  • In the present study, a numerical formulation procedure fer free vibration analysis of thin-walled horizontally curved multi-girder bridges is presented. The presented finite element procedure consists of curved and straight beam elements including warping degree of freedom. The homogeneous solutions of curved beam equations were used for shape functions in numerical formulation to achieve good convergence. In the straight beam element, the third order hermite polynomials were used fer shape functions. The Gupta method was used to solve the eigenvalue problem efficiently. The developed numerical procedure was applied to investigate the characteristics of free vibration of horizontally curved multi-girder bridges with varing subtended angle.

Primary angle-closure glaucoma, a rare but severe complication after blepharoplasty: Case report and review of the literature

  • Blepharoplasty is one of the most commonly performed aesthetic procedures. Surgical complications are rare, but can have severe consequences, such as permanent vision loss. In this report, we describe a patient who developed primary angle-closure glaucoma (ACG) with associated vision loss after a oculoplastic procedure using local anesthesia. So far, six similar cases have been described in the literature. It is believed that acute ACG is triggered by the surgical procedure in patients with predisposing risk factors such as a cataract. Surgical triggering factors include the use of buffered lidocaine/xylocaine with adrenaline/epinephrine, stress, and coverage of the eyes postoperatively. Due to postoperative analgesic use, the clinical presentation can be mild and atypical, leading to a significant diagnostic delay. Acute ACG should therefore be excluded in each patient with postoperative complaints by assessing pupillary reactions. If a fixed mid-wide pupil is observed in an ophthalmologic examination, an immediate ophthalmology referral is warranted. Surgeons should be aware of this rare complication in order to offer treatment at an early stage and to minimize the chance of irreversible vision loss.

Optimization of ride comfort for a three-axle vehicle equipped with interconnected hydro-pneumatic suspension system

  • The aim of this study is the optimization of the parameters of interconnected Hydro-Pneumatic (HP) suspension system of a three-axle vehicle for ride comfort and handling. For HP suspension systems of equivalent vertical stiffness and damping characteristics, interconnected HP suspension systems increase roll and pitch stiffness and damping characteristics of the vehicle as compared to unconnected HP suspension systems. Thus, they result in improved handling and braking/acceleration performances of the vehicle. However, increased roll and pitch stiffness and damping characteristics also increase roll and pitch accelerations, which in turn result in degraded ride comfort performance. Therefore, in order to improve both ride comfort and vehicle handling performances simultaneously, an optimum parameter set of an interconnected HP suspension system is obtained through an optimization procedure. The objective function is formed as the sum of the weighted vertical accelerations according to ISO 2631. The roll angle, one of the important measures of vehicle handling and driving safety, is imposed as a constraint in the optimization study. Upper and lower parameter bounds are used in the optimization in order to get a physically realizable parameter set. Optimization procedure is implemented for a three-axle vehicle with unconnected and interconnected suspension systems separately. Optimization results show that interconnected HP suspension system results in improvements in both ride comfort and vehicle handling performance, as compared to the unconnected suspension system. As a result, interconnected HP suspension systems present a solution to the conflict between ride comfort and vehicle handling which is present in unconnected suspension systems.

Cosmetic Lateral Canthoplasty: Lateral Canthoplasty to Lengthen the Lateral Canthal Angle and Correct the Outer Tail of the Eye

  • There are many women who want larger and brighter eyes that will give a favorable impression. Surgical methods that make the eye larger and brighter include double eyelidplasty, epicanthoplasty, as well as lateral canthoplasty. Double eyelidplasty produces changes in the vertical dimension of the eyes, whereas epicanthoplasty and lateral canthoplasty create changes in the horizontal dimension of the eyes. Epicanthoplasty, a surgical procedure which enlarges the eye horizontally, is performed at the inner corner of the eye, whereas lateral canthoplasty enlarges the outer edge of the eye. In particular, if the slant of the palpebral fissure is raised and the horizontal dimension of the palpebral fissure is short, adjusting the slant of the palpebral fissure through lateral canthoplasty can achieve an enlargement of eye width and smoother features. Depending on the patient's condition, even better results can be achieved if this procedure is performed in conjunction with other procedures, such as double eyelid-plasty, epicanthoplasty, eye roll formation surgery, fat graft, and facial bone contouring surgery. In this paper, the authors will introduce in detail their surgical method for a cosmetic lateral canthoplasty that lengthens the lateral canthal angle and corrects the outer tail of the eyes, in order to ease the unfavorable impression.

  • This paper presents a method for estimating the useful output of solar D.H.W. system. Heating load calculations, climate data and various conditions are used in this procedure to assess the fraction of the monthly solar energy and the actual solar energy supplied by solar energy for particular system. The design procedure presented in this paper referred to the f-Chart Method. The results of analyzing of this study by Fortran programming are as follows ; 1 . The amount of actual solar energy required to the hot water system is slowly rised to the ascend of tilt angle within the range of $45^{\circ}$, with is decreased since $45^{\circ}$. 2. The fraction of solar energy is superior when collector area is $8.64m^2$. 3. At the tilt angle with the range of $37.6^{\circ}\~45^{\circ}$, the amount of actual solar energy established the best results. 4 Both the fraction of solar energy and the actual solar energy are the most suitable during the storage volume is $300{\iota}$.

Distal Soft-Tissue Procedure with or without Proximal Metatarsal Osteotomy for Mild to Moderate Hallux Valgus (중등도 이하의 변형을 보이는 무지외반증에서 연부조직 교정술과 근위부 절골술의 비교)

  • Multiple surgical procedures for the correction of hallux valgus have been described. The surgical algorithm is based on the degree of deformity, patient's age, and the presence or absence of arthritis. It is known that the role of an isolated distal soft-tissue procedure is limited and the procedure must be used in conjunction with the proximal metatarsal osteotomy to correct most hallux valgus. We reviewed retrospectively the clinical & radiological results of distal soft-tissue procedure with or without proximal metatarsal osteotomy, performed in 18 patients (29 cases ) who had mild to moderate degrees af hallux valgus deformity. The results were as follows : In 20 cases with isolated soft-tissue procedures, the preoperative hallux valgus angle (HV) averaged 30.6 degrees and the postoperative HV averaged 7.3 degrees. The preoperative intermetatarsal angle (IM) averaged 12.5 degrees and postoperative IM averaged 10.4 degrees. 2. In 9 cases with the proximal metatarsal osteotomy added, the preoperative HV averaged 32.5 degrees, and the postoperative HV averaged 8.0 degrees. The preoperative IM averaged 12.5 degrees, and postoperative IM averaged 10.1 degrees. 3. There was no significant difference of loss of correction, presence of complication, patients own satisfaction, between two groups. In conclusion, as for the patients with mild to moderate degrees of hallux valgus deformities, the clinical and radiological results following distal soft tissue procedure and proximal metatarsal osteotamy are not proved to be superior to those following isolated soft-tissue procedures.

Distal Soft Tissue Procedure in Hallux Valgus Deformity: Comparison of Modified Mcbride Procedure and Trans-Articular Approach (무지외반증에서의 원위 연부 조직 유리술: 변형된 맥브라이드 술식과 경관절 접근법의 비교)

  • Purpose: "Hallux valgus" is a common disease encountered in clinical practice and is accompanied by foot deformities. Conservative treatment is commonly used in the early stages of hallux valgus. On the other hand, surgical treatment often becomes necessary as the deformity progresses. Surgical treatments involve various osteotomy methods or joint fusion procedures combined with soft tissue release, and outcomes from these surgical treatments are generally favorable. This study compared two soft tissue release techniques in the hallux region. Materials and Methods: This study conducted a retrospective cohort study on 48 participants who underwent surgical treatment for hallux valgus at a single institution from March 1, 2018, to March 31, 2023. A scarf osteotomy was performed in all cases, and the "Modified Mcbride procedure" or "Trans-articular approach" was done for soft tissue release. Hallux valgus angle (HVA), intermetatarsal angle (IMA), and the degree of subluxation of the lateral sesamoid were measured through simple foot radiographs taken before surgery and one year after surgery. Results: In the Modified Mcbride procedure group, HVA, IMA, and the sesamoid position grade decreased from 34.94° to 9.98°, 15.64° to 5.44°, and 2.47 to 0.44, respectively. In the trans-articular approach group, HVA, IMA, and the sesamoid position grade decreased from 33.42° to 7.34°, 15.06° to 6.03°, and 2.17 to 0.58, respectively. There was no significant difference in these changes between the preoperative and one-year postoperative measurements for both techniques (p-value>0.05). Conclusion: A radiological assessment of soft tissue release through the Modified Mcbride procedure and trans-articular approach in hallux valgus did not show significant differences. Therefore, both surgical techniques can be considered in the distal soft tissue release for a hallux valgus correction.

  • The presence of an unbalanced mass is originated the common source of vibration in machines with rotating rotor. In this study, the unbalanced rigid rotor mounted on an overhang shaft is balanced using the static balancing procedure, and the compensation mass is estimated by the phase angle method and four run method. Also, the reduction of vibration level before and after balancing is examined. In the experimental results, it is shown that the vibration due to the unbalanced mass is decreased by eliminating the effect of the unbalance. Above all, the four run method is proved more effective on the ability of vibration reduction, in small unbalanced mass, the phase angle method.

Sublabial Autologous Ear Cartilage Grafting for Increasing the Nasolabial Angle

  • Background The loss of nasal tip support is caused by many factors and eventually results in the collapse and eventual dropping of the nasal tip. This reduces the nasolabial (NL) angle and negatively affects respiratory functions and one's appearance. Methods The aim of this retrospective study, which was conducted on 52 patients, was to present and popularize a simple and effective method for the reconstruction of a weakened columella by inserting an autologous ear cartilage graft using a sublabial approach. Results Of all the patients, three patients experienced transplant rejection. The period of follow-up observation was one to five years (mean, 27 months). The results were objectively evaluated by measuring the NL angle in standardized photos before and after the procedure at different time intervals over the follow-up period. We observed a significant increase of the NL angle (mean, $20^{\circ}$), and found these results to be durable over the long term. Of the 52 patients included in this study observed patients, three were dissatisfied (due to immediate infection and shifting of the strut), 28 were satisfied, and 21 were very satisfied. Conclusions The surgical method described here is simple and can be learned quickly. It has very good results with few complications, and is our method of choice for complex and serious cases seen in everyday rhinosurgical practice.

New mathematical approach to calculate the geometrical efficiency using different radioactive sources with gamma-ray cylindrical shape detectors

  • The geometrical efficiency of a source-to-detector configuration is considered to be necessary in the calculation of the full energy peak efficiency, especially for NaI(Tl) and HPGe gamma-ray spectroscopy detectors. The geometrical efficiency depends on the solid angle subtended by the radioactive sources and the detector surfaces. The present work is basically concerned to establish a new mathematical approach for calculating the solid angle and geometrical efficiency, based on conversion of the geometrical solid angle of a non-axial radioactive point source with respect to a circular surface of the detector to a new equivalent geometry. The equivalent geometry consists of an axial radioactive point source with respect to an arbitrary elliptical surface that lies between the radioactive point source and the circular surface of the detector. This expression was extended to include coaxial radioactive circular disk source. The results were compared with a number of published data to explain how significant this work is in the efficiency calibration procedure for the γ-ray detection systems, especially in case of using isotropic radiating γ-ray sources in the form of point and disk shapes.
