철도사고 위험도평가를 위한 철도 인간신뢰도분석 방법의 개정과 전산 소프트웨어의 개발 (Revision of the Railway Human Reliability Analysis Procedure and Development of an R-HRA Software)

  • 본 논문은 크게 두 가지 내용을 담고 있다. 하나는 기 개발된 철도 인간신뢰도분석 방법(R-HRA)의 개정에 관한 내용이며, 다른 하나는 개정된 R-HRA 방법에 기반한 R-HRA 지원 시스템의 개발이다. 개정된 R-HRA 방법은 분석자간 일관성을 유지하기 위한 직무분석 지침의 제공과 영향인자의 분류에 특징을 두고 있으며, R-HRA 지원 시스템은 인간신뢰도분석을 위한 정보의 수집, 내 외적 오류유형을 포함한 정성적 오류분석, 오류확률의 정량화, 전체 분석결과의 문서화 작업 등을 지원하고 있다. 개정된 R-HRA 방법과 지원 소프트웨어는 철도 사고 시나리오에서 발생 가능한 인적오류 가능성을 효과적이고 효율적으로 분석할 수 있도록 지원할 수 있을 것으로 기대된다.

  • The railway human reliability analysis(R-HRA) plays a role of identifying and assessing human failure events in the framework of the probabilistic risk assessment(PRA) of the railway systems. This study introduces a case study that was performed to select an appropriate R-HRA method. Three HRA methods were considered in the case study: (1) the K-MRA(THERP/ASEP-based) method, (2) the HEART method, (3) the RSSB-HRA method. Two case events were selected based on the review of the railway incidents/accidents, which include (1) a real-end collision event, which occurred on the railway between the Gomo and Kyungsan stations in 2003, (2) the signal passed at danger(SPAD) events, which are caused from a variety of factors. The three HRA methods were applied to both case events, and then the strengths and limitations of each method were derived and compared with each other from the viewpoint of the applicability of a HRA method to the railway industry.

  • The railway human reliability analysis (R-HRA) plays a role of identifying and assessing human failure events in the framework of the probabilistic risk assessment (PRA) of the railway systems.   • Recently, many safety measures are developing for the prevention of human error, which is main factors of railway accident. For the efficient management of human factors, many expertise on design, conditions, safety culture and staffing are required. But current safety management activities on safety critical works are focused on training, due to the limited resource and information. In order to establish railway human factors management, a systematic review model is required. Based on system engineering and nuclear industry model, a program review model is proposed in this study. The model includes operating experience review, task analysis, staffing and qualification, human reliability analysis, huma-system interface design, procedure development, training program, verification and validation, implementation and monitoring. Results can be applied for the review of safety measures relating to human factors.

열차 운전 종사자를 대상으로 한 인적오류의 개선 방안 연구 (A Study on the Prevention Measures of Human Error with Railway Drivers)

  • In this study, the causes of human error were identified through the survey of the drivers of the three organizations: Seoul Metro, Seoul Metropolitan Rapid Transit Corporation, and Korail. It was started with the aim of finding and eliciting causes in various directions including human factors, job factors, and environmental factors. The Cronbach alpha value was 0.95 for the reliability significance of the stress-induced factors in the operational area. The significance probability for organisational factors was shown to be 0.82, and the significance of the sub-accident experience and the driving skill factors in operation was 0.81 In addition, the analysis results showed that stress-induced in the field of driving is higher than the human factors in the reliability analysis. The results of the analysis confirmed that the reliability of the organizational and operational stress-induced factors was higher than other causes. In order to reduce urban railroad accidents, this paper suggests a method for operating safe urban railroad through the minimization human errors.

철도사고의 인적오류 분석을 위한 수행도 영향인자 분류 (Taxonomy of Performance Shaping Factors for Human Error Analysis of Railway Accidents)

  • Enhanced machine reliability has dramatically reduced the rate and number of railway accidents but for further reduction human error should be considered together that accounts for about 20% of the accidents. Therefore, the objective of this study was to suggest a new taxonomy of performance shaping factors (PSFs) that could be utilized to identify the causes of a human error associated with railway accidents. Four categories of human factor, task factor, environment factor, and organization factor and 14 sub-categories of physical state, psychological state, knowledge/experience/ability, information/communication, regulation/procedure, specific character of task, infrastructure, device/MMI, working environment, external environment, education, direction/management, system/atmosphere, and welfare/opportunity along with 131 specific factors was suggested by carefully reviewing 8 representative published taxonomy of Casualty Analysis Methodology for Maritime Operations (CASMET), Cognitive Reliability and Error Analysis Method (CREAM), Human Factors Analysis and Classification System (HFACS), Integrated Safety Investigation Methodology (ISIM), Korea-Human Performance Enhancement System (K-HPES), Rail safety and Standards Board (RSSB), $TapRoot^{(R)}$, and Technique for Retrospective and Predictive Analysis of Cognitive Errors (TRACEr). Then these were applied to the case of the railway accident occurred between Komo and Kyungsan stations in 2003 for verification. Both cause decision chart and why-because tree were developed and modified to aid the analyst to find causal factors from the suggested taxonomy. The taxonomy was well suited so that eight causes were found to explain the driver's error in the accident. The taxonomy of PSFs suggested in this study could cover from latent factors to direct causes of human errors related with railway accidents with systematic categorization.

  • 기존 연구결과에 따르면, 시스템의 안전성에 가장 큰 영향을 주는 요인 중 하나는 인적수행도의 저하인 것으로 밝혀졌다. 따라서 인적수행도 저하 방지를 통한 사건/사고의 방지는 원자력이나 철도와 같이 안전을 우선적으로 고려해야 하는 산업체의 경우 반드시 고려되어야 하는 조건이다. 이를 위해서는 인적수행도 저감의 원인의 체계적 파악 및 이에 근거한 효과적인 관리가 선행되어야 하기 때문에, 대표적인 인적수행도 저하 원인으로 알려져 있는 직무복잡도를 객관적으로 평가할 수 있는 방법의 개발이 필요하다. 이러한 필요성에 따라, 본 논문에서는 원자력발전소의 절차화된 직무에 대한 복잡도를 평가할 수 있는 TACOM 척도를 사용하여 KTX 기장이 수행해야 하는 운전직무의 복잡도를 정량적으로 평가한 후 그 결과를 주관적 직무부하 평가 방법인 NASA-TLX 결과와 비교하여 적합성을 검증하였다.

철도시스템 이상진단 및 예지정비를 위한 FMEA 분석 방안 연구 (A Study on FMEA Analysis Method for Fault Diagnosis and Predictive Maintenance of the Railway Systems)

  • With the advent of industrialization, consumers and end-users demand more reliable products. Meeting these demands requires a comprehensive approach, involving tasks such as market information collection, planning, reliable raw material procurement, accurate reliability design, and prediction, including various reliability tests. Moreover, this encompasses aspects like reliability management during manufacturing, operational maintenance, and systematic failure information collection, interpretation, and feedback. Improving product reliability requires prioritizing it from the initial development stage. Failure mode and effect analysis (FMEA) is a widely used method to increase product reliability. In this study, we reanalyzed using the FMEA method and proposed an improved method. Domestic railways lack an accurate measurement method or system for maintenance, so maintenance decisions rely on the opinions of experienced personnel, based on their experience with past faults. However, the current selection method is flawed as it relies on human experience and memory capacity, which are limited and ineffective. Therefore, in this study, we further specify qualitative contents to systematically accumulate failure modes based on the Failure Modes Table and create a standardized form based on the Master FMEA form to newly systematize it.

도시철도운영기관의 서비스지향성이 직무만족과 고객지향성에 미치는 영향 (The Effects of the Service Orientation on Job Satisfaction and Customer Orientation of the Urban Railway-operating Company)

  • 본 논문에서는 도시철도운영기관의 서비스지향성이 직원의 직무만족 및 고객지향성에 미치는 영향을 파악하고자 설문을 통한 실증연구를 하였다. 이를 위해 요인분석 및 신뢰도 분석, 종속변수에 영향을 미치는 독립변수의 관계 분석을 위한 회귀분석을 실시하였으며, 기관별 차이분석을 위하여 독립표본 t-test분석 및 직무분야별 차이분석을 위한 일원배치분산분석을 실시하였다. 분석결과를 요약하면 첫째, 인적자원관리, 직원권한, 서비스리더십, 서비스시스템 순으로 직무만족에 정(+)의 영향을 미치는 것으로 나타났다. 둘째, 고객지향성의 경우는 서비스시스템, 인적자원관리, 서비스리더십이 정(+)의 영향을 미치며, 직원권한이 부(-)의 영향을 미치는 것으로 분석되었다. 셋째, A사와 B사의 직원들 간에는 직무만족과 고객지향성에 유의한 차이가 있는 것으로 분석되었다. 넷째, 직무분야별로는 차량분야 직원의 만족도와 고객지향성이 가장 높았으며, 역무분야 직원의 평균값이 가장 낮게 나타났다.

인적오류 저감을 위한 인간 신뢰도 분석 체계 및 평가에 관한 연구 (Development and Evaluation of Human Reliability Analysis Model for the Reduction of Human Errors)

  • Almost all companies have paid much attention to the safety management ranging from maintenance to operation even at the stage of designing in order to prevent accidents, but fatal accidents continue to increase throughout the world. In particular, it is essential to systematically prevent such fatal accidents as fire, explosion or leakage of toxic gas at factories in order to not only protect the workers and neighbors but also prevent economic losses and environmental pollution. In addition, HRA may be used to detect the human errors which may cause accidents or trace back to any mistake on the part of workers. Usually, HRA technique is used in association with other risk assessment techniques. Moreover, it can serve to enumerate the human errors which may occur during operation or down-time or correct the existing system to reduce the mistakes. This work focuses on the coincidence of human error and mechanical failure for management of human error, and on some important performance shaping factors to propose a method for improving safety effectively of the process industries.
