  • Since the establishment of the national medical licensing examination board in 1992, the medical licensing examination system has changed enormously and this has had a number of impacts on examination services. All those reforms were aimed at improving the relevance and reliability of the test. Several attempts of the testing system have appeared in the new examination service, and which have also brought about the changes in the medical school curriculum such as introducing integrated courses instead of traditional subjects, using test scores as a reference to the post-graduation selection test. Some examples of changes in the examination system are as follows: 1) choosing three integrated test subjects and outlines of their reference content instead of 15 academic subjects, 2) adjusting the ratio of multiple choice question items to focus more on the problem solving level, 3) introduction of 'one-best answer' single set and 'extended matching type items, 4) item construction based on real clinical cases and real clinical materials. Recently, a clinical skill test system has been introduced to measure examinees' basic clinical skills competencies. Despite continuing efforts, the examination system still has many issues remaining to be solved. These problems include the differential weighting of test items, appropriate threshold for passing, and practicality of pre-testing to stabilize the passing rate and avoid the hazards of newness and undesirably difficult test items.

  • The aim of this study lies in answering the question "How to form weighting cells to enhance sample representativeness in telephone, Internet and mobile surveys\ulcorner". For this, we explored 2% raw data of Year 2000 Population and Housing Census of Korea looking for meaningful patterns for ownership of telephones, the usage of Internet and/or mobile phones. We found that telephone coverage rates vary significantly by household size; 84.6% for one member households, contrasting 98.5% for two-or-more member households. Thus, telephone survey samples need to be weighted differently in sub-groups by household size for proportional representation of target population. Searching socio-demographic factors influencing the use of Internet by C5.0 tree models, we found that education levels and the occupation (or housing type, the automobile ownership) are two most important factors in addition to gender and age. Thus, surveyor might form weighting cells by such factors at the stage of post-stratification or set quotas, a priori, proportional to size of the cells by such factors. For mobile surveys, we approached similarly and found that education levels and the occupation (or the automobile ownership, marriage status) are two additional factors that may be used in forming weighing cells or in setting quotas for cells.

  • Objectives : There has been a considerable debate about how to best control for placebo effects in clinical trials of acupuncture. Recently several sham needles were developed and validated. This study aimed at summarising the validation studies of these needles and evaluating the outcomes of the randomised controlled trials (RCTs) using them. Methods : Computerised literature searches were performed using 'acupuncture' AND 'placebo OR sham' with a limitation of the results to RCTs in Medline via PubMed and the Cochrane Library. Only formally validated sham needle controlled studies were included. Data were extracted regarding study design, condition, sample size, credibility testing, intervention and outcomes. Methodological quality was assessed using a modified Jadad scale. Results : Three validated sham needles by Streitberger, Park, and Fink, were identified. Acupuncture's effectiveness for various conditions was tested using these needles in 12 RCTs. Real acupuncture was superior to sham acupuncture for rotator cuff tendonitis and hypertension. No significant differences between real acupuncture and sham acupuncture emerged for chemotherapy-related nausea and vomiting, postoperative nausea and vomiting, menstrually related migraine, acute stroke rehabilitation, chronic/episodic tension-type headache, neutrophil respiratory burst in healthy volunteers, alcohol withdrawal symptoms and chronic poststroke leg spasticity. Conclusions : The new sham devices have been adequately validated and may be useful tools for investigating specific research question. In spite of the sham needle's limits, the results of RCTs using such devices tend to suggest that the clinical effects of acupuncture are largely due to a placebo response.

  • Knowledge base means a library stored in computer system providing useful information or appropriate solutions to specific area. Knowledge base associated with autism is the complex multidimensional information set related to the disease autism for its pathogenic factor and therapy. This paper focuses on the knowledge of biological molecular information extracted from massive biomedical texts with the aid of widespread used machine learning methods. Six classes of biological molecular information (such as protein, DNA, RNA, cell line, cell component, and cell type) are concerned and the probability statistics method, conditional random fields (CRFs), is utilized to discover these knowledges in this work. The knowledge base can help biologists to etiological analysis and pharmacists to drug development, which can at least answer four questions in question-answering (QA) system, i.e., which proteins are most related to the disease autism, which DNAs play important role to the development of autism, which cell types have the correlation to autism and which cell components participate the process to autism. • The paper studies the fluid flow profile contained between the orthotropic plate and rigid wall under the action of the moving load on the plate and main attention is focused on the fluid velocity profile in the load moving direction. It is assumed that the plate material is orthotropic one and the fluid is viscous and barotropic compressible. The plane-strain state in the plate and the plane flow of the fluid is considered. The motion of the plate is described by utilizing the exact equations of elastodynamics for anisotropic bodies, however, the flow of the fluid by utilizing the linearized Navier-Stokes equations. For the solution of the corresponding boundary value problem, the moving coordinate system associated with the moving load is introduced, after which the exponential Fourier transformation is employed with respect to the coordinate which indicates the distance of the material points from the moving load. The exact analytical expressions for the Fourier transforms of the sought values are obtained, the originals of which are determined numerically. Presented numerical results and their analyses are focused on the question of how the moving load acting on the face plane of the plate which is not in the contact with the fluid can cause the fluid flow and what type profile has this flow along the thickness direction of the strip filled by the fluid and, finally, how this profile changes ahead and behind with the distance of the moving load.
