치과용 지르코니아 착색 후 건조시간에 따른 반투명도의 차이 (Difference of Translucency according to Drying Time after Staining of Dental Zirconia)

  • 치과보철물의 심미적 결과를 조절하는 중요한 요인은 반투명도이며, 지르코니아 착색제의 건조 정도가 반투명도에 어떠한 영향을 미치는지 알아보고자 한다. 실험을 위해서 지르코니아 블록별로 원형 시편을 총 90개 제작하였다. 그 다음 지르코니아 시편을 착색제로 180초 침적 후 미건조(0초), 중간건조(30초), 완전건조(30분)의 건조시간(Dry time)을 부여한 후 지르코니아 소결로를 이용하여 원형 시편을 소결하였다. 분광광도계(Spectrophotometer CM-3600A, Konica Minolta, Japan)를 이용하여 흑색표준타일과 백색표준타일 위에 시편을 놓고, 표준광원 D65의 조건에서 정반사광 제거방식으로 지르코니아 원형 시편을 각각 측정하였다. 시편당 총 3번씩 반복측정하였다. 지르코니아의 건조시간에 따른 반투명도를 비교 분석하기 위하여 One-way ANOVA를 시행하였다. 지르코니아 종류별, 착색제에 따라 반투명도는 통계적으로 차이가 있었고(P < 0.001), 건조시간에 따라서는 통계적으로 차이는 없었다(P > 0.922). 그러나, 완전건조인 상태일 때, 반투명도 값이 작은 것으로 나타나 건조가 잘 될수록 지르코니아 색상이 투명하다는 것을 알 수 있었다.

DENTIS 내부연결형 서브머지드 임플란트에서 지대주 선택에 따른 성공률의 후향적 연구 (A retrospective randomized study of success rates according to abutment selection in DENTIS submerged implant with an internal hex connection)

  • PURPOSE. The purpose of this study is to determine the efficacy of the DENTIS submerged-type implant with an internal hex connection and to build corresponding abutment-selection criteria. MATERIAL AND METHODS. A total of 204 patients received submerged implant fixtures with an internal hex connection at the Dong-A University Hospital Dental clinic in Busan from January 2013 and May 2016. Three specific abutments, UCLA abutments, customized abutments, ready-made abutments, were randomly selected. Implant success was defined as the basis of the International Congress of Oral Implantologists(ICOI, 2007) criteria. The relationship between the implant success rate and the abutment factor was analyzed using the Kruskal-Wallis test(P<.05). RESULTS. A total of 508 implants were placed in 204 patients. After a mean observation period of 38.6 months, 493 out of 508 implants were in normal function, yielding an overall success rate of 97.05%. A total of 15 implants failed: 10 in the maxillary molar area, 4 in the mandibular molar area, and 1 in the mandibular incisal area. All of the implant failures occurred in a single-implant prosthesis, especially high in the maxillary molar area. The Kruskal-Wallis analysis showed that abutment selection has no significant correlation with implant failure(P>.05). CONCLUSION. DENTIS submerged implants with an internal hex connection showed predictable results with a success rate of 97.05%. It is no influence on the success rate in the selection of submerged implant abutment with an internal hex connection.

Assessment of effect of accelerated aging on interim fixed dental materials using digital technologies

  • PURPOSE. This study assessed the physical and mechanical properties of interim crown materials fabricated using various digital techniques after accelerated aging. MATERIALS AND METHODS. Three groups of interim dental restorative materials (N = 20) were tested. The first group (CO) was fabricated using a conventional manual method. The second group (ML) was prepared from prefabricated resin blocks for the milling method and cut into specimen sizes using a cutting disc. The third group (3D) was additively manufactured using a digital light-processing (DLP) 3D printer. Aging acceleration treatments using toothbrushing and thermocycling simulators were applied to half of the specimens corresponding to three years of usage in the oral environment (N = 10). Surface roughness (Ra), Vickers microhardness, 3-point bending, sorption, and solubility tests were performed. A 2-way analysis of variance (ANOVA) and Fisher's multiple comparison test were used to compare the results among the groups. RESULTS. The mean surface roughness (Ra) of the resin after accelerated aging was significantly higher in the CO and ML groups than that before aging, but not in the 3D group. All groups showed reduced hardness after accelerated aging. The flexural strength values were highest in the 3D group, followed by the ML and CO groups after accelerated aging. Accelerated aging significantly reduced water sorption in the ML group. CONCLUSION. According to the tested material and 3D printer type, both 3D-printed and milled interim restoration resins showed higher flexural strength and modulus, and lower surface roughness than those prepared by the conventional method after accelerated aging.

Evaluation of Color Stability according to Shade of Temporary Crown Resin Using Digital Spectrophotometer: In Vitro Study

  • Background: Temporary crown resins are used prior to prosthesis placement, indicating the importance of aesthetics. The aim of this study was evaluate the color stability of various staining solutions according to the color of temporary crown resins using VITA Easyshade V. Methods: The temporary crowns used were the powder-liquid type and included four shades. A total of 36 specimens were fabricated in the form of disks with a diameter of 1.8 mm and a depth of 2 mm. They were divided into four groups of nine each, and staining was performed for seven days by precipitation in 3 mL of three staining solutions composed of distilled water, black coffee, and red wine. Color and color stability evaluations were performed by a trained examiner using a digital spectrophotometer (VITA Easyshade V). Color stability was analyzed using the ΔE value. Results: Because of the color stability evaluation using the ΔE value, the difference between three and seven days was significant in the specimen I and III groups (p<0.05). Further, post hoc analysis showed that the ΔE value of red wine was significant, indicating that the color stability in red wine was low. The ΔE values in group II between days three and seven were statistically significant (p<0.05). Post hoc analysis showed that distilled water, coffee, and wine had the highest ΔE values on day three. On day seven, the ΔE value for wine was significant, and the color stability was low. There was no significant difference in group IV according to the staining period and staining solution; therefore, color stability was high (p>0.05). Conclusion: This study showed that most temporary resin restorations exhibited color stability in the staining solution. The darker the color of the temporary resin restoration, the higher the color stability against extrinsic staining.

Effect of connector configuration on the fracture load in conventional and translucent zirconia three-unit fixed dental prostheses

  • PURPOSE. The purpose of this study was to determine the effect of the connector configuration on the fracture load in conventional and translucent zirconia of three-unit fixed dental prostheses (FDPs). MATERIALS AND METHODS. Six different three-unit FDPs were prepared (n = 6) from three types of zirconia (3Y-TZP (Katana ML®), 4Y-TZP (Katana STML®), and 5Y-TZP (Katana UTML®)) in combination with two connector configurations (4 × 2.25, 3 × 3 mm). The CoCr master models were scanned, and the FDPs were designed and fabricated using CAD-CAM. The FDPs were cemented on the metal model and then loaded with a UTM at a crosshead speed of 1 mm/min until failure. Two-way ANOVA and Tukey's test were used for statistical analysis (α = .05). RESULTS. Fracture loads of 3Y-TZP (2740.6 ± 469.2 and 2718.7 ± 339.0 N for size 4 × 2.25 mm and 3 × 3 mm, respectively) were significantly higher than those of 4Y-TZP (1868.3 ± 281.6 and 1663.6 ± 372.7 N, respectively) and 5Y-TZP (1588.0 ± 255.0 and 1559.1 ± 110.0 N, respectively) (P < .05). No significant difference was found between fracture loads of 4Y-TZP and 5Y-TZP (P > .05). The connector configuration within 9 mm2 was found to have no effect on the fracture loads on all three types of zirconia (P > .05). CONCLUSION. Fracture loads of three-unit FDPs were affected by the type of zirconia. The fracture loads of conventional zirconia were higher than those of translucent zirconia. However, it was not affected by the connector configuration when the connector had a cross-sectional area of 9 mm2.

Tilted implants for implant-supported fixed hybrid prostheses: retrospective review

  • Objectives: This review assessed the performance of implant-supported fixed hybrid prostheses in 21 patients who received a total of 137 implants between 2003 and 2010. The implants were evaluated for marginal bone resorption, complications, success rate, and survival rate based on their vertical angularity, type of bone graft, and measured implant stability. Materials and Methods: One-way ANOVA and chi-square tests were used to analyze the relationships among long-term evaluation factors and these variables. The mean initial bone resorption in the implant group with a vertical angle of more than 20° was 0.33 mm and mean final bone resorption was 0.76 mm. In contrast, the mean initial bone resorption in the implant group with a vertical angle of less than 10° was 1.19 mm and mean final bone resorption was 2.17 mm. Results: The results showed that mean bone resorption decreased with an increase in the vertical placement angle of the implants used in fixed hybrid prostheses, as well as in the group without additional bone grafts and those with high implant stability. The success rate of implants placed after bone grafting was found to be higher than those placed simultaneously. Conclusion: These results suggest that implant-supported fixed hybrid prostheses may be an effective treatment option for edentulous patients, and intentionally placing implants with high angularity may improve outcomes.

사지구제술에서 언제 재활용 자가골 이식술이 유용한가? (When do we use the Recycling Autograft in Limb Salvage Surgery?)

  • 목적: 악성 근골격계 종양의 치료로 재활용 자가골 이식술을 이용하는 사지구제술에서 종양의 절제 및 재건 방법과 자가골의 재활용 처치 방법에 따라 그 유용성과 임상적 적용에 대하여 후향적 조사를 통하여 알아보고자 한다. 대상 및 방법: 본원에서는 1995년 12월에서 2006년 2월까지 재활용 자가골 이식술을 시행한 58례의 악성 근골격계 종양 환자들을 대상으로 하였다. 나이는 평균 36.5세(5~74세)였고, 성별은 남자가 34례, 여자가 24례였다. 58례 중 47례는 체외 방사선 조사(extracorporeal irradiation)를 시행하였고 11례는 저온 열처리(pasteurization)를 하였다. 재활용 자가골의 절제 및 재건 방법은 조각삽입(fragmentary) 3례, 분절삽입(intercalary) 8례, 골연골 삽입(osteoarticular) 18례, 자가골-종양대치물 복합체(recycling-autograft-prosthesis composite) 23례, 전 관절(total joint) 5례, 아킬레스건(achilles tendon) 1례였다. 결과는 절제 및 재건 방법과 자가골의 재활용 처치 방법에 따라 방사선학적 유합과 기능적인 평가(Musculoskeletal Tumor Society)에 따른 결과를 분석하였고 각각의 합병증에 대하여 조사하였다. 결과: 접합부의 유합은 체외 방사선 조사에서 15.0개월, 저온 열처리에서 12.6개월로 관찰되었다. 절제 방법에 따라 분절삽입 12.8개월, 조각삽입 6.0개월, 자가골-종양대치물 복합체 10.0개월, 골연골 삽입 23.3개월, 전 관절 15.6개월로 관찰되었다. 기능적 결과 점수는 체외 방사선 조사에서 59.6%, 저온 열처리에서 63.5%였고 절제 방법에 따라 분절삽입 60.8%, 조각삽입 65.5%, 자가골-종양대치물 복합체(골반제외) 62.8%, 골연골 삽입 66.0%, 전 관절 이식 66.6%였다. 저온 열처리에서 합병증은 2례(18.1%)로 감염 1례, 비구 돌출 1례가 관찰되었고 체외 방사선 조사에서 22례(46.8%)의 합병증(심부 감염 3례, 불유합 8례, 골절 2례, 성장판 문제 2례, 관절 불안정성 5례, 국소 재발 2례)이 발생하였다. 절제 및 재건 방법에 따라 분절삽입에서 불유합 3례(37.5%), 자가골-종양대치물에서 합병증 9례(50.0%, 불유합 4례, 심부감염 1례, 종양대치물 주위 골절 1례, 성장판 문제 1례, 국소재발 1례, 비구 돌출 1례), 골연골 삽입에서 합병증 6례(33.3%, 심부감염 2례, 불유합 2례, 성장판 문제 1례, 병적 골절 1례), 전 관절 이식에서 합병증 5례(100%, 관절 불안정성 5례), 아킬레스건에서 국소재발 1례(100%)가 발생하였다. 결론: 재활용 자가골 이식술의 유용성은 절제 및 재건 방법에 따라서 조각 및 분절 재건술이 사지의 기능이 우수하고 접합부의 빠른 유합을 보여 가장 좋은 적응증으로 사료되며, 자가골-종양 대치물 복합체의 재건술에서는 절제 후 남은 골이 부족할 때 종양 대치물의 안정성을 위해 고려해 볼 만하다. 자가골의 재활용 처치 방법에서는 저온 열처리법이 체외 방사선 조사법보다 더 우수한 것으로 사료된다.

Digital Shade Analysis System을 이용한 전치부의 색조 특성에 관한 연구 (SHADE ANALYSIS OF ANTERIOR TEETH USING DIGITAL SHADE ANALYSIS SYSTEM)

  • Statement of problem : A scientific examination and understanding of overall aspects of the natural dentition is the first step involved in making a satisfactory selection in the shade of an aesthetic prosthesis. Proper natural shade selection of the prosthetic restoration that is in harmony with the remaining dentition is as important aesthetically, as harmony of form and function in the anterior dentition. Clinically, the most commonly applied method of shade selection has been visual, but because of the subjective nature inherent to this method, shade selection results are variable and can be influenced by such factors as the technician, the type of shade guide used, and the type and intensity of the lighting. Purpose : The purpose of this study was to develop a more objective and scientific approach to examining and understanding the shade of teeth, which has in turn lead to the development of a number of shade analysis devices that present a more objective method of shade analysis. Material and Method : In this study, the shades of healthy anterior teeth were examined and analyzed using the recently developed digital shade analysis of the $ShadeScan^{TM}$ System. The study examined 80 individuals in their twenties, 40 males and 40 females, presenting 6 healthy, unrestored maxillary anterior teeth. Tooth brushing and oral prophylaxis were performed prior to evaluation. The ShadeScan handpiece was used to acquire images of the 6 maxillary anterior teeth. These images were analyzed using the Vita/Classical mode of the $ShadeScan^{TM}$ Software, and shade maps of each tooth were acquired and divided into cervical, middle, and incisal thirds. The shade distribution of each third, left and right symmetry, and gender differences were investigated and analyzed. Results : The results of the study are as follows : 1. An overwhelming majority of the examined teeth were found to possess shades belong to Group A, with the greatest variations occurring at the middle and cervical thirds of the maxillary central and lateral incisors, in both male and female subjects. 2. Canines of both male and female subjects showed left and right symmetry with uniform shade distribution of A4 and C4, while the lateral and central incisors showed left-right symmetry of the incisal 1/3 with a uniform shade distribution of A2 and A3 shades 3. No significant differences in shade distribution were seen between genders in maxillary canines, whereas maxillary central and lateral incisors showed differences at the middle and cervical thirds between male and female subjects The results of this study show that with the exception of maxillary canines, maxillary anterior teeth display a diverse shade distribution as well as gender differences. Conclusion : Clinically, when making a shade selection using the existing shade guide, one must consider the fact that even a single tooth consists of a variety of shades. The results of this study show that when selecting a shade from a number of groups is difficult, shades from A group are the most consistent with the natural shade or maxillary anterior teeth.

임플랜트 지지 고정성 국소의치의 임상적, 방사선학적 평가 (CLINICAL AND RADIOGRAPHICAL EVALUATION OF IMPLANT-SUPPORTED FIXED PARTIAL PROSTHESES)

  • Statement of problem: A conventional 3-unit fixed partial denture design with a pontic between two retainers is the most commonly used. However in cases where the mental nerve is in close proximity to the second premolar, a cantilever design can be considered. As such, logical and scientific evidence is lacking for the number and position of implants to be placed for partially edentulous patients, and no clear-cut set of treatment principles currently exist. Purpose : The purpose of this study was to evaluate prognosis of implant-supported fixed partial dentures and to compare changes in bone level which may rise due to the different factors. Material and method : The present study examined radiographical marginal bone loss in patients treated with implant-supported fixed partial dentures (87 prostheses supported by 227 implants) and evaluated the influence of the span of the pontic, type of the opposing dentition. Clinical complications were studied using a retrospective method. Within the limitation of this study. the following result were drawn Result, 1. Seven of a total of 227 implants restored with fixed prostheses failed, resulting in a 96.9% success rate. 2. Complications encountered during recall appointments included dissolution of temporary luting agent (17 cases), porcelain fracture (8 cases), loosened screws (5 cases), gingival recession (4 cases), and gingival enlargement (1 case). 3. Marginal bone loss, 1 year after prosthesis placement, was significant(P<0.05) in the group that underwent bone grafting, however no difference in annual resorption rate was observed afterwards. 4. Marginal bono loss, 1 year post-placement, was greater in cantilever-type prostheses than in centric pontic protheses (P<0.05). 5. Marginal bone loss was more pronounced in posterior regions compared to anterior regions (P<0.05). 6. The degree of marginal bone loss was proportional to the length of the pontic (P<0.05). Conclusion: The success rate of implant-supported fixed partial dentures, including marginal bone loss, was satisfactory in the present study. Factors influencing marginal bone loss included whether bone graft was performed, location of the pontic (s), location of the surgical area in the arch pontic span. Long-term evaluation is necessary for implant-supported fixed partial dentures, as are further studies on the relationship between functional load and the number of implants to be placed.

APPLICATION OF FINITE ELEMENT ANALYSIS TO EVALUATE IMPLANT FRACTURES

  • Statement of problem. Higher fracture rates were reported for Branemark implants placed in the maxilla and for 3.75 mm diameter implants installed in the posterior region. Purpose. The purpose of this study was to investigate the fracture of a fixture by finite element analysis and to compare different diameter of fixtures according to the level of alveolar bone resorption. Material and Methods. The single implant and prosthesis was modeled in accordance with the geometric designs for the 3i implant systems. Models were processed by the software programs HyperMesh and ANSA. Three-dimensional finite element models were developed for; (1) a regular titanium implant 3.75 mm in diameter and 13 mm in length (2) a regular titanium implant 4.0 mm in diameter and 13 mm in length (3) a wide titanium implant 5.0 mm in diameter and 13 mm in length each with a cementation type abutment and titanium alloy screw. The abutment screws were subjected to a tightening torque of 30 Ncm. The amount of preload was hypothesized as 650 N, and round and flat type prostheses were 12 mm in diameter, 9 mm in height were loaded to 600 N. Four loading offset points (0, 2, 4, and 6 mm from the center of the implants) were evaluated. To evaluate fixture fracture by alveolar bone resorption, we investigated the stress distribution of the fixtures according to different alveola. bone loss levels (0, 1.5, 3.5, and 5.0 mm of alveolar bone loss). Using these 12 models (four degrees of bone loss and three implant diameters), the effects of load-ing offset, the effect of alveolar bone resorption and the size of fixtures were evaluated. The PAM-CRASH 2G simulation software was used for analysis of stress. The PAM-VIEW and HyperView programs were used for post processing. Results. The results from our experiment are as follows: 1. Preload maintains implant-abutment joint stability within a limited offset point against occlusal force. 2. Von Mises stress of the implant, abutment screw, abutment, and bone was decreased with in-creasing of the implant diameter. 3. With severe advancing of alveolar bone resorption, fracture of the 3.75 and the 4.0 mm diameter implant was possible. 4. With increasing of bending stress by loading offset, fracture of the abutment screw was possible.
