간호직 공무원의 코로나19 스트레스, 충동성, 가족건강성이 자살생각에 미치는 영향요인 (Influencing Factors of COVID-19 Stress, Impulsiveness, and Family Strength on Suicidal Ideation of Public Nursing Officials in Community Health Center)

  • Purpose: This study was conducted to identify factors of COVID-19 stress, impulsiveness, and family strength on suicidal ideation of public nursing officials in community health centers. Methods: This study was a descriptive analysis study the subjects of this study were a total of 145 public nursing officials from community health centers in Busan. The data were analyzed using IBM SPSS/WIN 23.0 version. Result: The result of the predictor analysis showed that the motor impulsiveness (β=.383, p<.001), workload (β=.222, p=.003), cognitive impulsiveness (β=-.205, p=.012), bond (β=-.169, p=.033). The regression model showed an explanatory power of 24.1%. Conciusion: it is necessary to increase impulse and stress control ability, and to adjust the workload. In addition, it is necessary to systematically guarantee a working environment where practical vacation can be used, but it may not be a realistic alternative in a disaster crisis such as COVID-19, so indirect alternatives such as reinforcing infectious disease experts, improving the work environment, and psychological support to prevent suicide in advance are required.

보건소 건강증진사업 담당자의 직무교육에 대한 인식 및 교육요구도 (Analysis of Perceived Job Training and Educational Needs for the Public Health Promotion Program Personnel)

  • Objectives: This study is for understanding the perceived job training of the public health promotion program officials and analysis the educational needs in order to serve as the basis for the development of the future educational programs. Methods: To this end, we have developed a survey based on references and consultations with experts. We had explained the purpose and the intent of the survey to nationwide public health directors in advance and an online questionnaire was conducted for health promotion program and service providing personnel; the results from a total of 763 survey respondents were used in the final analysis. Collected data were analyzed through SPSS Win 21.0. Results: The results shows that the tasks of public health promotion personnel are 'business practices,' 'planning and evaluation related work' and so on in order of importance while the duties of service providing personnel are 'counseling,' 'education' and so on. One of the factors affecting field applications of education is 'awareness of the connection between education and career development' which accounts for 33% of explanatory power; the selected six other factors constitute 41.7%. Necessary educational contents for public officials are 'public health service planning,' 'report writing skills' and 'project evaluation methods.' On the other hand, for service providers, the contents are 'counseling methods,' 'development of educational materials,' 'monitoring health status' and so on. Conclusions: In order for trained service personnel to accord with the changed health promotion environment and the demand of local residents, Public Health Promotion officials should increase educational opportunities based on the competencies for each job, provide continuous learning opportunities and information even after the training, and finally, create a system that can link to career development.

일부 공무원들의 흡연유형에 관한 고찰 (A Review on Smoking Patterns among Public Officiers)

  • The purpose of this study is to identify the smoking patterns and their relevant factors of the public officials who are placed to guide people's way of thinking as conservative middle-income group and thereupon, provide for basic data which can be used to reduce the diseases and death rate caused by smoking in terms of preventive medicine. For this purpose, 170 public officials working for the government agencies located in seoul were surveyed for Jan. 10-Jan. 20, 1995. The results of survey can be analyzed/summarized as follows; 1. In terms of daily smoking frequencies, 50 officials $(29.4\%)$ are driven by stress to smoke. Among these offcials, 250nes $(50.0\%)$ smoke 20-30 times a day. Meanwhile, 22 officials $(12.9\%)$ answered that they smoke for pleasure and comfort, and among them 6 officials $(21.4\%)$ smoke 20-30 times a day. 26 officials $(15.3\%)$ are found to be nicotine­addicted physically or psychologically, and to 10 of them $(35.7\%)$ smoke 20-30 times a day. On the other hand, 10 officials $(5.9\%)$ have habit of smoking and 4 of them $(40.0\%)$ smoke 20-30 times a day. 2. Out of 50 officials $(24.9\%)$ who began smoking on impulse, 44 actually stopped smoking, and the rest 6 officials did not even implement their plan. In view of successful cigarette-stopping, only 8 officials $(15.4\%)$ out of 44 impulse-driven smokers succeeded in stopping cigarettes, while the rest 36 failed all. Meanwhile, out of those 84 officials $(64.6\%)$. Who smoked for reduced stress, 28 ones succeeded, while the rest 56 ones failed. Out of 20 officials who were nicotine-addicted but attempted to stop smoking, only 2 smokers succeeded and the rest 18 were all failures. 3. Factors involving smoking and Their patterns 1) By age group (30's, 40's and 50's), the smoking patterns can be grouped into stress-removing, nocotine-addictive and habitual ones, respectively. 2) By acadeic background, our officials smoke on impulse or due to nocotine addition. 3) The length of career is co-related with nicotine addition. 4) The ownership of house is related with smoking for comfort or less stress. 5) The number of children is related with impulse-driven smoking or smoking for comfort.

보건소 공무원의 코로나19 대응 경험 (Experiences of Public Officials for the COVID-19 Response in the Community Health Center)

  • Purpose: The purpose of this study was to examine the experiences of public officials working for the Coronavirus Disease-19 (COVID-19) response in community health centers in South Korea. Methods: A qualitative thematic analysis was conducted using data collected from three focus groups and two individual interviews. Results: The participants performed quarantine tasks in a poor working environment with several problems, such as significant workload, lack of manpower, and inappropriate compensation system. Participants experienced obstacles in performing quarantine works, which had the lack of the detailed quarantine guidelines, work support and cooperation system. Participants suffered from civil complaints. Participants endured sacrifices in their personal lives while partaking in COVID-19 response work without holidays, and subsequently experienced health problems. And also participants said that it was necessary to secure expertise and effective communication for infectious disease management. Conclusion: The study results suggest that policies are required that are aimed to improve the working environment and the recruitment of experts in infectious diseases. In addition, the job stress related to the COVID-19 response by public officials in community health centers must be evaluated, and the relation of their job stress to physical and mental problems, as well as psychosocial stress, must be examined.

보건소 공무원의 지역보건의료계획 수립 참여실태 및 태도 (Attitude and Participation Status on District Health Planning in Officials of Health Centers)

  • 광역시의 7개 구 보건소(180명), 광역도의 9개 시 보건소(262명), 광역도의 12개 군 보건소(232명) 등 28개 보건소의 공무원 674명을 대상으로 2002년 9월에 지역보건의료계획에 대한 인식과 태도를 조사함과 동시에 이들 보건소의 보건소장 또는 보건사업과장에게 지역보건의료계획 수립과정과 계획서에 따른 업무추진 내용, 제3기 지역보건의료계획서 작성에 관한 사항 등에 대해 설문조사 한 결과는 다음과 같다. 제2기 지역보건의료계획을 구체적으로 끝까지 읽어본 공무원은 13.6%에 불과하였고, 구체적으로 알고 있는 비율도 12.5%에 불과하였다. 대상공무원의 56.9%가 제2기 지역보건의료계획서를 가지고 있지 않았고, 2002년도 계획서를 소지하고 있지 않은 공무원은 63.5%였다. 응답자의 35.4%가 지역보건의료계획에 대해 지방자치단체장의 관심이 높다고 하였고, 22.4%는 지방의회의원의 관심이 높다고 하였다. 보건소장의 관심이 높다고 응답한 공무원은 77.8%였으며, 44.9%는 보건소내 타 공무원의 관심이 높다고 하였고, 43.9%가 본인의 관심이 높다고 하였다. 응답자의 58.6%가 지역보건의료계획이 필요하다고 하였고, 44.8%가 지역보건의료계획이 업무에 기여한다고 하였으며, 38.0%가 활용정도가 높다고 하였다. 응답자의 36.9%가 제2기 지역보건의료계획에 참여하였고, 49.6%가 제3기 지역보건의료계획에 참여하였다고 하였다. 15.7%가 제2기 지역보건의료계획 작성을 위한 교육에 참여하였고, 23.6%가 제3기 지역보건의료 계획 작성교육에 참여하였다고 하였다. 지역보건의료계획의 가장 큰 문제점으로는 인력 및 예산부족(39.8%), 직원의 인식부족(21.4%) 순이었다. 지역보건의료계획의 내용 중 가장 중요한 것을 보건소 업무의 추진현황과 추진계획이라고 하였다. 전체 보건소의 14.3%만이 제2기 지역보건의료계획에 따라 인력확충 계획을 실행하였다고 하였고, 28.6%가 조직개편계획을 실행하였다고 하였으며, 25.0%가 자체평가를 하였다고 하였다. 지역보건의료계획 목표설정시 내부의견 수렴을 제대로 거치지 않은 경우가 제2기 계획때는 42.9%였으나 제3기 때는 17.9%로 줄어들었다. 지역보건의료계획 시 가장 어려운 점은 '서식이 많고 복잡함', '직원의 전문지식 부족', '직원과 기관장의 관심부족' 순이었고, 개선방안으로는 '기획전담부서 확보', '계획수립시기 조정', '의회의결폐지', '직원교육확대', '서식 개선' 등이 제시되었다. 따라서 인력 및 예산확보, 기관장과 직원의 관심도 제고, 보건소 실정에 맞는 지침정비, 지역보건의료계획 수립제출시기 조정 등의 법규정비, 직원교육 등을 확대하여 지역실정에 맞는 체계적이고 효율적인 지역보건 의료계획을 수립하도록 하여야 할 것이다.

일 지역사회 일반직 공무원의 정신건강지식, 정신건강상태, 정신건강복지센터 및 정신건강사업에 대한 인식조사 (The Perception of Mental Health Literacy, Mental Health Status, Mental Health Welfare Center and Mental Health Business and of among Local Public Officials)

  • 본 연구는 일 지역사회 일반직 공무원의 정신건강지식, 정신건강상태, 정신건강복지센터와 정신건강사업에 대한 인식 정도를 알아보기 위한 서술적 조사연구이다. 조사대상은 G시 일반직 공무원 762명을 대상으로 구조화된 설문지로 2019년 10월 1일부터 2019년 10월 26일까지 자료를 수집하였으며, IBM SPSS 24.0 통계프로그램으로 분석하였다. 그 분석 결과, 대상자의 정신건강상태는 '심각한 스트레스' 28.1%로 가장 많았고, 정신건강지식의 평균 점수는 40.21점으로 보통보다 낮았으며, 정신건강상태의 평균 점수는 1.66점으로 매우 낮아 정신건강상태가 매우 좋은 것으로 나타났다. 정신건강복지센터의 인지 여부에서 44.6%가 '들어본 적이 없다'로 가장 많이 응답하였고, 정신건강사업의 경우 정신 건강 상담의 전화에 대해 '들어본 적이 없다'가 57.0%로 가장 많았으며, 정신건강 혹은 정신질환에 대한 정보를 얻는 경로로 'TV'가 65.2%로 가장 많았다. 그리고 정신건강 및 정신질환 관리를 위한 정부예산 증가는 85.0%가 '필요하다'고 응답하였다. 따라서 지역사회 정신건강복지센터의 전문인력을 활용하여 일반직 공무원의 특성에 맞는 맞춤형 정신건강 증진교육 프로그램 개발과 정신건강 향상을 위한 교육과 홍보가 지속적으로 더욱 강화되어야 할 것이다.

신종 인플루엔자 대유행의 확산과 영향 모델링 (Modelling the Impact of Pandemic Influenza)

  • The impact of the next influenza pandemic is difficult to predict. It is dependent on how virulent the virus is, how rapidly it spreads from population to population, and the effectiveness of prevention and response efforts. Despite the uncertainty about the magnitude of the next pandemic, estimates of the health and economic impact remain important to aid public health policy decisions and guide pandemic planning for health and emergency sectors. Planning ahead in preparation for an influenza pandemic, with its potentially very high morbidity and mortality rates, is essential for hospital administrators and public health officials. The estimat ion of pandemic impact is based on the previous pandemics- we had experienced at least 3 pandemics in 20th century. But the epidemiologi cal characteristics - ie, start season, the impact of 1st wave, pathogenicity and virulence of the viruses and the primary victims of population were quite different from one another. I reviewed methodology for estimation and modelling of pandemic impact and described some nations's results using them in their national preparedness plans. And then I showed the estimates of pandemic influenza impact in Korea with FluSurge and FluAid. And, I described the results of pandemic modelling with parameters of 1918 pandemic for the shake of education and training of the first-line responder health officials to the epidemics. In preparing influenza pandemics, the simulation and modelling are the keys to reduce the uncertainty of the future and to make proper policies to manage and control the pandemics.

한의약건강증진사업 현황 및 인식도 조사 : 보건소 건강증진사업담당자를 대상으로 (A National-wide Survey of Public Health Promotion Programs in Traditional Korean Medicine Targeted on Public Officials)

  • Objectives : This study aimed to examine the status of public health promotion programs in Traditional Korean Medicine (PHPP-TKM) from program managing public officials. Methods : The survey was a self-administered questionnaires based on internet service, survey monkey. The survey included the conducting status, obstacles, and improvement points of PHPP-TKM. Results : A total number of 179 officials participated the survey (response rate 71.0%), comprising 92.8% female. The survey demonstrated that 70.3% of participants conducted PHPP-TKM. The most frequent PHPP-TKM were stroke and bone/joint program. The reasons for not conducting PHPP-TKM were lack of financial support and program contents. The obstacles during conducting PHPP-TKM were insufficient contents and assessment index in PHPP-TKM. To develop necessary program for local residents and to secure financial support were required for managing future PHPP-TKM. Conclusions : The results suggests that various contents, validated assessment index, stable funds are to promote PHPP-TKM.

일 지역사회 공무원의 정신건강지식 및 정신건강사업에 대한 태도조사 (Mental health literacy of local public officials and the mental health business attitude survey)

  • 본 연구의 목적은 우리나라 지역사회 공무원의 정신건강지식 및 정신건강사업에 대한 인식을 알아보기 위해 시행된 서술적 조사연구이다. 조사대상자는 강원도 3개 군에 근무하는 공무원 273명을 대상으로 시행하였으며, 구조화된 설문지를 통해 2018년 7월 1일부터 8월 15일까지 자료를 수집하였다. 자료 분석은 SPSS 22.0 프로그램을 이용하였다. 연구결과 공무원의 정신건강지식은 101.45점(범위 35-160)이었다. 정신건강지식정도는 직업, 정신건강 교육 유무, 정신건강 치료경험 유무, 가족의 정신건강 치료경험 유무에서 통계적으로 유의한 차이가 있었다. 정신건강사업에 대한 인식으로 정신건강복지센터에 대해 알고 있는지에 대해 응답자의 39.9%가 이름정도만 알고 있다고 응답하였으며, 확대해야 할 정신건강서비스는 우울 등 조기발견 서비스가 35.9%로 가장 많았다. 정신건강사업으로 인한 지역사회의 변화에 대해 약간 좋아지고 있다는 응답자가 44.7%로 가장 많았다. 조사대상자들의 정신건강복지센터에 대한 인식과 정신건강 지식 점수와의 관계를 분석한 결과 정신건강지식점수에 따라 정신건강복지센터에 대한 사전지식 유무와 정신건강 관리를 위한 정부의 관리투자에 관한 문항은 통계적으로 유의한 것으로 나타났으므로(p=.012) 지역사회 공무원들의 정신건강지식을 향상시키기 위해 정신건강간호사를 활용한 교육 프로그램의 개발을 제안한다.

건강보험 빅데이터를 통한 전체 근로자 및 공무원 근로자의 암 발생률 분석 (Analyzing Cancer Incidence among Korean Workers and Public Officials Using Big Data from National Health Insurance Service)

  • Objectives: This study aimed to establish a control group based on the big data from National Health Insurance Service. We also presented presented the number of incidences for each cancer, and analyzed the cancer incidence rate among Korean workers. Methods: The cohort definition was separated by 'baseline cohort', 'dynamic cohort', and 'fixed- industry cohort' according to the definition. Cancer incidence was calculated based on the Korean Standard Classification of Disease code. Incidence rate was calculated among the group of all workers and public officials. Based on the study subjects and each cohort definition, the number of observations, incidences, and the incidence rate according to sex and age groups was calculated. The incidence rate was estimated based on the incidence per 100,000 person-year, and 95% confidence intervals calculated according to the Poisson distribution. Results: The result shows that the number of cancer cases in the all-worker group decreases after the age of 55, but the incidence rate tends to increase, which is attributed to the retirement of workers over 55 years old. Despite the specific characteristics of the workers, the trend and figures of cancer incidence revealed in this study are similar to those reported in previous studies of the overall South Korean population. When comparing the incidence rates of all workers and the control group of public officials, the incidence rate of public officials is generally observed to be higher in the age group under the age of 55. On the other hand, for workers aged 60 or older, the incidence rates were 1,065.4 per 100,000 person-year for all workers and 1,023.7 per 100,000 person-year for civil servants. Conclusions: This study analyzed through health insurance data including all workers in Korea, and analyzed the incidence of cancer of workers by sex and age. In addition, further in-depth researches are needed to determine the incidence of cancer by industry.
