 고환을 발견 못한 16례를 대상으로 복강경 검사를 실시하여 16례 고환 모두의 위치와 상태를 알 수 있었다. 결 론 : 신체검사상 고환을 촉지할 수 없는 비촉지성 정류고환에서 초음파검사, 전신마취하 신체검사, 복강경검사 순으로 진행하는 것이 비용과 효과 면에서 가장 좋은 진단과정으로 생각한다.

마샬 안정도시험기를 이용한 아스팔트 혼합물의 파괴에너지 측정시스템 개발 (Development of Fracture Energy Measurement System of Asphalt Mixture Using Marshall Tester)

  • 본 연구는 마샬 시험기를 이용하여 아스팔트 혼합물의 균열저항성을 평가할 수 있는 보다 간편하고 합리적인 측정시스템을 개발하는데 목적이 있다. 균열저항성 평가를 위한 파라메타로서 파괴에너지를 이용하였다. 마샬 시험기는 기본적으로 공시체 외부에 거치한 LVDT를 이용하여 수직변형률을 측정하는 시스템이며, 이 같은 외부 수직변형률 측정방식은 하중 스트랩 부분에서 발생하는 국부적인 변형으로 인해 측정오차를 야기할 가능성이 있다. 따라서 추가적인 계측 시스템을 설치하지 않고 기본적인 마샬 시험기를 이용하여 혼합물의 파괴에너지를 측정하기 위해서는 공시체 외부에 거치한 LVDT를 이용한 수직변형률 측정값이 파괴에너지 산정에 적용가능한지 여부를 검증하여야 한다. 이를 위해 본 연구에서는 공시체 중앙부분에서의 수평변형률과 외부에 거치한 LVDT를 이용한 수직변형률을 측정하는 두 가지 방식의 간접인장강도실험을 수행하여 그 차이를 비교 분석하였다. 실험결과, 외부 수직변형률 측정의 문제점으로 지적되었던 하중 스트랩 부분에서 발생하는 국부적인 변형은 파괴시점 이전에는 극히 적은 것으로 나타나 파괴에너지 계산에 오차를 유발하지 않음을 보여주었다. 또한 외부 수직변형률 측정의 실험변동성을 확인한 결과, 변동계수가 15% 이하로 마샬시험기를 이용한 균열저항성 평가시스템에 이용 가능함을 알 수 있었다.

환혼산(還魂散)이 실험적(實驗的)으로 유발(誘發)한 종양(腫瘍)에 미치는 영향(影響) (Influence of Hwanhonsan Extract against Chemically Induced and Xenografted Mice Tumor)

  • Hwanhonsan has been used for curing tumor as a Oriental medicine without any experimental evidence to support the rational basis for their clinical use. This experiment was carried out to evaluate the possible therapeutic or antitumoral effects of Hwanhonsan extract against cancer, and to study some mechanisms responsible for its effect. Some kind of tumors were induced by the typical application of 3-methylcholanthrene(MCA) or by the implantation of malignant tumor cells such as leukemia cells(3LL cells) or sarcoma cells(S180 cells) and FasII cells. Treatment of the Hwanhonsan extract(daily 1 mg/mouse, i.p.) was continued for 7 days prior to tumor induction and after that the treatment was lasted for 20 hrs. Against squamous cell carcinoma induced by MCA, Hwanhonsan decreased. not only the frequency of tumor production but also the number and weight of tumors per tumor bearing mice(TBM). Hwanhonsan also significantly suppressed the development of 3LL cells and S180 cells implanted tumors by frequency and their size, and some developed tumors were regressed by the continuous treatment of Hwanhonsan extract into TBM. However, when tumor was induced by FsaII cells implantation, the growth of implanted cells in mice was delayed by the water extract of Hwanhonsan until 7 days and then rapid growth ensued. In vitro treatment of Hwanhonsan extract had no inhibitory effect on the tumor induced by some kind of cell lines such as A431 cells strain but it significantly inhibited the proliferation of 3LL cells, S180 cells. These results suggested that Hwanhonsan extract exhibited a significant prophylactic benefits against tumors and its antitumor activity was manifested depending on the type of tumor cells.

한 병원이 지역사회에 미치는 경제적 영향 분석 (The Economic Impact of a Rural Hospital to Local Economy)

  • Demand for high quality medical care has recently been increasing in step with high level of income and education. Patients prefer the use of large general hospitals to small community hospitals. Large hospitals, usually located at urban area, expand their capacities to cope with the increasing demand, therefore, they easily secure revenue necessary for growth and development of hospitals. However, small community hospitals are facing with serious financial difficulties caused from the reduction of patients in one hand and the inflation of cost in another. If small rural hospitals were closed, the closure would have negative impacts on local economies in addition to the decrease in access to medical care. Community leaders should have an insight on the contribution of community hospitals to local economies. They could make a rational decision on the hospital closure only with the understanding of hospital's contribution to the community. This study is designed to develop an economic model to estimate the contribution of rural hospital to local economies, and also to apply this model with a specific hospital. The contribution of a hospital to local economies consists of two elements, direct effect and multiplier effects. The direct impacts include hospital's local purchasing power, employee's local purchasing power, and the consumption of patients coming from outside the community. The direct impact induces multiplication effect in the local economy. The seed money invested to other industries grows through economic activities in the region. This study estimated the direct effect with the data of expenditure of the case hospital. The total effect was calculated by multiplied the direct effect with a multiplier. The multiplier was drown from the ratio of marginal propensity of income and expenditure. Beside the estimation of the total impacts, the economic effect from the external resources was also analyzed by the use of the ratio of patients coming outside the region. The results are as follows. 1. The direct economic contribution of the hospital to the local economy is 1,104 million won. 2. The value of multiplier in the region is 2.976. 3. The total economic effect is 3,286 million won, and the multiplication effect is 2,182 million won. 4. The economic contribution from the external resources is 245 million won which is 7.5% of the total economic effect.

우리나라 국민의 정신질환으로 인한 질병부담 추정 (Estimating the Burden of Psychiatric Disorder in Korea)

  • Objectives: This study estimated the burden of disease especially caused by psychiatric disorders in Korea by using DALY, a composite indicator that was recently developed by the Global Burden of Disease study group. Methods: First, 11 of the major psychiatric disorders in Korea were selected based on the ICD-10. Second, the burden of disease due to premature death was estimated by using YLLs (years of life lost due to premature death). Third, for the calculation of the YLD (years lived with disability), the following parameters were estimated in the formula: the incidence rate, the prevalence rate and the disability weight of each psychiatric disorder. Last, we estimated the DALY of the psychiatric disorders by adding the YLLs and YLDs. Results: The burden of psychiatric disorder per 100,000 people was attributed mainly to unipolar major depression (1,278 person-years), schizophrenia (638 person-years) and alcohol use disorder (287 person-years). For males, schizophrenia (596 person-years) and alcohol use disorder (491 person-years) caused the highest burden. For females, unipolar major depression (1,749 person-years) and schizophrenia (680 person-years) cause the highest burden. As analyzed by gender and age group, alcohol use disorder causes a higher burden than schizophrenia in men aged 40 years and older. For females, unipolar major depression causes the highest burden in all age groups. Conclusions: We found that each of the psychiatric disorders that cause the highest burden is different according to gender and age group. This study's results can provide a rational basis to plan a national health policy regarding the burden of disease caused by psychiatric disorders.
