Demographic review of aesthetic surgery for patients with facial palsy

  • Background: This study analyzed the demographic characteristics of patients with facial palsy who were treated using either dynamic or static procedures. This study aimed to compare the frequency of procedure implementation and age distribution between the two groups. Methods: This study retrospectively analyzed the medical records of patients treated for facial palsy at a single institution from 2014 to 2022. Among cases included in our study, dynamic procedures involved cross-facial nerve graft and latissimus dorsi or gracilis muscle flap transfer. Static procedures included gold weight insertion, canthopexy, browlift, and thread lift/static slings. Results: Among the 31 patients included in our study, eight (25.8%) incorporated dynamic techniques, and the average age of patients was 44.75 years (range, 24-68 years) with a male to female ratio of 1:4. The remaining 23 patients (74.2%) underwent a static procedure, of which the average age was 59.17 years (range, 23-81 years) which was statistically significantly higher than the average age of 44.75 of dynamic patients (p= 0.013). Regarding the timing of treatment after diagnosis, no patient underwent dynamic procedures more than 20 years after initial diagnosis. A greater diversity in the timing of treatment was observed in the static group. All patients who underwent dynamic procedures were treated using static procedures during the study period. Conclusion: Because aesthetics-based static techniques are typically quick outpatient procedures that can be performed under local anesthesia, our study shows that these are often preferred treatments for all age groups, especially for debilitated or older patients. Further research is required to investigate the long-term functional outcomes of these surgical techniques in a wider population of patients.

In-depth exploration of machine learning algorithms for predicting sidewall displacement in underground caverns

  • This paper delves into the critical assessment of predicting sidewall displacement in underground caverns through the application of nine distinct machine learning techniques. The accurate prediction of sidewall displacement is essential for ensuring the structural safety and stability of underground caverns, which are prone to various geological challenges. The dataset utilized in this study comprises a total of 310 data points, each containing 13 relevant parameters extracted from 10 underground cavern projects located in Iran and other regions. To facilitate a comprehensive evaluation, the dataset is evenly divided into training and testing subset. The study employs a diverse array of machine learning models, including recurrent neural network, back-propagation neural network, K-nearest neighbors, normalized and ordinary radial basis function, support vector machine, weight estimation, feed-forward stepwise regression, and fuzzy inference system. These models are leveraged to develop predictive models that can accurately forecast sidewall displacement in underground caverns. The training phase involves utilizing 80% of the dataset (248 data points) to train the models, while the remaining 20% (62 data points) are used for testing and validation purposes. The findings of the study highlight the back-propagation neural network (BPNN) model as the most effective in providing accurate predictions. The BPNN model demonstrates a remarkably high correlation coefficient (R2 = 0.99) and a low error rate (RMSE = 4.27E-05), indicating its superior performance in predicting sidewall displacement in underground caverns. This research contributes valuable insights into the application of machine learning techniques for enhancing the safety and stability of underground structures.

Correlation between litter decomposition rate of Quercus mongolica leaf and microclimatic factors at Mt. Jeombongsan (점봉산 신갈나무 낙엽의 분해율과 미기상요인과의 상관관계 분석)

  • To understand functional changes of forest ecosystems due to climate change, correlation between decomposition rate of leaf litter, an important function of forest ecosystems, and microclimatic factors was analyzed. After 48 months elapsed, percent remaining weight of Quercus mongolica leaf litter was 27.1% in the east aspect and 37.0% in the west aspects. Decay constant of Q. mongolica leaf litter was 0.33 in the east aspect and 0.25 in the west aspect after 48 months elapsed. Initial C/N ratio of Q. mongolica leaf litter was 38.5. After 48 months elapsed, C/N ratio of decomposing Q. mongolica leaf litter decreased to 13.43 in the east aspect and 16.72 in the west aspect. Average air temperature and soil temperature during the investigation period of the research site were 8.2±9.0 and 9.1±9.3 in the east and 8.5±7.4 and 9.3±7.3℃ in the west aspect, respectively, with the west aspect showing higher air and soil temperatures. Soil moisture showed no significant difference between east and west aspects (average soil moisture: 19.4±11.0% vs. 20.5±5.7%). However, as a result of analyzing the correlation between decomposition rate and microclimatic factors, it was found that the decomposition rate and soil moisture has a positive correlation(r=0.426) in the east aspect but not in the west aspect. Our study shows that the correlation between decomposition rate and microclimatic factors can be significantly different depending on the direction of the aspect.

Extracorporeal Membrane Oxygenation in Neonates with Congenital Diaphragmatic Hernia: a Preliminary Experience (체외막 산소화 요법을 적용한 선천성 횡격막탈장 치료의 초기 경험)

  • Extracorporeal membrane oxygenation (ECMO) has been utilized in congenital diaphragmatic hernia (CDH) patients with severe respiratory failure unresponsive to conventional medical treatment. We retrospectively reviewed 12 CDH patients who were treated using ECMO in our center between April 2008 and February 2011. The pre ECMO and on ECMO variables analyzed included gestational age, sex, birth weight, age at the time of ECMO cannulation, arterial blood gas analysis results, CDH location, timing of CDH repair operation, complications and survival. There were 9 boys and 3 girls. All patients were prenatally diagnosed. Mean gestational age was $38.8{\pm}1.7$ weeks and mean birth weight was $3031{\pm}499$ gram. Mean age at the time of ECMO cannulation was $29.9{\pm}28.9$ hours. There were 4 patients who survived. Survivors showed higher 5 min Apgar scores ($8.25{\pm}0.96$ vs. $7.00{\pm}1.20$, p=0.109), higher pre ECMO mean pH ($7.258 {\pm}0.830$ vs. $7.159{\pm}0.986$, p=0.073) and lower pre ECMO $PaCO_2$ ($48.2{\pm}7.9$ vs. $64.8{\pm}16.1$, p=0.109) without statistical significance. The hernia was located on the left side in 10 patients and the right side in 2 patients. The time interval from ECMO placement to operative repair was about 3~4 days in 5 early cases and around 24 in the remaining cases. There were 3 cases of post operative bleeding requiring re operation and 2 cases of abdominal compartment syndrome requiring abdominal fascia reopening. ECMO catheter reposition was required in 4 cases. Three cases of arterial or venous thrombosis were detected and improved with follow up. Our data suggests that ECMO therapy could save the lives of some neonates with CDH who can not be maintained on other treatment modalities. Protocolized management and accumulation of case experience might be valuable in improving outcomes for neonates with CDH treated with ECMO.

A study of the metabolites for 7-keto-DHEA-acetate in human urine (II) (뇨시료에 함유된 7-keto-DHEA-acetate의 대사체에 관한 연구 (II))

  • The 7-keto-DHEA-acetate is converted to 7-keto-DHEA, a metabolite of DHEA, and similar to its metabolism. We studied the metabolite M3, M4, and M5 of 7-keto-DHEA-acetate. The estimated molecular weight of M3 and M4 was 304 which were supposed to have more 3 hydroxyl and/or ketone groups. We could know that M3 is the 7-OH-DHEA which has the hydroxyl groups on 3 and 7-carbon and a ketone group on 17-carbon. In case of M4, it is the 7-oxo-diol metabolite which has the hydroxyl groups on 3 and 17-carbon and a ketone group on 7-carbon. The M5 was supposed that the molecular weight is 320 and has the three hydroxyl groups on 3, 6, and 16 carbon and the ketone group on 17-carbon. After dosing, 7-OH-DHEA showed the maximum urine flow in human urine after 5 hr and decreased rapidly. But we could find it until 58 hr why is a higher remaining substance.

Effective Herbicide Application on Dry Seeded Paddy Rice (벼 건답휴립직파재배(乾畓畦立直播栽培)에서 효과적(效果的)인 제초제(除草劑) 사용법(使用法))

  • Several rice herbicides were evaluated to determine the herbicidal efficacy on dry seeded paddy rice. The time of herbicide application was within 1 day after seeding(DAS), 18 DAS, and 40 DAS. Under the upland conditions 30 DAS(before permanent flooding) dominant weed species were Echinochloa sp., Digitaria sp., Leptochla sp., weedy rice and other several annual broadleaved weeds belonging to Cruciferae, but after flooding Echinochloa sp., Aeschynomene sp., Eleocharis kuroguwai, Cyperus sorotinus, Ludwigia prostrata, Aneilema japonica were dominat. Early application (1 DAS) of thiobencarb, pyrazolate/butachlor, or chlomethoxyfen/butachlor resulted in very good herbicidal efficacy only during the first 30 days, but not thereafter. Application of propanil+butachlor on 18 DAS did not control the late occurring weeds effectively. Application of bentazone/quinchlorac on 40 DAS successfully suppressed weed growth throughout the remaining rice growing season and showed similar grain yield as hand weeded plot. Weed growth reduced the plant height, panicle number, 1000-grain weight, percent ripened grains, and grain yield of rice by 7, 82, 19, 55 and 87% respectively. There was a positive linear relationship between weed weight and lodging index of rice.

PILOT SCALE FISH PRODUCTION IN WATER RECYCLING SYSTEM (순환여과식 사육수정화 재사용방법에 의한 Pilot 규모의 어류양식시험)

  • Fish were grown in a water recycling rearing system consisting of a filter system, of which the area is about $30\;m^2$ and the volume of filter gravels is about $15\;m^3$, 6 rearing and 2 supplementary tans about $7\;m^2$ of surface area each. The results for 180 days from April 18 to October 14,1980 are summarized as following: One rearing tank was used for main experiment stocked with an equal number, and weight of each Israeli strain and $F_1$, hybrid between this strain and Korean local strain of common carp. Gross total production was 461 kg with a net production of 395 kg, and the Israeli strain grew to about 8 times initial weight and the hybrid about 6 times. Based on an analysis of results of the above mentioned main experiment and the other miscellaneous fish grown in the remaining tanks, it was considered to be feasible to implement a commercial production in this system by improving parts of the system and adding a number of the units. A special problem seems to be an adoption of more effective aeration device when fish loading increases. A solid waste removal system was deviced by use of by-pass flow ana this system worked very efficiently in view of supply water economy. A serious problem encountered in this experiment was heavy outbreaks of the gill-fluke, Dactylogyrus sp. which was relatively easily eradicated with Dipterex or DDVP treatment in the first trials, but later on strains which were resistant to any of these chemicals appeared.

The Relationship between Food Habit, Nutrient Intakes and Bone Mineral Density and Bone Mineral Content in Adult Women (성인여성의 식습관과 영양섭취상태와 골밀도 및 골무기질함량과의 관계)

  • Bones are important parts in sustaining the shape of the body, but they are also metabolic organs which undergo bone remodeling by constant bone resorption and formation. Osteoporosis, the typical metabolic bone disease, is characterized by a reduction in bone mineral density (BMD). Women more than men are at risk fir osteoporosis-related fractures, especially in the lumbar spine, wrist, and hip region. Risk of fracture depends on one's BMD, which open determined by the peak bone mass value achieved at skeletal maturity and followed by subsequent age-and menopause-related bone loss. Genetic and environmental factors are known to play a key role in bone metabolism and diet is considered as one of the important environmental factors. The purpose of the present study was to assess the status of BMD and bone mineral content(BMC) to clarify the relationships between dietary intakes and the risk of osteoporosis in adult women in Taegu. Subjects were 130 healthy females in between 20 and 69 years of age. BMD and BMC of the lumbar spine(venebrae L2-4) of the subjects were measured by dual energy X-ray absorptiometry. The average age of the subjects was 47.4${\pm}$11.7 years old, the average weight was 57.2${\pm}$8.4kg, the average age of menarche was 16.6${\pm}$1.9 years old and the average age of menopause was 48.4${\pm}$5.3 years old. The nutrient intakes of the subjects measured by the convenient method were generally lower than the level of RDA. The result of nutrient intake assessed has shown that the average energy intake was 1701${\pm}$316kca1 which is 85.1% of the RDA and the average calcium intake was 485.4${\pm}$172.3mg which is 69.3% of the RDA. The intakes of protein, vitamin A, vitamin B$_1$, niacin were greater than the RDA, whereas the remaining nutrient intakes were lower than the RDA. The average BMD of the subjects was shown to be 1.06${\pm}$1.09g/$\textrm{cm}^2$. The highest BMD of 1.24${\pm}$0.14g/$\textrm{cm}^2$ was noticed in the subjects of 30s compared to 20s, 40s, 50s, 60s. The BMD values were compared by the relative body weight(R3W) of the menopause subjects, and it was found that the underweight group had significant lower BMB while the rest of the groups did not have any differences in BMD. The most strongly correlated nutrient with BMD among the menopause subjects appeared to be calcium. The women whose Ca intakes were higher than 500mg showed the significantly higher BMD than those with Ca intakes lower than 500mg. This study suggests that the most effective way to prevent osteoporosis and to reduce the incidences of fractures seems to be minimizing bone loss through the adequate intake of calcium as well as avoiding underweight, especially in menopausal women. Clinical, Laboratory Biomarkers and Imaging Findings of Pancreatic Adenocarcinoma in Iran

  • Background: Pancreatic cancer has a high mortality rate because it is usually diagnosed late. Since little is known about this cancer in Iran, with the aim of improving this knowledge deficiency, we evaluated clinical, laboratory biomarkers, imaging findings and treatment modalities in Iranian patients with pancreatic adenocarcinoma. Materials and Methods: 131 cases of pancreatic adenocarcinoma in 2010-2013 were obtained from the Taleghani Hospital Record Department. Cases confirmed by histopathology from CT-guided biopsy, EUS-FNA and surgery examination were included. We excluded those with incomplete medical records. Results: The study included 131 subjects between 24 and 97 years of age and a mean age of $63{\pm}13.4$ years. Eighty (61.1%) were male and 51 (38.9%) female. Previous history included diabetes mellitus in 36 (27.5%), alcohol drinking in 5 (3.9%), smoker in 28 (21.4%) and opium addiction in 13 (10%). The common presenting history included weight loss in 79 (60.3%), abdominal pain in 77 (58.8%), fever in 11 (8.4%), nausea in 30 (22.9%), jaundice in 72 (55%), pruritus in 52 (39.7) and anemia in 33 (25.2%). CA19-9 levels with cut offs of 50, 100 and 200 U/ml were increased in 81%, 72% and 66% of patients, respectively. Tumor staging was: stage I, 3 (2.3%); stage II, 10 (7.6%); stage III, 58 (44.3%); and stage IV, 60 (45.8%). From 45 patients, 17 received ERCP inserted metallic stents and 22 plastic stents, the remaining 6 failed that PTC was done. Whipple surgery and chemotherapy were conducted for 10 and 29 patients, respectively. Conclusions: This disease affected older people and there was a male preponderance. The commonest risk factors were diabetes mellitus, smoking and cholelithiasis. The majority of patients presented with loss of appetite, loss of weight, jaundice, abdominal pain and discomfort. Almost all presented at late stages of the disease so that curative surgery was impossible. Also chemotherapy was only performed in a few patients as a neoadjuant treatment.

Performance of Crossbred Cattle Fed Chopped Green Sugarcane Tops and Supplemented with Wheat Bran or Lentil Chuni Concentrates

  • Fifteen crossbred cattle bulls of about 22-24 months age (mean body weight ranged from 291.0 to 298.1 kg) were randomly divided into 3 groups of 5 animals following randomized block design. Animals in group 1 were fed concentrate mixture containing wheat bran 97%, mineral mixture 2% and salt 1% plus ad libitum chopped green sugarcane tops (SCT) as a roughage source. Animals in group 2 were fed concentrate mixture containing lentil chuni 97%, mineral mixture 2% and salt 1% while group 3 concentrate contained wheat bran 48.5%, lentil chuni 48.5%, mineral mixture 2% and salt 1% plus ad libitum chopped green SCT to meet their maintenance requirements. During metabolism trial period, the mean total intake and digestibility of DM in groups 1, 2 and 3 was $8.09{\pm}0.41$, $8.41{\pm}0.49$ and $7.86{\pm}0.16kg/d$ and 60.24, 63.24 and 65.05%, respectively. The intake of CP (p<0.05) and EE (p<0.01) was significantly higher in group 2 and group 1, respectively as compared to remaining 2 groups, which were comparable. Digestibility of CP and EE was significantly (p<0.01) higher in group 3 and that of CP was comparable with group 2. The intake and digestibility of total carbohydrates, NDF, ADF, Hemi-cellulose and cellulose were similar among 3 groups. The mean body weight changes in group 1, 2 and 3 were 173.4, 253.4 and 203.4 g/d, respectively, which was significantly (p<0.05) higher in group 2. The total nitrogen (N) and phosphorus (P) intakes were significantly higher in group 2 and group 1, respectively. The total volatile fatty acids, ammonia-N, trichloro-acetic acid precipitable nitrogen (TCA ppt.-N; p<0.05), total-N concentrations and pH were significantly (p<0.01) higher in group 3, however, pH and TCA ppt.-N was comparable with group 2 and 1, respectively. From the results, it may be deduced that the feeding of chopped green SCT supplemented with a concentrate mixture having both wheat bran (48.5%) and lentil chuni (48.5%) has shown a better performance with respect to intake and digestibility of nutrients and growth as well.
