영남권역에서 가정용 인공호흡기를 사용하는 환자 가족간병인의 간병 부담과 삶의 질 (Survey on the Care Burden and Quality of Life in Family Caregivers of Patients Using Home Mechanical Ventilator in Yeongnam Region, Korea)

  • 목적 : 본 연구는 영남권역 내 가정에서 인공호흡기 사용 환자를 돌보는 가족간병인이 느끼는 간병 부담과 삶의 질을 알아보고자 하였다. 연구방법 : 영남권역 내에 거주하며 가정에서 인공호흡기를 사용하는 환자를 돌보는 주간병인을 대상으로 설문조사를 시행하였고 가족간병인의 응답 내용을 분석하였다. 설문지는 환자의 정보, 간병 실태 및 추가 지원 요구 사항 등으로 구성하였다. 가족간병인의 간병 부담과 삶의 질을 평가하기 위해 각각 Korean version of Short Form Zarit Burden Interview(K-ZBI-12)과 한국형 가중치 모형에 따라 교정한 3-Level version of EuroQol-5 Dimension(KEQ-5D-3L)을 사용하였다. 통계적 유의수준은 p<0.05로 하였다. 결과 : 총 150명 중 주간병인이 가족간병인인 98명을 분석하였다. 환자의 연령은 54.10±17.94세, 성별은 남자 74명(75.5%), 여자 24명(24.5%)이었고 주 진단은 신경근육계 질환이 83명(84.7%)으로 가장 많았다. 가족간병인의 K-ZBI-12 점수는 33.08±10.34로 간병에 대한 고부담을 보였고 EQ-5D-3L 점수는 0.71±0.25로 두 항목은 음의 상관관계를 보였다(p=.038). 환자의 연령, 인공호흡기 사용기간, 의료비에 대한 가족간병인의 경제적 부담, 간병비에 대한 가족간병인의 경제적 부담 및 전문간병인의 참여 시간은 K-ZBI-12 점수와 각각 통계적으로 유의한 상관관계를 보였다. KEQ-5D-3L 점수는 가정용 인공호흡기의 사용기간과 음의 상관관계를 나타냈다(p=.017). 환자가 침습적 인공호흡기를 사용하는 경우 비침습적 인공호흡기를 사용하는 경우보다 가족간병인의 KEQ-5D-3L 점수가 낮았다(p=.008). 간병에 참여하는 인원이 두 명 이상인 경우가 한 명인 경우보다 K-ZBI-12 점수가 낮았다(p=.001). 가족간병인이 추가지원을 바라는 요구사항 중 경제적 지원에 대한 요구도가 가장 높게 조사되었다. 결론 : 영남권역에서 가정용 인공호흡기를 사용하는 환자들의 보호자가 느끼는 간병 부담이 크고 삶의 질이 낮다는 것을 알 수 있다. 가족간병인의 간병 부담과 삶의 질을 개선하기 위해서 지역에서 실질적으로 필요한 사회경제적 지원 및 환자와 가족들의 요구 사항을 확인하고 추가적인 지원을 위한 노력이 필요하다.

BRCA 돌연변이 가계의 심리상태 및 삶의 질 평가 (Evaluation of Psychosocial Impact and Quality of Life in BRCA Mutation Family)

  • 목 적: 본 연구는 한국인 BRCA 유전자 돌연변이 가계구성원들을 대상으로 암진단 및 돌연변이 보유 여부가 심리 상태와 삶의 질에 미치는 영향을 알아보기 위해 시행되었다. 대상 및 방법: BRCA 유전자 돌연변이를 가진 13가계에서 암에 이환된 보인자 17명, 이환되지 않은 보인자 16명, 건강한 비보인자 13명이 본 연구의 분석에 포함되었다. 이 세 군을 대상으로 우울, 불안, 낙관, 유전성 유방암관련 지식수준과 삶의 질을 설문을 통하여 평가하였다. 결 과: 설문시기는 유전자 검사 후 평균 21개월(6-35)로 세 군 간의 차이는 없었다(P=0.254). 세 군 간의 우울, 낙관, 육체적 삶의 질은 유사했다. 불안은 세 군 모두에서 일반인 보다 상승되어 있었다. 이환된 보인자의 정신적 삶의 질은 암에 이환된 보인자가 다른 두 군에 비해 유의하게 낮았다(P=0.009, P=0.017). 다변량 분석 결과 정신적 삶의 질에 영향을 미친 인자는 암이환여부(P=0.043)와 직업유무(P=0.008) 였다. 결 론: 같은 돌연변이 가계 내에서 돌연변이 유무는 우울, 불안, 낙관에서 심리적반작용을 일으키지 않았으나, 돌연변이 가계 구성원의 불안 수준은 돌연변이 유무에 관계 없이 높았다. 본 연구는 소규모 표본을 대상으로 한 단면적 연구이나, BRCA 유전자 검사에 수반될 수 있는 심리적 스트레스 및 그에 대한 대처법을 수립하는 데 기초연구로 의의를 가진다.

우리나라 대형 종합병원의 아웃소싱 실태 (Outsourcing in Hospital Services: Experience of Korean Hospitals)

  • This study was conducted to investigate the degree of utilization of outsourcing in large hospitals in Korea. We also investigated the outcome and the level of satisfaction for adopting outsourcing in these hospitals. Types of work areas that were currently operated by outsourcing and were planned to adopt outsourcing in the future were identified. A total of 83 hospitals were eligible for this study, which had more than 500 beds, and were identified from the 2003 National Hospital List published by the Korean Hospital Association. A self-administered Questionnaire survey was conducted between April 25th and May 20th in 2003 with a personnel being charged of arrangement of outsourcing in each hospital. Among the 58 hospitals responding the survey(response rate=69.9%), 49 hospitals(84.5%) utilized outsourcing in at least one work field in their organizations. The largest proportion of the hospitals(85.7%) using outsourcing responded that the biggest outcome after introducing outsourcing were cost reduction(49.0%), followed by improved efficiency in operating the organization or human resources(34.7%) and the improved quality of the work(6.1%). The degree of satisfaction for outsourcing among the hospital managers(3.43) was significantly higher than that among the employees(3.l4) on a S-point Likert-type scale(p<0.05). Among the 7 work areas, the hospitals used outsourcing most frequently in facility management(housekeeping, building maintenance, hospital security and parking management), followed by non-medical profit business(funeral, convenient store, and cafeteria), logistics(provision of patient meal, in-house delivery, and purchasing), and information and computing system(hospital information system, maintenance of personal computers and printers). The work areas that the hospitals planned to adopt or expand the outsourcing in the future most frequently were facility management, non-medical profit business, logistics, and information and computing systems. In conclusion, outsourcing was highly diffused in large Korean hospitals, particularly in the work field of facility management and non-medical profit business. The satisfaction for outsourcing was not high yet in Korean hospitals.

종합병원의 비보험환자 처치행위 양상과 수가분석에 관한 연구 (An Analysis of Nursing Behavior and Unit of Treatment Cost of Non- Insurance Patients)

  • The medical care insurance system, being put into practice nearly for three years, seem to have brought about some considerable problems as serious for the government as to consider a revision of that system. As one of the most serious problems of present system, the treatment cost of insurance patients is so remarkably low in comparison with than of non-insurance cases that normal operation of hospitals is threatened and care services of low quality are induced. The researcher carried out this survey to analyze and bring to light several aspects of treatment cost of non-insurance patients as a material for a re-assessment of the cost of insurance cases which shows a a considerable difference in amount at the standpoint of hospitals with than of non-insurance cases and further, hoping the significant blind spot of present insurance system(that is, the absence of regulations' for cost assessment by patterns or types of health care treatment) will be mended in near future. The survey was carried out with the treatment invoice sheets of total 902 in-hospital Patients of a general hospital in Seoul during the period of the 2 nd quarter of the year(1979). Among total 902 patients, 694 cases were used for analysis, because those disease or syndromes shared by less than 10% of the patients were put aside before procession. The data were analyzed by kinds or types of diseases, demographic characteristics of patients, hospitalization patterns, types of nursing treatment, etc. The result of analysis was as follows 1. Among all the non-insurance cases, those who received one or more kinds of nursing treatment mounted up to 96. 7 %. The invoice issue frequency per person was 7.2 times, while that frequency per day for a person was 0.8, : the treatment cosr per person was ₩22,650 while its daily average was ₩2,430, due to the average 9.3 in-hospital days per person. 2. As to the nursing treatment types by the demographic characteristics of patients and hospitalization patterns. a. The unit cost female patients was generally more expensive them that of males, and independent nursing service was more given than other types of treatment. As to age, higher age groups received independent nursing service most, while the youngest group received instrumental and integrated nursing services. b. As to room grade, the unit cost of I.C.U. cases was the highest : and the cast of private room patients was higher than that of public room patients. By in-hospital days, the curve of function showed L. type : that is, the longer stay, the lower function. 3. State of treatment types by kinds of disease were ; a. Dependent nursing service showed comparatively high availability in surgical and neurologic disease and independent nursing service was most received by medical, obstetrical and urological patients, while instrumental and integrated services were most available for respiratory disease and obstetrical and neurologic diseases next. b. The invoice issue frequency per day for a patient was highest in obstetrical disease 3.8 times, and the unit cost(per one invoice sheet) was also highest in obstertrical disease(₩10,880) and next in neurologic cases(₩ 4,690 ). 4. As to the pertained departments. a. Cost amount per person was highest in department of Psychiatries daily cost was highest in obstetrical cases : while the invoice issue frequency was highest in obstetrics and next in pediatrics. b. In departments in need of surgical operation, dependent nursing care was highly availabl : while in internal medicine and obstetrics, independent service was higher. Psychiatrics showed the highest the of integrate nursing while pediatrics and obstetrics higher of instrumental services. The variation co-efficien of treatment cost came out to be relatively in high in special surgery, opthalmology and internal medicine. 5. State of treatment cost by types of nursing behavior was. a. The average frequency of invoice issue was 3.5 (times). Among the type four types of treatment, instrumetal service (4.3) and independent nursing behavior(3.9) showed higher frequency than average respectively. But as to unit cost (per invoice). dependent (₩5,200) and integrated (₩5,340) nursing care services were higher than average and considerably higher than the other two types. b. In repect patient distribution. independent nursing behavior(80.3% ) was the highest and depend ent nursing (31.7% ) the lowest. The variation co-efficient of treatment cost appeared highest in dependent nursing be havior as a whole, and among that, doctor's diagnosis showed the highest coefficient value (100.7). In conclusion, the variaty of treatment cost(treatment itself ) by various characteristics and treatment types pro- that treatment various sort of patients and treatment cost of various types of nursing behavior cannot be uniform. Therefore, to attain the equalization of health care service and its cost both for insurant and non-insurant patients, a more specific provision for assessment of cost should be added to the present medical care insurance system and, in addition, the cost of nursing treatment is desired to be inserted into the treatment invoice.

슬관절 전치환술 환자의 조기퇴원 후 가정간호 운동 프로그램이 슬관절기능 상태와 삶의 질에 미치는 효과 (The Effects of Home Care Exercise Program on Knee Joint Function and Quality of Life in Patients with Total Knee Replacement Arthroplasty)

  • Purpose: The aim of this study was to investigate the effects of home care exercise program on knee joint function(WOMAC) and the quality of life in patient with total knee replacement arthroplasty. Method: Forty-one subjects were sampled according to research criteria, and divided into two groups : 21 out of experimental group and 20 out of control group. The patients having treatment of total knee replacement arthroplasty were out of the hospital. After surgery executed, it took 8days for them to recovere their health conditions since 10th day, the experimental group received the home care exercise program per regularly four days interval. Results: 1) After applying home care exercise program, the total score on the WOMAC Index for the control group was $3.09{\pm}.76$. The score of the experimental group was $3.55{\pm}.55$ which is statistically higher than that of the control group. (P=.007). 2) After applying home care exercise program, the score of the quality of life for the control group was $3.09{\pm}.50$. The score of the experimental group was $3.46{\pm}.35$ Which is statistically higher than that of the control group. (P=.007). Conclusion: This figures show that home care exercise program has good results. These findings also indicate that the services of home care exercise program are alternatives for the hospitalization.

Quality of Life and Psychological Well-Being of Colorectal Cancer Survivors in Jordan

  • Background: Colorectal ranked first among cancers reported in males and ranked second amongst females in Jordan, accounting for 12.7% and 10.5% of cancers in males and females, respectively. Colorectal cancer patients can suffer several consequences after treatment that include pain and fatigue, constipation, stoma complications, sexual problems, appearance and body-image concerns as well as psychological dysfunction. There is no published quantitative data on the health-related quality of life and psychological wellbeing of Jordanian colorectal cancer survivors. Method: This project was a cross-sectional study of colorectal cancer survivors diagnosed in 2009 and 2010. Assessment was performed using the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30), the colorectal cancer specific module (EORTC QLQ-CR 29) and the Hospital Anxiety and Depression Scale (HADS). Data on potential predictors of scores were also collected. Results: A total of 241 subjects completed the study with mean age of $56.7{\pm}13.6$. Males represented 52.3% of study participants. A majority of participants reported good to high overall health; the mean Global health score was $79.74{\pm}23.31$ with only 6.64% of study participants scoring less than 33.3%. The striking result in this study was that none of the study participants participated in a psychosocial support group; only 4 of them (1.7%) were even offered such support. The mean scores for HADS, depression score, and anxiety score were $8.25{\pm}9$, $4.35{\pm}4.9$ and $3.9{\pm}4.6$, respectively. However, 77.1% of study participants were within the normal category for the depression score and 81.7% were within this category for anxiety score; 5.4% of participants had severe anxiety and 5.4% of them had severe depression. Discussion: Patients with colorectal cancer in Jordan have a good quality of life and psychological wellbeing scores when compared with patients from western countries. None of the colorectal cancer patients managed at the Ministry of Health received any formal counselling, or participated in psychological or social support programmes. This highlights the urgent need for a psychosocial support programme, psychological screening and consultations for patients diagnosed with colorectal cancer at the Ministry of Health Hospitals.

해양원격의료의 서비스 특성이 선원만족도 및 충성도에 미치는 영향 (The Effects of Service Characteristics of Maritime Telemedicine on Seafarer's Satisfaction and Loyalty)

  • 본 연구는 해양원격의료의 서비스 특성(서비스 질, 서비스의 인지 여부, 건강 상담 서비스의 경험 여부)이 선원만족도에 얼마나 영향을 미치는지 분석하고, 또한, 선원만족도가 충성도에 얼마나 영향을 미치는지를 분석하였다. 이를 위해, 해양원격의료 시범사업에서 수행했던 2017년도 서비스 대상자 설문조사에서 수집된 데이터를 활용하였다. 다만, 본 연구에서는 해양원격의료 중 화상원격(Tele-Monitering) 서비스를 받은 선원 103명을 연구의 대상으로 실증 분석을 하였다. 본 연구의 주요 결과는 다음과 같다.ⅰ) 서비스 질이 높을수록 만족도는 높았다, ⅱ) 만족도가 높을수록 충성도는 높았다, ⅲ) 30세 미만보다 50세 미만의 선원들이 만족도가 낮았다. ⅳ) 총 승선경력에서, 5년 이상에서 10년 미만인 선원이 5년 미만인 선원보다 만족도가 높았다, ⅴ) 10년 이상에서 15년 미만인 선원이 5년 미만인 선원보다 만족도가 높았다, ⅵ) 15년 이상에서 20년 미만인 선원이 5년 미만인 선원보다 만족도가 높았다, ⅶ) 또한, 20년 초과인 선원이 5년 미만인 선원보다 만족도가 높았다. 결론적으로,ⅰ) 해양원격의료 서비스 질이 높을수록 만족도는 높았고, 만족도가 높을수록 충성도는 높았다. 따라서 서비스 질의 향상은 만족도 및 충성도를 높이는데 중요한 요소일 뿐만 아니라, 해양원격의료에 대한 긍정적인 이미지 형성에 중요한 역할을 한다고 하겠다. ⅱ) 나이가 젊고, 대체적으로 총 승선경력이 낮은 선원이 만족도가 높았다. 이는 원격의료에 대한 이용 의사가 있고, 다른 사람에게 추천할 의도가 있는 선원은 젊은 층이라고 사료 된다.

폐경비만여성의 골밀도와 골대사 지표 및 식사의 질 상관성 조사 (Relationship between Bone Mineral Density and Bone Metabolic Biochemical Markers and Diet Quality Index-International(DQI-I) in Postmenopausal Obese Women)

  • Objectives: This study compared the differences of postmenopausal women's bone mineral density in relation to the degree of obesity, metabolism index and dietary factors that affect bone mineral density. Methods: The subjects included in the study are 39 postmenopausal women of normal weight with body mass index less than $25kg/m^2$ and 32 postmenopausal who are obese. Anthropometry and biochemical analysis were performed and nutrient intakes and DQI-I were assessed. Results: Normal weight women were $56.03{\pm}3.76years$ old and obese women were $58.09{\pm}5.13years$ old and there was no significant difference in age between the two groups. The T-score of bone mineral density was $0.03{\pm}1.06$ in normal weight women and $-0.60{\pm}1.47$ in obese women and this was significantly different between the two groups (p<0.05). Blood Leptin concentration was significantly lower in normal weight women ($6.09{\pm}3.37ng/mL$) compared to obese women in ($9.01{\pm}4.99ng/mL$) (p<0.05). The total score of diet quality index-international was $70.41{\pm}9.34$ in normal weight women and $64.93{\pm}7.82$ in obese women (p<0.05). T-score of bone mineral density showed negative correlations with percentage of body fat (r = -0.233, p=0.05), BMI (r = -0.197, p=0.017), triglyceride (r = -0.281, p=0.020) and leptin (r = -0.308, p=0.011). The results of multiple regression analysis performed as the method of entry showed that with 22.0% of explanation power, percentage of body fat (${\beta}=-0.048$, p<0.05), triglyceride (${\beta}=-0.005$, p<0.05) and HDL-cholesterol (${\beta}=0.034$, p<0.01), moderation of DQI-I (${\beta}=-0.231$, p<0.05) affected T-score significantly. Conclusions: The results of the study showed that obese women have less bone density than those with normal weight women. In addition, the factor analysis result that affect bone mineral density showed that intake of fat is a very important factor. Therefore, postmenopausal women need to maintain normal weight and manage blood lipid levels within normal range. They also need to take various sources of protein and reduce consumption of empty calorie foods that have high calories, fat, cholesterol and sodium.

치과의료서비스 향상을 위한 환자 기대도 조사연구 (A Study on the Patient's Expectation Toward Quality Care of Dental Services)

  • 2001년 4월 25일부터 6월 9일까지 서울시내 치과대학 병원 2곳과 서울시내 치과의원 2곳을 임의적으로 선정하여 그 곳에 방문한 환자에게 설문조사를 실시하였다. 설문내용은 치과 병 의원에 방문한 환자의 일반적인 특성, 치과 의료 환경에 대한 기대, 치과 인력에 대한 기대, 편의시설에 대한 기대, 치과정보에 대한 기대, 예약/정산에 대한 기대, 대기시간에 대한 기대로 구성되어 있다. 환자의 일반적 특성에 따라 치과의료서비스 기대를 분석한 결과는 다음과 같다. 1. 치과 의료서비스의 기대도 분석결과는 치과 병 의원의 환경에 대한 기대와 인력에 대한 기대가 가장 높게 나타났다. 2. 치과 의료서비스의 각 요인별 기대수준은 다음과 같다. 1) 환경에 대한 기대요인 중 '치과 의료기관의 편리한 교통'의 기대가 가장 높았다. 2) 인력에 대한 기대요인 중 '치과의사의 친절'의 기대가 높게 나타났다. 3) 편의시설에 대한 기대요인 중 '어린이만을 위한 시설'에서 기대가 가장 높게 나타났다. 4) 치과정보제공에 대한 기대요인 중 '치료방법 및 치료비 설명'에서 기대가 가장 높았다. 5) 예약/정산에 대한 기대요인 중 '예약/정산의 신속성'에서 기대가 가장 높았다. 6) 대기시간에 대한 기대요인 중 '대기시간을 알려주는 배려'에서 기대가 높게 나타났다. 3. 환자의 일반적 특성에 따른 총 치과 의료서비스 기대분석 결과로서는 여자가 남자보다, 연령이 낮아질수록, 그리고 교육수준이 높을수록 치과의료서비스의 기대가 높았다. 본 연구 결과를 통해 소득의 증대와 의료서비스 이용의 기회가 증가하면서 의료에 대한 개념이 바뀌고 고객들이 원하는 요구도 매우 다양해지고 복잡해지고 있음을 추측할 수 있었다. 이에 맞추어 치과 병 의원은 치료서비스 이외에 환자가 요구하는 여러 가지 부가적인 서비스를 포함하여 포괄적인 개념의 의료서비스를 제공하는 방법으로 접근을 시도해야 한다. 그리고 병원자체의 의료서비스 평가를 통해서 고객의 기대와 실제로 환자에게 제공된 의료서비스간의 갭을 파악하고 그 갭을 줄이기 위해 다양한 방향으로 노력하여 고객만족을 실현해야 할 것이다.

병원의 활동기준원가를 이용한 총체적 질관리 모형 및 질비용 산출 모형 개발 (Development of the Model for Total Quality Management and Cost of Quality using Activity Based Costing in the Hospital)

  • Healthcare service organizations can apply the cost of quality(COQ) model as a method to evaluate a service quality improvement project such as Total Quality Management (TQM). COQ model has been used to quantify and evaluate the efficiency and effectiveness of TQM project through estimation between cost and benefit in intervention for a quality Improvement to provide satisfied services for a customer, and to identify a non value added process. For estimating cost of quality, We used activities and activity costs based on Activity Based Costing(ABC) system. These procedures let the researchers know whether the process is value-added by each activity, and identify a process to require improvement in TQM project. Through the series of procedures, health care organizations are service organizations can identify a problem in their quality improvement programs, solve the problem, and improve their quality of care for their costumers with optimized cost. The study subject was a quality improvement program of the department of radiology department in a hospital with n bed sizes in Metropolitan Statistical Area (MSA). The principal source of data for developing the COQ model was total cases of retaking shots for diagnoses during five months period from December of the 1998 to April of the 1999 in the department. First of the procedures, for estimating activity based cost of the department of diagnostic radiology, the researchers analyzed total department health insurance claims to identify activities and activity costs using one year period health insurance claims from September of the 1998 to August of the 1999. COQ model in this study applied Simpson & Multher's COQ(SM's COQ) model, and SM's COQ model divided cost of quality into failure cost with external and internal failure cost, and evaluation/prevention cost. The researchers identified contents for cost of quality, defined activities and activity costs for each content with the SM's COQ model, and finally made the formula for estimating activity costs relating to implementing service quality improvement program. The results from the formula for estimating cost of quality were following: 1. The reasons for retaking shots were largely classified into technique, appliances, patients, quality management, non-appliances, doctors, and unclassified. These classifications by reasons were allocated into each office doing re-taking shots. Therefore, total retaking shots categorized by reasons and offices, the researchers identified internal and external failure costs based on these categories. 2. The researchers have developed cost of quality (COQ) model, identified activities by content for cost of quality, assessed activity driving factors and activity contribution rate, and calculated total cost by each content for cost for quality, except for activity cost. 3. According to estimation of cost of quality for retaking shots in department of diagnostic radiology, the failure cost was ₩35,880, evaluation/preventive cost was ₩72,521, two times as much as failure cost. The proportion between internal failure cost and external failure cost in failure cost is similar. The study cannot identify trends on input cost and quality improving in cost of qualify over the time, because the study employs cross-sectional design. Even with this limitation, results of this study are much meaningful. This study shows possibility to evaluate value on the process of TQM subjects using activities and activity costs by ABC system, and this study can objectively evaluate quality improvement program through quantitative comparing input costs with marginal benefits in quality improvement.
