  • In oriental medicine, it is possible to classify each person into eight kinds of constitutions based on the eight constitutional medicine theory. We developed a piezoelectric 3-channel tactile sensor using PVDF (polyvinylidene fluoride) film for pulse detection of the radial artery. High frequency buffer (impulse buffer), amplifier, 60 Hz noise notch filter and low pass filter were integrated on three sheets of PCB board. The pulses of the radial artery at three points were checked using our system. Each constitution of the eight ones has different combinations of pulses.

Peripheral Blood Flow Velocity and Peripheral Pulse Wave Velocity Measured Using a Clip-type Pulsimeter Equipped with a Permanent Magnet and a Hall Device

  • We measured radial arterial pulse signals using a prototype of a clip-type pulsimeter equipped with a permanent magnet and a Hall device, which produced signals through a voltage-detecting circuit. The systolic peak time and the reflective peak time for a temporally pulsed signal were analyzed for an arbitrary pulse wave at one position of a small permanent magnet. The measured value of the peripheral pulse wave velocity was about 1.25-1.52 m/s, demonstrating the accuracy of this new method. To measure the peripheral blood flow velocity, we simultaneously connected the radial artery pulsimeter to a photoplethysmography meter. The average value of the peripheral blood flow velocity was about 0.27-0.50 m/s.

A study for measurement of radial artery oxygen saturation system using photoelectric plenthysmography (광전용적맥파를 이용한 요골동맥 산소포화도 측정 시스템의 연구)

  • In this study, the pulse of radial artery and oxygen saturation are detected using photoelectric plethysmograph method. Using our device designed reflection type, we can detect the reflected light by radial artery and by switching circuit, we can also separate to 625nm band signal and 940nm band signals. The separated signals are converted as a pulse data by the pulse signal processing circuit. In this study, the reflection type of oxygen saturation calculation method is applied instead of the transmission type because of the reflection type sensor is used to measure the radial artery. As a result, we can detect about 97% accuracy of the oxygen saturation compare with the conventional method. For the accurate signals, the wrist band with sensor was designed and fixed on the radial artery. As a result, this wrist band type sensor was applicable to prevent position errors from motion artifact and could increase the accuracy during the measuring.

  • In order to standardize the pressure/depth against radial artery, the externally-applied-pressure measuring system was fabricated and evaluated. Based on the resistance-variable characteristic of the very thin($10{\mu}m$) film conductive tape along the pressure of a tip of a examiner's hand, this system was designed. The change of the pressure was processed through voltage regulator and Matlab S/W, then showed on computer monitor. The signal output through voltage regulator, and Matlab S/W was evaluated on various conditions. The evaluation was executed on these cases; an examiner slowly increases and decreases the pressure, rapidly increases and decreases the pressure, sequentially increases and decreases the pressure, sustains the pressure, micro-changes the pressure. As a pulse examiner varies the pressure on the radial artery of the examinee, the system's real-time output consistently varies according to the pressure. From the results, it is concluded that this system consistently shows the pressure of the tip of a examiner's hand in real time without interrupting the evaluation of the radial artery pulse. Therefore this system is expected to standardize the value of the pressure/depth externally applied by an examiner.

AIx Change According to Pressing Angle and Pressing Force of the Radial Artery Pulse by Tonometry (토노메트리 방식 맥파 측정의 가압 각도와 가압력에 따른 AIx 변화)

  • A radial artery pulse wave is measured while pressing an artery with constant force. However, pulse waveform measurements vary depending on pressing force and direction. Accurate pulse waveform measurements are important for analysis. Thus, it is necessary to define the measurement range of the permissible force and direction from which a correct pulse waveform is derived. In this study, pulse waves were generated by a pulse wave generator for accurate control. The pulse waves generated for different angles and pressing forces were analyzed. The augmentation index (AIx), which is the most commonly used index for evaluating vascular stiffness, was analyzed. The AIx was measured within ${\pm}6^{\circ}$ of the vessel direction and within ${\pm}8^{\circ}$ perpendicular to the vessel direction with a force that was 25% or more of the pressing force at which the maximum pressure wave was generated. We identified the applicable pressing force and angle range by analyzing the effect of pressing angle on the pulse wave. The AIx analysis performed using the pulse wave measurement device is reliable and reproducible.

  • One prototype product of clip-type pulsimeter equipped with magnetic field sensing semiconductor Hall device after one permanent magnet attached "Chwan" position in center of a radial artery was developed. The clip-pulsimeter was composed of the hard ware system measuring to voltage signals. To measure the blood velocity, the radial artery pulsimeter is simultaneously connected the PPG (photoplethysmograph). Analysis and comparison of two pulse waves data has done obtained from a clinical test of forty subjects of 20 ages. The value of a blood velocity simultaneously measured from a radial artery puls wave and PPG is an average value of 0.8m/s. The usage of this research results is possible to store the biomedical signals for health care.

A Study on the Characteristics of Pulse Waves according to Eight Principle Pattern Identification in 500 Women (500명 여성을 대상으로 한 팔강변증에 따른 맥파 특성 연구)

  • This study was conducted to find objective diagnostic indicators for the Eight Principle Pattern Identification using a pulse wave analyzer. Typology Complexion Pulse and Symptom data from 500 women over the age of 18 were used. Five experts made a diagnosis of Eight Principle Pattern Identification and divided the subjects into 'Yin-Yang', 'Exterior-Interior', 'Cold-Heat', 'Deficiency-Excess' groups. Their pulse waves were measured in the left and right radial arteries, and it was investigated whether there was a significant difference between groups in the pulse wave parameter values. 'Yin' group showed a significantly lower value in the left radial artery for Ener, Emin, EIX, T4T, T4TT parameters and in the Right for T2, T2T, T5. The Vmag, As and Ad parameters were significantly different between the 'Exterior' and 'Interior' groups. 'Heat' group showed a significantly higher value in the right radial artery for RAI/t parameter. 'Deficiency' group showed a significantly higher value in the right radial artery for W, Angl parameters. Through this study, significant pulse wave parameters were found, and they can be used as objective diagnostic indicators for Eight Principle Pattern Identification.

Pulse Wave Velocity Measured by Radial Artery Clip-type Pulsimeter Equipped with a Hall Device and Electrocardiogram (홀소자가 구비된 요골동맥 집게형맥진기와 심전도로 측정된 맥파전달속도)

  • The clip-type pulsimeter equipped with a magnetic sensing Hall device and the most popular body sign of the electrocardiogram (ECG) were investigated in order to analyze the pulse wave velocity (PWV). The PWV simultaneously calculated by means of time difference between the maximum peak of ECG pulse wave and the starting point of radial artery pulse wave, and distance difference between the heart position and the radial wrist position. The PWV analyzed from the clinical data was a wider scope of 5~7 m/s with an average value of 6 m/s. By the prediction of blood vessel's elasticity from the analysis of PWV, it may be useful for developing an oriental-western diagnostic medical signal device for a U-health-care system in the future.

The Heart Rate Monitoring System using Inverted Photoplethysmography (반전 용적맥파 신호를 이용한 심박 모니터링 시스템)

  • Photoplethysmogram is widely used to measure heart rate and arterial blood oxygen saturation in human. This paper describes implementation a photoplethysmography monitoring system that uses 780nm and 940nm length infrared light in radial artery. That system used combinations of 8 LEDs and 2 photoelectricities. When recorded on the skin over radial artery, the radial pulse wave was inverted. The mechanism of inverted pulse wave skin, which was reduced during systolic period and increase during diastolic period of the cardiac cycle. Through this system, we discovered optimal environments and combinations of sensors in both penetration type and reflection type. These results suggest that radial arterial wall way reflect infrared ray.

Observational Clinical Study on Mibyeong Based on Korean Medicine Diagnosis, Questionnaire, and Radial Artery Tonometry (한의사의 진단, 설문지, 맥진을 이용한 미병 관리에 관한 관찰적 임상연구)

  • Objectives : This study was conducted to reveal the relationship between multiple factors of traditional Korean Medicine diagnosis and consider the further probabilities of treating people with physical and mental problems not defined as diseases, which is called 'Mibyeong' in traditional Korean Medicine. Methods : 40 healthy participants were included in the observational clinical trial. The participants were asked to complete health questionnaires (e.g. State-Trait Anxiety Inventory, Pittsburgh Sleep Quality Index, Stress Response Inventory) and they went through a traditional diagnosis process, including four stages of diagnosis (looking, listening/smelling, inquiring, and pulse taking), by a Korean Medicine doctor. Both the Korean Medicine doctor and an artery tonometry device performed the pulse diagnosis. Results : Although all participants were healthy people with no history of disease, more than half of participants had a problem related with severe level of fatigue (n=19), sleep disturbance (n=26) and stress (n=27) status according to the related questionnaires. Participants diagnosed with phlegm syndrome by the Korean Medicine doctor showed significantly greater score in phlegm pattern questionnaires than participants who were not. However, there was little agreement between the doctor's pulse diagnosis and radial artery tonometry results. Conclusions : We conducted a pulse diagnosis and measured health-related information along with the traditional Korean Medicine diagnose procedure, including four stages of diagnosis, and we found a linkage between diagnosis of phlegm and the phlegm pattern questionnaire score. The results suggest that a number of healthy participants, with no disease diagnosed, have Mibyoung symptoms which need further clinical management. Thus, we suggest that Mibyoung management programs based on qualified diagnosis tools and traditional Korean medicine diagnosis procedures be developed, and that future research using various diagnostic tools be carried out on a large population.
