Y의료원의 물류 공급체계 중앙화 관리에 대한 연구 (A Study of an effective centralization of medical supply system. In Y University Medical Center)

  • Since the late 1980s, there have been radical changes in the managerial environment of Y University Medical Center(YUMC). Externally, the competition among hospitals has intensified due to the establishment of universal health insurance in 1939 and the entrance of large enterprises into the health care industry in the early 1990s. In addition, government regulation of medical institution is becoming stricter. Also, consumer groups have continued to demand the respect for patient rights and improvement of the quality of medical services. Internally, the financial condition of YUMC has worsened, not only because weak control and poor mediation in its large-scale structure have made its operation inefficient, but also because the rates of increase in the prices of goods and labor have grown faster than any increases in revenues. This study on materials management at YUMC presents a way for YUMC to reduce costs and increase its productivity, thereby overcoming its financial difficulties and dealing with external pressures. This study utilized the case studies of the materials purchasing and medical supply management in the United States and the comparative analysis of management to suggest short-term and long-term alternatives for innovation in YUMC. The goals of the short-term alternatives for innovation are to centralize the purchasing and supply departments and to simplify the decision-making processes. Through these attempts, it is estimated that YUMC's costs could be reduced by $600,000 per year. In the long-term, it is necessary to consider introducing a Supply Processing Distribution(SPD) system and setting up a centralized electronic system for supply and inventory management, although it is difficult to estimate the effect of cost-cutting because of the lack of analysis data. Thus, YUMC should thoroughly analyze initial investment costs and economical efficiency generated from long-term alternatives.

치과클리닉에 대한 의료 소비자의 브랜드 이미지가 충성도에 미치는 영향 - 수도권의 의료 소비자 중심으로 - (The Effects of Brand Image of Dental Clinics Perceived by Healthcare Service Users)

  • Purposes : The purpose of this study is to analyze empirically the consturct development of brand trust in dental clinic service and its influence on both satisfaction and brand loyalty of clinic users in Seoul area. Methodology : To test the hypotheses, the structured instrument was employed to question to patients and their guardians who had used dental clinics in the metropolitan area. 207 cases were analyzed with structural equation method through SPSS 18.0 and AMOS 22.0, and findings of the test as follows. Findings : Firstly, both functional and associative images had a positive impact on the brand trust. Especially we found the influence of functional images was greater than that of the brands' associative images. Secondly, the satisfaction of dental clinic users had greater positive influence on the clinics with greater trust. Thirdly, the higher the satisfaction of health service consumers, the more positive effect on brand loyalty. This relationship between the satisfaction of dental clinic users and brand loyalty match the findings of previous studies on relations of similar variables. Practical Implications : This study shows that with greater satisfaction, the referral rate and the revisit rate of users would increase. This implies that dental clinics need to take appropriate actions to build brand trust with advertising and/or PR strategies for a good brand image. It would be necessary to research further expected variables that influence users' brand loyalty in the service, such as the dentists' level of expertise, their relationship level with customers, the quality of dental services and et al..

  • 병원 내의 정보시스템의 도입은 업무운영 효율의 혁신적인 향상을 가져 올 수 있으며 운영 경비의 절감과 환자에 대한 양질의 서비스를 제공한다. X-ray필름을 포함한 의료 영상 정보를 효율적으로 관리하는 PACS는 현대화된 병원 정보 시스템에 있어서 필수적인 구성요소로 등장하고 있다. PACS 시스템은 각종 장비에서 서로 다른 형태의 영상 정보를 표준 의료 영상 파일 형식으로 표준화시켜 네트워크로 연결시켜 주어야한다. 본 논문에서는 이를 해결하기 위해서 DICOM 컨버터를 설계하여 Non-DICOM 파일 형식을 표준화된 파일 형식으로 변환시켜 주는 Mini-PACS를 설계하였다. 이 시스템은 웹 기반에서 운영되고 있으며, 클라이언트 모듈과 서버모듈인 두개의 서버로 운영되고 있는 기존의 시스템과 달리 두개의 서버 기능을 하나로 통합하여 시스템 상호간의 강력한 결합을 이룰 수 있도록 설계 및 구현하였다.

  • In these days, HIS(Hospital Information System) raise the quality of medical services by effective management of medical records. As computing environment was developed, it is possible to search information quickly. But, standard medical data exchange is not completed between medical clinic and another organ so far. In case of patient transfer, past medical record was not efficiently transmitted. It be feasible treatment delay or medical accident. It is trouble that medical records is transferred by a person and communicate with each other. Extensible Markup Language (XML) is a simple, very flexible text format derived from SGML. Originally designed to meet the challenges of large-scale electronic publishing, XML is also playing an increasingly important role in the exchange of a wide variety of data on the Web and elsewhere. Form in system of company product, relative organs that handle bio-signal data is each other dissimilar and integration and to transmit to supplement bottleneck this research uses XML. In this study, it is discussed about sharing of medical data using XML web technology to standard medical record between hospital and relative organization The data structure model was designed to manage bio-signal data and patient record. We experimented about data transmission and all-in-one between different systems (one make use of MS-SQL database system and the other manage existent bio-signal data in itself form in file in this research). In order to search and refer medical record, the web-based system was implemented. The system that can be shared medical data was tested to estimate the merits of XML. Implemented XML schema confirms data transmission between different data system and integration result.

간호사의 근거중심실무역량에 대한 영향요인 (Factors Influencing Competency in Evidence-based Practice among Clinical Nurses)

  • Purpose: The aim of this study was to describe competency in evidence-based practice (EBP) among clinical nurses and to explore critical thinking disposition (CTD) and organizational culture (OC) as possible predictors for competency in EBP. Also this study was conducted to identify baseline data for purpose of developing evidence-based OC to ensure safety of patients and to improve quality of nursing services in clinical setting. Methods: With a descriptive correlation study design, a competency in EBP questionnaire was administered to a convenience sample of nurses (N=393) from a single general hospital in Gyeonggi Province. Results: The overall mean score for competency in EBP was $3.03{\pm}0.53$. Factors influencing competency in EBP were education (${\beta}=0.23$, p<.001), attendance at lectures on EBP (${\beta}=0.18$, p<.001), intellectual eagerness & healthy skepticism (${\beta}=0.27$, p<.001) among CTD, and hierarchy-oriented OC (${\beta}=0.14$, p=.005), which explained about 35% of the total variance. Conclusion: Critical thinking and competency in EBP should be part of nursing education especially for undergraduate students as well as strengthening clinical practice by activation of EBP. The results of this study that hierarchy-oriented OC, is a significant predictor for competency in EBP is difficult to accept. Its seemed to require further exploration.

농촌의 보건의료문제 해결을 위한 원격보건 : 미국과 한국의 경험 (Telehealth for Rural Health Problems: Experiences in The U.S.A and Korea)

  • Telehealth is widely tested in the U.S.A and other developed countries. This system is expected to solve rural health problems reducing professional isolation. Some demonstration projects showed that the system can provide quality care in reasonable prices to rural residents. However, few study has been done on whether telehealth system can attract physicians by reducing professional isolation. The system is not available to most of the rural hospitals because the price for the equipment and telephone charges are not low enough. It is promising that the system cost and telephone charges are decreasing gradually. As time passes, rural hospitals will be more viable for the system. Satisfaction of the physicians and patients is a key factor for the implementation of the system. The physicians need to understand more about telecommunication and computer systems. After physicians are well-versed about the system, we can expect wide use of telehealth in rural areas. Effort for the confidentiality and standardization should be devoted to assure patient's privacy and compatibility of patient records and exam results. In Korea, two projects are being operated in Uljin and Kurye. A study evaluated the economic efficiency of the projects suggesting that increase of the number of patients up to three times of current number or decrease in hardware costs and telecommunication charges into two thirds of the current costs. The hardware and telecommunication charges are decreasing. Another area telehealth system can be applied is psychiatric accommodation facilities. Establishment of telehealth in the psychiatric facilities will increase the access of psychiatric care for patients and is expected to be economically efficient.

  • 고품질의 맞춤형 서비스는 사회가 고령화, 초핵가족화, 재난 취약 계층의 증가로 인하여 급격히 늘어나게 되었다. 급성 질환, 심뇌혈관 질환, 자살 등 예방 가능한 사망률이 선진국에 비해 높으므로 빠르고 전문적인 구조 및 구급 서비스가 요구된다. 본 논문에서는 응급 환자가 발생하여 병원에 도착하기 전에 환자의 정보를 이용하여 응급처치가 가능하도록 한다. 그리고 응급 의료 기관에서는 환자 진료 준비를 사전에 갖출 수 있도록 하여 응급 환자 진료의 효율성을 높였다. 사회적 인지도가 높은 119번호를 이용하여 다양한 복합 응급 신고를 접수하고 있고, 유관기관과 통합적 대응 체계를 구축하는 효율적 응급의료서비스 선진화 전략을 제시하고자 한다.

국민안심병원에서 근무하는 간호사들의 직무스트레스와 회복탄력성이 소진에 미치는 영향 (Influence of Job Stress and Resilience on the Burnout of Nurses Who Works at the Designated Public Relief Hospital)

  • 본 연구는 장기화된 팬데믹 상황에서 COVID-19관련 환자를 돌보는 간호사들의 직무스트레스와 회복탄력성이 소진에 미치는 영향을 알아보는 것을 목적으로 하였다. 간호사 162명을 대상으로 구조화된 설문지를 배포하여 분석하였고, 직무스트레스의 보상부적절, 직무요구, 직장문화, 회복탄력성의 철학적 패턴, 관계적 패턴이 통계적으로 유의하게 나타났다. 직무스트레스와 회복탄력성이 소진에 미치는 효과를 단계적다중회귀분석 방법으로 파악한 결과 최종적으로 유의한 변수는 직무스트레스였다. 간호사의 소진은 간호서비스 질 저하, 병원 경쟁력의 감소, 나아가 방역체제에 영향을 줄 수 있다. 이에, 본 연구는 간호사의 처우 및 근무환경 개선 등 적절한 보상의 정책적인 지원이 필요함을 제언한다.

  • 최근 RFID, NFC, 스마트 기기 등 첨단 IT 기술의 융합을 통한 의료 서비스 및 프로세스 혁신에 대한 관심이 커지고 있다. 본 논문에서는 이러한 첨단 IT 기술을 활용하여 스마트 건강검진 시스템을 구축한 프로세스 혁신 사례를 제시한다. 종이 차트 및 수작업에 의존하는 기존의 비효율적인 방식에서 탈피하여 능동형 RFID 및 스마트 기기를 사용하여 건강검진 프로세스를 혁신하였다. 병원의 경쟁력을 강화하기 위해 새롭게 개발된 스마트 건강검진 시스템은 2013년 4월부터 삼성서울병원의 건강검진 센터에서 성공적으로 활용되고 있다. 새로운 시스템 도입을 통해 고객 서비스 품질 및 고객만족도 제고, 직원들의 업무 부하 경감, 업무 정확도 제고, 병원 경쟁력 강화 등의 효과를 보이고 있다.

중소병원 간호사가 인식하는 행복감, 임상간호인성 및 직무만족도와의 관계에 관한 융합연구 (Convergence study on the relationship between happiness, clinical nursing personality and job satisfaction perceived by small and medium hospital nurses)

  • 본 연구는 간호사의 행복감, 임상간호인성 및 직무만족도와의 관계를 파악하고 직무만족도 영향요인을 확인하여 임상 현장에서 간호서비스의 질 향상과 간호사의 직무만족도 향상을 위한 프로그램의 개발의 기초자료를 제공하고자 시도되었다. 연구의 대상자는 M시 소재 2곳의 병원에 근무하는 간호사로, 자료 수집은 2020년 4월 27에서 5월 11일까지였다. 수집된 자료는 SPSS WIN 23.0 program을 활용하여 분석하였다. 연구결과 대상자의 직무만족도에 미치는 영향요인을 분석한 결과 행복감이 영향력을 나타내었으며, 설명력은 20.4%였다. 결론적으로 간호사의 행복감을 향상시키고 직무만족도를 향상시킬 수 있는 구체적이고 체계적인 프로그램을 개발하는 것이 필요하다고 사료된다.
