Pressure Sore and Necrosis over the lateral malleolus of the Ankle (족근 관절 외과 부위의 압박궤양과 괴사)

  • Pressure sores are an ulceration and necrosis of the skin and underlying tissue usually occur after prolonged or repeated pressure by interruption of blood flow from the small. vessels to the skin and deep tissue. The management of pressure sores is mostly difficult and requires prolonged hospitalization or repeated surgical interventions with a high recurrence rate. In this article we reviewed 14 cases of National Pressure Ulcer Advisory Pannel staging III, IV over the lateral malleolar area of the ankle in 2 years period from January 1999 to October 2001. The pressure over lateral malleolar area was mainly due to unique Korean sitting position with cross legs at flexed hips and knees or supine position of patient with external rotation of low extremity. Male to female ratio was 11: 3 and ages were between 36 and 83 (mean age: 67.1 years). Associated diseases were DM(7 cases), Hemiparesis caused by CVA(2 cases), Liver cirrhosis(2 cases), disarticulation of opposite hip due to squamous cell ca.(1 case), Intertrochanteric Fx.(1 case). Wound cultures reported Staphylococus, Pseudomonas and others. Abnormally elevated ESR and CRP were seen in 6 cases. Operative treatments were irrigation and debridement, direct closure with gravity drainage and skin grafting. The most important aspect of pressure sore treatment is pressure relief of the lateral malleolar area. Pressure-relieving Cast or Brace was helpful for local management and preventing recurrence.

  • This study was analyzed on relationship of blood vessel pressure massage and skin and management method on blood vessel pressure for skin care with Yin-yang 5 factors theory, Yin-yang 5 factors theory of blood vessel pressure massage with related Korean medicine is essential for descriptions of the physiology of human body and disease. Six elements(wind, heat, fire, dryness, wetness, and cold) and seven emotions(anger, happiness, thought, worry, sadness, surprise, and fear) effected on skin care and the five viscera and the six stomach. Blood vessel pressure massage related with skin consists of the five viscera and the six stomach and is improved blood circulation and is retarded aging of skin by controls of hormone and free nerve system. Blood vessel pressure massage for skin care improved in the intestine system and blood circulation and got healthy. The blood vessel pressure massage treatment of beauty art can aid the function of bio-rhythm of a human body and make our body health by healing the problems of the five viscera organs and the six stomach. It also help circulate of the blood flow and vigor. The study expects the related researches to improve the various treatments through this treatment. The researcher encountered many problems with the lack of concerned materials and former studies but expects this study to be a study to retard aging the skin and prevent the diseases through the study of the blood vessel pressure massage.

Validation of the mobile wireless digital automatic blood pressure monitor using the cuff pressure oscillometric method, for clinical use and self-management, according to international protocols

  • The purpose of this study was to evaluate the accuracy of a mobile wireless digital automatic blood pressure monitor for clinical use and mobile health (mHealth). In this study, a manual sphygmomanometer and a digital blood pressure monitor were tested in 100 participants in a repetitive and sequential manner to measure blood pressure. The guidelines for measurement used the Korea Food & Drug Administration protocol, which reflects international standards, such as the American National Standard Institution/Association for the Advancement of Medical Instrumentation SP 10: 1992 and the British Hypertension Society protocol. Measurements were generally consistent across observers according to the measured mean ${\pm}SD$, which ranged in $0.1{\pm}2.6mmHg$ for systolic blood pressure (SBP) and $0.5{\pm}2.2mmHg$ for diastolic blood pressure (DBP). For the device and the observer, the difference in average blood pressure (mean${\pm}$SD) was $2.3{\pm}4.7mmHg$ for SBP and $2.0{\pm}4.2mmHg$ for DBP. The SBP and DBP measured in this study showed accurate measurements that satisfied all criteria, including an average difference that did not exceed 5 mmHg and a standard deviation that did not exceed 8 mmHg. The mobile wireless digital blood pressure monitor has the potential for clinical use and managing one's own health.

Pattern Analysis in Patients with Hypertension grades (고혈압 단계별 변증특성 분석)

  • To investigate and characterize basic patterns of prehypertension. Participants were divided into three groups; ideal blood pressure(n=40), prehypertension(n=62), and stage 1 hypertension groups(n=68) according to the JNC 7 standards. They answered questionnaire asking various symptoms and received clinical examination. Data were collected and analyzed in the focus of three Zheng patterns, cold-heat, deficiency-excess and four major hypertension types. Analysis of variance was used to find differences among groups. In addition, relationship between the cold-heat trends and risk factors of hypertension were analyzed using Pearson's correlation analysis. Three risk factors; age, body mass index (BMI), and fasting blood sugar (FBS) showed intergroup differences. Statistical significances were revealed in the cold-heat pattern and two hypertension subtypes. The cold scores decreased from $4.8{\pm}1.84$ to $3.9{\pm}1.88$ and $3.7{\pm}1.27$ (p=0.022), while the heat increased from $1.9{\pm}1.32$ to $2.8{\pm}1.72$ and $2.8{\pm}1.48$ (p=0.009). Additionally two hypertension subtypes, the excessive liver fire, and the yin-yang deficiency showed significant differences. Cold had negative correlations with blood pressure (both systolic and diastolic), BMI, triglyceride, and FBS. Heat had positive correlations with systolic blood pressure, BMI, triglyceride, and FBS. Prehypertension could be characterized by using the cold-heat patterns. The cold-heat are correlated with Blood pressure, BMI, blood lipids and sugar.

A Study on Effectiveness of Chronic Disease Online Program Due to COVID-19 (COVID-19로 인한 만성질환 관리 비대면 프로그램의 효과성 연구)

  • The purpose of this study is to apply a chronic disease management non-face-to-face program for the prevention and management of chronic disease and to confirm the effect of a one-group pre-and post-design similar experimental study. The program consisted of nursing, nutrition, and exercise programs for 12 weeks. The subjects were 69 adult men and women using the chronic disease management center at public health centers, and the period was from May 3 to August 6, 2021. Systolic pressure, diastolic pressure, fasting blood sugar, BMI, depression, and quality of life were measured before and after the program. Data were analyzed with SPSS 20.0. As a result, systolic pressure, diastolic pressure, fasting blood glucose, and quality of life were found to be significant. Depression was not significant, but mild depression was reduced. Developing, applying, and managing various non-face-to-face programs for chronic disease management will contribute to the prevention of chronic diseases and the promotion of health maintenance.

  • Diesel engines have the advantages of good fuel efficiency and low emissions. Therefore, car makers have been developed various kinds of diesel engine management system to clean up emissions while improving fuel efficiency. One of them is the common rail system. In the common rail system, diesel fuel is injected into the combustion chamber at ultra high pressures up to 1,800 bar to ensure more complete combustion for cleaner exhaust gas, and highly precise multiple injection reduces NOx emission, combustion noise and vibration. Generally speaking, common rail system consists of booster pump, high pressure pump, common rail, injectors, control valves, and sensors. The high pressure pump receives low pressure fuel from the booster pump and supply high pressure fuel to injectors through the high pressure common injection rail. Therefore, high pressure pump has an important role in common rail system. In this paper, we have investigated the performance of high pressure pump of common rail system.

High Hydrostatic Pressure Extract of Red Ginseng Attenuates Inflammation in Rats with High-fat Diet Induced Obesity

  • Chronic low-grade inflammation is associated with obesity. This study investigated effect of high hydrostatic pressure extract of red ginseng (HRG) on inflammation in rats with high-fat (HF) diet induced obesity. Male, Sprague-Dawley rats (80~110 g) were randomly divided into two groups, and fed a 45% HF diet (HF) and a 45% HF diet containing 1.5% HRG (HF+HRG) for 14 weeks. At the end of the experiment, the serum leptin level was reduced by the HRG supplementation. The mRNA expression of genes related to adipogenesis including peroxisome proliferator-activated receptor-gamma and adipocyte protein 2 was down-regulated in the white adipose tissue (WAT). The mRNA levels of major inflammatory cytokines such as tumor necrosis factor-${\alpha}$, monocyte chemoattractant protein 1, and interleukin-6 were remarkably down-regulated by the HRG in WAT. These results suggest that HRG might be beneficial in ameliorating the inflammation-associated health complications by suppressing adipogenic and pro-inflammatory gene expression.

Factors Related to the Outpatient Visits for Blood Pressure Management in Patients diagnosed with Hypertension (고혈압 진단자의 혈압 관리를 위한 외래 방문 영향요인)

  • Background: Regular doctor visits are vital for hypertension patients, especially for who have never received hypertension medication or non-pharmacologic therapy. This study purposed to study factors affecting outpatient visits for patients diagnosed with hypertension. Methods: This study included 59,009 respondents with hypertension over 30 from 2019 Community Health Survey data. Outpatient visits were defined by having hypertension treatments such as medication or non-pharmacologic therapy. Logistic regression was used to examine the factors affecting outpatient visits using SAS ver. 9.3. Results: 57,081(96.73%) patients with hypertension were identified as those having a outpatient visit for hypertension treatments, whereas 1,928(3.27%) patients did not have visits. Patient's characteristics such as gender, age, periods of hypertension, education level, perception of the blood pressure, hypertension management education, place of living, body mass index, depression and diabetes were found to have statistically significant relationship with the outpatient visits. Practical Implications: There is a need to select patients with high blood pressure who are unlikely to visit for hypertension treatments based on the study results. For those, establishing a personalized management plan such as health education and counseling programs will be helpful for the successful implementation of national chronic disease management program.

A Study on the Relationship between Health Behavior Factors and Blood Pressure of Workers (혈압 건강군과 비건강군 근로자의 건강실천 관련 요인 조사연구)

  • This study was examined 718 workers who had consistent blood pressure results in 2001 and 2002 general health examinations that were held at a work places managed by a health care agency in Seoul. Significant results are found as follows by analysing SPSS 11.0 on the result of self-recorded questionnaires investigated from Mar 1, 2003 to April 30, 2003. 1. A sampled healthy group and a sampled unhealthy group had significant differences in four variables out of possible nineteens that are sex, age, marriage and occupation. The unhealthy group had more males than females, more aged (over 50 years old) than youngers (under 50 years old), more married than singles, more manufacturing workers than non-manufacturing workers. In the case of systolic blood pressure, as the healthy group had 16.52mmHg while that of the other group had 149. 58mmHg, 33.06mmHg of difference between those groups were detected. In the case of diastolic blood pressure, 74.93mmHg of the healthy group and 96.53mmHg of the unhealthy group yielded 21.60mmHg of difference between them. This result implies that a guidance of health care is required to be aware of 20-30mmHg volatility in blood pressure rate or to understand and treat properly own blood pressure. as it is difficult to detect hypertension in early stage due to no initial symptom. According to the result. an establishment of management system of workers, companies and health care agencies is required for consist health care. 2. In terms of risky habits to health, the unhealthy group had more proportion of past smokers, over-twice-a-week drinkers, people with higher obesity rate. However, in terms of excercise, the proportion of regularly exercising people is higher in the unhealthy group while that of non-exercising people is higher in the healthy group. On the other hand. the average grade of health practicing behaviour in two groups are not significantly different as the health group had 3.00 out of possible 6.00 while the other had 3.10. This result means that as workers are not interested in health practicing behaviour. health promoting programmes must be developed in such a way of various method of motivations and incentives. Particularly this implies that distortional objectives of exercises should be readjusted through health guidance. 3. Systolic blood pressure in the healthy group can be explained by sex and the obesity rate while that in the unhealthy group can be explained by subjective health awareness and the obesity. Diastolic blood pressure in the healthy group can be explained by sex and the obesity rate like the former. The obesity rate was significant variable affecting the blood pressure of both groups, and particularly the effect to the unhealthy group was remarkably higher than that to the healthy group. Therefore, this research identified that the health care on the blood pressure of workers is not only limited to hypertension patients, but also extended to all workers. In order for consistent care, an establishment of management system of workers, companies and health care agencies is required.

A Study on the Prevalence Rate of Hypertension and the Actual Conditions of Control (일 지역 성인의 고혈압 유병률 및 관리 실태)

  • In order to analyse the prevalence rate of hypertension and the actual conditions of control, we selected five districts out of eleven eups and myuns in Chinan Country. We administered structured questionaries to 309 adults above the age of 40, computerized the data using SPSS - PC+. More than 40.1% of adults over 40 in Chinan County have health disorders ranging from high blood pressure to hypertension including alert high blood pressure at 36.2%, relatively high. Among general characteristics, differences in the rate of hypertension were influenced by age, occupation and places of residence. Over 71 who are engaged in agriculture, who don't have jobs, who reside in Sungsu, Jungchun, Chinan-eup all have higher hypertension rates than other groups. Accordingly, the control of hypertension should be focused on these people. As a result of the control of blood pressure, the survey showed 93.0% of the subjects were checked mainly at hospitals clinics, health centers subhealth centers and community health posts more than once a year, relatively high level of blood pressure management. However, the difference between their blood pressure measurements at ordinary times and the level of blood pressure at the time of research was quite considerable. Only 47.3% of the subjects diagnosed with high blood pressure and 70.3% of the subjects with normal blood pressure recognized their blood pressure accurately 52.7% of the subjects diagnosed with high blood pressure showed errors in understanding their blood pressure at normal times. Because these errors can cause problems in the control of blood pressure, proper management should be executed through a systematic examination. As a result of the high blood pressure control condition, the average period of hypertension was 74.5( ${\pm}92.8$) months, 92.3% of the subjects were diagnosed with high blood pressure at hospitals clinics, health centers subhealth centers community health posts, but only 29.5% were examined after a general check up on high blood pressure was completed. 70.5% were diagnosed with high blood pressure only after measuring their blood pressure. 14.1% of the subjects were hospitalized because of falls influenced by high blood pressure. 33.3% attended hospitals and health centers regularily for medical treatment and this shows how low the rate of the control of blood pressure. Most people did not undergo medical treatment, because they had no painful symptoms (46.7%), they didn't need to take the medicine(28.9%), or they forget to take the medicine(20.0%). These problems in the control of hypertension were discovered in the process of diagnosing high blood pressure at health medical institutions. Many people did not recognize the need for consistent control of blood pressure. That is, although the diagnosis for high blood pressures performed at hospitals clinics, health centers subhealth centers and community health posts, was 92.3%, more than 70.5% of the subjects were not examined completely with regard to blood pressure. Accordingly, heath medical institutions must diagnose high blood pressure not only by only measuring blood pressure but also by using systematic process of examination. As for the people diagnosed with high blood pressure, one should perform consistent medical approaches and help them to recognize the importance of the continuous control of blood pressure through subject-oriented education. Problems the subjects experienced were the following numbness in the limbs easily paralyzed stitches in their shoulders which felt painful, stiff necks, occiputs felt heavy, headaches when they got up in the morning, felt dizzy when standing and moving their heads and poor eyesight. The rate of knowledge related to high blood pressure was 78.7 points, comparatively low. Whether they had normal blood pressure or hypertension made no difference. These results are not desirable. Adult-oriented education forgot the prevention and management of high blood pressure should be implemented. Hypertensive-oriented education should be especially reinforced. Because there was a difference in the level of knowledge according to age, academic career, occupation or place of residence, education related to hypertension should be intensified and focused on those over the age of 71 those who did not attend school, those who do not have jobs and are engaged in agriculture and residents living in Bugui, Jungchun regions. The degree of healthy life practice in hypertensives is poor, particularly weight control, as opposed to people who have normal blood pressure. It makes no difference in smoking, the amount of daily smoking, drinking, the control of salt because each result means that they are not practicing healthy life or modifying their life-style. The development and programs to improve a healthy life should be executed.
