The purpose of this study is to analyze the equity of health care utilization by income groups in terms of both quantity and quality of care, which is measured by expenditure, type of care, and type of health care institutions. Equity in health care utilization is measured by HIwv index, based on the survey of 1,480 Gwangju-Jeonnam residents. Health care utilization in terms of the probability and quantity of outpatient and inpatient care show equitable or pro-poor inequitable distribution, whereas the distribution of health care expenditure, which can account for the quality of care, is pro-rich inequitable, implying that the better off tend to use more expensive medical care. In terms of the types of care, simple visits for basic care show equitable distribution, whereas the distribution of the utilization of traditional tonic medicine, comprehensive health examination, CT, MRI, and ultrasound is pro-rich inequitable. Utilization of general hospitals and traditional health institutions show pro-rich inequitable distribution, hospitals and dental care institutions equitable, and physician clinics and public health centers pro-poor inequitable.
Purpose: This study was to describe and develop public health nursing. Methods: Data were collected from 19 nurses sampled from 5 public health centers. The grounded theory of Strauss and Corbin (1990) was applied. Results: As a result, 48 concepts, 23 subcategories, and 11 categories were deduced from open coding. In axial coding, casual conditions 'facing difficulties of practical administration' and 'attaching importance to health support for residents' and context condition 'assessing of residents' service needs' impacted phenomenon 'searching for efficient performing methods.' Intervening conditions 'forming of support system' and 'working with positive,' and action-interaction conditions 'improving of resident's health and medical accessibility' and 'striving for self-development' lead to consequences 'carried out promptly for field-based problem solving,' 'mastered of successful program planning methods' and 'solving the issues with existing style.' The periods of process were divided into 4 stages, confirming program-contents, probing program-method, developing program-strategies, and applying program-competencies. The core category, ‘strengthening of practical-planning work competencies' incorporated the relationship between and among all categories and explained the process. Conclusion: This study described public health nurses' performance in Korea. These findings have important implications for the practice and must be considered to develop competencies for planning and practice of public health.

Objectives: To evaluate the effect on education for child rearing using child-growth developmental screening program that hat had been developed by the maternal-child health services team in a public health center, Busan area, the rates of practicing proper child rearing between educated group and non-educated group were compared. Methods: Subjects were 596 mothers whose children were between 2 and 15 months old and who had visited two public health centers in Busan area for vaccination. Subjects checked the items that they are practicing for child rearing, the rates of items properly be practiced were compared between educated group and non-educated group by chi-square test. The effect of education in 2, 4, 6, 12 months old were observed in 4, 6, 12, 15 months old, respectively. Results: The percentages of the items that practice rate of proper child rearing was over 50% were not different by monthly age. In 4 months age, the proper practice rates of three items among eleven items were significantly higher in the educated group than in non-educated group. In 6 and 12 months age, those of two items and one item among fourteen items were higher respectively in the educated group than in non-educated group. In 15 months age, there was no item among thirteen that is different between two groups. Conclusions: As monthly ages are increasing, the practice rates of proper child rearing were decreased. In a few items, the practice rates of proper child rearing were higher in the educated group than in non-educated group. It suggests that the program that increase the effect on education of child rearing was developed and performed in public health centers.
Purpose: The purpose of this study was to estimate the demand and supply of visiting nursing services provided by health centers in urban area, aiming at strengthening infrastructure, which may improved the quality of life and health status of vulnerable population in the community. Methods: This study was conducted through nominal group discussion, focus group study. The demand and supply of visiting nursing were estimated by health economists based on the secondary analysis data from 25 health centers in Seoul. Result: Primary targets for the visiting nursing must be people who are homebound in the community. They can be classified into: a group of Level I: chronic patients who need visiting nursing care at least once a week: and a group of Level II: vulnerable families that need management periodically e. g. twice a month. Based on the estimation of demand for visiting nursing services in the community, the estimated supply required was $651{\sim}770$ visiting nurses including home health nurses in visiting nursing programs based on health centers in Seoul. Conclusions: The estimated demand and supply of visiting nursing are expected to provide basic data for establishing alternative policies on visiting nursing infrastructure that might be accomplished through demand-based visiting nursing programs by districts.

This study was conducted to investigate the effects of the public dentist's professional education for 81 dentists employed at the public health centers and dental clinics in the National hospitals. The purpose of this education was to improve public dentist's abilities to perform their duties. The 3days education was from 28 May until 30 May in 2008. The total 81 participants consisted of 8 medical position dentists, 20 employment position dentists, and 53 public health dentists. The survey provided appraisal of the education and suggestion of the development after this education. The obtained results were as followings : l. The public dental health project for the disabled people(91.4%) was a top priority. The second rank was the public dental health education project (87.6%) and the public dental health project for the aged people(86.4%) was followed. 2. The participants agreed that public dentist's professional educational programs were necessary(87.6%). 3. The participants did not have difficulties in attending this education(56.8%). 4. The participants who participated in 2008 public dentists' professional education were satisfied with this educational programs(81.4%). 5. Not only did the public dentists need the preventive dentistry programs and the treatment of the disabled people but also needed the health administrative programs and the health statistical programs in the future.
Although the public health centers have been initiating health education recently, it is not extending as expected and the participation of the program is low. The reason is that the office workers have few opportunities to receive health education since there are few public health centers in relation to the population. Much time is required for travel and attendance of the classes. In order to solve this problem, the aim is to increase participation in health education, improve the overall public health awareness, expand the number of health education locations to reduce medical expenses, vitalize the lifelong educational health programs, and improve the national health insurance. In order to research about the health education expansion plan for public health improvement, a study focused on men and women above age 20 who need health education. The research sample was selected through random sampling that targeted people who participated in the health programs or the health education. The period of this survey was from September 1st to September 30th, 2014. A total of 509 participants completed the survey data for the actual analysis to propose the health education expansion plan for the public health improvement.

Objectives: The Regional Cardiocerebrovascular Center (RCCVC) Project designated local teaching hospitals as RCCVCs, in order to improve patient outcomes of acute cardiocerebrovascular emergencies by founding a regional system that can adequately transfer and manage patients within 3 hours. We investigated the effects of RCCVC establishment on treatment volume and 30-day mortality. Methods: We constructed a panel dataset by extracting all acute myocardial infarction cases that occurred from 2007 to 2016 from the Health Insurance Review and Assessment Service claims data, a national and representative source. We then used a panel fixed-effect model to estimate the impacts of RCCVC establishment on patient outcomes. Results: We found that the number of cases of acute myocardial infarction that were treated increased chronologically, but when the time effect and other related covariates were controlled for, RCCVCs only significantly increased the number of treatment cases of female in large catchment areas. There was no statistically significant impact on 30-day mortality. Conclusions: The establishment of RCCVCs increased the number of treatment cases of female, without increasing the mortality rate. Therefore, the RCCVCs might have prevented potential untreated deaths by increasing the preparedness and capacity of hospitals to treat acute myocardial infarction patients.
Objectives: This study thus set out to examine the health literacy and self-care behaviors of community-dwelling elderly patients with hypertension and investigate their relations and influential factors, thus providing basic data to develop a nursing intervention program to promote self-care behaviors among hypertension patients. Methods: The subjects include 180 hypertension patients using community health centers, public senior centers, and welfare centers in Jeju. Results: The subjects scored a mean of 49.8 (${\pm}12.4$) points on health literacy and 55.8 (${\pm}6.7$) points on self-care behaviors. In the results of hierarchical regression analysis, health literacy independently predicted self-care behaviors (${\beta}=.12$, p=.050) after controlling for general characteristics, hypertension-related knowledge and self-efficacy. Conclusions: These results suggest that interventions for improving health literacy are important to enhance elder's ability of self-care behaviors. It is necessary to understand the health literacy of elderly patients with hypertension before providing them with educational intervention and information and to develop appropriate educational materials and intervention programs.
